1.Correlation between the changes in cardiac structure and function and bone age in children with congenital atrial septal defect

Objective To investigate the changes in physique and the bone age,and the correlation between the changes in cardiac structure and function and the bone age index (BAI) in atrial septal defect (ASD) children.Methods The experimental group included 48 cases of ASD patients,and 30 cases of healthy children were assigned as the control group;the children of both groups were measured for height and weight.Philips ie33 ultrasonic detector was used to detect the indicators of cardiac structure and function,and Philips digital X-ray machine was used to radiograph the left hand and the wrist.Then the bone age was assessed in order to calculate the BAI.Fourteen cases of ASD patients who underwent repairing surgery were measured for orifice area in surgery.The differences in physique and the bone age between the 2 groups were analyzed,and Pearson correlation analysis method was used to observe the correlation between the cardiac structure and function indicators,defect area and BAI in the experimental group.Results There was no significant difference in the age,height,bone age (t =-0.02,-1.31,-1.69,all P > 0.05),while the weight and BAI were significantly different between the 2 groups (t =-2.28,-9.06,all P < 0.05).The height,weight and BAI were significantly different in different pulmonary artery systolic pressure (PASP) patients (F =27.630,23.537,16.704,all P < 0.01),while the age and weight had no difference (t =-1.218,-0.046,all P > 0.05).The height and BAI were significantly different in patients with different defect size(t =2.561,2.191,all P < 0.05).There was no correlation between the age and the BAI in children with ASD(r =-0.175,P > 0.05).The defect size,defect size/atrial septal stretched diameter,PASP were all negatively correlated with BAI (r =-0.349,P < 0.05 ; r =-0.412,P <0.01 ;r =-0.539,P <0.01).The defect area was positively related to the defect size in 14 cases of children with ASD underwent surgery (r =0.599,P < 0.05),and the defect area was negatively correlated with BAI (r =-0.655,P < 0.05).Conclusions The physical development in ASD patients evidently lagged behind the healthy children,particularly for weight.The BAI of ASD patients is significantly smaller than that of the healthy children,in other words,the growing speed of bone in ASD patients is significantly slower than that of the healthy children at the same age.The lagging extent of physical development and the bone age are related to the PASP and defect size.PASP has more influence on the height,yet the defect size has influence on height and weight of ASD children.There is no correlation between physical development and the bone age with the course of disease.
2.Inhibitory effects of jiajianzhujing decoction containing serum on proliferation of rat choroidal vascular endothelial cells

Background Choroidal neovascularization (CNV) of macular zone is one of blinding eye diseases.Conventional treatment methods include photodynamic therapy and intravitreal injection of anti-vascular endothelial growth factor (VEGF) drugs.However,there are some adverse effects following these treatments.Researches showed that fiajianzhujing decoction,a Chinese herbal medicine,can inhibit CNV.Objective This study was to investigate the impact of drug serum offiajianzhujing decocation (drug serum) on the proliferation of rat choroidal vascular endothelial cells (RCVECs).Methods Jiajianzhujing decoction (0.525 g/ml) was used by intragastric administration with the dosage 20 ml/kg for 3 days,and the abdominal aortic blood was collected 2 hours after last dosage to prepare the drug serum.Cultured RCVECs were divided into 4 groups.CoCl2 with the concentration of 100 μmol/L (100 μl) was added in the medium to establish the anoxic models in the CoCl2 group,and 20% drug serum was added in the anoxic culture medium in the drug serum+CoCl2 group.Regular culture medium was used in the blank control group,and 20% pure serum was added in the anoxic culture medium in the pure serum+CoCl2 group.The cells were incubated for consecutive 24 hours,and the growth of the cells was detected by monotetrazolium (MTT) assay as absorbance (A).The expressions of gene and protein of hypoxia-inducible factor-1 α (HIF-1 α) and VEGF in the cells were detected by reverse transcription PCR and Western blot,respectively.This study was approved by the Ethics Committee of Eye Hospital of China Academy of Chinese Medical Sciences.Results Cultured cells grew well with fusiform shape.Hypoxic cell models were created by adding CoCl2.The mean A values were 0.659± 0.051,0.757±0.553,0.683±0.037 and O.731 ±O.038 in the blank control group,CoCl2 group,drug serum+CoCl2 group and pure serum+CoCl2 group,respectively,and the A value in the CoCl2 group was significantly elevated in comparison with the blank control group and drug serum+CoCl2 group (both at P<0.O1).The significant differences were found in the relative expressions of HIF-1 α and VEGF among the blank control group,CoCl2 group,drug serum+ CoC12 group and pure serum+CoCl2 group (HIF-1 α:F =3.100,P<0.05;VEGF mRNA:F =3.420,P<0.05;HIF-1 α protein:F=470.600,P =0.000;VEGF protein:F =146.700,P =0.000),and the relative expressions of HIF-1α mRNA,VEGF mRNA,HIF-1α protein and VEGF protein in the CoCl2 group were significantly higher than those in the blank control group and drug serum+CoCl2 group (all at P<0.05).Conclusions Drug serum ofjiajianzhujing decoction can inhibit the growth and proliferation of hypoxic RCVECs by down-regulating the secretion of HIF-1α and VEGF.
3.Study on Restrictive Critical Factors and Research of Traditional Chinese Medicine Enterprises Become Bigger and Stronger

