1.Construction and Application of Traditional Chinese Medicine Efficacy Concept Network

This study was aimed to establish Chinese medicine efficacy concept network in order to reveal the in-ner relation between traditional Chinese medicine (TCM) and efficacy as well as to suggest the application way of efficacy concept network. Through the collation of medicine efficacy recorded in the Pharmacopoeia of the People's Republic of China (2010 Edition, Volume I), the correlation relationship between TCM efficacy concept was systematically analyzed, and the efficacy concept network was constructed in order to extract specific effica-cy sub-network based on efficacy concept network and then to discover the efficacy groups by utilizing the net-work clustering algorithm. In the Chinese medicine efficacy concept network, 422 efficacy concepts were con-structed and 21 tightly efficacy groups were discovered. Using the efficacy of promoting blood circulation as an example, the application of sub-networks was discussed. Chinese medicine efficacy concept network can be used to reflect the basic theory and clinical experiences in Chinese medicine , to systematically reveal the relationship between TCM efficacy concepts, which can be beneficial to guide the reasonable application of Chinese medicine in disease treatment .
2.Effect of Ginkgo biloba extract EGb761 on apoptosis of K-ras mutational human colon cancer cells DLD1 and its mechanism

Objective To explore the effect of Ginkgo biloba extract (EGb761) on apoptosis of K-ras mutational human colon cancer cells DLD1(DLD1/G13D)and its mechanism. Methods Human colon cancer cell lines DLD1/G13D and DLD1 with K-ras wild type(DLD1/WT)were cultured in vitro,the cell proliferation and apoptosis after 24 h of EGb761 were measured. Proteins involved in related signal pathway were detected by Western blot or ELISA. Results EGb761 reduced cell proliferation and induced cell apoptosis in a concentration-dependent manner in DLD1/WT and DLD1/G13D cells. EGb761 downregulated the expression of RIP1, impaired the phosphorylation of IκB and decreased the level of NF-κB in DLD1/WT and DLD1/G13D cells[DLD1/G13D: (24±4)%, DLD1/WT: (29±9)%(P<0.05). Conclusion EGb761 restrains the proliferation and induces the apoptosis of DLD1/WT and DLD1/G13D cells. The mechanism may be related to the degradation of RIP-1 and inhibition of activation of NF-κB signaling pathway.
3.One-stage posterior approach via multifidus-Iongissimus cleavage planes for debridement combined with autogenous bone grafting and internal fixation in the treatment of thoracolumbar suppurative spondylitis in adults

Objectives:To explore the clinical effects of one-stage posterior approach via multifidus-longis-simus cleavage planes for lesion debridement,bone grafting and internal fixation in the treatment of thora-columbar suppurative spondylitis in adults.Methods:73 patients with thoracic or lumbar pyogenic spondylitis who underwent one-stage posterior approach via multifidus-longissimus cleavage planes for lesion removal and bone grafting and internal fixation in the Department of Orthopedics,the Affiliated Hospital of Southwest Medical University from February 2019 to February 2021 were retrospectively analyzed.There were 52 males and 21 females,aged 35-75 years(56.5±10.7 years);T1-T10 in 15 cases,T11-L2 in 32 cases,and L3-L5 in 26 cases.Obvious kyphotic deformity was found locally in all the patients.23 cases were accompanied with intraspinal epidural abscess,and 17 cases with paravertebral or psoas major muscle abscess.7 cases were combined with neurological impairment,and the American Spinal Injury Association(ASLA)classification of neurological function was detailed in 1 case of grade C and 6 cases of grade D.24 cases were complicat-ed with cardiovascular and cerebrovascular diseases,35 cases with diabetes,and 30 cases had a history of invasive operation.The venous blood was collected for bacterial culture before operation.All the patients were treated with one-stage posterior approach via multifidus-longissimus cleavage planes for lesion removal and bone grafting and internal fixation.The lesion tissue was taken for bacterial culture during operation.The postoperative irrigation lasted for 10-14d,and the anti-infection treatment lasted for 6-8 weeks.The operative time,intraoperative blood loss,and length of hospital stays,and complications were recorded;Erythrocyte sedimentation rate(ESR),C-reactive protein(CRP)and procalcitonin(PCT)were compared preoperatively and be-fore discharge;Visual analogue scale(VAS)scores of thoracolumbar back pain before operation and discharge and at final follow-up were compared.The Cobb angles of kyphosis of the lesion segment were measured on radiographs,and the rate of correction of kyphosis and loss angle of correction at the final follow-up were calculated;The recovery of spinal cord function(ASIA grade)and bone graft fusion at final follow-up were ob-served and evaluated.Results:All the patients underwent the surgery successfully.The operative time was 195.8±15.5min,the intraoperative blood loss was 562.1±45.5mL,and the length of hospital stays were 18.0±2.0d.Postoperative complications occurred in 9 patients(12.3%):2 patients had delirium syndrome,which gradually disappeared after oral administration of olanzapine;4 cases had lower limb pain and numbness,and the symptoms disappeared after nutritional nerve treatment;3 cases presented with pleural effusion and were cured after administration of closed chest drainage,anti-infection and strengthening of lung function exercises.51 cases were positive of bacterial culture,including 29 cases of Staphylococcus aureus,17 cases of Es-cherichia coli and 5 cases of Staphylococcus epidermidis.Preoperative ESR,CRP and PCT were 80.9±13.9mm/h,58.4±13.5mg/L,0.8±0.2ng/mL,and before discharge they were 29.6±8.9mm/h,15.0±7.2mg/L and 0.1±0.0ng/mL.ESR,CRP,and PCT were significantly decreased before discharge compared with preoperative values(P<0.05).The patients were followed up for 12-30 months(20.1±4.0 months).At preoperation,before dis-charge and final follow-up,the VAS scores were 7.6±1.1,2.2±0.6 and 1.6±0.6,respectively.Before discharge and final follow-up VAS scores were significantly lower than the preoperative score(P<0.05).The Cobb angle was 19.6°±1.6°,6.2°±1.5°,9.4°±1.0°,and the correction angle was 13.4°±0.6°,the correction rate was 68.7%±5.3%,which was significantly corrected before discharge(P<0.05).At final follow-up,the corrected angle loss was 3.2°±0.7° relative to before discharge,and the corrected angle loss rate was 24.0%±4.6%(P<0.05).At the final follow-up,one patient of preoperative ASIA grade C recovered to grade D,and six patients of grade D recovered to grade E.There was no change in ASLA grade after operation in patients without nerve injury before operation.Bone fusion was achieved in all patients at the final follow-up.Conclusions:On the basis of anti-infection,one-stage posterior approach via multifidus-longissimus cleavage planes for the treatment of thoracolumbar suppurative spondylitis enables removal of lesion,decompression of spinal canal,and fusion and fixation of intervertebral support implants with protection of paravertebral muscles as well as the posterior lig-amentous complexes,and reduces chronic thoracolumbar back pain after operation.
4.Can radical parametrectomy be omitted in occult cervical cancer after extrafascial hysterectomy?

