1.Study on the effect of bacillus licheniformis and live combined bifidobacterium and lactobacillus on chemotherapy- induced diarrhea

Objective To study the effect of bacillus licheniformis and live combined bifidobacterium and lactobacillus on chemotherapy-induced diarrhea (CID). Methods Seventy-two gastric cancer patients with CID were collected and the clinical data were analyzed retrospectively. The patients were divided into 4 groups according to different treatment methods with 18 cases each. The patients in group A were treated with montmorillonite powder, the patients in group B were treated with montmorillonite powder and bacillus licheniformis, the patients in group C were treated with montmorillonite powder and live combined bifidobacterium and lactobacillus, and the patients in group D were treated with montmorillonite powder, bacillus licheniformis capsule and live combined bifidobacterium and lactobacillus. The patients with severe diarrhea in 4 groups were treated with rehydration, maintaining water and electrolyte balance and nutritional support. The Karnofsky performance scale score (KPS score), diarrhea grading before and after treatment and treatment effect were recorded. Results The KPS score after treatment in group A, group B, group C and group D were significantly higher than that before treatment: (70.6 ± 10.6) scores vs. (62.2 ± 12.2) scores, (76.1 ± 7.8) scores vs. (61.7 ± 9.9) scores, (77.2 ± 7.5) scores vs. (61.1 ± 10.8) scores, (83.9 ± 5.0) scores vs. (63.9 ± 10.9) scores. Moreover, The KPS score in group B, group C and group D were significantly higher than that in group A, the KPS score in group D was significantly higher than that in group B and group C, and there were statistical differences (P<0.05). The diarrhea grading after treatment in 4 groups were improved compared with that before treatment. Furthermore, the diarrhea grading in group B, group C and group D were significantly better than that in group B and group C, and there were statistical differences (P<0.05). In group A, there were 2 cases with excellent effect, 10 cases with effect and 6 cases with no effect;in group B, there were 8 cases with excellent effect, 8 cases with effect and 2 cases with no effect;in group C, there were 7 cases with excellent effect, 9 cases with effect and 2 cases with no effect; in group D, there were 9 cases with excellent effect, 9 cases with effect and 0 case with no effect. The treatment effect in group D was significantly better than that in the other 3 groups, and there were statistical differences (Hc = 10.81, P<0.05). Conclusions Bacillus licheniformis and live combined bifidobacterium and lactobacillus is more effective in the treatment of gastric cancer patients with CID.
2.The immunomodulatory function and clinical applications of mesenchymal stem cells

BACKGROUND: Mesenchymal stem cells (MSCs) have the abilities of self-renewal, multidirectional differentiation and immunomodulation, and have become the focus of current research. OBJECTIVE: To summarize the immunomodulation of MSCs to different immune cells and the clinical applications of MSCs in the treatment of immune-related diseases. METHODS: The first author searched the PubMed and the CKNI databases for relative articles from January 1974 to December 2016. The key words were mesenchymal stem cells, immunomodulation, MSC1 and MSC2, autoimmune diseases in English and Chinese, respectively. Finally, 52 representative articles were included. RESULTS AND CONCLUSION: MSCs can inhibit the function of T lymphocytes, reduce the activation, proliferation and antibody secretion of B lymphocytes, affect the polarization of macrophages, inhibit the maturation of dendritic cells, inhibit the proliferation and toxicity of NK cells, so MSCs have the great potential in the treatment of immune-related diseases. However, MSCs exhibit the opposite immunomodulatory abilities under different inflammatory microenvironments, and moreover the definite and controllable mechanism of this phenomenon is still unclear, Therefore, future investigations may focus on the specific mechanism of MSCs in the clinical treatment of immune-related diseases.
3.Pareto Chart Analysis of Unreasonable Factors of Prescription in Outpatient and Emergency Departments of Our Hospital during 2013-2015

