1.Determination of matrine and phillyrin in Jieyinjing Suppositories by HPLC

AIM: To establish the content determination method of matrine and phillyrin in Jieyinjing Suppositories(Radix Sophorae Flavescentis,Fructus Forsythiae,Cortex Phellodendri Chinensis,Radix Stemonae,etc.) by HPLC. METHODS: HPLC was performed to determine matrine on ODS column,the mobile phase consisting of methanole-0.02% triethylamine(55∶45) and detection wavelength at 220 nm and the phillyrin of mobile phase consisting of acetonitrile-water(25∶75) and its detection wavelength at 277 nm. RESULTS: The linearity of matrine was in the range of 9.8 ?g?mL~(-1)-49.0 ?g?mL~(-1)(r=0.999 96),and the average recovery was(99.48%,) RSD=1.49%;The linearity of phillyrin was in the range of 5.4 ?g?mL~(-1)-54.0 ?g?mL~(-1)(r=(0.999 9),) and the average recovery was 100.07%,RSD=1.34%. CONCLUSION: This method is simple,sensitive and accurate,and available for control of Jieyinjing Suppositories.
2.Determination of Naringin in Bogu Pill by HPLC

Objective To establish a method for the content determination of naringin in Bogu Pill by HPLC. Methods At room temperature and by using ultrasonic extraction, HPLC was performed to determine naringin content on ODS chromatographic column.The mobile phase consisted of a mixture of acetonitrile and 8 % acetic acid (15 ∶ 85)and detection wavelength was at 283 nm.Results The linearity of naringin was in the range of 5.2 ? g / mL~ 31.2 ? g / mL (r=0.9999) and the average recovery was 98.72 % , RSD=1.55 % . Conclusion This method was simple, sensitive and accurate, and can be used for the quality control of Bogu Pill.
3.Effect of puerarin on the expression of HIF-1αin placenta of rats with preeclampsia

Objective To investigate the effect of puerarin on preeclampsia-like rat placental tissue hypoxia inducible factor (hypoxia-inducible factor,HIF)-1αexpression and study the mechanism of protective effects in rats.Methods 60 SD rats were randomly divided into control group,drugs intervention groups(low-dose,middle-dose,high dose)and model group.The rats in drugs intervention groups and model group were adopted endotoxin for induction of preeclampsia model.On the basis of the model group,low,medium and high dose groups were given 20.0,40.0,60.0 mg/kg puerarin by intraperitoneal injection,respectively.The blood pressure of tail artery,urine protein concentration were observed at different points and the expression of HIF-1αwere detected by PT-RCT in each group.Results The systolic blood pressure ,urine protein and HIF-1αprotein in model group were higher than control group(P<0.05 );serum NO concentration was lower than control group (P<0.05 );after the puerarin intervention,systolic blood pressure and urine protein concentration in drugs intervention groups were lower than model group (P<0.05 ),plasma NO concentration was higher than model group (P<0.05 )and HIF-1αprotein was lower than model group (P<0.05 );the placenta weight,weight and body length of newborn rats in drug intervention group were higher than model group (P<0.05 ).Conclusion Puerarin can reduce systolic blood pressure and urine protein levels,promote the level of NO in plasma and reduce HIF-1αlevel in placenta of rats.Puerarin can offer protection to rats with preeclampsia by decreasing the expression of HIF-1αin placental.

