1.Repairing facial nerve defect with autologous degenerative skeletal muscle

BACKGROUND: Facial nerve defect seriously impacts the mental and physical health of patients as well as their quality of life. Pediceled neck muscle transplant and autologous nerve transplant have been used in clinical to repair facial nerve defect and restore the facial muscle function. However,there are disadvantages and limits of above methods which have restricted their wide application of them. It is a research hotspot for looking for a new material to repair facial nerve defect.OBJECTIVE: To investigate the effects of autologous degenerative skeletal muscle to repair facial nerve defect and provide theoretic basis for further improving the functions of facial nerve.DESIGN: Randomized controlled study based on animals.SETTING: Anatomy department and neurological department in a medical college.MATERIALS: The experiment was completed in the Neuro-histochemical Teaching Section of Xianning Medical College from April to September 2002. Twenty-two Japanese big ear rabbits were randomly divided into 3 groups: 8 in group A to conduct autologous degenerative skeletal muscle transplant, 8 in group B to conduct autologous nerve transplant and 6 in group C as normal control.METHODS: Animal model was built by make 8 mm defect of zygomatic branch of left facial nerve in 22 rabbits. Electrophysiological examination and morphologic observation were conducted to the zygomatic branch of facial nerve and the muscle it controlled to rabbits after 20 weeks.MAIN OUTCOME MEASURES: Electrophysiological examination results and morphologic observation of each group.RESULTS: Electrophysiological examination: Neural stem action potential (NAP), compound facial muscles action potential(cMAP) and nerve conduction velocity(NCV) were compared between groups. There was no difference between group A and B( P > 0.05) while significant difference existed between group A and C, B and C ( P < 0.05) . Morphological image analysis:number of nerve fibers was 2 559 ± 1 683 in group A, 2 658 ± 1 295 in group B and 3 253 ± 1 564 in group C. Diameter of nerve fiber was(4. 01 ± 0. 88) μm,(4. 26 ±0.77) μm, (4.98 ± 0. 72) μm for the three groups, respectively.Average density of regenerated nerve fiber: was(220 ±30) /0. 013 89 mm2,(233 ±32) /0. 0138 9 mm2, (315 ±27)/0. 013 89 mm2, respectively. When comparing the above parameters between groups, there was no difference between group A and B( P > 0. 05), while there was significant difference between group A and C, B and C ( P < 0.05).CONCLUSION: Autologous degenerative skeletal muscle can effectively induce and stimulate the regeneration of nerve and repair the facial nerve defect with the equivalent effects of autologous nerve transplant.
2.Clinical analysis of pulmonary tuberculosis combined malignant lymphoma

Objective To discuss the Clinical character of pulmonary tuberculosis combined malignant lymphoma and its Pathogenesis,and to review the literature.Methods Eighteen cases of pulmonary tuberculosis combined malignant lymphoma from 1996 to 2003 were retrospectively analyzed by its clinical manifestations,X-ray features,diagnosis and treatments.Results 18 cases were all infiltrative pulmonary tuberculosis,13 them were calcification focals,5 were active pulmonary tuberculosis;5 of all were Hodgkin’s lymphomas,13 of all were non-Hodgkin′s lymphomas.16 cases were lymphomas after tuberculosis,2 cases tuberculosis after lymphomas,none were co-existent malignant lymphoma and tuberculosis.Tuberculosis may precede or complicate a lymphomatous process during the development of both diseases,This might is linked to immune deficiency and chronic inflammation;Lymphomas might cause pulmonary tuberculosis,it might cause the immune turbulence of an individual,Pulmonary tuberculosis infection occurring during or after the radiotherapy and chemotherapy of lymphoma.Conclusions It may pulmonary tuberculosis combined malignant lymphoma in the patients in the endemic areas of tuberculosis,Appropriate invasive biopsy procedures are necessary for early diagnosis.
3.Practice and Consideration of Teaching Foreign Students Clinical Medicine in English

