2.Study on the number alteration of endothelial progenitor cells from peripheral blood in patients with acute myocardial infarction of young man

Objective To compare the number of endothelial progenitor cells (EPCs) from peripheral blood in patients with acute myocardial infarction of young man and healthy man.Methods Eighteen young men (18 ~50 years old) with acute myocardial infarction (AMI) who were admitted at the First Affiliated Hospital of Dalian Medical University from June 2010 to April 2011 in young man were enrolled,aged (65 ~ 85 years old) men with acute myocardial infarction in 18 cases,within 24 hours of onset collected blood 2 ml.Ten cases of healthy young men (30 ~50 years old) were used as control group,fasting venous blood 2 ml.A volume (400 μl) of blood was taken to red blood cell lysis buffer hemolysis labeled with vascular endothelial growth factor (VEGF),CD34,and CD133 antibodies,and then analyzed with flow cytometry.Results The number of EPCs in peripheral blood was measured in young male AMI group.The number of EPCs in peripheral blood was (0.58 ±0.83)% in older men.The number of EPCs in peripheral blood of AMI group was (0.04 ± 0.03) %.For healthy controls,the number of EPCs was (0.02 ± 0.02)%.The number of EPCs was significantly higher in AMI patients compared to control group (P < 0.05).However,for AMI group,the increased number of EPCs in young men was significantly greater than young female (P <0.01).Conclusions The number of EPCs in peripheral blood in young man AMI patients is significantly increased within 24 hours.

Objective:To investigate the changes of gastric tissue COX2 after intestinal perforations due to abdominal firearm wound.And to study its relationship with plasma endotoxin levels and pathological change of gastric tissue.Methods:A total of 42 Chang-Bai piglets were assigned randomly into 7 groups:control group and wounded 1,2,4,8,12,24 hours group.The model of intestinal perforations due to abdominal firearm wound was established in wounded groups.Gastric tissue COX2 activity was measured with immunohisto-chemical staining and image analysis in all groups.The plasma endotoxin levels were measured by chromogenic limulus amebocyte lysate test.The alterations of gastric tissue were observed under light microscope in all groups.Results:The expressions of COX2 of gastric tissue in wounded groups were significantly increased compared with control group (P

Objective To compare the muscular relaxation effect and side effects of Cisatracuri-um and Succinylcholine for children on tracheal foreign body removal.Methods Forty ASA Ⅰ pediat-ric patients,undergoing electivetracheal foreign body removal,were randomly divided into two groups:group C,received cisatracurium 0.2 mg/kg for induction,0.05 mg/kg for maintenance,n=20;group S,received succinycholine 2 mg/kg for induction,0.4 mg/kg for maintenance,n=20.All patients were performed with controlled ventilation assisted by high-frequency jet ventilation (HFJV).We recorded bronchoscope condition,muscle relaxant supplements,extubation time and the incidence of muscle soreness in postoperative 24 h.Results There was no significant difference in bronchoscope condition between these two groups.The extubation time were significantly longer in group C than in group S (P<0.05).The incidence of muscle soreness in postoperative 24 h were sig-nificantly higher after the operation in group S than that in group C (P<0.01).All patients in group S needed muscle relaxant supplements,but none in group C (P <0.01).Conclusion Both Cisatra-curium and succinylcholine can provide good muscle relaxation effect on tracheal foreign body remov-al,but cisatracurium prolong the extubation time,and succinylcholine increase the incidence of muscle soreness.
5.Correlation of apoptosis with androgen receptor in prostate cancer

Purpose To investigate relationship between androgen receptor and apoptotic index and to further understand the tumor biology of prostate cancer. Methods Fifty-six patients with histologically proven prostate cancer and 20 cases with benign prostatic hyperplasia(BPH) were collected. Androgen receptor(AR) were stained by 2H12 monoclonal antibody using immunohistochemical method. The apoptotic index (AI) was determined by the terminal deoxynucleotidyl transterase-mediated dUTP biotin nick end labeling (TUNEL) technique on serial sections of formalin fixed, paraffin embedded tissues. Results AR was not found significantly difference (P>0.05) between prostate cancer and BPH. The association of AR with AI according to Gleason score were not observed in prostate cancer. AI were significantly higher in prostate cancer compared to BPH (P<0.05).AI were also significantly higher in AR-positive prostate cancer than in AR-negative prostate cancer(P<0.05). Conclusion AR expression can induce prostate cancer cells to become apoptosis, and may be a useful predictor in functional classification and endocrine response of prostate cancer.

7.Anesthesia of seven sevoflurane combined with remifentanil for laparoscopic cholecystectomy

Objective To study the clinical effect of sevoflurane combined with remifentanil in laparoscopic cholecystectomy (LC).Methods50 cases of laparoscopic cholecystectomy in Tianjin Haihe Hospital from February 2015 to September 2016 were selected as the research object, and randomly divided into the control group and the experimental group, each group had a total of 25 cases.The patients in control group were treated with continuous intravenous infusion of propofol.The experimental group received sevoflurane.Patients in the experimental group and the control group were given remifentanil after surgery.Compared with the experimental group and control group of patients with the degree of recovery and related Linchuan indicators.ResultsAfter the corresponding anesthesia, the control group of patients with recovery time was (13.4±3.1) minutes, the score was (4.1±1.2) points, extubation time was (14.6±3.7) minutes.However, the extubation time and recovery time of the control group were significantly longer than those of the experimental group, and the recovery score was lower than that of the experimental group, with statistical difference (P<0.05).There was no obvious adverse reaction in the experimental group and the control group, and there was no significant difference in the changes of cardiovascular factors.ConclusionSevoflurane combined with remifentanil for laparoscopic cholecystectomy can improve the anesthetic effect to a great extent, stable operation and postoperative patients with cardiovascular function, recovery time is short, the clinical significance of only one step promotion and application.
8. Survival status and prognostic factors of leptomeningeal metastasis from lung adenocarcinoma

