1.Clinical application of the obturator nerve block beside vessel in transurethral resection of bladder tumor

Objective To compare the operational feasibility and efficacy of inguinal obturator nerve block (ONB) beside the vessel by ultrasound combined with nerve stimulation and traditional ONB guided by nerve stimulation preventing obturator nerve reflex. Methods Forty patients with American Society of Anesthesiology (ASA)Ⅰ or Ⅱ, aged 35~91 years who underwent transurethral electric excision of bladder tumor in our hospital in 2014 were ran-domly divided into beside vessel group (group V, = 20) and traditional group (group T, = 20). Adductor strength was measured before and after the block. Frequencies of obturator nerve block puncture, operation time, visual ana-log scale (VAS) pain score, and incidence of complications were recorded during and after ONB. Results The suc-cess rate of the first puncture was significantly higher in group V than that in group T ( <0.01). The operation time in group V was significantly shorter than that in group T ( < 0.01). The decreased degree of adductor strength has significant difference of the two groups ( < 0.05 or < 0.01). The VAS pain score in group T was higher than that in groups V ( < 0.01). There are two cases contract blood vessel in group T, while none in group V ( < 0.01). There was no case with the local anesthetic toxicity and paresthesia in the area of the obturator nerve and the obtu-rator nerve injury in the two groups. Conclusion Compared with traditional ONB guided by nerve stimulation, in-guinal ONB beside the vessel by ultrasound combined with nerve stimulation showed more accurate positioning, less adjustment, less puncture time, more comfortable and safe.
2.Effect of Goal-oriented Repetitive Training on Upper Limb Motor Function after Stroke

Objective To explore the effect of goal-oriented repetitive training on motor function of upper limb in patients with stroke. Methods From March, 2014 to February, 2016, a total of 60 stroke patients were randomly divided into experiment group (n=30) and con-trol group (n=30). Both groups received routine rehabilitation, while the experiment group received goal-oriented repetitive training in addi-tion, 30 minutes once a day, 5 days a week for 4 weeks. They were assessed with simple Fugl-Meyer Assessment-Upper Extremity (FMA-UE) and modified Barthel index (MBI) before and after training. Results There was no difference in the scores of FMA-UE and MBI between two groups before training (Z<1.153, P>0.05). The scores of FMA-UE and MBI significantly improved in both groups after treat-ment (Z>5.645, P<0.001), in which the scores were higher in the experiment group than in the control group (Z>2.275, P<0.05), as well as the D-value of scores before and after training (t>5.770, P<0.001). Conclusion Goal-oriented repetitive training could promote the recovery of upper limb motor function in patients with stroke.
3.Establishment of a quality evaluation method for Angelica different processed products from genuine producing areas based on data mining.

The paper reports the development of a quality evaluation method for Angelica different processed products. The data of high-performance liquid chromatography, water, total ash and extract were analyzed with SPSS Clementine 11.0 software. Discriminant analysis (DA) established the classification model and parameter for Angelica different processed products. Fish's discriminant functions of Angelica different processed products were generated using 8 predictor variables selected from 59 indexes. The correct rate of discriminating back substitution is 96.7%. Angelica different processed products can be accurately and reliably recognized and validated with DA of SPSS Clementine 11.0 software.
4.Study on methodology of determination of Yizhi osmotic pump tablets active ingredient in Beagle dog plasma by LC-MS/MS.

OBJECTIVETo develop a sensitive and specific LC-MS/MS method for determination of Yizhi osmotic pump tablets active ingredient in Beagle dog plasma.
METHODBeagle dog plasma pre-treatment methods were established. Geniposide, notoginsenoside R1, ginsenoside Rg1 and Rb1 notoginsenoside molecular ions and fragment ions peaks were separated well and detected synchronously by LC-MS/MS with digoxin as internal standard.
RESULTUnder the selected LC-MS/MS conditions, the characteristic fragment ions of the four components could be well separated and quantified, and the calibration curves showed good linearity within a certain concentration range of each component; extraction recoveries of those four compounds in plasma were higher than 75%, method recoveries were higher than 90%; day precision (RSD <10%) and inter-day precision (RSD <15%) were generally fine; freeze-thaw and room temperature experiments showed good stability (RSD <15%).
CONCLUSIONThe method is proved to be suitable for pharmacokinetic studies of Yizhi active ingredients in Beagle dog plasma.

