1.Analysis of the Utilization of Perioperative Haemocoagulase Hemostatic Drugs in Our Hospital in 2014

OBJECTIVE:To provide reference for the rational use of haemocoagulase hemostatic drugs in the clinic by under-standing the perioperative utilization. METHODS:In retrospective study,1 200 discharge records of perioperative prophylactic ap-plication of haemocoagulase hemostatic drugs were collected randomly during Jan.-Dec. 2014.,and then analyzed statistically in re-spects of consumption sum and amount,application,irrational drug use,etc. The cost of hemocoagulase accounted for 7.39% of total drug cost,among which Haemocoagulase agkistrodon for injection had the highest consumption amount and sum,with utiliza-tion ratio of 51.44%. Average medication time of hemocoagulase during perioperative period was 2.36d;maximal average cost of hemocoagulase was 684.52 yuan in ENT and head and neck surgery department/ stomatology department;ENT and head and neck surgery department/ stomatology department,urinary surgery department had long average medication time,being 3.85d and 3.50d. RESULTS:Only 39.42% haemocoagulase hemostatic drugs were reasonably used during perioperative period. The unreasonable manifestation included:improper medication time(52.15%), too long medication course after operation(35.75%),wrong usage (5.61%)and replacing various haemocoagulase hemostatic drugs without reason(5.06%). CONCLUSIONS:The striking phenome-non of irrational use of haemocoagulase hemostatic drugs during perioperative period is severely worrying,so we need to strength-en the standard management and pharmaceutical supervision and intervention by clinical pharmacists.
2.The relationship between serum leptin and fibrosis in patients with chronic hepatitis B incorporate alcoholic liver disease

Objective To study the relationship between serum leptin and fibrosis in chronic hepatitis B incorporate alcoholic liver disease(CHB + ALD) patients.Methods Select 17 male patients with CHB+ALD,15 male patients with liver cirrhosis(LC) ,19 male patients with chronic hepatitis B(CHB) and 12 male healthy persons as normal control(NC).Serum leptin is detected by ELISA in 63 patients.Serum levels of fibrosis markers(HA,LN,PC HI ,IV-C) are determined at the same time.Results Serum leptin level in CHB +ALD are significantly higher than those of the NC(6.79±24.12) μg/L vs (4.27±7.18 ) μg/L (P<0.05).There is no significant difference between patients with CHB and the NC.The four fibrosis markers in CHB + ALD patients are significantly higher than that of men both in the CHB and the NC.There are significant correlations between both serum leptin level and quadrinomial fibrosis markers(P<0.05 or P<0.01 ).Conclusion That serum leptin level is one aecelerate gene in the fibrosis formation in patients with CHB+ALD.
3.Construction Effectiveness Analysis of Rational Drug Use Monitoring System in Our Hospital

OBJECTIVE:To explore the construction effect of rational drug use monitoring system on the improvement of clini-cal drug use in the hospital. METHODS:The data about the construction of rational drug use monitoring system were collected from our hospital,including pharmacist workstation prescription check(check before prescription/medical order charging to realize warning in advance),clinical pharmacy management system prescription comment(realize prescription/medical order comment after-wards)and adverse drug reaction/adverse drug event(ADR)reporting information platform(realize online ADR reporting)during 2014-2015. The data was analyzed to evaluate the effectiveness of system construction. RESULTS & CONCLUSIONS:There were a total of 3 417 329 prescriptions audited by the pharmacist workstation within 2 years through improving system functions,includ-ing 7 315 prescriptions were returned to doctors to be modified,the percentage of which declined from 0.39% to 0.08%. The pass rate of prescription comment pre-judgment increased from 81.2% to 90.4% in 2015 by clinical pharmacy management system. The valid case number of ADR monitoring and reporting increased from 186 cases to 267 cases,involving all department of our hospi-tal. The construction of rational drug use monitoring system in our hospital can improve the standardization of prescription/medical orders,the level of rational drug use and the case number of ADR,which has achieved the expected results.
4.Discussions on Problems about the Monitoring System for Rational Drug Use and Relevant Countermea-sures

OBJECTIVE:To analyze the problems about the prescription review by the monitoring system for rational drug use (PASS 1.6.1.7) and discuss the countermeasures. METHODS:A retrospective method was adopted. A total of 4 942 prescriptions of our hospital in the first quarter of 2015 were selected at random,and reviewed by PASS and the pharmacist respectively. The re-view results were analyzed,and then countermeasures were put forward for the problems found to exist in the software review. RE-SULTS & CONCLUSIONS:The conformity rates of prescriptions which had been determined as acceptable through PASS review and pharmacist review were 81.20% and 97.40% respectively. PASS misjudged 929 (18.80%) rational prescriptions as irrational ones,mainly concerning administration and dosage,simultaneous existence of Western medicine and Chinese patent medicine on one prescription,repeated drug use and use with caution,etc. PASS misjudged 122(2.47%)irrational prescriptions as rational pre-scriptions,mainly concerning administration and dosage,inconformity between diagnosis and drug use,etc. The above-said prob-lem was attributed to imperfect hospital management and insufficient software intelligence. It is suggested that the hospital should strengthen the data maintenance and management of information system and PASS developer should improve software rules to in-crease the accuracy rate of prescription review.
5.Gaucher disease II:a case report