Objective: To investigate the survival status and prognostic factors of patients with leptomeningeal metastasis (LM) from lung adenocarcinoma. Methods: The survival rate and prognostic factors of 65 patients with LM from lung adenocarcinoma, who had complete follow-up data, were retrospectively analyzed. Results: The median survival time of the 65 patients was 7.4 months,and the 1-year survival rate was 6.2% (4/65). Univariate analysis demonstrated that gender, age, smoking history, timing of LM and whether in combination with brain metastasis had no significant correlations with overall survival (all P > 0.05); while the Eastern Cooperative Oncology Group (ECOG) performance status (PS) score, ventriculo-peritoneal (V-P) shunt, epidermal growth factor receptor-tyrosine kinase inhibitor (EGFR-TKI) targeted therapy, systemic chemotherapy (SC), whole-brain radiotherapy (WBRT), clinical signs of LM and EGFR gene status were associated with the overall survival (all P < 0.05). Multivariate analysis showed that EGFR gene status, ECOG PS score, SC and V-P shunt were independent prognostic factors of the prognosis of patients with LM from lung adenocarcinoma (all P < 0.05). Conclusion: The overall prognosis of patients with LM from lung adenocarcinoma is poor. The prognosis of patients with LM bearing EGFR mutation is relatively good. EGFR-TKI targeted therapy, SC and V-P shunt can prolong the survival time and improve the prognosis of patients with LM metastasis from lung adenocarcinoma.
9.Features of full field electroretinogram responses in children with early type 1 diabetes

Objective To observe the features of the full field electroretinogram (FF-ERG) in type 1 diabetes (T1D) children without diabetic retinopathy (DR).Methods Retrospective case study.Forty-one T1D children and 25 age-matched normal controls underwent a complete ophthalmic examination,including best-corrected visual acuity,refraction,intraocular pressure,slit lamp,fundus photography,indirect ophthalmoscopy,and spectral domain optical coherence tomography to exclude DR.All FF-ERG tests were performed by an experienced technician.The ERG series includes six protocols:dark-adapted 0.01 ERG (r-b 0.01);dark-adapted 3 ERG (mix-a 3.0,mix-b 3.0);dark-adapted 10 ERG (mix-a 10.0,mix-b 10.0);dark-adapted oscillatory potentials (OPS);light-adapted 3 ERG (c-a 3.0,c-b 3.0);light-adapted 30 Hz flicker (30 Hz FP) ERG.To compare the amplitudes and implicit times of the FF-ERG between the T1D and control group children.Results Compared with the control subjects,the FF-ERG amplitudes decreased and the implicit times increased in T1D.Except for r-b 0.01 (t =-0.228,P>0.05),the amplitudes of other FF-ERGs were all significantly attenuated (t =-1.664,-3.645,-4.324,-6.123,-5.846,-12.9,-14.4,-5.23;P<0.05) in T1D children.The implicit times of mix-b 3.0,mix-b 10.0,c-b 3.0 and OP2 significantly increased (t=5.242,2.879,5.378,3.506;P<0.05).The implicit times of r-b 0.01,mix-a 3.0,mix-a 10.0,c-a 3.0 and 30Hz FP changes were not significantly (t=2.331,1.677,0.557,0.84,0.064;P > 0.05).Conclusion The FF-ERG amplitudes decreased and implicit times increased in T1D children compared with the control normal subjects.
10.Clinical study of one-stage lymphatics-venous anastomosis to prevent upper extremity lymphedema of breast cancer after radical resection

Objective To investigate the value of one-stage lymphatics-venous anastomosis in radical mastectomy of breast cancer to prevent post-mastectomy upper limb lymphedema.Methods Ninety patients requiring radical mastectomy of breast cancer in Tangshan Tumor Hospital Affiliated to North China University of Science and Technology from March 2010 to May 2013 were collected as the objects.They were divided into the control group (45 cases) and the treatment group (45 cases) using block randomized grouping (concealment of allocation).Both groups underwent radical mastectomy of breast cancer, and the treatment group was treated with one-stage lymphatics-venous anastomosis on the basis of radical mastectomy.The operation times, amount of bleeding, hospitalization times, postoperative complications and the numbers of axillary lymph node dissection of the patients in the two groups were compared, and the postoperative upper limb lymphedema incidence rates of the patients in the two groups were compared.Results The operative times of the patients in the treatment group and the control group were (152.82 ± 18.76) min and (78.92 ± 10.33) min respectively, and amount of bleeding were (416.64 ± 94.65) ml and (250.84 ± 63.17) ml, with statistical significances (t =-20.39, P =0.00;t =-4.48, P =0.00).The average hospitalization times of the patients in the treatment group and the control group were (14.91 ± 5.44) d and (13.45 ± 2.36) d respectively, the numbers of axillary lymph node dissection were 14.63 ± 3.37 and 14.37 ± 3.18, the numbers of postoperative complications occurred were 9 cases (20.00%) and 5 cases (11.11%), with no statistical significances (t =-0.47, P =0.64;t =0.75, P =0.46;x2 =1.35, P =0.38).Compared with the control group, the treatment group has lower incidence of upper extremity lymphedema (13.95% vs.40.91%) and lower swelling degree, with statistical significance (x2 =8.48, P =0.03).Conclusion One-stage lymphatics-venous anastomosis in radical masteetomy of breast cancer can effectively transfer lymph diversion to the venous circulation and reduce the incidence of limb lymphedema, which has significant preventive effect.
