1.Retrospective effect analysis of integration of traditional Chinese medicine and western medicine in treatment of multiple myeloma

Objective To find out a treatment with high remission rate,long living period,and a good quality of life for elderly patients with multiple myeloma.Methods All patients were recruited into a treatment group(treated with traditional Chinese medicine,routine chemotherapy,and thalidomide)and a comparison group(treated with traditional Chinese medicine and routine chemotherapy).Results The total effective rate of the treatment group was 71.4%,higher than the comparison group(38.5%),but showing 110 statistical difference(P=0.128>0.05).Median survive time of two groups were 21.8 months and 12 months respectively,(P=0.001<0.01).The survival rate of 3 years and 5 years were 28.6%,15.4%and 7.1%,0 in the two groups respectively,without significant difference(0.317,1.000,both P>0.05).Conclusion The treatment group showed higher results in both complete remission rate(CR)and very good partial remission rate(VGPR)than the comparison group,demonstrating a better results in improving the patient's quality of life.The treatment group also had a higher value of the median survive time and the median progression-free surial time than the comparison group.The combined therapy of traditional Chinese medicine,routine chemotherapy and thalidomide is an ideal choice for both aged Patients or young MM patients who had no transplant conditions.
2.Preparation of elastic porous cell scaffold fabricated with combined polydimethylsiloxane (PDMS) and hydroxyapatite (HA).

Polydimethylsiloxane (PDMS) and hydroxyapatite (HA) were combined in our laboratory to fabricate an elastic porous cell scaffold with pore-forming agent, and then the scaffold was used as culture media for rat bone marrow derived mesenchymal stem cells (rBMSCs). Different porous materials (square and circular in shape) were prepared by different pore-forming agents (NaCl or paraffin spheres) with adjustable porosity (62%-76%). The HA crystals grew on the wall of hole when the material was exposed to SBF solutions, showing its biocompatibility and ability to support the cells to attach on the materials.

3.Physician-nurse collaboration and its related factors in public Class Ⅲ general hospitals

Objective To explore the current status of physician-nurse collaboration in public Class Ⅲ general hospitals and analyze its related factors so as to provide a basis and data support for pushing forward physician-nurse collaboration and improving negatively related factors. Methods From April to June 2018, totals of 465 nurses and 249 physicians form 6 public ClassⅢ general hospitals in Beijing were selected as subjects by convenience sampling and cross-sectional study. The questionnaire survey was carried out with the Chinese version of Nurse-Physician Collaboration Scale(NPCS). The related factors of physician-nurse collaboration were analyzed with the simple correlation and multiple linear regression. Results The scores of NPCS of 465 nurses and 249 physicians were(78.81±17.68)and(84.86±15.22)respectively. Simple correlation showed that there were statistical differences in the scores of NPCS among physicians and nurses with the different personnel categories,physician-nurse working relationships,physician-nurse working mode, whether to carry out clinical pathways or not and income satisfaction(P< 0.05). Multiple linear regression revealed that the related factors of physician-nurse collaboration included the physician-nurse working mode, personnel categories,income satisfaction and clinical pathways with statistical differences(P < 0.05). Conclusions Public Class Ⅲ general hospitals have good physician-nurse collaboration. However,physicians and nurses lack communication in participating the treatment and nursing decision making for patients. Managers should further deepen the reform of personnel system,implement position management and performance incentive systems,carry out the physician and nurse responsibility system and physician-nurse working mode and positively develop clinical pathways so as to promote physician-nurse collaboration.
4.Depression contributed a dissatisfied cervical surgery outcome of the posterior decompression in cervical spondylotic myelopathy