Through this way,it is easier for the students to get interested in this chapter and grasped the content.
5.Exchange transfusion therapy of neonatal hyperbilirubinemia in 5 patients

Objective To investigate changes in serum bilirubin and internal environment,etc.of neonatal hyperbilirubinemia that before and after the treatment of peripherally arteriovenous synchronous exchange transfusion. Methods In the strict monitoring of vital signs,hyperbilirubinemia peripheral vessels exchange transfusion was applied in five cases of neonatal,and the serum bilirubin,routine blood test,electrolytes before and after the exchange transfusion were detected.Results By comparing the data before and after the exchange transfusion,the total bilirubin and indirect bilirubin decreased obviously,respectively[(414.02 ±56.04)μmol/L,(225.72 ±53.54)μmol/L,t =5.433 P =0.01,(381.82 ±44.68)μmol/L,(199.22 ±57.37 )μmol/L,t =5.615 P =0.01 ],the difference was statistically significant,and also the platelet count were decreased obviously,respectively [(199.00 ± 42.01)×109 /L,(102.00 ±17.72)×109 /L,t =4.758,P =0.001],the difference was statistically significant.After the exchange transfusion,white blood cells,hemoglobin,potassium,sodium,chloride,calcium,glucose,blood pressure, heart rate and blood PH value had decreased or increased slightly,there was no significant difference P >0.05.Cere-bral palsy and other complications did not occur after 12 months of follow -up of the five cases.Conclusion The outer peripheral arteriovenous synchronous exchange transfusion therapy for neonatal severe hyperbilirubinemia can quickly reduce serum bilirubin,and has less impact on the body environment,it is an important and effective salvage treatment of neonatal hyperbilirubinemia,thereby reducing damage to the nervous system of bilirubin.
6.Effects of Systemic Nursing Intervention on Constipation after Coronary Heart Disease

Objective To explore the clinical effect of systemic nursing intervention on constipation after coronary heart disease. Methods 145 patients were randomly divided into intervention and control group. Patients in invention group were given systemic nursing intervention, including diet, exercise, health education and so on. The patients in control group were treated with cathartic.Results There was significant difference between intervention and control group in incidence of constipation or related complication(P<0.05). Conclusion Systemic nursing intervention is helpful to reduce constipation and prevent the related complication.
7.A study on clinical pathological features and pathogenesis of liver fibrosis in children with Wilson’s disease

Objective To explore the clinical pathological features of Wilson's disease and the pathogenesis of liver fibrosis. Methods The clinical data and liver biopsy specimens obtained from 48 children with Wilson's disease were analysed. The pathological changes were studied with light microscopy, electron microscopy, combined with rhodanine and rubeanic acid for copper staining, Gordon-Sweet's staining for reticular fibers and Masson's staining for collagen fibers. Meanwhile, immunohistochemistry was used to investigate the expression of tissue inhibitors of metalloproteinase (TIMP) -1 and TIMP-2. The apoptosis of activated hepatic stellate cells (HSC) in liver tissues was illustrated with in situ end labeling (ISEL)(TUNEL POD method) and ?-SMA double staining. Results In all the cases, the mean onset age was 10.0?3.8 years, and the positive rates of family history, Kayser-Fleischer’s ring and decreased serum ceruloplasmin level were 29.2%, 68.8% and 93.0%, respectively. The levels of serum ALT, AST, ALP and ?-Glo were 3.6, 3.0, 2.7 and 2.0 fold of normal cutoff values. The major pathological changes in childhood patients with Wilson’s disease presented various chronic inflammatory changes in hepatic acini and portal tracts, interface hepatitis, focal or diffuse vesicular/ microvesicular steatosis, with large and irregular apoptotic bodies, Mallory's bodies, glycogenated nuclei, and eosinophilc granular hepatocytes. Among all the cases, 77.0% of liver specimens were positive for rhodanine and rubeanic acid staining for copper in hepatocyts, especially in the zone I of acinus. Ultrastructural observation showed swollen and unusual giant mitochondria, increased lysosomes and vesicular inclusions in hepatocytes. The incidence of hepatic fibrosis was 100%, presenting expanded portal tracts in the early, fibrotic septa in the moderate and cirrhosis in the late stage. The extent of TIMP-1 and TIMP-2 expression and number of activated HSC were increased in various degrees in all the liver specimens, while apoptotic HSCs were obviously decreased in the majority of cases. Conclusions The clinical and pathological changes of children with Wilson’s disease are varied and relatively obscure, and liver fibrosis appears early and progressive. Excessive activation and proliferation of HSC stimulated by liver injury and inflammation due to copper deposition and the decrease in activity of matrix degradation enzymes might be the important mechanisms underlying the occurrence and progression of hepatic fibrosis in Wilson's disease.
8.Comparison of the diagnostic value between ECG and dynamic ECG for coronary heart disease arrhythmias

