1.Influence of Needling Point Hegu(LI 4) on Colonoscopy

Purpose: To investigate the influence of needling bilateral points Hegu (LI 4) before colonoscopy on the examined patients and the time of inserting colonoscope. Methods: 185 patients to receive colonoscopy were randomly divided into an acupuncture group (93 cases) and a control group (92 cases) in order of examination. Bilateral points Hegu (LI 4) were needled before colonoscopy in the acupuncture group. No preventive measures were taken in the control group.Pain grades during the period that colonoscope passes the sigmoid and transverse colons were observed and the time of inserting colonoscope was recorded. Results: One hundred and sixty-five patients underwent colonoscopic insertion successfully, which were taken as the effective cases. Of them, 84 cases were in the acupuncture group and 81 cases in the control group. Statistics indicated that pain grades during colonoscope's passing the sigmoid and transverse colons were significantly lower in the acupuncture group than in the control group (P< 0.01); the time of inserting colonoscope was significantly shorter in the acupuncture group than in the control group (P < 0.05).Conclusion: Needling bilateral points Hegu(LI 4) before colonoscopy can effectively relieve the patients'discomfort and shorten the colonoscopic time during the examination.
2.THE SECONDARY AFFERENT PATHWAY OF PELVIC VISCERAL SENSATION

2?l 10% ricin were injected into the pelvic nerve of cats. After 3~5 survival days 20% HRP solution was injected into the ipsilateral lateral parabrachial nucleus, 2~3 days later the animals were perfused with paraformaldehyde and glutaraldehyde, the brain and spinal cord were removed and processed for light and electron microscopy. It was confirmed that the primary afferent terminals (degenerated) of pelvic nerve formed axo-dendritic and axo-somatic synapses with the retrograde labelled neurons projected to the lateral parabrachial nucleus which were located in the dorsal commissure nucleus (DCN), intermediolateral nucleus (IML) and lamina Ⅰ of sacral spinal cord. Hence, the secondary afferent pathway of pelvic visceral sensation, as. indicated at the ultrastructural level, might originate from DCN, IML and lamina Ⅰ of sacral spinal cord and project to the lateral parabrachial nucleus.
3.THE CYTOARCHITECTURE OF DORSAL COMMISSURE NUCLEUS OF THE SACRAL SPINAL CORD IN CAT

The Nissl and Golgi staining methods and electron microscopy were u(?)o investigate the architecture of dorsal commissure nucleus (DCN) of the sacral spinal cord in cat. A morphologically specific nucleus could be identified in the medial part of the lamina Ⅵ of dorsal horn. The nucleus located beside the dorsal funiculus at the lumbar level and gradually moved toward the midline of the dorsal gray commissure in the ventromedial direction as the dorsal gray commissure enlarged at the segments S_(1-2) and two ellipsoid nuclei was formed at the two sides of the midline. A large triangular nucleus was gradually formed by fusion of the two nuclei at S_3-Co_1. Golgi stain positive neurons could be classified into three types: (1) small-triangular neurons (60%); (2) fusiform neurons (30%); (3) oval and irregular neurons (10%). The cell organells of most neurons were plentiful. The cell nucleus was irregular and often with deep invaginations. A large number of free ribosomes and saccules could be found in the canals formed by the nuclear invagination. There were axo-dentritic, axo-axonic, axo-somatic synaptic contacts in the DCN with percentages of 89%, 6%, and 5% respectively. The serial synapse, axo-spinous synapse and crest synapses could also be found.
4.The Action Mechanism of Nucleus of Solitary Tract in Adjusting Gastric Motion by Acupuncture at "Zusanli"Point

Objective To research the function of the vagus nerve center nucleus of solitary tract(NTS)in the influence of gastric pressure by acupuncture at "Zusanli" in rats with motility disturbances of the stomach.Methods Adopting electrophysiological methods,the extracellular discharge signals of neuronic action in NTS were recorded with glass microelectrode.The influence of the discharges of NTS neurons and gastric pressure responding to the acupuncture at "Zusanli" point were observed in normal status and the condition of Atropine or Metoclopramide intravenous injection.Results In the normal status,acupuncture at "Zusanli" activated the discharges of NTS neurons and promote the gastric pressure remarkably.After Atropine intravenous injection,the background discharges of NTS neurons decreased.There was no statistical difference in the discharges responding to the effect of acupuncture before and after Atropine administration.And the gastric motion reinforced obviously by acupuncture at "Zusanli" after Atropine administration.But after intravenous injection of Metoclopramide,the background discharges of NTS neurons increased.There was also no statistical difference in the discharges responding to the effect of acupuncture before and after Metoclopramide administration.Moreover,the function of promoting gastric motion by acupuncture at "Zusanli" got more obviously after Metoclopramide administration.Conclusion The function of adjusting gastric motion by acupuncture at "Zusanli" point has close relation to the activation of NTS neurons,and the increasing amplitude of the discharges of NTS neurons wasn't influenced by cholinergic receptor blocker Atropine and dopamine-receptor antagonist Metoclopramide.
5.Expression of cytokeratin-19 as a stem cell marker in different-purity islet samples of rats