Teaching the foreign students clinical medicine in English in the earlier period of foreign students education has played a vital role but also has such problems as teachers,difficulties in the later teaching and so on.Through practice we find that it is necessary to transit the mode from teaching in English to bilingual education,and finally teaching in Chinese in the internship to conform with the present medical foreign students education situation.Therefore ability of teaching in English and proficiency in Chinese are needed to improved as soon as possible.
4.Analysis of drug-resistance to bedaquiline and relationships between phenotypes and genotypes in ;multidrug-resistant Mycobacterium tuberculosis strains

Objective To evaluate the resistance of multidrug-resistant Mycobacterium tuberculosis ( M. tb) strains to bedaquiline ( BDQ) and to analyze the relationships between their genotypes and BDQ-re-sistant phenotypes in order to provide a scientific basis for rational use of BDQ for the treatment of multidrug-resistant tuberculosis ( MDR-TB) in clinical practice. Methods A total of 387 clinical M. tb strains, inclu-ding 100 pan-susceptible strains and 287 strains isolated from patients with MDR ( MDR-TB strains) , were enrolled in this study. Of the 287 MDR-TB strains, 77 strains were collected in Chongqing in 2015 and the other strains were collected in a national drug-resistant tuberculosis survey conducted in China during 2007 to 2008. Minimum inhibitory concentrations (MIC) of BDQ against those strains were detected. Genotypes of those strains were analyzed by Spoligotyping. Differences in the resistant rates against BDQ between Beijing genotype and non-Beijng genotype MDR-TB strains were comparatively analyzed. Results MIC50 and MIC90 of BDQ against the 287 MDR-TB strains were 0. 03 μg/ml and 0. 25 μg/ml, respectively. Nineteen out of the 287 MDR-TB strains (6. 6%) were resistant to BDQ. Based on the Spoligotyping, 195 strains were clas-sified into Beijing genotype, and the other 92 strains belonged to non-Beijing genotype. Statistical analysis revealed that the BDQ-resistant rate in Being genotype strains (4. 6%, 9/195) was lower than that in non-Beijing genotype strains (10. 9%, 10/92, χ2=3. 955, P=0. 047). In addition, the MIC50 and MIC90 of BDQ against pan-susceptible strains were 0. 03 μg/ml and 0. 12 μg/ml, respectively. Sixty-three pan-sus-ceptible strains belonged to Beijing genotype and the other 37 strains belonged to non-Beijing genotype. None of the pan-susceptible strains was resistant to BDQ. Conclusion This study indicates that BDQ showed stronger in vitro antibacterial activity against the MDR-TB strains isolated in China. A correlation between non-Beijing genotype and BDQ resistance is observed in those MDR strains. MDR strains of Beijing genotype are more susceptible to BDQ than those of non-Beijing genotype.
5.Effect of electroacupuncture at Zusanli on liver injury during early stage after burn in rats

