2.Enzyme Kinetics of 6 Specific Substrates of Cytochrome P_(450) Enzymes

OBJECTIVE:To study the enzyme kinetics of six specific substrates of cytochrome P450 enzymes. METHODS: The transforming amount of the corresponding metabolites of six substrates was determined by LC-MS/MS. The incubation conditions of substrates in human liver microsomes were optimized and the parameters of enzyme kinetics were calculated based on substrate concentration-reaction velocity curve. RESULTS: The optimal incubation time was 20 min and the optimal enzyme concentration was 0.25 mg protein?mL-1. The Km(?mol?L-1) values of phenacetin, tolbutamide, omeprazole, dextromethorphan,chlorzoxazone and nifedipine were 21.6,163.4,10.4,4.6,41.9 and 9.4 respectively. The Vmax (nmol?min-1?mg protein-1) values were 2.08, 0.52, 1.02, 0.40, 2.36 and 1.00 respectively. CONCLUSION: The enzyme kinetics of six specific substrates of cytochrome P450 enzymes has been investigated in this study, and the study serves as a reference and support for the study and application of substrates in CYP.
3.Expression of promyelocytic leukemia gene in peripheral blood lymphocytes from patients with psoriasis vulgaris

Objective To detect the expression of promyelocytic leukemia(PML)gene in peripheral blood lymphocytes from patients with psoriasis vulgaris,and to investigate the relationship between PML gene and psoriasis.Methods This study included 50 patients with vulgaris psoriasis who were aged between 18 and 65 years and had never received systemic treatment,as well as 45 healthy controls.Peripheral blood samples were collected from these subjects,and lymphocytes were separated by density-gradient technique.Then,RNA was extracted from the lymphocytes by using RNAprep pure blood kit followed by real time fluorescence-based quantitative reverse transcription-PCR for the detection of PML mRNA expression.Flow cytometry was performed to quantify the expression of PML protein in the lymphocytes.Independent samples t test was carried out to compare the differences in PML expression between the patients and controls.Results The relative quantity (RQ)of PML mRNA was 14.98 ± 3.64 in peripheral blood lymphocytes from the patients,significantly higher than that in the healthy controls(5.50 ± 1.10,t =16.79,P < 0.05).Similarly,a significant increment was observed in the fluorescence intensity of PML protein in peripheral blood lymphocytes from the patients compared with the healthy controls(3.13 ± 0.27 vs.2.43 ± 0.21,t =6.93,P < 0.05).Conclusions PML is overexpressed at the protein and mRNA levels in peripheral blood lymphocytes from patients with psoriasis vulgaris,hinting that PML may play a certain role in the development of psoriasis.

Objective To understand the epidemiological characteristics of hypertension in Chifeng.Methods 6 015 subjects aged over 15 years old were selected with stratified randomized sampling.The history of hypertension and social-economic status were investigated by face-to-face interview and blood pressure,body height,body weight and waistline were measured at the same time.Results The prevalence of hypertension in Chifeng was 28.5%.The age-adjusted prevalence was 19.09%.Thereinto,the prevalence of hypertension in urban area,rural area,pasturing area were 23.15%,19.85%,32.03%,respectively.The Hypertension prevalence in male was increased with age.The prevalence in male was higher than in female among people who lived in urban and pasturing area(P0.05).The prevalence were higher in illiterate person and semiliterate than people in other educational level(P

BACKGROUND:Bone marrow mesenchymal stem cells can differentiate into osteoblasts under inducing condition that Zouguiwan and Youguiwan coordinate inducers, but the mechanism remains to be discussed. OBJECTIVE:To observe the effects of serum containing Zuoguiwan and Youguiwan on transforming growth factorβ1 and its signal transduction protein Smad2/3 message expression during the osteogenic differentiation of bone marrow mesenchymal stem cells. METHODS:A whole bone marrow adherence method was adopted to isolate and cultivate bone marrow mesenchymal stem cells from rats. The cellcultivation was processed in five groups:bone marrow mesenchymal stem cells were respectively cultured with blank serum, serum containing Zouguiwan, serum containing Youguiwan, positive serum containing progynova+inducer (dexamethasone, vitamin C, andβ-glycerophosphate), and inducer. Western blot was applied to detect the expression of type I col agen. The immunohistochemical assay was utilized to test transforming growth factorβ1 and Smad2/3 expression in the osteoblasts. RESULTS AND CONCLUSION:It was apparently more significant for serum containing Zuoguiwan and Youguiwan on type I col agen, transforming growth factorβ1 and Smad2/3 expression, compared with blank serum group and inducer group (P<0.05);moreover, serum containing Zuoguiwan was better than serum containing Youguiwan (P<0.05). Both of serum containing Zuoguiwan and Youguiwan are able to promote osteogenic differentiation of bone marrow mesenchymal stem cells. Moreover, Zuoguiwan is much more effective indicating that this method of traditional Chinese medicine about nourishing kidneys can be better to promote osteogenic induction of bone marrow mesenchymal stem cells.
6.A multi-factor analysis of the relationship between changes of item scores of MMSE and the incidence of dementia in community elderly