Objectives To investigate the clinical characteristics and diagnosis of Gaucher disease. Methods The clini-cal features, enzyme activities and genetypes of an infant with Gaucher disease II were analyzed. Results The main clinical manifestations of the infant included hepatosplenomegaly, ocular dyscinesia and mental retardation. Gaucher′s cells could be seen in the bone marrow aspirates. Glucocerebrosidase activity was low (0.3 nmol/g/min). Serum chitotriosidase activity was high (87317 nmol/L/min). GBA mutations were M85T (c.371T>C) and R120W (c.475C>T). Conlusions Main features of Gau-cher disease II are hepatosplenomegaly with nerve system injury. Glucocerebrosidase activity and gene analysis are important for the diagnosis of Gaucher disease.
6.Selection and analysis of targeted cell-binding mimicry peptides in endometrial adenocarcinoma cell line through random peptide phage display

AIM: To isolate targeted the peptides that binding and internalizing into endometrial carcinoma cell line EAC. METHODS: The tumor cells were screened five rounds of whole cell screen through the Ph.D.-12 phage display library. Analysis of the displayed peptides specific binding efficiency to the tumor cells was proceeded. The DNA of phages was extracted, sequenced and translated to the sequences of amino acid and analysis using computer software. RESULTS: After five biopannings, the isolated phages showed high specificity and strong affinity for their cognate cell types relative to different cell lines. Through sequencing, the sequences of displayed peptides were obtained. CONCLUSIONS: Whole cell screening against EAC cells through random phage peptide library can obtain phage peptides with a highly tissue specific binding and internalizing ability. The peptides do provide a basis for tumor's targeted delivery as therapy vector.
7.Clinical features of childhood eosinophilic gastroenteritis

ObjectiveTo analyze clinical manifestations and treatment of eosinophilic gastroenteritis (EGE) in children.MethodsThe clinical data of 10 children with EGE under the treatment from June 2014 to June 2012 were analyzed retrospectively.ResultsThere were 10 children (6 males and 4 females) aged from 9 months to 14 years. Clin-ical symptoms of eight preschool and school age children included abdominal pain (8 cases), vomiting (4 cases), diarrhea (3 cases), ascites (3 cases), rash (3 cases), fever (2 cases) and hypoalbuminemia (2 cases). Clinical symptoms of two in-fant and toddler were diarrhea and hematochezia. Peripheral blood eosinophils were increased in 10 children. IgE was elevated in 9 children. One child had very high C-reactive protein. Endoscopic examination showed mucosal congestion, swelling, erythema, erosion. Multiple sites of biopsy showed eosinophil count> 20/HP. One child was diagnosed by lap-aroscopic exploration. The treatment included diet avoidance, omeprazole, anti-allergic drug. Four children were treated with corticosteroids. All children had been followed up for 6 months to 2 years. One child had relapsed.Conclusion The clinical manifestations of EGE are various. Endoscopic biopsy is necessary for diagnosis. Laparoscopy also is help-ful for diagnosis. Dietary and glucocorticoid therapy were effective.
8.Electrical stimulation relieves ischemia of rat skeletal muscle via angiogenesis

Objective To investigate the effects of electrical stimulation on the expression of vascular endothelial growth factor(VEGF)mRNA and receptor FLK 1/KDR in a ischemic model of rat hindlimb. Methods The model of hindlimb ischemia on the right side was established by ligation of the superficial femoral artery in 10 rats. The rats were then randomized into an experimental group and a control group. The rats in the experimental group were intervened with electrical stimulation ( 25 Hz, 0.1 V) on the tibialis anterior (TA) of the right side, while those in the control group were not. RT PCR and immunohistological methods were used to detect the expressions of VEGF mRNA and protein in TA muscles. FLK 1/KDR was detected by means of Western blot and immunofluorescence. Results After 7 days of continuous stimulation, there was a significant increase in blood flow within the muscle. 9.Role of androgen receptor in prostate cancer cell proliferation by use of RNAi

AIM: To study the role of androgen receptor (AR) in hormone-dependent and hormone-independent prostate cancer cell proliferation by knocking down AR expression with adenovirus-delivered siRNA. METHODS: Four well-designed siRNAs were synthesized and inserted into the adenovirus plasmid pShuttle-H1-Ri. The recombinant pShuttle-H1-Ri-AR plasmid was then co-transfected with pcDNA-AR to HEK293 cell line and Western blot was used to detect the inhibitory efficiency of different siRNAs on AR expression. Recombinant adenovirus containing more efficient siRNAs were prepared and used to infect three different humane prostate cancer cell lines including LNCapC4-2B and CWR22Rv1. The efficiency of knocking down AR expression was detected by Western blot. The effect of AR-knocking down on cell proliferation was detected by MTT colorimetric assay. RESULTS: All of the four designed siRNAs could knock down AR expression in transient co-transfection. Infecting with recombinant adenovirus containing more efficient siRNAs in hormone-dependent and hormone-independent prostate cancer cell lines specifically knocked down AR expression with high efficiency. Knocking down AR expression significantly decreased the proliferation rate in all these prostate cancer cells. CONCLUSION: The suppressed expression of AR in prostate cell lines mediated by siRNA could efficiently inhibit the cell proliferation, and these results show that AR plays an important role in the proliferation of hormone-dependent and hormone-independent prostate cancer cells. AR is an important therapeutic target for the treatment of prostate cancer.
10.Whole cell screening of phage-display peptide library for mimicry peptides of glioma SWO-38

AIM and METHODS:The Ph.D.-7 phage display library was used to isolate peptides specific for glioma SWO-38 cell by whole cell screening.Moreover,binding efficiency analysis was carried out to test the binding specificity of the clones obtained. RESULTS: After three rounds of biopanning,a high concentration of phage clones was obtained and two of them were found to be highly specific to glioma SWO-38. CONCLUSION: Highly specific clones against neurtral glioma cells can be obtained from a phage display library by simple procedures.