Objective To. investigate the effect of electroacupuncture at acupoint Zusanli (ST36) on the liver injury during the early stage after bum in rats. Methods Forty adult male SD rats weighing 220-250 g were randomly divided into 5 groups ( n = 8 each) : group sham operation (group Ⅰ ) ; group burn (group Ⅱ ) ; group acupoint at Zusanli (ST36) (group Ⅲ ); group non-acupoint stimulation (group Ⅳ ) and group ST36 + alphabungarotoxin (alpha7 nicotinic acetylcholine receptor antagonist) (group Ⅴ ). Rats were subjected to 3rd degree burn covering 30% of the total body surface area. Rats were resuscitated with lactataed Ringer's solution according to Parkland formula (4 ml/kg per 1% body surface area) immediately after burn. Bilateral acupoints Zusanli were stimulated with constant voltage (3 V, 3 Hz,2ms) for 20 min 3 times a day for 2 days starting immediately after resuscitation in H and V groups. In group V alpha-bungarotoxin 1.0 μg/kg was administered iv immediately after fluid resuscitation before acupuncture. In group Ⅳ same electric stimulation was performed at a point 0.5 cm lateral to Zusanli. The animals were sacrificed at 48 h after burn. The content and expression of high mobility group box 1 (HMGB1) protein in liver were measured. Liver specimens were obtained for microscopic examination (with light and electronic microscope). Results Compared with group Ⅰ , hepatic HMGB1 protein level significantly increased in Ⅱ and Ⅳ groups. There were significant ultrastructural changes in the liver in burn rats in group Ⅱ and group Ⅳ. Electric stimulation of ST36 significantly attenuated the histologic changes in the liver and decreased the hepatic HMGB1 protein level in group Ⅲ . Pretreatment with specific alpha.7 nicotinie acetylcholine receptor antagonist alpha-bungarotoxin reversed the beneficial effect of electroacupuncture at Zusanli. Conclusion Electric stimulation of acupoint ST36 can ameliorate liver injury during the early stage of burn by activating alpha7 nicotinic acetylcholine receptor-mediated pathways for anti-inflammation.
6.Pretreatment of dialyzer with high-dose vitamin C reduces oxidative stress of patients withhemodialysis

Sixty patients who had hemodialysis over half year were randomly divided into 3 groups with 20 cases in each group:group A received conventional hemodialysis,group B received hemodialysis with oral administration of compound danshen (salvia miltiorrhiza bunge) tablets and vitamin E,group C had the same treatment as group B but the dialyzer was pretreated with solution containing 5 g vitamin C ; 20 healthy adults served as normal controls. Serum levels of advanced oxidation protein products (AOPP) and superoxide dismutase (SOD) were measured in all groups.The levels of AOPP in groups A,B,C were higher and SOD levels were lower than those of normal controls ( all P < 0.01 ).The levels of AOPP in groups B,C were lower and SOD were higher than those in group A at the same time points (P <0.01 ).The levels of AOPP in group C were lower and SOD were higher than those in group B at the same time points (P <0.0l ).The results suggest that pretreatment of the dialyzer with high dose vitamin C plus vitamin E and compound danshen tablets can alleviate oxidative stress in patients with maintaining hemodialysis.
7.Role of angiotensinⅡin perfluoroisobutylene-induced acute lung injury in rats

Objective To investigate tentatively the role of angiotensionⅡ( AngⅡ) in perfluoroisobutylene ( PFIB)-in-duced acute lung injury ( ALI) in rats.Methods Twenty-eight male Wistar rats were randomly divided into one control group(0 h) and six PFIB-exposed groups which were executed at 1, 2, 4, 8, 16 and 24 h after PFIB exposure (n=4). The PFIB-exposed groups inhaled PFIB at a concentration of 145 mg/m3 for 8 min in a flow-past header while the control group was exposed to the filtered air in a similar manner .After execution at the corresponding time-point, the samples of the lung, serum and brochoalveolar lavage fluid (BALF) were harvested.The measurement of the lung wet-to-dry weight ratio ( W/D) and total protein content in BALF , and the histopathological examination of the lung were carried out to evalu -ate the degree of lung injury .The over-time changes in the content of AngⅡin the lung homogenates and blood plasma and the activity of angiotensin converting enzyme ( ACE) in the lung tissue were observed .Results The lung W/D and total protein content in BALF were increased significantly at 16 h after PFIB exposure with severe acute lung edema and abun-dant neutrophil exudation to the alveoli , which were alleviated dramatically at 24 h after PFIB exposure .The content of AngⅡin the lung homogenate showed a tendency of increase during the first 8 hours with significant decrease at 16 and 24 h after exposure.However, the content of AngⅡin the plasma and the activity of ACE in the lung experienced of fluctuations , but without significant difference compared to the control group .Conclusion There is no obvious correlation between the extent of lung injury and that of AngⅡin the lung.The pathological significance of AngⅡin PFIB-induced ALI needs to be further clarified.