BACKGROUND: Pancreatic stem cells can be differentiated into endocrine functioning cells under a suitable cultivation condition, suggesting that pancreatic stem cells can be used as a new source of islet in treating diabetes mellitus.OBJECTIVE: To discuss the expression of stem cell in different purities of pancreatic islets.DESING: A controlled observation based on cells.SETTING: Third People's Hospital of Mianyang City.MATERIALS: The experiment was conducted in the Center Laboratory (Key Laboratory of Chongqing City), the First Affiliated Hospital of Chongqing Medical University from March 2005 to March 2006. Thirty healthy male SD rats, of clean grade, aged 50 days were used in this study. During the experiment, the disposal of animals corresponded to Animal Ethical Standard. Cytokeratin (CK)-19 and β-actin were designed by Shanghai Invitrogen Biological Co., Ltd.Brdu reagent, mouse anti-rat Brdu monoclonal antibody, rabbit anti-mouse (RAM) CK-19 polyclonal antibody and Brdu immunohistochemical kit were from Boster Co., Ltd. (Wuhan); HistostainTM -DS double immunohistochemical staining kit was purchased from Beijing Zhongshan Company.METHODS: Rats were intraperitoneally injected with Brdu for labeling, and subjected to perfusion with Ⅴ-type collagenase via pancreatic ducts, with pancreas being excised, ripped, beaten up, digested and centrifuged to obtain pancreatic islet sediments. After being precipitated, the sediments were divided into 3 groups. Group A: The isolated sediments of islets were not purified; Group B: The sediments were added slowly with 25% Ficoll-400 and Hanks solution for purification; Group C: The sediments were slowly added with 25% Ficoll-400, 11% Ficoll-400 for purification in order.MAIN OUTCOME MEASURES: Expressions of Brdu and CK-19-positive cells in the islet samples were detected by immunohistochemical method, and mRNA expression of CK-19 in the different purities of islets was detected by reverse transcription-polymerase chain reaction.RESULTS: ① The purity of islets in the group A was the lowest. The purity of islets obtained by different purification techniques showed significances among the three groups (F =89.42, P < 0.05). ②Positive expressions of Brdu and CK-19 in the group A were significantly higher than those in the group B and group C (F =18.64, 22.12, 38.61. P <0.01). ③Expression of CK-19 mRNA in the group A was significantly higher than that in other groups, with group C showing the lowest.CONCLUSION: Expressions of Brdu and CK-19-positive cells exist in different purities of islets, suggesting the existence of more stem cells in the low purify of islets.
6.Effect of Cupping plus Medication on Quality of Life and Estrogen Level in Perimenopausal Syndrome Patients

Objective To observe the effect of cupping plus medication on the quality of life and estrogen level in perimenopausal syndrome patients. Method A total of 40 eligible perimenopausal syndrome patients were randomized into a cupping-medication group and a Chinese medication group, 20 cases in each group. The Chinese medication group was intervened by Er Xian decoction, the cupping-medication group was intervened by moving cupping on the back in addition to the treatment given to the Chinese medication group, once a week. Each item of the Menopause-specific Quality of Life (MENQOL), the levels of estradiol (E2), follicle-stimulating hormone (FSH) and luteotropic hormone (LH) in serum were observed before and after the treatment, and the results were analyzed. Result After two treatment courses, the vasomotor symptoms, mental state, physiological state, and sexual life scores in the two groups were significantly improved compared to the scores before the treatment (P<0.05,P<0.01), and there was a significant difference between the two groups in comparing the MENQOL total score (P<0.01). The between-group comparison after the treatment showed that the cupping-medication group was significantly superior to the Chinese medication group (P<0.05). After two treatment courses, the serum E2 levels in the two groups were significantly higher than those before the treatment (P<0.01), and the increase in the cupping-medication group was more significant than that in the Chinese medication group (P<0.05). After the treatment, the serum FSH and LH levels in the two groups were lower compared to those before the treatment, but without statistical significance (P>0.05). Conclusion Moving cupping on the back plus Er Xian decoction can effectively improve the quality of life of perimenopausal syndrome patients and significantly increase the serum E2 level, superior to using Er Xian decoction alone.
7.Application value of CT-guided percutaneous lung biopsy in lung disease