The literature study and survey revealed existed problems and factors such as the difficulties to maintain quality of traditional Chinese medicine (TCM), sustainable development, homogenization and indeterminacy of clinical orientation diagnosis, lack of favorable evidence-based medicine and pharmacoeconomics data to support, lack of ev-idence of safety evaluation, which restrict the development of TCM enterprises. Based on problems and factors men-tioned above, recommendations were made on the healthy development of TCM enterprises, which include guarantee of the quality of TCM herbs and the sustainable development of its resource; conduct systematic evidence-based pharmacoeconomic research, identify the clinical orientation; carry out safety assessment systematically, improve the specification; improve clinical effectiveness-oriented quality standards.
4.The efficacy of internet-based cognitive behavior therapy on blood pressure for comorbid hypertension and insomnia

Objective To evaluate the effectiveness of internet-based cognitive behavior therapy (CBT-I) on blood pressure in patients with hypertension comorbid with insomnia.Methods One hundred and six patients met the JNC-8 diagnostic criteria for hypertension and DSM-5 diagnostic criteria for insomnia were selected,and randomly assigned to CBT-I group (n=53) and regular treatment group (n=53).All the patients accepted relevant treatment for 8 consecutive weeks.Changes of systolic blood pressure (SBP),diastolic blood pressure (DBP),the sleep parameters and the anxiety and depression levels were observed between baseline and 8 weeks of follow-up.Results Compared with pre-treatment,SBP and DBP decreased significantly in both CBT-I group and regular treatment group (P<0.01),the decrease was more significant in CBT-I group than in regular treatment group (P<0.01).The sleep latency,sleep efficiency,bed time and rating anxiety scale were improved more significantly in CBT-I group than in regular treatment group (44.49 ± 22.75min vs.55.50 ± 34.96min;72.31% ± 9.15% vs.57.70% ± 11.53%;302.65 ± 43.76min vs.262.70 ± 50.64min and 17.14 ± 6.45 vs.21.02 ± 6.64,respectively),the differences were of statistical significance (P<0.05).In addition,no significant difference was found in the level of depression between CBT-I group and regular treatment group.Conclusion CBT-I can effectively shorten sleep latency,improve sleep efficiency,extend bed time and improve anxiety,meanwhile effectively lower the blood pressure of patients with hypertension comorbid with insomnia by optimizing sleep quality.
5.Establishment of a diagnostic model for charactering solitary pulmonary nodule based on 18F-FDGPET/CT and clinical data