Objective To investigate the effect of depression symptoms on surgical outcome of posterior decompression among cervical spondylotic myelopathy (CSM) patients. Methods Between October 2006 and October 2011 in our hospital, lami?nectomy or laminoplasty was performed in 396 cases that were enrolled in the study. There were 132 males and 264 females with an average age of 60.2 years(ranged,39-84 years). All patients were divided into depression group and non?depressed group by the 21?item Beck Depression Inventory (BDI). There were no statistically significant differences between groups in age, sex, smok?ing status, duration of symptoms, and employment status (whether in the current working). The Japanese Orthopedic Association (JOA) scores, Neck disability index (NDI) and visual analogue scale (VAS) were compared after 1.5 months postoperatively. Re?sults All of 396 cases were followed up. The mean follow up duration was 32 months (range,24-50 months). There were no statistically significant differences in the CCI decline (7.1%±2.1% versus 6.8%±1.5%), expansion degree[(130.9±7.0) mm2 versus (150.8 ± 5.2) mm2] and the drift?back distance of the spinal cord [(5.7 ± 1.2) mm versus (6.2 ± 0.8) mm]. However, pa?tients with continuous depression showed poorer improvement than non?depressed patients in the surgery outcome: JOA im?proved (1.42±0.56 versus 6.76±3.12); NDI declined (7.31±2.18 versus 21.11±11.36); and VAS lightened (16.08±19.76 versus 23.85±20.79). Conclusion Depression contributed a dissatisfied surgery outcome after posterior decompression on functional re?covery, disability index and pain scores among patients of the cervical spondylotic myelopathy.

7.Investigation on evidence-based nursing ability of nurses and influencing factors analysis in a tertiary general hospital in Beijing

Objective? To investigate the status and influencing factors of nurses' evidence-based nursing ability in a tertiary general hospital in Beijing. Methods? A total of 463 nurses from a tertiary hospital in Beijing were selected for research by convenience sampling method. Evidence-based nursing practice ability rating scale was used to evaluate nurses' evidence-based nursing ability, and a self-designed influencing factor questionnaire was used to investigate and analyze the influencing factors. 463 questionnaires were collected and 463 were valid. Results? The total score of 463 nurses with evidence-based nursing ability was (43.05±24.25). The average score was (1.87±1.05). The score of the evidence retrieval and evaluation ability dimension was (1.72±1.18), the score of the evidence integration ability dimension was (1.54±1.21), the score of the evidence dissemination ability dimension was (1.89±1.22), and the score of situational evaluation and evidence application dimension was (2.20±1.04). Univariate analysis showed the scores of nurses' evidence- based nursing ability were different with 10 different characteristics:initial education, final education, working years, English level, whether received evidence-based training, knowledge of evidence-based nursing, and whether considered evidence-based nursing is helpful for clinical work, considered whether it is necessary to carry out evidence-based practice projects, whether to consult the literature, whether you have tried evidence-based nursing practice, the differences were statistically significant (P< 0.05). Multiple linear regression analysis showed years of work, whether to consult the literature, and considered whether it is necessary to carry out evidence-based practice projects, whether you have tried evidence-based nursing practice were the factors affecting the nurse's evidence-based ability score. Conclusions? In this study, the level of evidence-based nursing ability of nurses needs to be improved. The nurses, who are with longer working years, limited literature consulting, consider no need to carry out evidence-based practice, and have no evidence-based practice experience, have lower ability in evidence-based practice. It is suggested that nursing managers should pay attention to the cultivation of evidence-based nursing ability of senior nurses, organize literature review and evidence-based nursing training to increase knowledge, create an organizational environment for evidence-based nursing to change attitudes, give full play to leadership to support evidence-based nursing behaviors of nurses, and improve their evidence-based nursing ability.
8.Working status of wound, ostomy and continence specialist nurses in China

