1.Changes of leukotriene B4 in chronic obstructive pulmonary disease and effects of theophylline on leukotriene B4

Objective : To investigate the changes of leukotriene B4 (LTB4) in both the airways and the blood circulation in an animal model of chronic obstructive pulmonary disease (COPD) and the effects of theophylline on LTB4. Methods: Thirty-two male Wistar rats were divided into four groups by random number meter: Group A (normal controls), group B (COPD), group C (smoking+low dose theophylline), and group D (smoking+high dose theophylline ),with 8 rats in each group. Pulmonary functions of the rats were assessed, and pathological changes of airways were scored. Cell counts and cell differentiation in bronchoalveolar lavage fluid (BALF) were also performed. Concentrations of IL-8 and LTB4 in BALF, plasma and lung tissue were measured by enzyme-linked immunosorbent assay (ELISA) and enzyme immunoassay (EIA). Results: LTB4 concentrations in BALF of COPD rats increased significantly as compared with the normal controls, and positively correlated with the percentage of neutrophil and IL-8 in BALF (r=0.794,0.863 ; P=0.007,0.012), and negatively correlated with peak expiratory flow (PEF) (r=-0.718, P=0.028). Positive correlations were also found between levels of LTB4 in BALF and the scores of inflammatory cell infiltration of COPD rats (r=0.836, P=0.036). Treatment with theophylline decreased the percentage of neutrophil and the concentrations of LTB4 in BALF of the COPD rats, attenuated the pathological changes of small airways, such as airway occlusion, goblet-cell metaplasia, inflammatory cell infiltration, and fibrosis and smooth muscle proliferation. The effect was more significant on goblet-cell metaplasia and inflammatory cell infiltration in the high dose group. However theophylline had little effect on PEF. Conclusion: Our results suggest that LTB4 is involved in airway inflammation in COPD. Theophylline is effective in decreasing the levels of LTB4 in BALF of COPD rats, reducing the percentage of neutrophil, and attenuating pathological changes of small airways.

Objective To explore the expression of leptin and its receptors in liver tissue of nonalcoholic steatohepatitis(NASH)patient and its significance in pathogenesis.Methods Samples of 59 liver biopsies of NASH patients were divided into three groups G,S and F according to their degree of inflammation and steatosis and stage of fibrosis.The expression of leptin and its receptors including Ob-R,the short form of Ob-R(Ob-Ra)and the long form of Ob-R(Ob-Rb)were semi-quantitatively determined at the protein and/or mRNA expression levels by using immunohistochemistry,in situ hybridization and image analysis.Results The expressions of leptin in NASH liver tissues were found in KC,HSC and MNC,which were mainly situated in zone 3 near steatotic hepatocytes and perisinusoidal or portal fibrosis areas.Leptin mRNA(Ob)positive cells were distributed in hepatocytes in zone 3.The expression of leptin at protein and mRNA levels in the NASH liver tissues was significantly increased in NASH compared with normal liver tissues(P
3.Interventional effect of fluoxetine on the quality of life in depressive patients with breast cancer

BACKGROUND: There are many studies on the quality of life (QOL) of patients with breast cancer at home and abroad at present. But the effect of antidepressants on the QOL of depressive patients with breast cancer is not clear yet.OBJECTIVE: To investigate the effect of fluoxetine on the QOL of depressive patients with breast cancer.DESIGN: A self-controlled and observational comparative study of taking the patient as the subjects.SETTING: The research institute of medical psychology and psychiatric specialty in two universities and a psychiatric department in a city hospital.PARTICIPANTS: Sixty-three female post-operative patients with breast cancer hospitalized in the First Affiliated Hospital and Second Affiliated Hospital of China Medical University and Dalian Tumor Hospital, with an average age of (50 ± 9) years.INTERVENTIONS: Sity-three depressive patients with breast cancer were treated with fluoxetine for 8 weeks. Treatment emergent symptom scale (TESS) and relative laboratory examinations were used to evaluate the safety of fluoxetine to depression and anxiety of depressive patients with breast cancer. QOL of WHO scale was used to assess the QOL of the patients before and after administration.MAIN OUTCOME MEASURS:①Score of every index of QOL of WHO;②Side effects and adverse effects.RESULTS: The scores of physiology, psychology and independence in QOLafter 8 weeks' administration(16.30 ±2.17, 14.41 ± 1.85, 16. 90 ± 1.53)were significantly higher than those before grouping (13.61 ±2.46,12.98 ± 2.65, 13.65 ± 2.45) ( t = 6.52, P < 0.01; t = 2.41, P< 0.05; t= 8.93, P < 0.01 ). No obvious side effect was observed during administration.CONCLUSION: Fluoxetine can significantly improve the QOL of patients with breast cancer and there are fewer side effects during the treatment.
4.Expressions of TNF-?,TGF-?_1 and leptin in liver tissues with nonalcoholic lipoid hepatitis and their significance

