1.Observation and nursing of patients after minimally invasive ventriculo-atrial shunt

Objective To summarize the observation points and nursing experience after minimally invasive ventriculo-atrial shunt. Methods 45 patients after minimally invasive ventficulo-atrial shunt were given preoperative psychological care and preparations, postoperative observation of vital signs, con-sciousness, pupil, with or without intracranial hypertension, inadequate shunt or transitional symptoms, in-fectious symptoms, specific and basic care, awareness of postoperative complications and detailed guidance for discharge and follow-up jobs. Results Half month after shunt 28 cases regained consciousness, 9 cases with alleviated consciousness dysfunction, 8 cases with unchanged consciousness dysfunction, 5 cases with shunt blockage; 3 cases with over-shunt; 5 cases with inadequate shunt, 1 case with blood-borne in-fection, 1 case with shunt exposed, no intracranial infection, air embolism, complications such as endocardi-tis took place. Conclusions Adequate preoperative preparation and close postoperative observation, ef-fective specific care and basic care can increase success rate of ventriculo-atrial shunt and improve the quality of life of patients.
2.Identification of the Specific Proteins in Two Hydrophis with a Simple Differential Proteins Display Method

Objective To identify the specific proteins in two hydrophis (l#and 2#) with a simple differential proteins display method. Method By means of SDS-PAGE and Western blot, combined with analysis, the specific proteins in two hydrophis were obtained and been sequencing.Result The specific proteins of l#are 14.6kD and 8.7kD components and of 2#are 16.7kD. 10.2 kD components. The amino acid sequences of 14.6 kD and 10.2 kD components were assayed respectively. Conclusion Differential proteins can be obtained and used to identify the Chinese traditional medicinal crops by means of one-dimensional electrophoresis combined with antigen analysis .
3.Comparisons of hemodynamics in 1-way and 2-way CABG.

For the purpose of improving the hemodynamics of CABG, the authors presented a new amelioration measurement of symmetric 2-way bypass graft. The physiological blood flow in 1-way and 2-way bypass graft of coronary is simulated with finite element method, and the data on computational hemodynamics of these two cases were compared. The temporal-spatial distributions of hemodynamics during the cardiac cycle such as flow patterns and wall shear stress in the vicinity of anastomosis were analyzed. The study results showed that 2-way bypass graft was of more rational hemodynamics when compared with 1-way bypass graft, and it could improve the flow conditions and decrease the probability of restenosis.

4.An alternative method of strain analysis on arterial walls.

As is well known, the zero stress state of an artery is brought by cutting a ring of the artery radically and then the ring segment springs open into a sector. The zero stress state is a foundation of strain analysis of an artery. There are two methods to measure the dimensions of a blood vessel in the zero stress state. First, it is a way to measure immediately an opening angle. Second, it is a method of assuming that a cut-open, stress-free specimen forms a circular sector and there are two edge angles. In this paper, we proposed a new method to measure the dimensions of an arterial wall in the zero stress state. The new method is to measure immediately the curve length of an arterial wall in zero stress state by computer image process (CIP). We used the curve lengths to define a standard circle for the strain analysis. We compared these three methods on descending thoracic aortas, common carotid arteries, and femoral arteries of white rabbits. Conclusions are: (1) the new method, that is the strain analysis method from the curve length, is the best method among the three methods of strain analysis; (2) the lengths of a ring segment (the length between D/2 and 2D) do not affect the dimensions of the zero stress state.

5.Comparison of biomechanical properties between human nasal periosteum and fascia.

There has been a lot of controversies on which layer the silastic implants should be inserted in the augmentation rhinoplasty, i.e. subperiosteal or deep subfascial. This study is to investigate the biomechanical properties of human nasal periosteum and deep fascia, including tensile strength, stress-strain and stress relaxation characters under uniaxial tension system. The periosteum is stronger in tensile strength than that of the fascia, but it is less elastic. Under a sudden increase of load, the periosteum relaxes far less than the fasia. Therefore, in view of biomechanics, the periosteum is thicker, tougher, stiffer and less relaxation than facia, thus has a better fixation effect.

6.A new approach to detection of QRS complexes based on histogram.

A brief comment on the current methods of detecting QRS complexes from ECG(electrocardiogram) including the wavelet transform method is given, and the shortcomings of those methods are discussed. A simple and effective new approach for detecting QRS complexes based on histogram is developed and presented in this paper with a description of the theory of histogram and the match principle for detecting QRS complexes. This new method is checked up on the data from the MIT/BIH ECG database and that from the Beijing Polytechnic University Hospital.