Objective:To understand the current situation of use and management of wound, ostomy and continence specialist nurses in China, and provide a basis for further improving the use and management of specialist nurses.Methods:Using the convenient sampling method, a questionnaire survey was conducted among 1 020 wound, ostomy and continence specialist nurses trained by 21 provincial nursing societies and 12 international enterostomal therapist schools across China from December 2018 to January 2019. On the basis of literature review, we designed the Questionnaire of Work Status of Specialist Nurses of Wound Ostomy Incontinence, which was divided into three parts: general information, work status and hospital management with 33 items in total. The research group communicated with the leaders of 12 international stomatological therapists schools and the chairman of the professional committee of wound stomatological incontinence of 21 provincial nursing societies in China. After informed consent was obtained, the electronic questionnaire link was pushed and sent to the corresponding specialist nurses by the leaders and chairman of each school.Results:In this survey, 1 020 questionnaires were collected and invalid questionnaires were eliminated. A total of 885 valid questionnaires were collected. 81.5% (721/885) of nurses were engaged in specialized work part-time, among which clinical specialized nursing, nursing consultation, difficult case discussion and nursing ward round were the main work undertaken by specialist nurses. In terms of education, specialized nurses mainly undertake the work of health education for patients, teaching of continuing education projects and training and teaching of in-hospital specialized nurses. In terms of scientific research, 40.8% (361/885) of specialized nurses undertook scientific research projects and 41.2% (365/885) of specialized nurses published papers as the first author or corresponding author. In terms of specialist consultation, other nursing staff , patients or family members and doctors consulted specialist nurses for specialist questions. In terms of professional promotion, specialist nurses organized specialist studies, participated in standard setting and served on professional committees. Wound, ostomy and continence specialist nurses had difficulties in management support and lack of knowledge and skills. The hospitals where 37.1% (328/885) specialized nurses worked had no relevant rules and regulations for wound, ostomy and continence specialist nurses.Conclusions:Wound, ostomy and continence specialist nurses play an important role in many aspects and their role development is restricted. It is necessary to give more time, space, training and management support to specialist nurses. It is urgent to improve the job management of specialist nurses, improve relevant rules and regulations and promote the professional development of wound, ostomy and continence specialist nurses.
9.Effect of the tight-knit medical alliance model of precise assistance on nursing quality in assisted hospitals

Objective:To evaluate the effect of the tight-knit medical association model of precise assistance on nursing quality in assisted hospitals.Methods:The tight-knit medical association model of precise assistance was applied to the assisted hospital, the first stage assistance was provided from January 2015 to December 2016 and the second stage assistance was provided from January 2017 to December 2018. Before the assistance (December 2014) , after the assistance (December 2016) and the second year after the assistance (December 2018) , a total of 210 nurses and 210 patients were respectively selected to investigate the satisfaction with the convenient sampling method. The incidences of falls, in-hospital stress injuries and unplanned extubation in the assisted hospital before and after the assistance were compared.Results:In the second year after assistance, the satisfaction rate of nurses in the assisted hospital was 90.476% (190/210) , which was higher than 69.524% (146/210) before assistance and 78.571% (165/210) after assistance, and the differences were statistically significant ( P<0.01) . In the second year after the assistance, the satisfaction rate of patients in the assisted hospital was 93.810% (197/210) , which was higher than 72.381% (152/210) before the assistance and 82.857% (174/210) after the assistance, and the differences were statistically significant ( P<0.01) . After the assistance, the incidences of falls, in-hospital stress injuries and unplanned extubation of patients in the assisted hospital were lower than those before the assistance, and the differences were statistically significant ( P< 0.05) . Conclusions:The tight-knit medical alliance model of precise assistance can improve the satisfaction of nurses' and patients' satisfaction with nursing work in the assisted hospitals and reduce the incidences of falls, in-hospital stress injuries and unplanned extubation, which can improve nursing quality in assisted hospitals.
10.Construction of nursing quality evaluation index system for the disabled elderly in medical-nursing combined institutions

Objective:To construct the nursing quality evaluation index system for the disabled elderly in medical-nursing combined institutions, so as to provide a basis for nursing managers to evaluate nursing quality of the disabled elderly in medical-nursing combined institutions.Methods:A total of 20 experts were selected by the convenient sampling method. From May 2020 to January 2021, the research team used literature analysis to construct the evaluation index framework of nursing quality for the disabled elderly in medical-nursing combined institutions and conducted two rounds of expert consultation among 20 experts through Delphi expert consultation method to determine the evaluation index system and weight of nursing quality for disabled elderly people in medical-nursing combined institutions.Results:The recovery rate of the two rounds of expert consultation questionnaire was 100.00% (20/20) , and the expert authority coefficient was 0.880. The Kendall's concordance coefficients of the first and second rounds were 0.138 and 0.128, respectively ( P<0.01) . Finally, the evaluation index system of nursing quality for the disabled elderly in medical-nursing combined institutions was constructed, including 3 first-level indicators, 8 second-level indicators and 43 third-level indicators. Conclusions:The nursing quality evaluation index system for the disabled elderly in medical-nursing combined institutions established in this study is scientific and reliable, which can provide a basis for nursing managers to quantitatively evaluate the nursing quality of the disabled elderly.
