1.Study of Yanshu Injection on Enhancing Effects of Chemotherapy and Reducing Toxicity and Immune Effect in Tumor-bearing Mice

Objective To investigate the effects of Yanshu injection for the chemotherapy, toxicity- reducing and immunization in mice implanted tumor. Methods Bulid up the mice models with S180 solid tumor transfered by ascites tumor, and observe the effect on tumor weight, blood WBC and bone marrow nucleated cells when combined use Yanshu injection and cyclophospamide (Cy). Determine OD value of plague forming cell by spectrophotometry, measure T-lymphocyte transformation by 3H-TDR incorporation, and survey natural killer cell (NKC) activity by colorimetric method, all these can help to study the role of Yanshu injection on the immune effect in the mice models. Results Yanshu injection can strengthen antitumor activity of Cy, antagonize the inhibition of Cy to WBC and bone marrow nucleated cell, improve OD value of plague forming cell, promote T-lymphocyte transformation, also can enhance NKC activity and killing rate. Conclusion Yanshu injection can increase the chemotherapeutic effects, reduce toxin, and enhance immune function of mice implanted tumor.
2.Effective nursing care of cardiac arrest rescue caused by amniotic fluid embolism

Objective To report successful nursing experience of cardiac arrest caused by amniotic fluid embolism.Methods 3 pregnant women with cardiac arrest caused by amniotic fluid embolism from January to August 2012 were successfully rescued,the nursing measures included observation of labor course,mechanical ventilation,rapid establishment of intravenous injection route,anti-shock and prevention of DIC and renal failure.Results 3 cases of pregnant women were given timely rescue and pulled through.All rehabilitated and were discharged.Conclusions Amniotic fluid embolism is the most serious,dramatic,and the most difficult to control complications in obstetric department.Prevention is the key for amniotic fluid embolism,once happened,medical workers should carry out rescue work calmly and implement effective interventions.
3.Thinking of prescriptions corresponding to syndromes in construction of syndrome differentiation system

Syndrome factors and their combinations based on syndromes are the important links of construction of syndrome differentiation system,in which the theory of prescriptions corresponding to syndromes plays a key role.The basic requirements of good clinical eff icacy are'syndrome factors'seen as the basic connotation of'syndromes'in prescriptions corresponding to syndromes,keeping the compatibility of effect classical prescriptions,emphasizing further detailed formula indication and dynamic evolution,and increasing the conformity of prescription and syndrome.These are also the basis of construction of syndrome differentiation system.Method of evidence-based medicine should be used for reference in clinical research design,and reverse thinking of conjecturing syndrome based on prescription be used for guidance in the constructing process.
4.Effect of PQQ on the hippocampal neurons of aging rat induced by D-galactose

Objective To investigate the effects of pyrroloquinoline quinone(PQQ) on the hippocampal neurons of aged rats induced by D-galactose(D-gal).Methods D-gal was used to induce the model of aging rat,PQQ was administered into rat lateral intracerebroventricle.After 50 days the metamorphosis of hippocampal neurons was observed by H-E and Nissl's staining.The apoptosis rate of hippocampus was tested by flow cytometry.The contents of free radical and C-FOS protern were measured.Results Compared with the control group,the size of the neurosoma was slightly changed,the optical density of Nissl's was decreased,the content of free radical and the apoptosis rate increased markedly in D-gal group.After PQQ injection with D-gal,the size of neurosoma and the optical density of Nissl's were markedly increased,the content of free radical and the apoptosis rate of hippocampus did not change.PQQ improved the expression of C-FOS protern.Conclusion PQQ can slow down the aging progress of hippocamal neurons induced by D-gal.
5.Medical coping strategies and impact factors of patients with coronary heart disease in Wuhan

Objective To explore the medical coping strategies and impact factors of patients with coronary heart disease in Wuhan.Methods Both medical coping strategies and impact factors of 90 patients with coronary heart disease in Wuhan were analyzed by using Spearman coefficient and multiple linear regression analysis,and compared with the norm of 701 cases.Results Compared with the norm,patients with coronary heart disease in Wuhan preferred less facing coping strategies and more avoidance coping strategies:(17.89±2.67) points vs.(19.48±3.81) points,(16.95±2.54) points vs.(14.44±2.97) points,and there were significant differences (t=2.146,P=0.035;t=1.994,P=0.048).The yielding coping strategies was indicated insignificant differences (t=1.758,P=0.173).The facing coping strategies were correlated with age (r=-0.212,P=0.036),education levels (r=0.190,P=0.041),family monthly income (r=-0.106,P=0.049) and disease course (r=0.327,P=0.017).The avoidance coping strategies were correlated with marital status (r=-0.304,P=0.013).The yielding coping strategies were correlated with length of disease(r=0.294,P=0.023)and therapy methods (r=0.308,P=0.017).Conclusions Based on patients' characteristics and impact factors,nursing staff should guide and encourage patients to use positive medical coping styles.
6.Clinical analysis of therapies for native valve endocarditis