BACKGROUNDOccult invasive cervical cancer discovered after simple hysterectomy is not common, radical parametrectomy (RP) is a preferred option for young women. However, the morbidity of RP was high. The aim of our study is to assess the incidence of parametrial involvement in patients who underwent radical parametrectomy for occult cervical cancer or radical hysterectomy for early-stage cervical cancer and to suggest an algorithm for the triage of patients with occult cervical cancer to avoid RP.
METHODSA total of 13 patients with occult cervical cancer who had undergone RP with an upper vaginectomy and pelvic lymphadenectomy were included in this retrospective study. Data on the clinicopathologic characteristics of the cases were collected. The published literature was also reviewed, and low risk factors for parametrial involvement in early-stage cervical cancer were analyzed.
RESULTSOf the 13 patients, 9 had a stage IB1 lesion, and 4 had a stage IA2 lesion. There were four patients with grade 1 disease, seven with grade 2 disease, and two with grade 3 disease. The median age of the entire patients was 41 years. The most common indication for extrafascial hysterectomy was cervical intraepithelial neoplasia 3. Three patients had visible lesions measuring 10-30 mm, in diameter and ten patients had cervical stromal invasions with depths ranging from 4 to 9 mm; only one patient had more than 50% stromal invasion, and four patients had lymph-vascular space invasion (LVSI). Perioperative complications included intraoperative bowel injury, blood transfusion, vesico-vaginal fistula, and ileus (1 case for each). Postoperative pathologic examination results did not show residual disease or parametrial involvement. One patient with positive lymph nodes received concurrent radiation therapy. Only one patient experienced recurrence.
CONCLUSIONSPerioperative complications following RP were common, whereas the incidence of parametrial involvement was very low among selected early-stage cervical cancer patients. Based on these results, we thought that patients with very low-risk parametrial involvement(tumor size ≤ 2 cm, no LVSI, less than 50% stromal invasion, negative lymph nodes) may benefit from omitting RP. Further prospective data are warranted.

5.Efficient lung cancer-targeted drug delivery a nanoparticle/MSC system.

Low targeting efficiency limits the applications of nanoparticles in cancer therapy. The fact that mesenchymal stem cells (MSC) trapped in the lung after systemic infusion is a disadvantage for cell therapy purposes. Here, we utilized MSC as lung cancer-targeted drug delivery vehicles by loading nanoparticles (NP) with anti-cancer drug. MSC showed a higher drug intake capacity than fibroblasts. In addition, MSC showed predominant lung trapping in both rabbit and monkey. IR-780 dye, a fluorescent probe used to represent docetaxel (DTX) in NP, delivered MSC accumulated in the lung. Both MSC/A549 cell experiments and MSC/lung cancer experiments validated the intercellular transportation of NP between MSC and cancer cells. assays showed that the MSC/NP/DTX drug delivery system exerted primary tumor inhibition efficiency similar to that of a NP/DTX drug system. Collectively, the MSC/NP drug delivery system is promising for lung-targeted drug delivery for the treatment of lung cancer and other lung-related diseases.
6.Preparation and Quality Evaluation of Pogostone Transfersomes

OBJECTIVE: To prepare pogostone transfersomes, and to evaluate its quality. METHODS: Film dispersion method was used to prepare pogostone transfersomes. Using the accumulative penetration volume (Qn) and accumulative penetration ratio (PR) of pogostone as evaluation indexes, the types of surfactant, formulation were screened in respects of the dosage of surfactant and the dosage of pogostone. The pogostone transfersomes were prepared with optimal formulation; the morphology, particle size distribution and Zeta potential were observed and the entrapment efficiency was measured. RESULTS: The optimal formulation was as follows as the sodium cholate was selected as surfactant; the dosage of sodium cholate was 0.25 g; the dosage of pogostone was 15 mg. The optimal pogostone transfersomes were ivory-white suspension; average particle size was (115.6±3.65) nm (RSD=3.20%,n=3); PDI was 0.185±0.008 (RSD=4.30%, n=3); Zeta potential was (-13.76±0.225) mV (RSD=1.70%,n=3); entrapment efficiency of pogostone was (46.01±0.40)% (RSD=0.87%,n=3); Qn was (378.76±0.61) μg/cm2 (RSD=0.20%,n=3); PR was (89.02±0.96)% (RSD=1.10%,n=3). CONCLUSIONS: Prepared pogostone transfersomes are in line with quality requirements, which can provide reference for the further study of new dosage form of pogostone.
