1.Purification,renaturation and antiviral effects of recombinant cyanovirin-N on herpes simplex virus type 1

Objective To purify,renature and explore the antiviral effects of recombinant cyanovirin-N(CV-N)on herpes simplex virus type 1(HSV-1).Methods The recombinant CV-N was purified with Ni Sepharose column and renatured by dilution method.The antiviral activities of CV-N were carried out in Vero cells by observing cytopathic effect(CPE)and by using MTT colorimetric assay for vital cell rate.Results SDS-PAGE showed that the purified protein was in the position of 11KDa with only one clear band.The renatured CV-N had little cytotoxic effect on Vero cells,it could not directly inactivate HSV-1 infectivity.CV-N not only interfered in adsorption of HSV-1 to Vero cells but also inhibited HSV-1 biosynthesis in the cells,which were more effective than the positive control Acyclovir.Conclusion CV-N exhibited pronounced antiviral activities agaist HSV-1,further development of CV-N might yield novel candidates of antiviral drugs.
2.Effects of Different Doses of Diphtheria Toxin on the Cochlear Structure and AuditoryFunction in Wildtype Auditory-Mature Mice

Objective To investigate the effects of different doses of diphtheria toxin on cochlear structure and auditory function of adult wildtype mice.Methods The auditory-mature wild type C57BL/6J mice 4 weeks old were randomly devided into 50 ng/g group, 100 ng/g group and control group.C57BL/6J mice in the 50 ng/g or 100 ng/g group were injected 50 ng/g or 100 ng/g diphtheria toxin intraperitoneally for one time, respectively, and the control mice were injected equal volume of normal saline for one time.Then we investigated the ABR threshold change and morphological change of inner and outer hair cell and spiral ganglion neuron 7 days after the injection.Results At 7 day post diphtheria toxin injection compared with those of in control group, in the 50 ng/g group, there was no threshold elevation across frequencies(8 kHz ABR threshold was 20.0±3.78 and 20.83±2.04 dB SPL for 50 ng/g and control respectively), and no loss of inner and outer hair cells (for both groups, the HC loss rates were 0.3%~1%) or SGN (the SGN density was 39.45±3.65, 41.03±3.73/105 μm2, in 50 ng/g and control, respectively).However, the 100 ng/g group, compared with those of in control group, the ABR threshold (8 kHz ABR threshold was 63.0±4.47 dB SPL, respectively)was significantly elevated across each frequency(t=19.62,P<0.001), and there was significant loss of outer hair cell (the loss rate of IHC and OHC was 0.5%±0.1%, 10.7%±0.3%, respectively), which was 10% loss in the apical, middle and basal turn(t=42.219,P<0.001).And the loss of spiral ganglion neuron (the SGN density was 25.55±3.66/105 μm2) was 38%, which was significantly different from the control(t=10.985,P<0.001).Conclusion High dose injection of diphtheria toxin can cause loss of outer hair cell and spiral ganglion neuron in wild type auditory-mature C57BL/6J mice.
3.The efficacy of metacognitive training in inpatients with schizophrenia: a randomized controlled study

Objective To explore the feasibility and efficiency of metacognitive training (MCT) in inpatients with schizophrenia,as well as the short-term effects of metacognitive training on symptoms,cognitive function,and insight.Methods Sixty inpatients that met the DSM-Ⅳ criteria for schizophrenia were randomly assigned to either MCT (research group,n =30) or an active controlled intervention (control group,n =30).Positive and Negative Syndrome Scale (PANSS),Schizophrenia Cognition Rating Scale (SCoRS),Insight and Treatment Attitude Questionnaire(ITAQ) and beads task are assessed at baseline and after completion of the training.Results After treatment,the scores of the two groups in PANSS,SCoRS,ITAQ and the bead task were significantly improved(P<0.05).The improvement of positive symptoms,cognitive function and insight in research group (12.8 ± 4.2,31.7 ± 13.9,9.2 ± 6.1 respectively) was significantly better than that in control group (15.6±5.9,38.1± 18.1,5.5±5.8 respectively),and the difference was statistically significant (P< 0.05).Conclusion Metacognitive training can effectively improve the clinical symptoms,cognitive function and insight for inpatients with schizophrenia.
4.Effects of Creatine Phosphate Sodium on Plasma Cardiotrophin-1 and N Terminal Probrain Natriuretic in Elderly Hypertensive Patients with Heart Failure