5.Analysis of the application effect with energy spectrum CT multi -parameter quantitative values in pulmonary qccupying lesions

Objective To evaluate the application value with Energy Spectrum CT multi -parameter quanti-tative value in differentiating pulmonary occupying lesions (>0.5cm in diameter).Methods Those were retrospec-tively analyzed on 58 cases in pulmonary occupying lesions performed spectrum dual -phase enhanced CT scan and confirmed by pathology,including malignant group 48 cases;10 cases of benign group.Iodine content(IC)was meas-ured in arterial phase(AP)and venous phase (VP)in iodine based on the image and the level of the aorta,and nor-malized iodine concentrations were calculated in the two phases lesions compared with aorta,and the difference between the two normalized iodine concentrations ICD (ICD =NICvp -NICap).To measure the CT value of lesions on 50keV and 100keV energy image,and calculate the energy attenuation curve slope,namely |Hu (100 kev -50kev)/Hu50 |.The differences of NIC,curve slope,and ICD were compared between benign and malignant pulmonary occupying lesions using independent sample t test method.Results In two phase scan,NIC,curve slope of the venous phase and ICD in malignant group were significantly higher than the benign group,(NIC,curve slope of the venous phase and ICD of the malignant group:NICap:0.180 ±0.051,NICvp:0.463 ±0.086,1.696 ±0.475,ICD:0.284 ± 0.071;NIC,curve slope of the venous phase and ICD of the benign group:NICap:0.123 ±0.062,NICvp:0.290 ± 0.119,1.169 ±0.582,ICD:0.166 ±0.073,),but there was no significant difference between the patients with benign and malignant lesions in curve slope of the arterial phase.Conclusion Energy spectrum CT dual -phase enhanced scan can differentiate the nature of benign and malignant pulmonary occupying lesions,and has certain clinical application value.
6.Immune effectiveness of A/H1N1 influenza vaccine immunization in 505 health care workers within three months after vaccination

Objective To investigate the immunization effect of influenza A/H1N1 vaccine in health care workers (HCW) in Inner Mongolia Greater Khingan Mountains area. Methods Five hundred and five HCW who received A/H1N1 influenza vaccination (immunized group) and 129 staffs who didn't receive the vaccination (unimmunized group) were randomly sampled for semiquantitative testing of serum H1N1 antibody (IgG) levels by enzyme-linked immunosorbent assay (ELISA).Results were analyzed and stratified by age, sex, occupation and the time interval between the time of vaccination and serum sample collection. The antibody positive rates of the two groups were compared by x2test. Results There were 401 (79. 4%) HCW whose H1N1 antibody were positive and 50 (9.9%) whose antibody were weak positive among 505 immunized HCW. While among 129 unimmunized HCW, there were 59 (45.7%) whose antibody were positive and 15 (11.6%) whose antibody were weak positive. The seroconversion rates of specific antibody were not significantly different among the different age groups after receiving A/H1N1 influenza vaccine (P> 0.05).However, there were statistical differences of the seroconversion rates among different sex groups (men 95.7% vs women 87.4% in immunized group, x2=6.40, P<0.05; and men 73.3% vs women 52.5% in unimmunized group, x2 =4.07, P<0.05) and different occupation groups (doctor 86.0% vs nurse 94.5% in immunized group, x2 = 9. 16, P<0.01; and doctor 43. 8% vs nurse 75.0% in unimmunized group, x2=12.61, P<0.01 ). The seroconversion rate was 81.5% after 80 to 89 days of vaccination, which was significantly lower than those after 30 to 39, 50 to 59 days and 60 to 69 days of vaccination, which was 100.0%, 94.7% and 93.6%, respectively (x2 =3.96, P <0.05; x2=7.15, P <0. 01; x2 = 9. 98, P<0. 01). Conclusions A/H1N1 influenza vaccination can induce effective immune response in HCW in Greater Khingan Mountains area of Inner Mongolia. However,the level of specific antibody significantly reduces after 80 to 89 days of vaccination.
7.Quality control and discrimination of angelica different processed products based on HPLC fingerprints combined chemometrics methods.