Objective To summarize experience in treating patients with native value endocarditis ( NVE) , and discuss the effect of timing of surgery on the infection control , cardiac function recovery and long-term mortality among patients . Methods Eighty NVE cases with antibiotic treatment were recruited between January 2006 and May 2013 .According to the timing of surgery, the subjects were divided into the early surgery group (38 cases) and the late surgery group (42 ca-ses) .The infection-related markers and cardiac function indexes before and after surgery , and 1-year, 3-year and 5-year mortality after surgery were analyzed .Results The infection control and recovery of cardiac function in the early surgery group were better than in the late surgery group (P<0.05).The difference in 3-year and 5-year mortalities between the two groups was significant .Conclusions For NVE patients with routine antibiotics treatment , the surgical effect and long-term survival rate are much better in the early surgery group than in the late surgery group .
7.Experimental Study of Prevention and Therapeutic Effect of Jinyebaidu Granules on Cytomegalovirus Infection During Mid-pregnancy

Objective To study the effect of Jingyebaidu granules on treating cytomegalovirus ( CMV) infection during mid-pregnancy. Methods The sexually mature guinea pigs with no CMV infection history served as the subjects. Put the male and female ones in the same cages. Then the female ones were randomly divided into three groups during mid-pregnancy. Model control group:15 guinea pigs which were inoculated 1 mL suspension of GPCMV intraperitoneally. Jingyebaidu Medicine group:15 guinea pigs which were treated with Jingyebaidu(3. 09 mL·kg-1 ) through stomach perfusion after inoculation for 14 days. Normal control group:15 normal mid-pregnant guinea pigs. Viremia rates were examined 7 days after infection. All animals were sacrificed 20 days after infection. The placenta infection rate, pup infection rate, still-born rate were examined. Results Compared with the normal controls, the still-born rate was increased in model control group(8. 33% vs 34. 55%, P<0. 05). In comparison to the model control group, the GPCMV maternal infection rate(86. 67% vs 33. 33%), placenta infection rate (91. 67% vs 61. 22%), pup infection rate(90. 91% vs 48. 28%), still-born rate(34. 55% vs 15. 52%) were significantly decreased in the Jinyebaidu group (all P<0. 05). Conclusion Jinyebaidu granules could reduce maternal infection,pup loss, and placenta infection caused by CMV inoculation during mid-pregnancy.
8.Ameliorative effects of atractylodin on intestinal inflammation and co-occurring dysmotility in both constipation and diarrhea prominent rats.

Intestinal disorders often co-occur with inflammation and dysmotility. However, drugs which simultaneously improve intestinal inflammation and co-occurring dysmotility are rarely reported. Atractylodin, a widely used herbal medicine, is used to treat digestive disorders. The present study was designed to characterize the effects of atractylodin on amelioration of both jejunal inflammation and the co-occurring dysmotility in both constipation-prominent (CP) and diarrhea-prominent (DP) rats. The results indicated that atractylodin reduced proinflammatory cytokines TNF-α, IL-1β, and IL-6 in the plasma and inhibited the expression of inflammatory mediators iNOS and NF-kappa B in jejunal segments in both CP and DP rats. The results indicated that atractylodin exerted stimulatory effects and inhibitory effects on the contractility of jejunal segments isolated from CP and DP rats respectively, showing a contractile-state-dependent regulation. Atractylodin-induced contractile-state-dependent regulation was also observed by using rat jejunal segments in low and high contractile states respectively (5 pairs of low/high contractile states). Atractylodin up-regulated the decreased phosphorylation of 20 kDa myosin light chain, protein contents of myosin light chain kinase (MLCK), and MLCK mRNA expression in jejunal segments of CP rats and down-regulated those increased parameters in DP rats. Taken together, atractylodin alleviated rat jejunal inflammation and exerted contractile-state-dependent regulation on the contractility of jejunal segments isolated from CP and DP rats respectively, suggesting the potential clinical implication for ameliorating intestinal inflammation and co-occurring dysmotility.

