1.Rationality Analysis on Traditional Chinese Medicine Injection for Activating Blood Circulation Usage in Our Hospital

Objective To evaluate the usage situation and the trend of traditional Chinese medicine injection for activating blood circulation in our hospital. Methods The sales amount and drug use frequency (DDDs) and drug utilization index (DUI) of traditional Chinese medicine injection for activating blood circulation were sorted and analyzed in our hospital from the fourth quarter of 2011 to the third quarter of 2012. Results The total sales amount of traditional Chinese medicine injection for activating blood circulation gradually increased in our hospital during that time. The proportion of varieties with the ratio DUI sum/DDDs sort of closed to 1 (0.75≤order ratio≤1.25) in the three quarters of 2012 accounted for 53.33%, 53.33%, and 66.67%, respectively. This indicated that the usage of traditional Chinese medicine injection for activating blood circulation was more unreasonable, but tended to be reasonable. Conclusion The usage of traditional Chinese medicine injection for activating blood circulation was basically rational in our hospital, and efficacy of clinical pharmacist intervention was obvious. Effective and safe evaluation of traditional Chinese medicine injection can be further strengthened, in order to improve the efficacy and reduce the adverse reaction.
2.Kinetics of LBP/CD14 system and its significance in patients vnth bemodialysis

To explore the kinetics of LBP/CD14 system in patients with hemodialysis, and further to analyse its role in (he development of systemic inflammation response based on the changes of plasma endotaxin, TNF and IL-6 levels in patients with uremia treated with hemodilysis. Methods Sixteen patients with end-stage renal failure (8 cases with hemodialysis, 8 cses without hemodialysis) were selected for this study. limulus amebocyte lysate chromogenic assay, ELISA and cell in situ hybridization were used to determine the changes in endotoxin, LBP, TNF and IL-6 levels in plasma, and expression of CD14 mRNA in the monocytes. Results (l)Plasma LBP levels in patients with hemodialysis were significantly higher than those in patients without hemodialysis. The expression of CD14 mRNA in the monocytes in patients with hemodialysis was also more obvious. Both of them incresed much more in hemodialysis. (2)Plasma endotoxin levels in patients with hemodialysis, though being significantly higher than in patients without hemodialysis, were in low-level (61.7 ? 10.6 pg/ml). (3) Plasma TNF and IL-6 levels were markedly increased in patients with hemodialysis, which were significantly correlated with plasma LBP levels. Conclusion LBP/CD14 system in patients with hemodialysis is markedly up-regulated, which might be the important mechanism for low-level endotoxemia to exert its effects in hemodialysis.
3.Evaluation of myocardial perfusion abnormalities in patients with hypertrophic cardiomyopathy by myocardial contrast echocardiography

Objective To evaluate the microcirculation abnormalities in the patients with hypertrophic cardiomyopathy(HCM). Methods Twenty patients with HCM and 20 healthy volunteers were included in the study. Two-dimensional images on the apical four-chamber, two-chamber and long-axis views, including 5 cardiac cycles before 'flash' and 15 cardiac cycles after 'flash', were reeorded and stored for off-line analysis in EchoPAC workstation. Then,contrast time-intensity curves were obtained for each available left ventrieular segment. Results There were significant differences in the A,k and the product A×k between the hypertrophic wall segments (P = 0.01, P<0.001, P = 0.036,respectively) and nonhypertrophic wall segments (P<0.001,respectively) and healthy subjects. A,k and the product A×k in hypertrophic wall segments were significantly different from those in nonhypertrophic wall segments in patients with HCM (P = 0.021, P = 0.016, P = 0. 001,respectively). Accordingly,the normalized value of A and the product A × k were lower in hypertrophied wall segments (P = 0.031, P<0.001, respectively) and nonhypertrophied wall segments(P = 0.002, P<0.001,respectively) in patients with HCM than those in normal segments of healthy controls. The normalized value of A and the product A × k were lower in hypertrophic wall segments than nonhypertrophic wall segments in patients with HCM(P = 0.045, P = 0.021,respectively). Conclusions Myocardial contrast echocardiography is a useful method to investigate myocardial perfusion abnormalities in patients with HCM.
4.Evaluation of the relationship between carotid plaque instability and coronary heart disease by contrast-enhanced ultrasound

Objective To evaluate the relationship between carotid plaque instability and coronary heart disease by contrast-enhanced ultrasound. Methods Thirty-five patients with acute coronary syndrome (ACS) and 32 patients with stable coronary artery disease(sCAD) were included. Inclusion criteria were at least 1 carotid atherosclerotic plaque with thickness larger than 2.0 mm. Contrast-agent enhancement in the plaque was evaluated by visual interpretation and quantitative analysis. Results The percentage of soft plaque in ACS group was significantly higher than that in sCAD group ( P <0.001 ). The proportion of contrast-agent enhancement in patients with ACS was significantly than that in patients with sCAD( P =0. 037). The enhanced intensity in the plaque and the ratio of enhanced intensity in the plaque to that in the carotid artery lumen in patients with ACS were significantly larger than those in patients with sCAD ( P <0.001, P = 0.026, respectively). Sensitivity and specificity of prediction ACS were 74% and 60%,respectively,for enhanced intensity in the plaque and 86% and 67%, respectively, for ratio of enhanced intensity in the plaque to that in the lumen of the carotid artery. Conclusions The subjects with ACS had more intense contrast-agent enhancement than the subjects with sCAD. Contrast-enhanced ultrasound can be used to evaluate the relationship between carotid plaque instability and coronary heart disease.
5.The Influence of Naikan Therapy on the Family Function in Patients with Anxiety Disorder of Cerebral Infarction

