1.Comparative Study on Allocation Levels of Health Resources in Different Provinces of China

Objective:Through comparing per capita allocation level of health resources in 31 provinces of China, the characteristics and the influencing factors of health resource allocation in each province are reveled. Methods: Selecting 7 representative indexes and using cluster analysis methods to divide per capita allocation level of 31 provinces into 5 grades; the influencing factors of per capita allocation level are proposed based on 5 grades and the ordered Probit model is built and estimated. Results: Beijing, Shanghai and Tibet respectively marked the first, the fourth and the fifth grade, 7 provinces ( including Tianjin) belong to the second grade, 21 provinces ( including Hebei ) belong to the third grade; four factors such as per capita GDP , family disposable income and so on all significantly affect per capita allocation level. Conclusion: Economic development and per capita allocation level of health resources are not balanced. The proportion of government health expenditure is negatively correlated with per capita allocation level of health resources.
2.OAZI-1 enhances the immunogenicity of Melan-A vaccine in mice

Objective To investigate whether ornithine decarboxylase antizyme inhibitor-1(OAZI-1) can enhance the immunogenicity of Melan-A and induce antitumor immune effect in the experimental animals.Methods The eukaryotic expression plasmid pcDNA3.1(-) /OAZI-1, pcDNA3.1 (-)/Melan-A and pcDNA3.1(-)/Melan-A-OAZI-1 were constructed and used to immunize BALB/c mice.The spleen lymphocytes were prepared from the immunized mice and then used to determine the lymphocyte subsets by flow cytometry assay and tumor-killing activity by LDH release assay.The blood samples were collected from the immunized mice and used to test the serum INF-γ by ELISA.Results The eukaryotic expression plasmid pcDNA3.1(-)/OAZI-1, pcDNA3.1(-)/Melan-A and pcDNA3.1(-)/Melan-A-OAZI-1 were successfully constructed.All three gene vaccines could increaseCD4+ T cell ratio (P<0.05), among of them, the ratio in the pcDNA3.1(-)/Melan-A-OAZI-1 and pcDNA3.1(-)/Melan-A immunized groups increased more significantly than other groups but no obvious differences was observed between these two groups.Similarly, all three gene vaccines could also increased CD8+T cells ratio significantly (P<0.05), but, comparing with all other groups, the highest increase was observed in the pcDNA3.1(-)/Melan-A-OAZI-1 immune group (P<0.05).The pcDNA3.1(-)/Melan-A-OAZI-1 gene vaccines significantly increased cytotoxic activity of the spleen lymphocyte in the immune mice(P<0.05).Among the three gene vaccines only pcDNA3.1(-)/Melan-A-OAZI-1 could significantly increased the INF-γ level in the mice serum (P<0.05).Conclusions OAZI-1 can improve antitumor immunity by promoting tumor antigen presentation.
3.PATHOLOGICAL AND ETIOLOGICAL FINDINGS IN A DEAD CASE OF SEVERE ACUTE RESPIRATORY SYNDROME OF CHINA

Objective To explore the pathological features and the causative particles of severe acute respiratory syndrome (SARS) for providing evidences of SARS prevention and clinical treatment. Methods A dead case of SARS of China was studied by light microscopy, electron microscopy, histochemical and immunohistochemical stain. Results The major pathological changes of lung in the SARS case were acute pulmonary interstitial exudative and leakage inflammation, with predominant lymphocyte infiltration. The hyaloid membranes were formed in 20%~30% pulmonary alveoli. The diffuse pulmonary epithelial injury was observed, and virus like inclusions were found in about 30% of total alveolar epithelia by histochemical stain, but chlamydia like inclusions were found occasionally. Meanwhile, the extra pulmonary organs, such as lymph nodes and spleen, showed extensive haemorrhagic necrosis inflammation, accompanied macrophage/histocyte reactive proliferation with erythrocytophage. The double adrenal glands also presented focal haemorrhagic necrosis inflammation. Under the electron microscopy observation, virus like particles with 100 ~150 nm diameter and halo or garland envelopes were found in more than 30 % alveolar epithelial cells, endothelial cells in lung tissues, and also in a part of cardiomyocytes, lymphocytes and macrophages in lymph nodes. The virus like particles were mainly located in cytoplasm and dilated reticular endoplasm. In contrast, chlamydia like particles were commonly visualized in multiple extra lung organs such as liver, but very few in the lung. Immunohistochemistry showed the positive reactions in the lung tissues with the serum IgG and/or IgM from the dead case himself and other SARS convalescent stage cases from Guangdong province of China. Conclusion In the severe SARS case, predominant acute interstitial exudative and leakage inflammation, often with the formation of hyaloid membranes in pulmonaryalveoli, and the haemorrhagic necrosis inflammation of immune organs might be pathological features of SARS. According to the structures, diameter and location of the virus like particles found in this case, combined with the pathological changes, we should consider that those virus like particles might be a new kind of coronavirus, and this kind of virus might be the main causative agent of SARS. However, the chlamydia like particles frequently observed in extra lung organs also suggested the potential new kind of coronavirus might be coexist and synergicallly cause SARS. Our findings in this study provide several evidences for SARS clinical therapy such as application of corticosteroid and enhancement of immune ability and combination of anti virus and anti chlamydium drugs.
4.STUDIES OF PATHOLOGICAL CHANGES OF EXTRA-LUNG ORGANS IN A CASE OF SEVERE ACUTE RESPIRATORY SYNDROME