9.HLA- haploidentical donor hematopoietic transplantation for severe aplastic anemia achieved comparable outcomes with HLA- unrelated donor transplantation.

OBJECTIVETo evaluate the efficacy of HLA- haploidentical donor hematopoietic transplantation (Haplo- HSCT)for severe aplastic anemia (SAA)by compared with the same period of unrelated donor transplantation (UD- HSCT).
METHODSOf a cohort of 50 SAA patients between September 2012 and July 2014, 26 patients underwent UD- HSCT and 24 patients Haplo- HSCT.
RESULTSOS rate was 91.3% with a median follow-up of 9 (2-26)months. According to transplant type, there was no significant difference between UD- and Haplo-HSCT (96.1%vs 86.0%,P=0.30). 3 of 50 (6%)patients had primary engraft failure. Haplo- HSCT developed higher significantly incidence of Ⅱ- Ⅳ aGVHD (37.5%vs 3.83%,P=0.003)and cGVHD (37.5%vs 15.3%,P=0.030)than UD-HSCT. Haplo-HSCT also had significantly higher incidences of CMV viremia (78.2%vs 46.1%,P=0.005)and EBV viremia (43.1%vs 16.0%,P=0.040), respectively than UD-HSCT. But the incidences of hemorrhagic cystitis were similar between two transplant types (39.1%vs 23.0%,P=0.120).
CONCLUSIONThis study showed favorable outcome of Haplo-HSCT for SAA, which was comparable with UD-HSCT.

10.Efficacy of volume therapy guided by stroke volume variability in patients undergoing surgery for severe traumatic brain injury

Objective To evaluate the efficacy of volume therapy guided by stroke volume variabil-ity ( SVV) in the patients undergoing surgery for severe traumatic brain injury. Methods Thirty patients of both sexes with severe traumatic brain injury, aged 18-64 yr, of American Society of Anesthesiologists physical status Ⅲ, who were admitted to the hospital within 24 h after injury, with Glasgow Coma Scale ( GCS) score≤8, were divided into control group ( C group, n=15) and SVV group ( n=15) using a ran-dom number table method. In group C, conventional fluid administration was performed to maintain mean arterial pressure at 65-110 mmHg, central venous pressure at 5-12 cmH2 O and urine volume>1 ml·kg-1 ·h-1 . Fluid was given according to SVV, maintaining SVV≤13% and mean arterial pressure at 65-110 mmHg in group SVV. Immediately after skin incision ( T0 ) , immediately after opening cerebral dura mater ( T1 ) , at 1 h after opening cerebral dura mater ( T2 ) , immediately after suturing cerebral dura mater ( T3 ) and at the end of operation ( T4 ) , blood samples were collected from the radial artery and inter-nal jugular venous bulb for blood gas analysis, the jugular venous oxygen partial pressure, jugular venous bulb oxygen saturation, blood lactate, arterial oxygen partial pressure, arterial oxygen saturation and Hbwere recorded, and the cerebral artery and arteriovenous blood O2 content difference and cerebral O2 extrac-tion rate were calculated. Blood samples were collected from the internal jugular venous bulb at T0-2 , T4 and 24 h after operation ( T5 ) for determination of S100β protein concentrations by enzyme-linked immunosor-bent assay. The intraoperative volume of fluid intake and output and consumption of vasoactive drugs were recorded. GCS scores were recorded immediately after admission to the operating room, and at 1, 3, 7 and 14 days after operation. The development of postoperative length of hospitalization and complications ( pul-monary infection and brain edema) was recorded. Glasgow Outcome Scale Score was used to assess the early postoperative quality of life. Results Compared with group C, the urine volume was significantly in-creased, the consumption of vasoactive drugs was reduced, jugular venous bulb oxygen saturation was in-creased at T2,3 , the cerebral O2 extraction rate was decreased at T2-4 , the serum S100β protein concentra-tion was decreased at T2 , and the GCS score was increased at day 3 after operation ( P<0. 05) , and no sig-nificant change was found in blood lactate, postoperative Glasgow Outcome Scale score or length of hospital-ization at each time point in group SVV ( P>0. 05) . Conclusion SVV-guided volume therapy can improve cerebral oxygen metabolism, ensure adequate tissue perfusion and reduce craniocerebral injury in the pa-tients undergoing surgery for severe traumatic brain injury.