7.Comparison of endoscopic balloon dilation with surgical therapy in management of esophageal achalasia

Objective To compare the effects and safety of endoscopic balloon dilation with surgical therapy in treating esophageal achalasia. Methods The patients who were diagnosed as achalasia of cardia from Jan. 2002 to Dec. 2007 were consecutively collected. All patients were divided into endoscopic balloon dilation group (endoscopy group), thorascopic / laparoseopic surgery group (telescopy group) and standard thoracotomy group (operation group). The efficacy, relapse rate and safety were compared among three groups. Results Ninety eight patients were enrolled. There were 57 patients in endoscopy group, 24 patients in telescopy group, and 17 patients in opreation group. All of the patients completed the treatment. There was one case (1.6%) in endoscopy group complicated with esophageal rupture, two (8.3%) in telescopy group complicated with esophageal fistula, and one (5.9%) in operation group complicated with severe post-operation pneumonia infection, acute respiratory distress syndrome and infectious shock. The rest patients had symptomatic relief of dysphagia in different degrees after therapy, and that was further confirmed by symptoms assessment, x-ray, or endoscopic examination after one month. Five patients (8.9%) in endoscopy group relapsed, with a duration of 5 to 31 months (mean 8 months), and underwent re-dilation or stent placement successfully. One patient (4.2%) in telescopy group relapsed after 6 months and underwent re-dilation successfully. No one relapsed in operation group. Conclusions Compared with standard operation group and telescopy group, endoscopic balloon dilation is deserved to use widely in clinical application with its advantages of simple, convenient and low complication. In spite of its relapse, it still can be remedied well by endoscopic re-dilation or stent placement.

The change of PNA receptors on the surface of thymocytes in protein deficient mice was investigated. After the mice were fed with protein deficient diet, the body weight continuously declined and the thymus gradually a-trophied. Qualitative test of FITC-PNA indicated the number of PNA positive cells was reduced and the fluorescent antibody reaction on the surface of PNA positive cells weakened. 9.Osteotomy for malunion of intra-articular ankle fracture assisted by 3D printing relay template

Objective To explore the effectiveness of our self-designed 3D printing relay template in the treatment of malunion of irregular intra-articular ankle fractures.Methods From October 2013 to October 2015,our self-designed 3D printing relay template was used in 14 patients with severe malunion of intra-articular ankle fracture to assist their intra-articular osteotomy.They were 10 males and 4 females,with an average age of 40.5 years (from 34 to 59 years).Thin slice CT scan was preformed preoperatively for all the ankles involved.The computer fictitious tibial and fibular models for location and osteotomy navigation templates were acquired from the data of CT scan after processing,design and reconstruction by software.After the navigation templates which fit the size of solid ankle were printed by a 3D printer,they were sterilized and reserved.The guide plates were used in turn during operation.The irregular malunited bone blocks in the posterior malleolus were completely resected along the osteotomy template.After fracture reduction,the bone blocks were stabilized with internal fixation.Fracture union time was recorded.Scores of visual analogue scale (VAS),American Orthopaedic Foot and Ankle Society (AOFAS) and the short form-36 (SF-36),ankle ROM and complications were also recorded at the final follow-ups.Results The 14 patients obtained an average follow-up of 28.6 months (from 14 to 35 months).Bony union was achieved after an average of 3.7months (from 3 to 6 months).At the final follow-ups,on average,VAS score (2.2 ± O.8),AOFAS ankle and hindfoot score (81.1 ± 6.8),SF-36 score (82.5 ± 5.2),dorsal extension (27.5 ° ± 2.5°),and plantar flexion (24.2° ±6.3°) were significantly improved compared with the preoperative values (6.3 ± 1.8,43.1 ± 12.0,37.6 ± 9.9,14.1° ± 3.8° and 14.4° ± 3.9°,respectively) (P < 0.05).No implant failure,soft tissue complications or post-traumatic arthritis occurred during the follow-up.Conclusion Since our self-designed 3D printing relay templates for osteotomy can accurately position the fracture lines of the distal tibia via limited incision and invasion,they help a lot in precise osteotomy and anatomical reduction of malunited intra-articular ankle fracture,preventing incidence of traumatic arthritis.
10.The new method for transfection of macromolecules into cells using protein transductions domain

Objective To construct a fusion protein to transfect some cell lines that were difficult to be transfected such as neoplastic cells, nerve cells, stem cells. Methods PCR was performed to amplified protein transductions domain(PTD),G4S and streptavidin (Strep).Enzymatic digestion and ligation were used to construct pAYZ-PTD-Strep plasmid. Fusion protein was induced to express by AP5 medium and was isolated by E-tag affinity chromatography. Fusion protein was identified by Western blot. eGFP was trasfected into U937 cells by pAYZ-PTD-Strep. FACS was performed to detect transfection percentage. Results Fusion protein PTD-G4S-Strep was expressed as soluble protein.The concentration of fusion protein was about 0.7 mg/L,and purity was over 90 %. The protein could carry plasmid into a suspended cell line, U937 cells. The transfection-efficiency of protein was higher than monometer PTD.Conclusion The protein PAYZ-PTD-Strep could carry macromolecules into blood tumor cells,and its biological activity may be expected to develop into a highly efficient and reliable transfection method.
