Medical imaging plays a more important role in diagnosis and treatment of diseases with the rapid development of big data technologies and hospital information systems.It is thus quite necessary to develop the effective and accurate medical imaging retrieval systems.The key techniques for text-based, contents-based and semantic-based medical imaging retrieval methods were thus elaborated and analyzed with the prospects put forward for the future development direction of medical imaging retrieval techniques.

Clinical guidelines,coming into being with the development of evidence-based medicine,are of great significance in reducing medical malpractice and regulating medical behaviors.However,more effective and intelligent measures should be taken for their development and application due to their low utilization and poor compliance of doctors in their implementation.The emergence and structure of clinical guidelines were elaborated with related problems summarized.The knowledge expression models of digital clinical guidelines were analyzed and compared with the challenges faced by the current knowledge expression models of clinical guidelines pointed out.

Objective:To explore the genetic polymorphism of cytochrome P450 enzyme 17(CYP17) and its relationship with the pathogenesis ofpolycystic ovary syndrome(PCOS).Methods:A total of 260 patients with PCOS of uyhan and han nationality in Xinjiang,who were admitted to Chinese Medicine Research Institute of Xinjiang Uygur Autonomous Region from January 2015 to February 2017,were chosen as PCOS group;the other 237 healthy fertile women,who were examined in the outpatient department during the same period,were chosen as control group.The CYP17 gene polymorphism in the two groups of subjects was detected by the restriction fragment length polymorphism polymerase chain reaction (PCR-RFLP) technique;the distribution of alleles and gene frequencies was compared between the two groups;combining with its clinical data,the relationship between CYP17 gene polymorphism and the pathogenesis of PCOS was analyzed.Results:Body Mass Index(BMI) and Follicle-stimulating hormone(FSH) levels in the PCOS group were lower than those in the control group;Luteotropic hormone(LH),Testosterone(TES) and LH/FSH levels in the PCOS group were higher than those in the control group,and the differences were statistically significant (P＜0.05).The frequencies of A1A1,A1A2,and A2A2 of CYP17 gene in the PCOS group were 34.62％,41.92％,23.46％,respectively,compared with 34.18％,43.88％,21.94％ in the control group,the difference was not statistically significant (P＞0.05).The allele frequency of A1 and A2 in the PCOS group were 55.58％,and 44.42％ respectively,compared with 56.12％ and 43.88％ in the control grouP,the difference was not statistically significant (P＞0.05).There was not statistical significance in FSH,LH and LH/FSH levels of different CYP17 genotypes in the PCOS group (P＞0.05).There was statistical significance in BMI level A2A2＞A1A2＞A1A1 and TES level A2A2＜A1A1＜A1A2 in the PCOS group (all P＜0.05).The BMI,FSH,LH,TES and LH/FSH levels of different CYP17 genotypes in the control group were not statistically significant (P＞0.05).Conclusion:CYP17 gene-34bp T/C nucleotide polymorphisms is a common base replacement in the population,which is not significantly related to the pathogenesis of PCOS.

Objective To analyze the voice quality of children post operation for complete cleft palate through subjective and objective evaluation and explore the correlation between them. Methods Electroglottography analysis was used in the natural sound/a/of 30 postoper-ative children with complete cleft palate and 34 normal children, and the difference was compared. The natural prolonged sound/a/was as-sessed with GRBAS (grade, roughness, breathiness, asthenia, strain) in the children with complete cleft palate, and the correlation between the subjective and objective results was analyzed. Results F0, harmonic to noise ratio (HNR) were less, and the shimmer, jitter, normalized noise energy (NNE) and nasalance was greater in postoperative children with complete cleft palate than in normal children. For the children with complete cleft palate, G and R mainly rated as levels 1 and 2, B also rated as levels 1 and 2 in less than half of the children. There were low or very low correlation between all subjective and objective results, excepted G, R, B and F0, which showed moderate correlation. Con-clusion The vocal cords vibrate slowly and irregularly in postoperative children with complete cleft palate, suggesting mild and moderate voice disorder, and they sound hoarsely with roughness and breathiness. There are poor correlations between the subjective and objective as-sessment, and need to be used in combination for the children with complete cleft palate.

ObjectiveTo elucidate the association of immune response to hepatitis B vaccination with HLA-DRB1 * 12 allele as well as the level of IL-4 and IFN-γ.MethodsSeventy-four healthy college students from Guangxi province who had non- or hypor -response to recombinant hepatitis B vaccination and 64 medium- or hyper-responders with the conditions of similar were selected randomly and involved in this study.HLA-DRB1 * 12 was detected by PCR-SSP,the level of IFN-γ and IL-4 cytokines were examined by ELISA.Results(1)The allelic frequencies of HLA-DRB1 * 12 was lower in the non- or hypor-responders than that in the medium- or hyper-responders ( 10.8％ vs 32.8％,P=0.002) ; (2)The expression level of IFN-γ in the non- or hypor-responders ( 7.21±7.92 ) ng/ml was much less than that of the medium- or hyper- responders ( 16.36± 11.00) ng/ml ( P=0.000).(3) The expression level of IL-4 in the non- or hyporresponders (3.18±4.45) ng/ml was much less than that of the medium- or hyper- responders (7.76±5.71 ) ng/ml(P=0.000).(4)No significant differences was seen between the expression level of IFN-γ in the HLA-DRB1 * 12 positive ( 13.18± 11.24) ng/ml and the negative ( 11.00± 10.29 ) ng/ml ( P =0.349 ).(5)No significant differences was seen between the expression level of IL-4 in the HLA-DRB1 * 12 positive (5.947±4.530) ng/ml and the negative (5.132±5.800) ng/ml (P=0.423).ConclusionHLA-DRB1 * 12 might be the allele enhanced immune response to hepatitis B vaccination.The expression levels of IFN-γand IL-4 correlating to Thl/Th2 cells might affect on the immune response to hepatitis B vaccination.