OBJECTIVE:To investigate the current situation of unreasonable prescriptions in outpatient and emergency depart-ments of our hospital,and to provide reference for rational drug use in the clinic. METHODS:The evaluation of outpatient and emergency prescriptions in our hospital during 2013-2015 were summarized and analyzed. Irrational prescriptions were divided into 3 categories,involving 19 items. The main,secondary and general factors of irrational prescriptions were analyzed by Pareto chart. RESULTS:Among 19 types of irrational prescriptions,main factors included unsuitable drug selection,inappropriate usage and dosage,inappropriate drug combination,prescribing not in accordance with the antibiotics management regulations,incomplete di-agnosis. No indications of medication,drug dosage beyond the regulations ofthree urgent seven slowwere treated as the second-ary factors. Other 12 items were general factors. CONCLUSIONS:According to the main and secondary irrational drug use types, clinical pharmacists should carry out effective intervention and pharmaceutical care to improve the level of rational drug use and en-sure the safety of clinical drug use.
4.Evaluation of the effect of instant blood glucose monitoring system in perioperative blood glucose management in patients with hip fracture complicated with diabetes mellitus

Objective:To explore the effect of instant blood glucose monitoring system (FGM) in perioperative blood glucose management in patients with hip fracture complicated with diabetes mellitus, and to provide guidance for perioperative blood glucose management.Methods:A total of 100 patients with hip fracture complicated with diabetes mellitus treated in the Department of Orthopaedics, Gongli Hospital, Pudong New Area, Shanghai from July 2018 to July 2020 were selected as subjects.The patients were divided into experimental group and control group by random digits table method with 50 cases in each group. The patients in both groups adopted the mode of multidisciplinary cooperation.The experimental group was used to monitor blood glucose based on FGM, and traditional blood glucose monitoring was used in the control group. The waiting time before operation, the pain caused by blood glucose acupuncture, the satisfaction of patients and medical staff, the blood glucose level at different time points and the incidence of adverse reactions were compared between the two groups.Results:The waiting time before operation, the proportion of patients with moderate and severe pain after acupuncture and the incidence of hyperglycemia in the experimental group were (2.58 ± 1.30) d, 2% (1/50) and 30% (15/50), respectively, the control group were (3.67 ± 1.59) d, 22% (11/50) and 50% (25/50), respectively. The differences were statistically significant( t value was -2.087, χ2 values were 9.470, 4.170, P<0.05). The satisfaction of patients and medical staff in the experimental group were (46.43 ± 1.87), (46.58 ± 2.10) points, respectively, the control group were (40.67 ± 3.24), (43.84 ± 2.92) points, respectively.The differences were statistically significant( t values were 8.441, 8.087, P<0.05). There was no significant difference in fasting blood glucose, 2-hour postprandial blood glucose and pre-bedtime blood glucose between the two groups( P>0.05). Conclusions:The application of FGM can realize the continuous blood glucose monitoring and management of patients,making the blood glucose reach the standard more quickly and smoothly, meanwhile it may effectively shorten the waiting time before operation and reduce the acupuncture pain of blood glucose monitoring and recognize patients with concealed abnormal blood glucose. The application can improve the satisfaction of patients and medical staff, and promote the rapid recovery of patients as well.
5.Bilberry extract induces apoptosis of Hela cells and its action mechanism

Objective: Bilberry extract can inhibit the growth of cancer cells,but its effect on cervical carcinoma is rarely reported.This study was to explore the effects of bilberry extract on the expressions of Bcl-2,Bax,Fas and FasL in Hela cells.Methods: We determined the apoptosis of Hela cells by Hoechst33342/PI fluorescent staining and DNA ladder and Annexin V/PI double-staining after exposed to bilberry extract at the concentrations of 0.025-25 ?g/ml for 24 hours,and the protein expressions of Bcl-2,Bax,Fas and FasL were detected by Western blotting.Results: Obvious apoptosis-associated morphological changes and specific DNA ladder bands were observed in the Hela cells after exposed to bilberry extract at the concentrations of 0.025,0.25,2.5 and 25 ?g/ml,and the apoptosis rates of the cells were(4.13 ? 0.63)%,(5.41 ? 0.77)%,(8.74 ? 1.27)% and(12.05 ? 1.03)%,respectively.Western blot showed increased expressions of bax,Fas and FasL,and a decreased expression of bcl-2.Conclusion: Bilberry extract induces the apoptosis of Hela cells by up-regulating the protein expressions bax,Fas and FasL and down-regulating that of bcl-2.
6.Clinical Study on Compound Sophora Injection Combined with Java Brucea Fruit Oil Emulsion for Palliative Treatment of Advanced Malignant Tumors