Objective To investigate the expressions of pivotal cytokines such as TNF-? and TGF-?_1, and leptin in human nonalcoholic lipoid hepatitis (NALH), and to explore their potential roles in the pathogenesis of NALH. Methods Liver tissue obtained from 59 patients pathologically diagnosed as nonalcoholic lipoid hepatitis and 10 healthy adult liver tissues were examined in the present study. The respective expression of TNF-?, TGF-?_1 and Leptin was qualitatively and semi-quantitatively determined at the protein expression levels by using immunohistochemical method and image analysis. Results In NALH liver tissue the expression of TNF-? appeared mainly in inflammatory cells and lining cells of liver sinusoids in zone 3, lipogranulomas and portal tract, where obvious inflammation was found. Objective To explore the occurrence of surgical site infection(SSI)in patients in different levels of hospitals.Methods SSI among patients in 47 hospitals at 0:00-24:00 of May 16,2012 were investigated by medi-cal record reviewing,doctor inquiry,and bed-side visiting.Results A total of 5 977 surgical patients were investiga-ted,SSI prevalence rate was 1 .76%.SSI prevalence rate in secondary hospitals was higher than tertiary hospitals (χ2 =9.337,P =0.002);SSI prevalence rates in clean-contaminated and contaminated incision in secondary hospi-tals were both higher than tertiary hospitals (χ2 =4.315,8.129,both P <0.05);departments with high SSI preva-lence rates were general surgery,orthopedic,and neurosurgery;the major isolated pathogens were Escherichia coli , Staphylococcus aureus ,and coagulase negative Staphylococcus .Conclusion SSI rates of different types of incision and different departments are varied,corresponding prevention and control measures should be taken.
6.Clinical significance of monocyte chemoattractant protein-1, tissue factor in elderly patients with acute exacerbation of chronic obstructive pulmonary disease

Objective To investigate the levels and clinical significance of monocyte chemoattractant protein (MCP)-1 and tissue factor (TF) in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods Forty-nine elderly patients with AECOPD(AECOPD group) and 30 healthy controls (control group) were selected,and AECOPD group was divided into procalcitonin (PCT) increased group (PCT ≥ 0.5 μ g/L,19 cases) and PCT normal group (PCT < 0.5 μ g/L,30 cases) according to the level of serum PCT.The levels of plasma TF,serum MCP-1 and PCT were determined by enzyme linked immunosorbent assay.Results The levels of plasma TF and serum MCP-1 were (203.6 ± 92.9),(152.8 ±99.9) ng/L in AECOPD group,and (136.9 ±24.3),(87.5 ±41.5) ng/L in control group.There were significant differences in the levels of plasma TF and serum MCP-1 between AECOPD group and control group (P<0.01).The level of plasma TF was positively correlated with serum MCP-1 (r =0.673,P=0.029).The levels of plasma TF and serum MCP-1 were (215.3 ±71.2),(181.1 ±61.6) ng/L in PCT increased group,and (192.4 ±79.7),(137.3 ±74.4) ng/L in PCT normal group.There were significant differences in the levels of plasma TF and serum MCP-1 between PCT increased group and PCT normal group (P<0.05).Conclusions In patients with AECOPD,hypercoagulability state is activated,and it is more severe in the patients with increased PCT.The level of plasma TF is positively correlated with serum MCP-1 in patients with AECOPD.To monitor the levels of plasma TF and serum MCP-1 is particularly important for elderly patients of AECOPD with hypercoagulability state to prevent cardiovascular,lungs and cerebrovascular thrombotic disease.
7.Laparoscopic sleeve gastrectomy with duodenal-jejunal bypass for the treatment of type 2 diabetes mellitus