OBJECTIVETo establish a chemical fingerprint method for reorganizing and validating angelica different processed products.
METHODA high-performance liquid chromatographic method was developed to establish the fingerprint. Principal component analysis, hierarchical cluster analysis and discriminate analysis were applied to study HPLC finger printing and chemical pattern reorganization.
RESULTThere were difference of characteristic peaks and its relative peak area of HPLC fingerprints between different processed products. Fish's discriminate functions were generated by using six selected predictor variables, the tested samples of different processed products were classified with 100% accuracy, and discriminate analysis plots for the five groups were well-resolved.
CONCLUSIONThe developed HPLC finger print, combined with chemometrics, can accurately identify and validate angelica different processed products, the research provide theoretical basis for the processing mechanism and quality assess of angelica different processed products.

8.Analysis of using Danhong injection to treatment coronary heart disease patients medicines based on real world HIS database.

OBJECTIVETo analyze the rules of medicinal groups that doctors treat angina pectoris using Danhong injection.
METHODTwo thousands and seventy cases from hospital information system of 10 hospitals in Beijing 3a grade hospital were collected. Then use frequencies to find the most common drug and use association rules with apriori algorithm to analyze doctor's prescription compatibility rules curing coronary heart disease.
RESULTThe compatibility among aspirin, oral nitrate, oral dose of clopidogrel and oral dose of Xintong was the most frequently used medicinal groups with Danhong injection in real world HIS database.
CONCLUSIONDanhong injection was used at treatment of coronary heart disease is often combination with similar pharmacological effects chemical drugs. Doctors should balance the benefits and the risks.

9.Analysis of clinical use of Danhong injection based on hospital information system.

OBJECTIVETo know how Danhong injection is used in clinical practice and to provide a reference for guiding clinical use of Danhong Injection.
METHODExtract Danhong injection's post-marketing re-evaluation data from the Hospital Information System of ten three grade III-A General Hospitals in Beijing, use basic statistical analysis methods to analyze Danhong injection's indications, usage and dosage, days of treatment etc. in clinical practice.
RESULTIn patients using Danhong injection, there were more than 60 percent patients were prescribed based on main-diagnosis, first-visit and other diagnosis, which were also coincided with Danhong injection's instruction. In clinical practice, 95.5 percent of Danhong injection's administration routes conformed to the instruction and more than 90 percent of Danhong injection's dosage were within the limits prescribed by the instruction. Danhong injection was used less than 20 days successively. Danhong injection was used in combination with drugs with the action of removing blood stasis, antianginal drug, antiplatelet drug, drugs for cerebrovascular disease and so on.
CONCLUSIONDanhong injection has been used according to instruction in practice almostly.

10.Exploring real world: analysis about Shenmai injection data extracted from hospital information system database in clinical practice.

OBJECTIVETo analyze the clinical application of Shenmai injection and to learn the rule of it in the real word so that we can provide inferences to doctors.
METHODCases using Shenmai injection in hospital information system from 10 grade A hospitals in Beijing were collected. Descriptive statistic methods were used to analyze the usage, dosage and medication time.
RESULTFor the clinical application of Shenmai injection, the average age of the patients using Shenmai injection was about 64.68, but most of them came from cardiovascular diseases department Shenmai injection was usually used to treat coronary disease in Western medicine and Xiongbi (Chinese medicine syndrome name) in traditional Chinese medicine. The course of treatment was from one day to seven days, which was in accord with the drug instruction as well as the dosage.
CONCLUSIONShenmai injection was usually used for the old people. In clinical practice, the use of Shenmai injection was basically in accord with the drug instruction by doctors. However, there are still some of the doctors who did not follow the instruction and should be careful of this.