Objective To study the pathological changes in organs remote from the lung in SARS patient. Methods The pathological changes in extra lung organs and potential coronavirus infection were studied by using light and electron microscopic examinations as well as special virus inclusion stains in the tissues obtained from an autopsy of a patient who died of SARS. Results Besides the lesions in the lung, pathological changes were found also in the central nervous system (CNS), including the cerebrum, cerebellum, thalamus, pons, and medulla oblongata, such as widening of the Virchow Robin′s space, infiltration of a few lymphocytes and macrophages in the parenchyma, vasodilatation and congestion. However, no significant neuron degeneration or necrosis was identified. Vasodilatation in the lamina propria of mucosa and submucosa of the digestive tract with some lymphocytes infiltration, and epithelial nuclear vacuolation, and occasional apoptosis were observed in the mucosal epithelial and glandular cells, as well as focal hemorrhage in segments of the small intestine. Mesenchymal edema and infiltration of a few lymphocytes in the pancreas were noted. Very mild lymphocyte infiltration, but no viral inclusions, was found in the convoluted seminiferous tubules of the testis. The patient who died of SARS was proved to have arteriosclerosis of the coronary arteries, and coronavirul particles were identified in the blood vessels under electron microscopic examination, however no coronavirul particles were found in the brain or the testis of the patient. Conclusion There were mild hypoxic changes in the tissue of CNS in the patient with severe SARS without invasion of the virus. It was confirmed that there were coronavirul particles in the blood of the patient at the acute stage of SARS. Since the patient who succumbed to the disease had a history of coronary arteriosclerosis, it was inferred that cardiovascular disease might be a contributory factor of mortality in this patient with severe SARS.
5.SARS CORONAVIRUS COULD CAUSE MULTI-ORGAN INFECTION

Objective To explore the target cells of SARS coronavirus infection in vivo and to provide the evidence of multi organ injuries produced by SARS coronavirus infection. Methods Three biotin labeling oligonucleotide probes were synthesized according to the published gene sequence of SARS coronavirus. The location, distributtion and quantity of SARS coronavirus in 2 autopsy cases of SARS were studied by in situ hybridization and electron microscopic examination. Results SARS coronavirus particles were identified in multiple organs. In lungs, SARS coronaviruses were located predominantly in the cytoplasm of bronchiolar and alveolar epithelial cells, in a part of macrophages and endothelial cells as well as a few infiltrated lymphocytes. In situ hybridization showed that in target cells SARS coronavirus distribution presented a cytoplasmic or inclusive pattern, and the mean number of positive cells in the pulmonary tissue was 80?25 per 200? field. Electron microscopic examination showed that the coronaviral particles were 100~150 nm in diameter, with low density electron cores with halo or garland envelopes. About 15% of renal tubular epithelial cells harbored SARS coronavirus, and a few parenchymal cells and sinusoid capillary endothelial cells of adrenal glands were hybridization positive. In the gastro intestinal tract, SARS coronaviruses were seen in the cytoplasm of mucosal and crypt epithelial cells, mostly in 2/3 of superficial mocosa. Under both electron microscopy and in situ hybridization observation, SARS coronaviruses were found focally distributed in some cardiomyocytes. The SARS coronavirus positive particles were also noted in macrophages/histocytes, sinusoid endothelial cells, as well as a few lymphocytes in thoracic and celiac lymph nodes. In addition, coronavirus particles were also seen in a few testicular epithelial cells and Leydig's cells. Conclusion SARS coronavirus could attack multiple target cells, implicating that SARS might cause multi organ damages, with lungs as the predominant organ of injury.
6.PTEN inhibits cell growth and increases chemosensitivity to doxorubicin of human breast cancer cells

Objective To study whether transient overexpression of tumor suppressor gene PTEN could lead to growth suppression and up-regulate the sensitivity to doxorubicin of human breast cancer MCF-7 cells. Methods The eukaryotic expression plasmid pEGFP-C 1-PTEN containing whole cDNA of PTEN was constructed and transfected into MCF-7 cells by Lipofectamine 2000 in vitro. Growth inhibition of the cells was observed by phase contrast microscope and flow cytometry. The clonogenic cell survival ability was studied by clony forming assay. MCF-7 cells′ chemosensitivity to adriamycin was studied with MTT assay. Results PTEN overexpression led to morphological changes characteristic of apoptosis of MCF-7 cells. PTEN overexpression also resulted in a significant increase in G 0/G 1 cell population (14.79%) and apoptosis (10.60%) detected by flow cytometry. The clonogenic survival rate of cells transfected with PTEN was significantly decreased after doxorubicin treatment compared with control. 7.Effects of oxidative stress and podocyte injury in renal injury of hyperthyroidism rats