Objective To observe the short-term clinical effect of compound sophora injection combined with Java brucea fruit oil emulsion injection for palliative treatment of advanced malignant tumors. Methods Totally 120 cases of advanced malignant tumor patients were randomly divided into treatment group and control group, each contains 60 cases. The control group was treated with Java brucea fruit oil emulsion, and the treatment group took compound sophora injection additionally by intravenous infusion one time daily. The cycle was 24 d. After 3 cycles, the curative effect, pain relief, improvement situation of quality of life and adverse reactions of the two groups were observed. Results Feasible curative effect evaluation of 112 patients showed:The disease control rate in the treatment group was 89.5% (51/57), and the control group was 74.5% (41/55). The total effective rate of cancer pain relief of the treatment group was 91.2%(52/57), and the control group was 70.9% (39/55). The improvement rate of life quality of the treatment group was 70.2% (40/57), and the control group was 49.1% (27/55). The differences between the two groups was significant (P<0.05). There was no difference in hematology toxicity, liver and kidney function, gastrointestinal adverse reactions and itchy skin incidence between the two groups (P>0.05). And the treatment group had a lower incidence of phlebitis than the control group (P<0.05). Conclusion Compound sophora injection combined with Java brucea fruit oil emulsion for palliative treatment can improve curative effect, significantly improve patients’ quality of life, it was a safe and effective way in treating advanced malignant tumors.

8.EFFECT OF PINE-SEED OIL ON LIPID METABOLISM-RELATED GENE EXPRESSION IN ADIPOSE TISSUE OF MICE

Objective To study the influence of pine-seed oil on lipid metabolism-related gene expression. Method The mice were fed by basic food,high fat food (80% basic food added 20% lard, HF) and HF added pine-seed oil (80% high fat food added 20% pine-seed oil, SHF). At 45 d and 90 d, the ACS mRNA, FATP mRNA, HSL mRNA and PPAR mRNA of adipose tissue were measured respectively by RT-PCR. Results At 45 d, compared with the basic group, ACS mRNA, FATP mRNA, HSL mRNA and PPAR-? mRNA expression level in SHF group increased by 24.2%, 8.8%, 10.9%, 21.6% respectively,and at 90 d,increased by 19.6%, 25.7%, 60.1%, 13.1% respectirely. Compared with the basic group,ACS mRNA, FATP mRNA, HSL mRNA and PPAR-? mRNA expression level in HF group increased by 27.9%, 41.7%, 51.1%, 23.9% respectively. Conclusion Pine-seed oil is significantly effective in regulation of lipid metabolism.
9.Psychological investigation and analysis of 116 earthquake patients from Sichuan

Objective:To investigate the psychological state of the patients from Wenchuan Earthquake treated in some local and military hospitals.Methods:We consulted the general data and pathogenetic condition of 116 patients from the earthquake-stricken area of Sichuan,studied their mental health using the Hamilton Anxiety Scale(HAMA) and investigated their psychological changes during the rescue and treatment with self-designed questionnaires.Results:The earthquake patients manifested obvious anxiety in the initial stage and their mental health was gradually improved towards the end of the rescue process.Conclusion:Wenchuan Earthquake caused great psychological trauma in the earthquake patients,for whom timely psychological intervention is of utmost importance.
10.Susceptibility to Antifungal Agents in 111 Candida Strains Causing Nosocomial Deep Fungal Infections

OBJECTIVE To study pathogen distribution and susceptibility to sevenral antifungal agents.METHODS The CHROMagar candida color medium was applied to isolate and identify Candida strains.Drug sensitivity was analyzed by the K-B diffusion method.The drug sensitivity to econazole(ECO),nystatin(NYS),itraconazole(ITR),ketoconazole(KET),fluconazole(FLU),flucytosine(FCT),and amphotericin B(AMB)was tested.RESULTS Candida albicans was the most frequent isolated organism accounting for 62.5%.C.tropicalis and others accounted for 21.62% and 8.1%,respectively.The respiratory tract infection accounted for 76.57%,and the urinary tract was the second(6.31%),the blood and genitourinary tract were the third(5.00%).The susceptibility of Candada spp strains to nystatin and amphotericin B was more active than to other antifungal agents.CONCLUSIONS The fungal infection is relative to some risk factors,such as using antibiotics,underlying diseases and aging.Therefore,using antibiotics reasonably and improving the immunity are the main measures of preventing the fungal infection.