Objective To establish a diagnostic model based on 18F-FDG PET/CT and clinical data and assess its diagnostic potency for characterizing SPN.Methods From November 2004 to May 2014,164 patients with SPN who underwent 18F-FDG PET/CT scan were retrospectively analyzed.The patients'clinical factors (age,gender,history of smoking and history of malignancy),information on CT (diameter,location and spiculated edge of the lesion) and metabolic information on PET imaging were collected to establish a diagnostic model by using the binary logistic regression.Then,the optimal operating point (OOP)of the established model was set.The diagnostic potencies of the established model and PET were assessed by ROC curve.Results Malignancy was diagnosed in 104 of 164 SPN patients.The rest 60 patients had benign diseases.The factors of age,spiculation(0:no spiculation,1:obvious spiculation) and metabolic information(0:≤ mediastinal blood pool,1:>mediastinal blood pool) were demonstrated to be useful for the establishment of the model (x2 =5.486,16.240,33.855,all P<0.05).However,the factors of gender,history of smoking,the diameter and location of lesions showed no influence for the model (x2 =2.452,0.453,0.127,0.390,all P>0.05) and rejected from the model established.The history of malignancy was excluded from statistical analysis because there were only 2 patients with history of malignancy.The established model was as follows:P=1/(1+e-Z),z=-5.512+0.061xage+2.208xspiculation+3.767×metabolic increase.The ROC AUC of the established model and PET using two-point scoring scale (TPSS) for charactering SPN were 0.92(95% CI:0.87-0.96)and 0.80(95% CI:0.73-0.86).The model had higher diagnostic efficacy compared with TPSS (z=4.369,P<0.05).When P=0.796 7 was set as an OOP,the diagnostic sensitivities of the model and PET for charactering SPN were 91.3% (95/104) and 94.2% (98/104) respectively,and no significant difference was found between them (x2 =0.800,P>0.05).However,significant difference was found between the diagnostic specificities of them (80.0% (48/60) vs 65.0% (39/60);x2 =7.111,P<0.05).Conclusions A new diagnostic model for characterizing SPN based on the information from 18FFDG PET,thin-section CT and clinical data is successfully established.Its sensitivity for diagnosis of lung cancer is high,and its specificity is superior to PET using with TPSS.This model has a potential value for clinical application.
6.Study on Rapid Elemental Analysis of Tibetan Medicine “GTso Thal” by LIBS Technique

Laser-induced breakdown spectroscopy (LIBS) was applied to perform qualitative research on Tibetan medicine “GTso Thal” in order to establish a rapid element analysis method . The Nd: YAG laser with the funda-mental frequency at 1064 nm was used. A high-power laser beam was focused on the surface of the sample. The spectrometer and CCD was used to detect its spectrum signals. Based on the National Institute of Standards and Technology (NIST) database, emission spectrum characteristics were differentiated. The LIBS spectra of “GTso Thal”showed multi-elements including Hg, Ca, Na, As, Fe, Mg, Al, K, Li, Pb, Ag, Au and S. Among them, Hg, Pb, Ag and Au were heavy mental elements. The results demonstrated that LIBS was a viable technique for analysis of Ti-betan medicine “GTso Thal”. LIBS provided reliable elemental analysis on Tibetan medicine “GTso Thal”. The de-tection was real-time, rapid and in situ. It had prospects in the elemental analysis of ethnic medicine study. LIBS had broad application prospects.
7.Inhibitory Effect of Oleanolic Acid on Lipid Accumulation in Cytoplasm of 3T3-L1 Adipocyte

