1.Analysis of risk factor and intervention of intracranial hemorrhage in the premature infants

Objective To explore the risk factors and seek effective intervention of intracranial hemorrhage in the premature in-fants .Methods Clinical data of the premature infants in our hospital from January 2009 to December 2013 was retrospectively ana-lysed and single factor analysis of 20 relevant factors was done for cases with intracerebral haemorrhage and without intracerebral haemorrhage .Logistic regression analysis were done for some influence factors of intracranial hemorrhage .Results 1 726 cases of premature babies were included in the study ,including 264 cases of intracranial hemorrhage .Logistic regression analysis results shown that the neonatal transport network and integrated active transport models are protective factors of intracranial hemorrhage in the premature infant .We found that basic-level hospital transport was an independent risk factor .Between January 2009 and De-cember 2011 ,142 of 714 premature infants were intracranial hemorrhage ,including 88 cases from 348 patients transported from bas-ic-level hospital ,the incidence of intracranial hemorrhage was 25 .29% ,and compared with the incidence of intracranial hemorrhage (14 .75% ) of our hospital ,the difference was statistically significant (P<0 .05) .From January 2012 ,we established perfect neonatal transport network and implementation of comprehensive active transport model .122 of 1 012 premature infants were intracranial hemorrhage ,including 75 cases of 490 patients from basic-level hospitals .The incidence was statistically significant different com-pared with the incidence of intracranial hemorrhage(9 .00% ) transported from our hospital(P<0 .05) .The incidence of intracranial hemorrhage in the premature infants transported from basic-level hospitals were statistically different before and after neonatal transport network and comprehensive active transport model was established (P<0 .01) .Conclusion It will effectively reduce the incidence of intracranial hemorrhage in the premature infant by establishing the perfect regional neonatal three-level network trans-port system and comprehensive active transport models .
2.Establishment of a Rat Model of Ulcerative Colitis by Immunological Sensitization Combined with Local Acetic Acid Irritation

Objective To establish a rat model of ulcerative colitis by immunological sensitization combined with local acetic acid irritation.Methods Thirty healthy 2-3-month-old Wistar rats were randomly divided into 3 groups:normal control group,model group and SASP-treated model group(n=10 rats per group).The ulcerative colitis in the model group and positive group was developed by immunological sensitization with rabbit colon mucosal proteins and local irritation with 5%acetic acid.The rats of the model and positive groups were fed with sulfasalazine,and rats of the control group fed with normal saline.The rats of positive control group were treated with sulfasalazine.Two weeks later,all rats were killed and colon mucosal and blood samples were collected.The concentrations of NO,NOS,MDA,SOD and GSH-Px in the colonic mucosal homogenate were measured by biochemistry,the contents of IL-4 and TNF-α were measured by RIA,and the serum concentration of IFN-γ was measured by ELISA.Results Compared with the normal control group,the concentration of MDA,NO,NOS in the colonic mucosa was increased,and SOD and GSH-Px levels significantly decreased in the model group.Compared with the model group,the concentrations of MDA,NO,and NOS in the colonic mucosal tissue were decreased,and the levels of SOD and GSH-Px decreased in the SASP-treated model group.The contents of IL-4 in the serum and colonic mucosal homogenate of model group were significantly lower, and the contents of TNF-α in the serum and colonic mucosal homogenate were significantly increased than those of the normal control group. The IL-4 levels in the serum and colonic mucosal homogenate was increased, and that of TNF and IFN-γ significantly decreased in the SASP-treated model group, compared with those of the normal control group.Conclusion The rat model of ulcerative colitis established by immunological sensitization combined with local acetic acid irritation, as described in the present study, can better simulate the characteristic chronic alterations in human ulcerative colitis,and may better serve studies and evaluation of treatment of this disease.
3.A prospective clinical study of endoscopic foam sclerotherapy combined with rubber band ligation in the treatment of grade Ⅱ-Ⅲ internal hemorrhoids (with video)

Objective:To evaluate the safety and long-term effectiveness of endoscopic foam sclerotherapy (FS) combined with endoscopic rubber band ligation (ERBL)in the treatment of grade Ⅱ-Ⅲ internal hemorrhoids.Methods:Consecutive patients diagnosed as having grade Ⅱ-Ⅲ internal hemorrhoids in Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from January to December 2020 were prospectively enrolled in the study, and randomly divided into ERBL group and FS combined with ERBL group. The 24 h visual analogue scale (VAS) for pain and 1-week degree of bleeding were evaluated after the treatment. After follow-up of 6 months, the effectiveness of treatment was evaluated.Results:A total of 84 patients with age of 54.4±7.9 years were enrolled, 57.1% (48/84) males, and 73.8% (62/84)grade Ⅱ internal hemorrhoids. Forty-three patients were assigned to the ERBL group and 41 to the FS combined with ERBL group. There was no significant difference between the two groups in baseline data ( P>0.05). In the FS combined with ERBL group, the mean amount of polidocanol foam was 13.8±2.5 mL, the mean number of injection site was 4.7±1.2, and the median scores of VAS was 0 (0, 3), which was significantly lower than that of ERBL group [2 (0, 4), Z=-2.116, P=0.034]. The bleeding rate 1 week after treatment in the ERBL and FS combined with ERBL group were 20.9% (9/43) and 29.3% (12/41), respectively, and mild bleeding was the main symptom. There was no significant difference between the two groups in the bleeding degree ( U=807.0, P=0.378). After 6 months of follow-up, the total effective rates in the ERBL group and the FS combined with ERBL group were 81.4% (35/43) and 90.2% (37/41), respectively ( U=684.5, P=0.044). Conclusion:FS combined with ERBL can effectively relieve post-treatment perianal pain, and improve the long-term effectiveness.
4. Rare primary proximal epithelioid sarcoma in skull base: clinical analysis of four cases

Objective:
To report the clinical and pathological features of primary proximal epithelioid sarcoma (PES) in skull base.
Methods:
The clinical and pathological features of four cases of PES in skull base from Sanbo Brain Institute of Capital Medical University and Kunming Sanbo Brain Institute were analysed retrospectively.
Results:
Three cases was female, and one male, the age ranged from 46 to 52 years.All cases occurred in skull base, and sellar region was the main site of involvement.Under the microscope, the tumor cells characterized by epithelioid cell changes, with or without rhabdoid tumor cells.Mitotic figure was active.Immunohistochemical staining showed that AE1/AE3, EMA and CD34 were variously expression in tumor cells.INI-1 protein was lost in all cases.Three cases were detected by FISH, and