Objective To study the effect of repairing limbs bone defect by using uncellular tissue engineering complex of autolegous red bone marrow wrapped by facial flap with vessels to provide evidence for clinical application. Methods Nineteen cases of limbs bone defect were chosen, among them, 3 cases were with benign bone tumor,6 cases were with open fracture causing bone defect and 10 cases were with bone hypotoxic infection after operation. Took autolegous red bone marrow to prepare uncelluar tissue engineering complex with osteoinduction active material (OAM) containing bone morphogenetic protein (BMP). Prepared a facial flap with capillary network originating from an anonymos vessel adjacent to the bone defect,wrappad the tissue engineering bone and filled the bone defect. On a certain time after the operation, the patients were tested by X-ray. Results All the 19 cases were followed up in 1st,3rd,6th, 10th, 12th and 18th month after the operation. There were 3 cases that the implanted tissue engineering bone completely replaced the bone defect in 6th month displayed on X-ray,6 cases in 10th month,9 cases in 12th month and 1 case in 18th month. None of the cases had bone infection or bone absorption. Osteanagenesis and bone molding had come true in all cases. Conclusions The uncellular tissue engineering complex of autolegous red bone marrow wrapped by facial flap with vessels shows double effects in inducing osteanagenesis and the vascularization, and it is feasible in the recovery of large area bone defect. The complex can promote bone recovery and advance the quality and quantity of osteanagenesis.
9.Solitary fibrous tumor of the seminal vesicle: a case report

The incidence of solitary fibroma of seminal vesicle is low, and the source of seminal vesicle is rare. A 38-year-old patient was admitted to hospital because of intermittent gross hematuria for more than one month. Seminal vesicle space occupying lesions can be seen in pelvic MRI. Laparoscopic resection of right seminal vesicle tumor was performed, and the pathological diagnosis tended to solitary fibroma. During the 5-month follow-up, the symptoms of hematuria disappeared and no recurrence.
10.Effect of diabetes mellitus on adenosine postconditioning-induced reduction of myocardial ischemia-reperfusion injury in rats

Objective To investigate the effect of diabetes mellitus (DM) on adenosine postcondi-tioning-induced reduction of myocardial ischemia-reperfusion (I∕R) injury in rats. Methods Adult male Sprague-Dawley rats, weighing 230-260 g, were used in the study. Type 2 DM was induced by high-fat diet and intraperitoneal l% streptozocin 35 mg∕kg and confirmed by fasting blood glucose concentration>16. 7 mmol∕L 72 h later. Eighteen rats with type 2 DM were divided into 3 groups (n= 6 each) using a ran-dom number table: sham operation group (DS group), I∕R group (DI∕R group) and adenosine postcondi-tioning group (DAP group). Eighteen healthy nondiabetic rats were selected and randomly divided into 3 groups (n= 6 each): sham operation group (NS group), I∕R group (NI∕R group) and adenosine postcon-ditioning group (NAP group). Myocardial I∕R was induced by 30 min occlusion of the left anterior descend-ing branch of coronary artery followed by 2 h of reperfusion. Venous blood samples were collected from the femoral vein at 2 h of reperfusion for measurement of plasma cardiac troponin I (cTnI) and creatine kinase-MB (CK-MB) concentrations (by enzyme-linked immunosorbent assay). The rats were then sacrificed im-mediately after blood sampling for determination of the myocardial ischemic area and infarct size. Results The plasma cTnI and CK-MB concentrations were significantly increased, and the percentage of myocardial infarct size was increased after myocardial I∕R in nondiabetic and diabetic rats. Adenosine postconditioning significantly decreased plasma cTnI and CK-MB concentrations and percentage of myocardial infarct size in nondiabetic and diabetic rats (P<0. 05). Compared with group NAP, the plasma concentrations of cTnI and CK-MB were significantly increased, and the percentage of myocardial infarct size was increased in group DAP (P<0. 05). Conclusion DM can weaken cardioprotection induced by adenosine postcondition-ing in rats.
