2.Controlled Clinical Study on Electroacupuncture at the Acupoints on Extremities Predominantly for Functional Dyspepsia

Objective To observe the therapeutic efficacy of electroacupuncture at the acupoints on extremities or abdominal acupoints and orally taking Mosapride in treating functional dyspepsia (FD) and the different effects on life style, for guiding the points selection in acupuncture-moxibustion treatment for FD.Method Sixty FD patients were randomized into an extremity-electroacupuncture group, an abdomen-electroacupuncture group, and a Western medication group by their visiting sequence, 20 cases in each group. In theextremity-electroacupuncture group, Zusanli (ST36), Sanyinjiao (SP6), Neiguan (PC6) and Gongsun (SP4) were selected. Bilateral Zusanli and Sanyinjiao were respectively connected to G6805-2 electroacupuncture apparatus, 30 min for each session, once everyother day, 10 sessions in total. In the abdomen-electroacpuncture group, Zhongwan (CV12), Tianshu (ST25), and Qihai (CV6) were selected. Bilateral Tianshu were connected to electroacupuncture apparatus, with same treatment time, frequency and sessions. The Western medication group was intervened by oral administration of Mosapride, 1 tablet each time, 3 times a day, for 20 d in total. The three groups were evaluated by using Nepean Dyspepsia Index (NDI) prior to the treatment, after the treatment and 1 month after the treatment (follow-up study), and the therapeutic efficacies were estimated after the treatment.Result The effective rate of the extremity-electroacupuncture group was higher than that of the abdomen-electroacupuncture group and Western medication group, while there was no significant difference between the abdomen-electroacupuncture group and Western medication group in comparing the effective rate. After the treatment, the improvement of Nepean Dyspepsia Symptom Index (NDSI) in the extremity-electroacupuncture group was more significant than that in the abdomen-electroacupuncture group and Western medication group (P<0.05), while there was no significant difference between the abdomen-electroacupuncture group and Western medication group (P>0.05). The 1-month follow-up showed that the improvement of symptoms in the extremity-electroacupuncture group was more significant than that in the abdomen-electroacupuncture group (P<0.05), and the improvement in the abdomen-electroacupuncture group wasmore significant than that in the Western medication group (P<0.05). After the treatment, the improvements of Nepean Dyspepsia Life Quality Index (NDLQI) in the extremity-electroacupuncture group and abdomen-electroacupuncture group were superior to the improvement in the Western medication group (P<0.05), while there was no significant difference between the two electroacupuncture groups (P>0.05). The 1-month follow-up study showed that the quality of life in the extremity-electroacupuncture group was still significantly improved compared to that before the treatment, while the improvement in the Western medication group was insignificant.Conclusion Electroacupuncture at the acupoints on extremities and abdominal acupoints both can more effectively and consistently improve the symptoms and quality of life of FD patients compared to orally taking Mosapride. Compared to electroacupuncture at the abdominal acupoints, electroacupuncture at the extremity points works better in improving the symptoms of FD butdoesn't show advantage in improving the quality of life.
3.Relationship of serum C reactive protein level with serum leptin concentration in type 2 diabetic patients

Objective To explore the association of serum high-sensitivity C reactive protein (hs-CRP) level with leptin in patients with type 2 diabetes mellitns (T2DM). Methods 35 cases of T2DM patients were involved in the study and 16 normal subjects were taken as controls. Fasting serum leptin and CRP were measured .The relationship between them were also analyzed. Results Fasting serum hs-CRP concentration in T2DM group was higher than that in healthy control group, but serum ieptin levels is the same (P > 0.05). Partial correlation analysis demonstrated that fasting hs-CRP level was positively correlated with leptin (r =0.512 ,P =0. 018). Conclusion Fasting serum hs-CRP level is significantly raised in T2DM ,which is associated with leptin. There is chronic inflammatory reaction in T2DM patients,and leptin is possibly involved in inflammatory pathogenesis of T2DM.
4.Effects of different doses of dexmedetomidine on EC50 of propofol given by target-controlled infusion at loss of consciousness

Objective To evaluate the effects of different doses of dexmedetomidine on the median effective concentration (EC50) of propofol given by target-controlled infusion (TCI) at loss of consciousness (LOC).Methods Eighty ASA Ⅰ or Ⅱ patients of both sexes,aged 18-64 yr,with body mass index ≤25 kg/m2,scheduled for operations under general anesthesia,were randomly allocated to one of four groups(n=20 each): control group (group C) and dexmedetomidine 0.4 μg/kg group (group D1),dexmedetomidine 0.5 μg/kg group (group D2) and dexmedetomidine 0.6 μg/kg group (group D3).Dexmedetomidine 0.4,0.5 and 0.6 μg/kg were infused intravenously over 10 min in groups D1-3,while the equal volume of normal saline was given instead of dexmedetomidine in group C.Propofol was then given by TCI and the EC50 was determined by up-and-down sequential trial.The target plasma concentration was set at 2.0μg/ml in the first patient in each group.The ratio of the target plasma concentration between the two consecutive patients was 1.1.Loss of response to eyelash stimulation and verbal command (2 times) was considered to be signs of LOC.The EC50 and 95% confidence interval (CI) of propofol causing LOC were calculated.Complications such as bradycardia,hypotension and respiratory depression were recorded.Results The EC50 (95% CI) of propofol causing LOC was 2.59 (2.51-2.67),2.09 (2.02-2.16),1.82 (1.70-1.95) and 1.60 (1.49-1.72) μg/ml in groups C and D1.3 respectively.The EC50 of propofol causing LOC was significantly lower in groups D1-3 than in group C.Dexmedetomidine significantly decreased the EC50 of propofol required for causing LOC in a dose-dependent manner in groups D1-3 (P < 0.05).The incidences of bradycardia and hypotension were significantly lower in groups D1.3 than in group C (P < 0.05).Compared with group D1,the incidence of bradycardia was increased in groups D2,3 and the incidence of hypotension was increased in group D3 (P < 0.05),There was no significant difference in the incidences of bradycardia and hypotension between groups D2 and D3 (P > 0.05).No patients developed respiratory depression.Conclusion The optimum dose for dexmedetomidine infused intravenously when combined with propofol given by TCI is 0.4 μg/kg and it can decrease the EC50 of propofol administered by TCI at LOC with no adverse reactions.
5.Association of serum leptin with intima-media thickness of carotid artery in type 2 diabetes patients