Objective To study the rule of dynamic change of cognitive function in community elderly and predicting for dementia through using items of MMSE. Methods A 10 year follow up of cognitive function survey was carried out in the non dementia old people using Chinese version Mini Mental State Examination(MMSE). The elder people were divided into two groups (dementia group and non dementia group) according to the DSM Ⅲ R diagnostic criteria 10 years later. Results (1) A total of 1 192 subjects completed all data sheets by two surveys and 124 cases(10 4%)were identified in the second survey 10 years later. (2)Almost all the baseline item scores and its change of MMSE of the dementia group were higher than that of the non dementia group. (3)Orientation of time, serial subtration by seven, reading items of MMSE were effective predictors for the dementia, resding and orientation of time were significant predictors for Alzheimer's disease through logistic regression model. Conclusions The item analysis of MMSE is important information for the epidemiological research.
7.Psoriasis Associated with Acute Myelogenous Leukemia and Treatment with Allogeneic Bone Marrow Transplantation:A Case Report

Objective To report a case of psoriasis vulgaris associated with acute myelogenous leukemia(AML)(type M4EO).Methods Clinical data from the patient were collected.Histopathologic examination,and examination of bone marrow and peripheral blood smear were performed.The immunologic types of bone marrow cells were analyzed with FACS.Chromosome and G-banding analyses were carried out with cultured bone marrow cells.Results A33-year-old woman had a history of chronic plaque psoriasis for20years.Her cousin had the same disease history.The patient was treated with various therapeutic regi-mens,most of which were traditional Chinese medicines.Recently the patient suffered from myalgia and chest bone pain,periodic bleeding on gums,fever and so on.The abnormal infantile monocytes and promye-locytes were found with bone marrow smear,and crassitude basophilic granules were noticed in eosinophils.The diagnosis of acute myelogenous leukemia type M4EO was made.The diagnosis was confirmed with the immunologic analysis of born marrow cells with FACS.Chromosome and G-banding analyses revealed her karyotype of46,XX,inv(16)/47,XX,inv(16),+8(2/22).The plaque lesions of psoriasis were regressed after allogeneic bone marrow transplantation and the symptoms of AML were resolved.Conclusion It is the first case report of psoriasis vulgaris associated with acute myelogenous leukemia M4EO which responded to allogeneic bone marrow transplantation.
8.Study on the Expression of Apoptosis-Related Molecule PDCD5 in the Skin Lesions of Psoriasis

Objective To study the relationship between apoptosis regulation and epidermal proliferation in psoriasis. Methods The expression of apoptosis molecule PDCD5 (programmed cell death 5) on psoriatic lesions was observed by direct immunofluorescence. The positive rates and the average of fluorescence intensity of PDCD5 were analyzed quantitatively with FACS and computer CELL Quest software. The expression of PDCD5 mRNA in psoriatic lesions was detected by RT-PCR. Results The expression of PDCD5 protein was obviously lower in psoriatic epidermal cells than that of the normal skin. 9.Clinical study of navelbine combined with cisplatin in the treatment of advanced non-small cell lung cancer.

BACKGROUNDTo evaluate the efficacy and toxicity of combination chemotherapy with navelbine (NVB) and cisplatin (DDP) in the treatment of advanced non-small cell lung cancer (NSCLC).
METHODSFrom 1998 to 2003, 46 patients with advanced NSCLC were treated with NVB plus DDP. NVB was given at a dose of 25 mg/m² on day 1 and 8, DDP at a dose of 65-80 mg/m² on day 1 or days 1-3. Twenty-one days was one cycle, each patient was treated at least for two cycles.
RESULTSThe overall response rate (RR) was 47.8% (22/46), 51.6% (16/31) for the first treated cases and 40.0% (6/15) for the retreated cases. RR of stage IIIB and IV was 54.5% (12/22) and 41.7% (10/24) respectively. RR for adenocarcinoma and squamous cell carcinoma was 50.0% (14/28) and 44.4% (8/18) respectively. The median survival time was 9.5 months. One-year survival rate was 39.1%. The common toxicities were myelosuppression, nausea/vomiting and phlebitis. Neutropenia accounted for 95.7% (grade III+IV was 28.3%), nausea/vomiting for 47.8% (grade III+IV was 8.7%) and phlebitis for 19.6%, but the last one were mild.
CONCLUSIONSCombination chemotherapy with NVB and DDP is an effective regimen to treat advanced NSCLC, and the toxicities are tolerable.
10.Role of nursing classification and marking management in ICU nursing management

Objective To probe into the role of nursing classification and marking management in ICU nursing management. Methods According to the admission date,the first 87 patients hospitalized in our department were assigned into the control group and another 84 into the intervention group.Then the human resources were divided based on the ABCD nursing classification and marking management for ICU.The two groups were compared in terms of adverse events.Results The incidences of pressure sores, non-planned extubation,improper clinostatism for using respirator,hypoglucycaemia during reinforced treatment with insulin, respirator-associated pulmonary pneumonia,tube-associated infection,and multidrug resistant bacteria in the intervention group were all significantly lower than those in the control group (allP<0.05).Conclusion The nursing classification and marking management for ICU nursing management can enhance the nursing quality.