Objective To study the influence of Naikan therapy on the family function in patients with anxiety disorder of cerebral infarction.Methods A total of 192 patients with anxiety disorder of cerebral infarction were divided into control group and study group according to the set sequence.The study group was treated with Naikan therapy and the control group received routine therapy.All patients were assessed by family assessment device (FAD),Hamilton depression scale (HAMD),and Hamilton anxiety scale (HAMA).Results After 6-week treatment,the HAMD,HAMA,the affective reaction and behavior control scores in the study group were significantly lower than those before the treatment (t =2.24,2.43,2.37,2.35,P < 0.05).The affective reaction and behavior control in the study group were obviously lower than that of the control group (t =2.51,2.41,P < 0.05).Conclusion Naikan therapy can improve family function in patients with anxiety disorder of cerebral infarction.
6.The Influence of Naikan Therapyon on Depression in Patients with Schizophrenia

Objective To study the influence of Naikan therapy on depression in patients with Schizophrenia.Methods 256 patients with Schizophrenia were divided into study group and control group according to the set sequence.The study group was simulated by Naikan therapy and the control group was given routine cure.All patients were assessed by Positive and negative symptom scale (PANSS),Simple coping style questionnaire (SCSQ),Defense way questionnaire (DSQ),Hamilton depression scale (HAMD),and Hamilton anxiety scale (HAMA).Restlts In the 6th week,the differences in HAMD score,Cover factor,negative coping were statistically significant between the two groups (t=2.37,2.33,2.36,P<0.05).The differencs in HAMA score,Cover factor,negative coping were statistically significant between the 0th and the 6 th week in the study groups (t =2.31,2.42,2.47,P< 0.05).The difference of PANSS score were statistically significant between the 0th and 6th week of the two groups.Concltsion The Naikan therapy can improve the mental symptoms and depressive symptoms in patients with Schizophrenia.
7.Comparison of acute toxicity of two preparations of norcantharidin in mice

OBJECTIVE: To compare the acute toxicity of common injection and sustained-release preparation of norcantharidin for mice so as to identify the attenuation of the sustained-release preparation of norcantharidin. METHODS: The poloxamer 407 was used as a sustained-release vehicle for topical administration of norcantharidin, and the acute toxicity of mice treated with common injection and sustained-release preparation of norcantharidin was observed. The median lethal dose (LD(50)) was calculated by Bliss software. RESULTS: The symptoms of mice were similar between the two groups, but the appearance of symptoms in norcantharidin/poloxamer 407 group was 4 hours later than that in norcantharidin group. The LD(50) of norcantharidin administered through vein injection was 12.6 mg/kg. The LD(50) of norcantharidin/poloxamer 407 administered through intraperitoneal injection, intrahepatic injection and intramuscular injection were 19.9, 19.1 and 20.9 mg/kg, respectively, and the LD(50) of the common preparation were 13.0, 13.1 and 15.1 mg/kg, respectively. CONCLUSION: The norcantharidin/poloxamer 407 is less toxic than the equivalent dose of norcantharidin, mainly because norcantharidin/poloxamer 407 may release norcantharidin sustainedly, thus reducing norcantharidin concentration in blood.
8.Evaluation of aortic valve function in patients with bicuspid aortic valve with echocardiography

Objective To evaluate the application of echocardiography in the diagnosis of bicuspid aortic valve and its pathophysiologic development and to demonstrate the degree of aortic valve dysfunction. Methods The echocardiographic characteristics and clinical data were analyzed retrospectively in 135 patients with bicuspid aortic valve to demonstrate the variation of aortic valve dysfunction in the progression of the course of the disease. Results Isolated aortic regurgitation was present in 68 (50%) patients, 19 (14%)patients had isolated aortic stenosis and 29 (22%)patients had combined aortic stenosis and regurgitation, 19 (14%) patients had neither stenosis nor regurgitation. Aortic regurgitation were all more commen within each age group, aortic regurgitation were also more commen within each age group in patients in their advanced course who went to surgical treatment. Conclusions Echocardiography is an effective method in the early diagnosis of aortic valve dysfunction.
9.Etiological identification of 50 isolates from fungal corneal ulceration by semi-nested PCR amplification of ITS2 region

Objective To identify the pathogens of 50 cases of fungal corneal ulceration by using semi-nested PCR amplification of ITS2 region.Methods Fifty isolates of fungal corneal ulceration and 3standard fungal strains cultures were collected and their DNAs were extracted.Their ITS2 regions were amplified by semi-nested PCR and sequenced.The results were compared with the nucleotide sequences in the NCBI GenBank.The pathogens of the fungi were identified and their distribution were analysed.Results The sequences results of the 3 standard fungal strains were consistent with the information in the GenBank.The pathological microorganisms of 50 cases of fungal corneal ulceration were:24 Fusarium (48%),including 17 Fusarium solani,6 Fusarium oxysporum and 1 Fusarium verticillioide; 10 Aspergillius (20%),including 5 Aspergillius flavus,3 Aspergillius sydowii and 2 Aspergillius nidulans; 6 Penicillium (12%),including 2 Penicillium citreo-viride,2 Penicillium multicolor and 2 Penicillium oxalicum ;5 Candida (10%),including 3 Candida albicans and 2 Candida parapsilosis; 3 Cladosporium (6%),including 2 Cladosporium herbarum and 1 case of Cladosporium cladosporioides ; 1 case of Neurospora crassa (2%) ;1 Alternaria alternata(2%).Conclusion Semi-nested PCR amplification of ITS2 region was proved to be a fast,simple and accurate method to identify pathogens of fungal corneal ulceration,and may be useful for personalized treatment and epidemiological investigation of local fungi.