A collection of 57 natural compounds derived from Chinese herbal medicines were evaluated for their an-ti-adipogenic effects. The lipid droplets in differentiated 3T3-L1 adipocytes treated with tested substrate were de-tected by an image-based assay. The results demonstrated that oleanolic acid (OA) inhibited lipid droplets accumula-tion in 3T3-L1 adipocytes in a dose-dependent manner with the IC50 value of 14.5 μmol·L-1. In addition, ToxInsight assay showed that OA was free of liver injury for HepG2 cells within 60 μmol·L-1. It was concluded that OA, which had an obvious anti-adipogenic effect, can be a candidate for hyperlipidemia therapy.
8.Study on Drug Pair of Cassia Twig and White Peony Root by HPLC/MS Analysis Based on Theory of“Xin-Gan Hua-Yang and Suan-Gan Hua-Yin”

This article was aimed to study the different clinical characteristics using drug pair of Cassia twig and white peony root with the contents ratio of 1:1 and 1:2. Based on the different clinical treatment of drug pair of Cas-sia twig and white peony root, different compositional ingredients in ratio of 1:1 and 1:2 were illuminated by HPLC/MS method. The drug pair of Cassia twig and white peony roots in ratio of 1:1 and 1:2 and single herbs were ex-tracted for HPLC/MS analysis. A protocol was followed, including acetonitrile - 0.1% acetic acid with gradient elution, positive mode, 350℃ capillary temperature and 300℃ vaporization temperature. The results showed that Procyanidol B2 and 2-Hydroxy cinnamal dehyde can be extracted from single Cassia twig, but 2-Hydroxy cinna-mal dehyde cannot be detected in drug pair. It showed the contents of Procyanidol B2 in 1:1 ratio was more than 1:2 ratio. Simultaneously, Palbinone, paeoniflorin sulfonate, 1,2,3,6-Tetra-O-galloyl-β-D-glucose, Paeoniflorin, Pae-oniflorin isomers, Benzoylpaeo-niflorin, and Benzoyl Paeoniflorin isomers can also be dissolved in white peony root. In addition, the contents of 1,2,3,6-Tetra-O-galloyl-β-D-glucose, Paeoniflorin, Benzoylpaeo-niflorin, and Benzoyl Paeoniflorin isomers in 1:1 were more than 1:2. The contents of Palbinone, paeoniflorin sulfonate and Paeoniflorin isomers in 1:2 were more than 1:1. It was concluded that Procyanidol B2, 1,2,3,6-Tetra-O-galloyl-β-D-glucose, Paeoniflorin, Benzoylpaeo-niflorin and Benzoyl Paeoniflorin isomers in 1:1 were more than 1:2. The contents of Pal-binone, Paeoniflorin sulfonate and Paeoniflorin isomers in 1:2 were more than 1:1. It provided a scientific basis for traditional Chinese medicine treatment using rational drug pair.
9.Optimization of solubilizing process of Qingkailing injection based on GA-BPNN modeling.

OBJECTIVETo optimize solubilizing process of Qingkailing injection.
METHODUniform design experimental method and GA-BPNN modeling method were applied to optimize the technical parameters, and the solubility of cholic acid in medium was taken as optimizing index.
RESULTThe type of solubilizer and pH of medium had greater impact on the index than the concentration for solubilizer. The optimum solubilizer was Tween-80. In a certain range the higher pH of medium was optimum; it was not necessary to use high-concentration solubilizer although there was positive correlation between the index and the concentration of solubilizer.
CONCLUSIONUniform design combined with GA-BPNN modeling method is suitable for optimization of solubilizing process of Chinese materia medica injection, which has certain practical application value.

10.Imaging observation of cerebral ischemia reperfusion injury after interventional therapy in acute middle cerebral artery occlusion

Objective To investigate the imaging changeof cerebral ischemireperfusion injury (CIRI) afteinterventional therapy in acute middle cerebral artery occlusion .Method32 patientwith acute middle cerebral artery occlusion in ouhospital from January 2013 to Novembe2014 were selected .16 casewere performed the recanalization therapy aftearterial thrombolysiand/omechanical thrombectomy(recanalization group) and 16 casewere notreated by thrombolytitherapy (non-recanalization group) .The differenceof brain imaging changes(onse,on 3 ,7 d afteonset) were analyzed and compared between the two group.