Objective To investigate the clinical effects of laparoscopic sleeve gastrectomy with duodenaljejunal bypass (LSG + DJB) for the treatment of type 2 diabetes mellitus.Methods The clinical data of 17 patients with type 2 diabetes mellitus who underwent LSG + DJB at the Xijing Hospital of the Fourth Military Medical University from March 2013 to February 2014 were retrospectively analyzed.The fasting blood glucose,postprandial 2-hour blood glucose,glycosylated hemoglobin (HbA1c) and body mass index (BMI) in 17 patients before operation were (9.2 ± 0.6) mmol/L,(14.4 ± 2.2) mmol/L,8.3% ± 1.2% and (29.4 ± 2.2) kg/m2,respectively.All the patients received LSG + DJB and were followed up by outpatient examination up to March 2015.The pre-and post-operative 12-month fasting blood glucose,postprandial 2-hour blood glucose,HbA1 c and BMI in 17 patients were compared.Measurement data with normal distribution were presented as-x ± s and analyzed by the t test.Results All the 17 patients received successful laparoscopic LSG + DJB without conversion to open surgery.The operation time,volume of intraoperative blood loss and recovery time of postoperative gastrointestinal function were (141 ±53)minutes,40 mL and 2.5 days.Of 3 patients with postoperative complications,1 patient with anastomotic leakage at postoperative day 5 received reoperation by laparoscopic Roux-en-Y gastric bypass,1 patient with digestive tract obstruction at postoperative day 10 released obstruction by reoperation and 1 patient with left subphrenic abscess and leakage at the upper of the stomach at postoperative week 2 was cured by the symptomatic treatment.The duration of hospital stay was 5.2 days.All the patients were followed up for a median time of 16 months (range,13-24 months).The postoperative 12-month fasting blood glucose,postprandial 2-hour blood glucose,HbA1c and BMI in 17 patients were (5.5 ± 0.7)mmol/L,(8.8 ± 1.7)mmol/L,5.1% ± 0.7% and (24.7 ± 2.3)kg/m2,which were significantly different from preoperative indicators (t =19.96,10.52,12.06,31.99,P < 0.05).During the follow-up,no anastomotic ulcer and stenosis,dumping syndrome and severe malnutrition were occurred.Conclusion LSG + DJB is safe and feasible for the treatment of type 2 diabetes mellitus,with a good short-term hypoglycemic effect.

9.Clinical efficacy of intra-articular dexmedetomidine mixed with ropivacaine for postoperative analgesia after arthroscopic knee surgery

Objective To evaluate the efficacy of intra-articular dexmedetomidine mixed with ropivacaine for postoperative analgesia after arthroscopic knee surgery.Methods Sixty ASA physical status Ⅰ or Ⅱ patients,aged 20-64 yr,weighing 50-90 kg,with body height 160-180cm,scheduled for elective arthroscopic knee surgery,were randomly assigned into 2 equal groups using a random number table:ropivacaine group (group R) and dexmedetomidine mixed with ropivacaine group (group RD).In group R,the mixture of noraml saline 1 ml and 19 ml of 0.25% ropivacaine was injected intra-articularly at the end of surgery.In group RD,the mixture of dexmedetomidine 1 μg/kg and 19 ml of 0.25% ropivacaine was injected intra-articularly at the end of surgery.VAS scores at rest and during activity were observed and recorded at 1,2,4,8,12,20 and 24 h after surgery.The duration of analgesia after sugery (from the time immediately after intra-articular administration to the time of first administration of fentanyl as an adjunct to analgesia) and consumption of fentanyl at 24 h after surgery were recorded.Results Compared with group R,VAS scores were significantly decreased at 1,2,4 and 8 h after surgery,the duration of analgesia after sugery was prolonged,and the consumption of fentanyl at 24 h after surgery was reduced in group RD (P < 0.05 or 0.01).There was no significant difference in VAS scores at 12-24 h after surgery between the two groups (P > 0.05).Conclusion Intra-articular dexmedetomidine can significantly improve the efficacy of ropivacaine for postoperative analgesia after arthroscopic knee surgery.
10.Iptakalim hydrochloride enhances glutamate uptake activity in synaptosomes from rat models of Parkinsons disease

AIM To study the relationship between the activity of glutamate transporters and Parkinson's disease (PD), examine whether a novel ATP sensitive potassium (K ATP) channel opener Iptakalim (Ipt) hydrochloride enhances glutamate uptake activity, and to investigate its mechanisms. METHODS Rats were stereotaxically injected with 6 hydroxydopamine (6 OHDA) in SNpc. The synaptosomes from normal and PD rats were isolated, and [ 3H] glutamate uptake in synaptosomes was measured by using liquid scintillation counting. RESULTS [ 3H] glutamate uptake by synaptosomes from striatum and cerebral cortex of PD rats decreased and the redution was recovered by administration with Ipt (10, 50, 100 ?mol?L -1 ). The protective effect of Ipt was blocked by co adiministration with glibenclamide (20 ?mol?L -1 ), an inhibitor of sulphonylurea receptors. CONCLUSION Our results demonstrate for the first time the protective role of K ATP channel in glutamate uptake of synaptosomes and conceptually support the view that Ipt may have potential and feasibility in therapy for PD.