Objective To investigate the efficiency and application value of CT-guided percutaneous puncture biopsy in the diagnosis of lung disease.Methods The clinical and imaging data of 60 patients with CT-guided percutaneous lung biopsy were analyzed retrospectively.Results Among all the 60 patients, 57 had finally to be proved by pathological diagnosis, including 47 primary lung cancer (21 squamous cell carcinoma, 18 adenocarcinoma, 3 small cell lung cancer and 5 bronchioloalveolar carcinoma), 7 metastatic lung cancer and 3 tuberculosis.In addition, 3 cases were chronic inflammation and did not make a definite diagnosis, in which 2 were proved to be caseous pneumonia and 1 was proved to be fungus infection by postoperative pathology.The successful rate of punctures,the diagnostic accuracy and the total complications rate of lung puncture biopsy in 60 cases was 100%, 95.0% and 21.7% [11.7% pneumothorax (7/60), 8.3% hemoptysis (5/60) and 1.7% needle bleeding(1/60)].Conclusion CT-guided percutaneous puncture biopsy is a safe, accurate and significant clinical method in the diagnosis and differential diagnosis of lung lesions.
8.Reflections on cancer treatment

Incidence of cancer is increasing year by year,diagnostic techniques are continually advanced,and the numbers of anti-cancer drugs are growing,but the efficacy has not been fundamentally improved.Surgery,chemotherapy and radiotherapy can only kill the part of mature tumor cells,but can not kill the cancer stem cells,and eventually leading to tumor recurrence.While acupuncture,qigong,psychology comprehensive treatment can inhibit tumor growth and reduce metastasis and recurrence,improve quality of life and prolong survival.We should update our recognition,to carry out the study on the treatment of tumor by TCM with large sample and multiple centers,to demonstrate its low toxicity,low costs and efficiency.
9.Correlation of tumor size and lymph node metastasis with the efficacy of postoperative radiotherapy in non-small cell lung cancer

Objective To analyze the factors affecting the efficacy of postoperative radiotherapy (PORT) in node-positive non-small cell lung cancer (NSCLC). Methods 480 patients with stage N_1-N_2 NSCLC after radical surgery were retrospectively reviewed. Of them, 267 patients received adjuvant chemotherapy and 121 received PORT. All patients were grouped based on the N stage, tumor size and lymph node positive ratio (the percentage of positive lymph nodes from the detected lymph nodes, LNPR). Group 1 included patients with tumor size ≤3 cm and LNPR ≤33%, group 2 was tumor size > 3 cm or LNPR > 33%, and group 3 was tumor size > 3 cm and LNPR > 33%. The endpoints were the local recurrence free survival (LRFS) and overall survival (OS). Kaplan-Meier method and Cox's proportional hazards regression model were used for the statistic analyses. Results PORT improved the overall survival only in patients with N_2 disease. Both tumor size and LNPR significantly influenced the efficacy of PORT. The 5-year LRFS for patients with vs. without PORT in the group 1, 2 and 3 were 55% vs. 60% (χ~2 = 0.03,P-0.869), 42% vs. 50% (χ~2 =0.31,P=0.547),and 62% vs. 52% (χ~2=4.25,P=0.036), respectively;and the corresponding OS were 22% vs. 50% (χ~2 = 1.65 ,P =0. 199), 26% vs. 22% (χ~2= 0. 13,P=0.786) and 42% vs. 16% (χ~2= 15.33,P=0.000), respectively. Conclusions Tumor size and LNPR significantly impact the efficacy of PORT . For patients with stage N_2 NSCLC , PORT could improve local recurrence free survival and overall survival when tumor size > 3 cm and LNPR >33%.
10.Clinical study on surgical treatment for injury of liver and vascular approaching to liver with 35 Cases

ObjectiveTo explore the surgical approach and therapeutic efficacy of injury of liver and vascular approaching to liver.MethodsThe clinical data from January 1997 to May 2010 of 35 patients with injury of liver and vascular approaching to liver were retrospectively analyzed.Results Among 35 cases,32cases were cured by surgery ( cure rate 91.43% ) and 3 cases died ( mortality rate 8.57% ).Postoperative complications occurred in 5 patients ( complication rate 14.29% ).There were 2 cases of pulmonary infection,1case of biliary fistula,1 case of multiple organ failure and 1 case of incision infection.Conclusion Urgent surgery need to be performed for injury of liver and vascular approaching to liver and effective recovery was important.In the operation,careful handling,complete dissociation and sufficient exposure were the key to successful repair.