OBJECTIVE: To verify the therapeutic effect and adverse reactions of ?-receptor blocker, domestic urapidil, on severe hypertension. METHODS: Observation was carried out in a multi-center, random sampling and controlled pattern. Drug was iv injected first and then infused. At the same time, the patients' systolic pressure, diastolic pressure, heart rate, EKG, blood & urine routine, serum GPT and urea nitrogen were measured and examined.RESULTS: Of 41 cases in this series. 16 satis- factory(39.0%), 23 improved (56.1%), 2 unsatisfactory(4.9%) .The total effective rate was 95.1%. Objective To compare the safety of laparoscopic live donor nephreetomy(LDN) and open live donor nephrectomy(ODN), evaluate the kidney function and blood pressure of living donors during 1 year follow-up. Methods Thirty cases of LDN and 30 eases of ODN were retrospectively reviewed. The operation time, warm ischemia time, operative blood loss, time to post-operative intake and time to ambulation of the 2 grouups were compared. According to the modified Clavien classifica-tion system procedure-related complications were described and compared. Serum creatinine(SCr) le-vels, blood pressure and 24-h urine protein excretion were measured before nephreetomy and 1 d, 7 d, 3 months, 6 and 12 months after nephrectomy. Glomerular filtration rate (GFR) were measured preo-pratively and at 6 and 12 months postoperatively. These data were statistically analyzed. Results The operation time was (98. 6+13. 6)rain and (96.3+19. 5)rain in the LDN and ODN groups, re- spectively. Warm ischemia time in the LDN group was (90.6±15.1)s, in the ODN group was (86.4±12.3)s. Operative blood loss was (105.2±634.8)ml and (206.3±126.4)ml in the LDN and ODN groups(P<0.01). For the time to post-operative intake and time to ambulation, LDN group was (28.5±2.9)h and (25.8±63.8)h, ODN group was (38.6±63.3)h and (36.5±65.3)h(P<0.01). Perioperative complications rates were 6.6%(2/30) and 23.3%(7/30) for LDN and ODN, respective-ly. SCr was (109.1±7.5), (105.4±69.5), (96.6±10.7), (89.4±11.5), (91.6±69.3)/zmol/L in the LDN group and (107.3±69.6), (103.3±68.4), (95.4±69.1), (90.5±13.6), (90.3±11.7)μmol/L in the ODN group 1 day, 7 days, 3 months, 6 months and 12 months after nephrectomy. The mean GFR of LDN and ODN was 64.7 and 65.8 ml/min at 6 months after nephrectomy, 65.9 and 67.5 ml/min at 12 months postoperatively, which were significantly different comparing with preoperative mean GFR in each group(P<0.05) but no significant difference was found between 6 months and 12 months after nephrectomy and between the 2 groups at the same time point respectively(P>0.05). Mean 24 h protein excretion was elevated after either LDN or ODN during 1 year followup, but was not significantly different either between predonation and 1 year after nephrectomy or between the 2 groups at the same period. Blood pressure increased or decreased slightly with the duration of follow-up,no significant blood presure changes were found before and after nephrectomy or between the two groups at the same period postoperatively. Conclusions LDN has the advantages of minimal trauma, less operative blood loss and quicker convalescence. It is safe and and has no adverse effects regarding kidney function and blood pressure during the first year after living kidney donation comparing to ODN.

OBJECTIVETo construct enterohemorrhagic Escherichia coli (EHEC) O157:H7 strains with delection espF gene and its nucleotide fragment and with espF gene complementation.

METHODSA pair of homologous arm primers was designed to amplify the gene fragment of kanamycin resistance, which was transformed into EHEC O157:H7 EDL933w strain via the PKD46 plasmid by electroporation. The replacement of the espF gene by kanamycin resistance gene through the PKD46-mediated red recombination system was confirmed by PCR and sequencing. The entire coding region of espF along with its nucleotide fragment was amplified by PCR and cloned into pBAD33 plasmid, which was transformed into a mutant strain to construct the strain with espF complementation. RT-PCR was used to verify the transcription of espF and its nucleotide fragment in the complemented mutant strain.

RESULTS AND CONCLUSIONWe established EHEC O157:H7 EDL933w strains with espF gene deletion and with espF gene complementation. Both espF and its nucleotide fragment were transcribed in the complemented mutant strain. The two strains provide a basis for further study of the regulatory mechanism of espF.

OBJECTIVETo test the hypothesis that psoralen induces rat mesenchymal stem cells (RMSCs) to differentiate towards male germ cells.

METHODSRMSCs were induced by psoralen at the final concentration of 3.0 µg/ml, and the morphological changes of the cells were detected microscopically and the cell proliferation changes were measured by MTT assay. The expressions of 7 representative marker genes of male germ cells, namely Oct-4, Stra8, RVLG, SCP3, TNP2, Itgb1, and Itga6, were investigated in the induced cells by real-time quantitative PCR.

RESULTSRMSCs showed no obvious morphological changes after induction with 3.0 µg/ml psoralen for 72 h. Psoralen induction for 5 days did not significantly affect the proliferation of RMSCs; Psoralen induction for 72 h, however, significantly up-regulated the genes Oct-4, Stra8, SCP3, and Itgb1 (P<0.05) without affecting the expressions of the genes RVLG, TNP2, or Itga6 (P>0.05).

CONCLUSIONPsoralen may participate in the differentiation of RMSCs towards male germ cells.
