1.Application of fluid management under the guidance of stroke volume variation in patients undergoing supratentorial neoplasms surgery

Objective To evaluate the effects of arterial pressure continuous output (APCO) derived from stroke volume variation (SVV)-guided fluid management in the patients undergoing supratentorial neoplasms surgery.Methods Sixty-three patients (29 males, 34 females, aged 18-65 years, ASA physical status Ⅰ or Ⅱ) undergoing elective supratentorial neoplasma surgery were randomly divided into control group (group C, CVP-guided fluid management, n=30) and GDT group (group S, SVV-guided fluid management, n=33).Before the induction of general anesthesia, the hydmxyethyl starch Voluven (130/0.4) bolus 3 ml/kg in the two groups was administered followed by infusion of crystalloid at the rate of physical requirement.Hydroxyethyl starch or vasoactive agents were administrated to achieve the goal of CVP≥8 mm Hg or MAP>80% of baseline in group C andto reach the value of SVV≤12% and MAP>70% of baselinein group S.Intraoperativecrystal, intraoperative colloids,total fluid volume, bleeding volume, volume of blood transfusion and urine volume were recorded.The radial artery and venous blood was sampled for blood gas analysis, measurement of lactate concentration and laboratory parameters at 30 min before anesthesia induction (T0), the dura mater cutted (T1), end of operation (T2) and postoperative 24 h (T3).Postoperative complications and the number of patients with complications in postoperative period, the length of ICU stay and postoperative days were assessed.Results Total infused fluid volume [(1 478±312) ml vs (1 183±294) ml] and intraoperative colloids [(775±236) ml vs (487±243) ml] were significantly higher in group S than those in group C (P<0.05).Compared with T0, the lactate concentration were decreased significantly in two groups at T1 and T2.The lactate concentration in group S was significantly lower than group C at T2 [(0.91±0.25) mmol/L vs (1.31±0.46) mmol/L](P<0.05).There was no significant difference of postoperative complications, the length of ICU stay and postoperative days between two groups.Conclusion Fluid management guided by SVV during supratentorial neoplasms surgery reduces lactate levels.
2.Effect of different types of counterion on the electronic structure of an antitumor polyoxomolybdate Mo7O246-.

The electronic structure of a Lindqvist type [Mo7O24]6- anion with 5 different types of counterion (K+, Na+, NH4+, [NH3Pr]+ and [NH3Pri]+) was calculated by using the Discrete Variational Method coupled with Density Functional Theory (DFT-DVM). It could be concluded through variance analysis to the calculated results that the type of counterion does not influence remarkably the electronic structure of [Mo7O24]6- anions. Perhaps it can be used to explain the experiment fact that the polyoxomolybdate structure of the Mo7O24 framework is apparently of critical significance to antitumor action. On these grounds we forecast that two other lindqvist type heptamolybadates(their counterions are Na+ and [NH3Pr]+ respectively) may also exhibit antitumor activities.

3.Changes of A20 expression in mesangial cells of LPS-induced diabetic nephropathy rat model

Objective To observe the changes of A20 in mesangial cells of diabetic nephropathy (DN) rat model in?duced by lipopolysaccharide (LPS)-rat, and to explore its possible mechanism. Methods (1)Thirty health male Wistar rats were randomly divided into two group. Model rats were given streptozotocin (STZ) at a dose of 60 mg/kg by intraperitoneal in?jection. Rats in the control group received the same volume of citrate buffer in the same way. Levels of blood glucose and uri?nary microalbumin were detected in two groups at the 6th and the 8th week. Changes of renal pathology were observed by HE staining. Changes of protein A20 were observed by immunohistochemistry. (2) Expression changes of gene and proteins A20, nuclear factor (NF)-κB, IκB, IKKγand MCP-1 in renal cells treated with LPS were determined after treatment with different time points (0, 2, 4, 6, 12, 24, 48 and 72 h) and different concentrations (0.1, 1 and 10μg/L). Results (1) Levels of blood glucose and urinary microalbumin were significantly increased in model group compared with those of control group ( P <0.01). HE stainig showed that hyaline degeneration in tubular epithelial cells was found in model group, especially at the 8th week. Results of immunohistochemistry showed that expression of protein A20 significantly decreased in kidney tubules and nearly disappeared in glomerulus in model group compared with that of control group, which expressed less at the 8th week. (2) There was no significant difference in the expression of IKKγbetween different concentrations and different times. Com?pared with 0 h, the expression of A20 protein was increased at 2 h and 4 h, except that the expression of A20 protein in?creased after 6 h (P<0.05). Meanwhile NF-κB expression increased and IκB expression decreased in different time points (P<0.05). In addition, the expressions of A20 and IκB were decreased concentration-dependently (P<0.05). The expres?sion levels of NF-κB and MCP-1 were increased concentration-dependently (P<0.05). Conclusion A20 may involve in the development of diabetic nephropathy by regulating the NF-κB pathway.
5.Correlation between sleep quality and parasympathetic intensity at nighttime

Objective To clarify the effect of cardio pulmonary coupling (CPC) technology on sleep quality and the effect of Shichen-based physiological function index analysis on sleep health assessment.Methods CPC and Shichen-based physiological function index were used to evaluate the sleep quality of 242 volunteers from 9:00 pm to 9:00 am for 12 hours.Based on CPC technology score,volunteers wree seperated to three sleep quality groups (scoring 85-100 in 63 volunteers,scoring 60-84 in 80 volunteers,scoring < 60 in 99 volunteers).The results of sleep quality and parasympathetic intensity over the whole nighttime and the different time period of each sleep quality group were compared and the correlation analysis was done.Results The whole nighttime sleep quality test showed that there was significant difference (t =-12.754-19.030,all P < 0.01) in the time to the stable sleep ratio (52.78% ± 10.32%,43.99% ± 14.48,20.95% ± 10.41%),the disturbed sleep ratio (25.65% ± 7.81%,34.18% ± 12.71%,53.65% ± 14.81%),and the apnea-hypopnea index (2.79 ±2.95,7.93 ± 6.61,23.94 ± 16.09)in the different sleep quality groups.The Shichen-based sleep quality test showed that there was significant difference among the stable sleep ratio,the disturbed sleep ratio during the 23:00 pm to 5:00 am period in the different sleep quality groups.There existed significant difference with parasympathetic intensity in the different sleep quality groups.There was positive correlation between the parasympathetic intensity and the stable sleep ratio in the same sleep quality group (r =0.508,P < 0.05).Conclusions CPC evaluation can commendably distinguish and evaluate different sleep quality groups on the whole night sleep.The Shichen-based physiological function index can not only distinguish the sleep quality,but also evaluate the multiple physiological function,the parasympathetic intensity between different sleep quality groups during different sleep periods.Thus they can be widely used to assess the sleep quality.