Objective To investigate and compare the clinical values of routine electrocardiogram and dynamic electrocardiogram in arrhythmia in coronary heart disease diagnosis.Methods 73 patients with coronary heart disease were collected and monitored by routine electrocardiogram and dynamic electrocardiogram,and two groups were e stablished including control group using routine electrocardiogram and observation group using dynamic electrocardiogram.The positive diagnosis rate and the specific diagnosis diresults of arrhythmia in coronary heart disease of both groups were compared.Results The positive diagnosis rate of arrhythmia in coronary heart disease of observation group(75.3% ) was significantly higher than that of control group(54.8% ) ( x2 =6.78,P <0.05).And the positive diagnosis rate of the observation group was also significantly higher than that of the control group in ventricular premature beats two,triple law,pairs of premature ventricular contractions,atrial premature beats two,triple law,atrial premature beats in pairs,atrioventricular block and paroxysmal ventricular tachycardia( all P < 0.05 ).Conclusion Compared with routine electrocardiogram,the dynamic electrocardiogram in anrnythmia in coronary heart disease diagnosis could obtain higher positive diagnosis rate,and it is worthy of clinical application.
9.Nursing cooperation of hand-assisted thoracoscopic surgery for simultaneous operation of bilateral lung metastasectomy

ObjectiveTo explore the points of nursing cooperation during hand-assisted thoracoscopic surgery for simultaneous operation of bilateral lung metastasectomy.Methods The preoperative preparation,nursing cooperation during the surgery,the cleaning and maintenance of surgical instruments and equipment after operation were retrospectively summarized in 72 patients who underwent hand-assisted thoracoscopic surgery for simultaneous treatment of bilateral pulmonary metastases. ResultsAll the procedures of surgery were smooth.No patients were converted to thoracotomy or died of surgery. Conclusions It is an important challenge for nursing quality in the operation room and the cleaning and maintenance of surgical instruments and equipment in carrying out hand-assisted thoracoscopic surgery.The skilled nursing cooperation and standardized operating procedures could improve the quality of intraoperative nursing cooperation.It could ensure the smooth process of simultaneous operation of bilateral lung metastasectomy.
10.Clinical Assessment of Chinese Recognition in Patients with Right Cerebral Hemisphere Lesion

Objective:TO provide theoretic evidence for Chinese rehabilitation training to study the characteristics of right hemisphere in recognizing Chinese and association between left and right cerebral hemispheres.Methods:102 cases up to the qualification,including 41 patients with right-hemisphere-lesion(RHL)、 31 patients with left-hemisphere-lesion(LHL) and 30 healthy individuals as control,were collected and tested with The Standardized Aphasia Battery in Chinese(ABC)、Writing-scopy of Chinese Character and Visual Spatial Structure-scopy.Results:14 aphasic patients,including 1 RHL and 13 LHLs,were found 11 of 72 patients had apparent reading disorder,including 7 LHLs with language reading disorder and 4 RHLs with neglect reading disorder;and 29 of 72 patients had apparent writing disorder,including 13 LHLs with language writing disorder and 16 RHLs among whom 9 cases had visual space writing disorder and 7 cases had language writing disorder;RHLs showed lighter listenning comprehension disorder than that of LHLs in the test.Conclusion:Right hemisphere is mainly involved in Chinese written language recognition and processing,and one of the most important reasons of reading and writing disorder is left side neglect、visual consciousness and spatial analysis dysfunction due to RHL.