Objective To investigate the effect of hosphocreatine therapy on the plasma cardiotrophin-1(CT-1) and N terminal probrain natriuretic (NT-proBNP) in elderly hypertensive patients with heart failure. Methods A total of 76 hy-pertensive patients with heart failure, aged 65 or over, were randomly divided into treatment group and control group (n=38 for each group). The control group received routine anti-heart failure treatment. The treatment group received conventional therapy plus creatine phosphate sodium for 2 weeks. The plasma levels of CT-1 and NT-proBNP were determined in two groups. The plasma CT-1 level was measured by a double antibody sandwich enzyme-linked immunosorbent assay (ELISA). The plasma level of NT-proBNP was tested by Rui Pu fluorescent dry quantitative analyzer. Results The plasma levels of CT-1 and NT-proBNP were significantly lower after treatment in two groups (P<0.01). The plasma levels of CT-1 and NT-proBNP were significantly decreased in treatment group than those in control group (P<0.05). The total effective rate was 89.47%in treatment group, which was significantly higher than that of control group (71.05%, P<0.05). Symptoms of heart failure improved in one week (21 cases in treatment group/9 cases in control group) and in two weeks (13 cases in treatment group/18 cases in control group). Conclusion The conventional therapy plus creatine phosphate sodium can decrease the plasma CT-1 and NT-proBNP levels in elderly hypertensive patients with heart failure, which improves symptoms of heart failure in a shorter period of time.
5.Effects of Low Dose Ultrashort Wave Therapy on Inflammation and Edema after Spinal Cord Injury in Rats

Objective To investigate the effect of ultrashort wave therapy on edema and inflammatory reaction after spinal cord injury (SCI) in rats. Methods 56 Sprague-Dawley rats were divided into sham-operated group (n=8), model group (n=24) and ultrashort wave group (n=24). The model was established with Allen's method. The sham-operated group was exposed endorhachis without hit. The ultra-short wave group was exposed to ultrashort wave radiation 7 minutes, once a day, 24 hours after modeling until the animals were sacrificed. Locomotors functional recovery was assessed every week post operation period by BBB score. Immunohistochemical staining was per-formed to observe the expression of the aquaporin-4(AQP-4) and ED-1. Results The BBB scores increased in the model group and the ultra-short wave group after treatment, and was higher in the ultrashort wave group than in the model group from 1 week after treatment (t>3.368, P<0.01). The expressions of AQP-4 and ED-1 were higher in the model group and ultrashort wave group than in the sham-operated group, and decreased as time went on. They were lower in the ultrashort wave group than in the model group(t>3.156, t>4.466, P<0.05). Conclu-sion Ultrashort wave therapy can alleviate the edema after SCI, reduce the activation and infiltration of inflammatory cells, and promote the recovery of neurological function.
6.Complementary self-treatment for posterior canal benign paroxysmal positional vertigo.

OBJECTIVE:
To examine the value of self-treatment for Posterior canal benign paroxysmal positional vertigo (PC-BPPV). The treatment effect was compared between patients treated with modified Epley in outpatient clinic combined with self treatment at home and patients treated by modified Epley alone.
METHOD:
A randomized controlled trial were carried out in the Department of Otolaryngology Head and Neck Surgery, the Affiliated Sixth People's Hospital of Shanghai Jiao Tong University from December 2012 to May 2013. 147 out of 150 patients with unilateral idiopathic BPPV-PSC were enrolled in follow-up. Among which, 73 patients were allocated in modified Epley-alone group and 74 were allocated in slef-treatment group.
RESULT:
The success rate was 53.4% in modified Epley-alone group vs 83.8% in self-treatment group (P < 0.01) after 1 week treatment. In the modified Epley-alone group,the success rate of hand repositioning group and chair-assisted repositioning group was 45.9% vs 61.1% (P > 0.05), the risk rate was 0.752, 95% CI (0.486 - 1.163). In the self-treatment group, the success rate of hand repositioning group and chair-assisted repositioning group was 87.5% vs 81.0% (P > 0.05), the risk rate was 1.081, 95% CI (0.888-1.316). Incidence rate of serious complications was 0% in the modified Epley-alone group and 1.3% in the self-treatment group (P > 0.05).
CONCLUSION
Complementary self-treatment with modified Epley maneuver treated PC-BPPV sooner and more effectively, Compared with modified Epley maneuver alone. And its incidence rate of serious complications didn't increase. Chair-assisted repositioning showed better result than hand repositioning alone, and self-treatment at home can reduce the effect of the gap. We found that complementary self-treatment with modified Epley maneuver had more benefits for patients with PC-BPPV.