Objective To investigate the relationship between serum leptin and carotid intima-media thickness in patients with type 2 diabetes mellitus. Methods 35 patients with type 2 diabetes mellitus (DM) and 16 normal controls were studied. Fasting serum leptin, fasting glucose(FBG), HbAlc, C-peptide, lipide, systolic(SBP), diastolic blood pressure(DBP), body mass index (BMI) and waist circ-umference (WC) were measured in all subjects. The intima-media thickness (IMT) of common carotid artery was measured by high-resolu-tion ultrasound imaging. Results IMT of T2DM was positively correlated with leptin concentration. Conclusion Serum leptin concentra-tion is independently associated with the IMT of carotid artery, which suggest that leptin may be an independent risk factor of atherosclerosis.
6.Study of relationship between liver function and the enhancement degree of liver parenchyma in the hepatobiliary phase with Gd-BOPTA

Objective To explore the relationship between the MRI enhancement ratios of liver parenchyma in hepatobiliary phase with gadobenate dimeglumine (Gd-BOPTA)and liver function.Methods Fifty-nine patients who underwent Gd-BOPTA-enhanced MRI were retrospectively enrolled in the study.The enhancement ratio of signal to noise ratio and enhancement ratio of the contrast ratio were calculated.The relationships between the enhancement ratio and CTP grading and MELD score were analyzed.Results The signal enhancement ratios in hepatobiliary phase in patients with CTP A classification were higher than those with CTP B classi-fication (P <0.01).Meanwhile,the ratios in patients with MELD scores less than 10 points were higher than those with MELD scores more than 10 points (P <0.01).Conclusion The MR enhancement degree of liver parenchyma in the hepatobiliary phase with Gd-BOPTA may reflect the liver function.
7.A Randomized Controlled Trial of Warming and Unblocking Acupuncture Treatment for Scapulocostal Syndrome

Objective To investigate the clinical efficacy of warming and unblocking acupuncture in treating scapulocostal syndrome.Method Eighty patients with scapulocostal syndrome meeting the inclusion criteria were allocated by simple randomization to warming and unblocking acupuncture and conventional acupuncturegroups, 40 cases each.The warming and unblocking acupuncture group received warming and unblocking acupuncture at points Tianzong(SI11), Jianjing(GB21)and Dazhui(GV14)on the affected side and affected chest Huatuo jiaji(Ex-B2)points and uniform reinforcing-reducing acupuncture at points Quchi(LI11), Hegu(LI4)and Waiguan(TE5).The conventional acupuncture group received uniform reinforcing-reducing acupuncture at the same points as those in the warming and unblocking acupuncture group after arrival of qi.Both groupswere treated once every other day, 10 days as a course, for a total of one course.Pain severity was assessed using the Pain Visual Analogue Scale(VAS) in the two groups of patients before and after treatment.Theclinical therapeutic effects were evaluated by follow-up at one month after the completion of treatment.Result The total efficacy rate was 95.0% (38/40) in the warming andunblocking acupuncture group and 80.0% (32/40) in the conventional acupuncture group; there was a statistically significant difference (P<0.05).The PainVisual Analogue Scale(VAS) score was significantly lower in the two groups of patients after treatment than before treatment; there was a statistically significant difference (P<0.05).The VAS score was decreased more in the warming and unblocking acupuncture group after treatment and at the follow-up one month after the completion of treatment (P<0.05).Conclusion Warming and unblocking acupuncture has clinically a better therapeutic effect onscapulocostal syndrome.

10.Analysis of influence factors on hospitalization expenses with hip arthroplasty

Objective To discuss the effect of rehabilitation nursing for total knee arthroplasty by discussing hospitalization expenses and its factors of rehabilitation nursing.Methods 305 patients who were underwent total knee arthroplasty from 2011 to 2013 were recruited and recovered according to rehabilitation nursing and training requirements established by our hospital.Patients admitted in 2011 were designated as control group.The comparison of American knee society knee score and knee rang of motion of patients were made and analyzed.Results The annual comparison of sores of pre-and post-operative KSS (t=5.30,7.99,11.20),and knee ROM of patients (t=8.21,4.57,7.86) showed that the difference was statistically significant (P<0.05).The total costs and rehabilitation nursing expenses inreased year by year (P<0.05).The proportion of rehabilitation and rehabilitation nursing expenses each year were 5.77% (3 157/54 679),4.60% (2 847/61 831),5.15% (3 341/64 930) and 0.51% (280/54 679),0.64% (393/61 831),0.52 %(338/64 930),respectively.Conclusions Pre-and post-operative correct rehabilitation exercises are one of the most important guarantee of curative effect.There is a lower percentage of rehabilitation nursing expenses in total costs,and the value of the nursing staff has not been fully reflected.