Objective To investigate effects of oxidative stress, expression change of transforming growth factor-β1 and Nestin in re-nal injury of hyperthyroidism rats. Methods The model of hyperthyroidism rat was made by giving them levothyroxine (L-T4). The activity of SOD, CAT, GSH-Px and the level of MDA on the 25th day, the 45th day, and the 60th day were determined by chromatometry in renal cor-tex.. The expressions of TGF-β1 and Nestin on the 25th day, the 45th day, and the 60th day were determined by immunohistochemistry. Results Along with the course of hyperthyroidism, it showed activity of SOD, CAT, GSH-Px and the expression of Nestin reduced, while the level of MDA and the expression of TGF-β1 increased. Conclusion Oxidative stress can increas the expression of TGF-β1 and podocyte in-jury may played an important role in hyperthyroidism renal injury.
8.Value of ultrasonography in predicting the birth weight discordance of twin pregnancy

Objective·To identify the relationship between the estimated fetal weight (EFW) discordance and birth weight (BW) discordance, and evaluate the accuracy of ultrasonography in predicting birth weight discordance (△BW) of twin pregnancy. Methods·The ultrasound was used to detect the growth of twins at different gestational weeks and the data were analyzed to compare the difference between EFW discordance (△EFW) and ?BW. Results·About 15.9 percent of twins (totally 277 cases) had severe △BW (△BW ≥20%) in our study, and the percentages of dichorionic-diamniotic twin (DCDA) and monochorionic-diamniotic twin (MCDA) were 68.2% and 31.8%, respectively (P>0.01). Compared with △EFW in other gestational weeks,△EFW last time before parturition was most consistent of △BW. Furthermore, the negative predictive value of MCDA was highest (94%) at 22-24 gestational weeks. Conclusion·Twin △BW was relatively better predicted by △EFW last time before parturition. Twin pregnancy without severe △EFW (△EFW<20%), especially MCDA with △EFW<20% at 22-24 weeks, had low probability of selective intrauterine growth restriction in the future.
9.Expression of IL-12 family subunit genes in mice C6 glioma cells

Objective:To observe the expression of IL-12 family subunit genes by real-time quantitative PCR in mice C6 glioma cells,construct the basis of the brain glioma research on IL-12 family in the future.Methods:Mice C6 glioma RNA was abstracted and reversed transcription cDNA.The mice C6 glioma cells mRNA expression influence of IL-12 family subunit genes was compared and analyzed by real-time quantitative PCR.Results: In mice C6 glioma cells, high expression abundances in IL-23a, IL-12a, midlde expression abundances in EBI3, IL-27, low expression abundance in IL-12b.Conclusion: IL-12 families are closely related to the occurrence and development of glioma,IL-12,IL-23 are regarded as the most potential anti-glioma cytokines among them,research de-velopments will bring a new way of brain glioma immune therapy.
10.Role of P-JNK and P-c-Jun of JNK transduction pathway on the nasal mucosa remodeling in allergic rhinitis rats.

OBJECTIVE:
To study the role of P-JNK and P-c-Jun of JNK (c-Jun N-terminal kinase) on nasal mucosa remodeling in allergic rhinitis rats.
METHOD:
Sixty male Wistar rats (weighing about 200-250 g) were randomly divided into AR group (A group) and B group(control group). The rats in A group were sensitized for inducing AR by intraperitoneal injection of ovalbumin and Al(OH)₃. Rats in group A were randomized into A4, A8 and A12 group (each had 10 rats). Ovalbumin was dropped in each nasal cavity of every rat for 4,8,12 weeks, respectively. Rats in group B were sensitized by saline instead of OVA, and were also divided into B4, B8 and B12 group. Each group had 10 rats. Pathological changes of nasal mucosa in each period were observed by hematoxylin and eosin stain dyeing. The phosphorylation of JNK and c-Jun were tested by immunohistochemistry.
RESULT:
In A8 group, mucosal congestion and edema thickening with inflammatory cells infiltration of eosinophils were observed in the eighth week, and the inflammatory changes were significantly increased as time went on. The mean absorbance values of P-JNK and P-c-Jun in A group were significantly higher than those in the corresponding B group (all P < 0.01). Moreover, the mean absorbance values of A12 group were significantly higher than A4 group and A8 group (all P < 0.01 ).
CONCLUSION
The expression of P-JNK and P-c-Jun in the process of nasal mucosa remodeling in allergic rhinitis rats were increased, which suggested that P-JNK and P-c-Jun played important roles in nasal mucosa remodeling of the allergic rhinitis rats.