7.Effects of Bone Marrow Stromal Cells Transplantation Combined with Ultrashort Wave Therapy on the Expression of GFAP and ED-1 after Spinal Cord Injury in Rats

Objective To investigate the effects of bone marrow stromal cells(BMSCs)transplantation combined with low dose ultrashort wave (USW)radiation on functional recovery and the expression of glial fibrillary acidic protein(GFAP)and ED?1 after spinal cord injury(SCI)in rats,and further discuss its action mechanism. Methods Female Sprague?Dawley rats(n=30)were randomly divided into 5 groups:sham?oper?ated,as well as control,USW,BMSCs,and USW+BMSCs that were subjected to spinal cord injury(SCI). Basso?Beattie?Bresnahan(BBB)tests were carried out before the operation and at 1 d,1 week,2 weeks,3 weeks,4 weeks after SCI. 4 weeks later,animals were sacrificed and tissues were collected to make paraffin section. Immunohistochemical staining was performed to observe the expression of GFAP and ED?1. Results 4 weeks after SCI,BBB scores were significantly higher in the USW and USW+BMSCs groups than in the control group(both P<0.001). No signifi?cant difference was observed between the BMSCs group and control group. On the expression of GFAP ,only USW+BMSCs group showed signifi?cantly decreased compared with the control group(P<0.05). All treatment groups exhibited lower ED?1 expression than the control group(all P<0.05). Conclusion Our results indicate that USW radiation alone can obviously improve neural functional recovery after SCI. The USW radi?ation and BMSCs transplantation treatment can reduce inflammation ,and USW radiation is more effective. The combination therapy did not show a synergistic action on promoting functional recovery ,but do have an effect on reducing the inflammatory response and glial scar formation.
8.The clinical and pathological features of Alport syndrome in children

Objective To analyze the clinical and pathological characteristics of Alport syndrome in children. Methods Clinical and pathological information gathered from 62 patients during March 1989 to August 2012 was retrospectively analyzed. Results Four autosomal recessive Alport syndromes (AR-AS) and 58 X-linked Alport syndromes (XL-AS) were analyzed. Of the XL-AS, 47 were boys and 11 were girls. Most of patients induced by upper respiratory tract infections, and onset with hematuria and proteinuria. There was no signiifcant gender difference in family history, impaired renal tubular proteins, hypertension, im-paired renal function, hearing loss, ocular abnormalities or renal pathological changes under light microscopy. However, extensive lamination and split of glomerular basement membrane (GBM) dense layers were found in 83.0%male and 18.2%female patients (P=0.000) and the rest patients were presented with limited distribution of typical GBM changes. Proteinuria progressed signiif-cantly with age in XL-AS males (r=0.501, P=0.000). Five XL-AS patients developed to end stage renal disease (ESRD) between 11 to 16 years old. Conclusions XL-AS is the main inherited type and severe changes of GBM are common in XL-AS males. Proteinuria increases remarkably with age. The detection of type IV collagen in renal tissue or skin is helpful to diagnose Alport syndrome and conifrm inheritance modes.

Non-alcoholic fatty liver disease (NAFLD) is a series of chronic liver diseases strongly associated with the metabolic disorder with an increasing rate of worldwide prevalence.Due to its complicated pathogenesis, only Saroglitazar has been approved by Indian Drug Controller General (DCGI) as a PPAR-α/γ dual agonist to treat non-cirrhotic non-alcoholic steatohepatitis.Combination therapy, which can target same or different signaling pathways of NAFLD pathogenesis, has been developed to achieve synergistic therapeutic efficacy.Currently, small-molecule drug combination, RNAi combination therapy, and chemogene therapy are proposed as promising strategies in NAFLD treatment.In addition, designing a smart, safe and effective drug delivery system is key to realizing the druggability, clinical translation and industrialization of small molecule drugs and gene drugs.This review summarizes the research status and delivery system of small-molecule drug combination, RNAi combination therapy, and chemogene therapy, in the hope of providing some novel insight for the treatment of NAFLD.
10.Study on extraction and clathration process of volatile oil in Tianma-Shouwu tablet

Objective To optimize the extraction and β-cyclodextrin clathration process of volatile oil in Tianma-Shouwu tablet.Methods The extraction of volatile oil was optimized through the orthogonal design L9(34) with the volatile oil extraction amount as assessment index. The saturated aqueous solution preparation of clathrate form was used. The volume of β-cyclodextrin and volatile oil, clathration temperature, clathration time was investigated by central composite design-response surface methodology, with the score of volatile oil inclusion ratio and utilization ratio as the index. The results were fitted by second-order polynomial equation, and then the best clathration process was determined. Results The optimization extraction process of volatile oil in Tianma-Shouwu tablet was as follows:to add 8 times water,immerse 30 minutes,steam distill 6 hours;and the best clathration process was as follows: the ratio of β-cyclodextrin and volatile oil was 9.50:1, clathration temperature 43 ℃, and clathration time 4.2 hours. Conclusions The optimized extraction and clathration process were reasonable, stabled and repeatable.
