1.The expression and clinical significance of vascular endothelial growth factor-C and nm23-H1 in colorectal carcinoma

Objective To discuss the expression and clinical significance of vascular endothelial growth factor (VEGF)-C and nm23-H1 in colorectal carcinoma.Methods Immunohistochemical staining was employed to determine the expression of VEGF-C and nm23-H1 in the carcinoma tissues of 120 cases with colorectal carcinoma and in the adjacent mucosal tissues of 55 cases as control,and analyzed their correlation with cliniopathological features and prognosis.Results The positive expression of VEGF-C in the carcinoma tissues was 71.7% (86/120),significantly higher than that in the adjacent mucosal tissues [21.8 % (12/55)],and there was significant difference (x2 =35.186,P < 0.01).The positive expression of nm23-H1 in the carcinoma tissues was 57.5% (69/120),significantly lower than that in the adjacent mucosal tissues [90.9% (50/55)],and there was significant difference (x2 =18.351,P < 0.01).The expression of VEGF-C was significantly correlated with lymph node metastasis (P < 0.05),and the expression of nm23-H 1was significantly correlated with lymph node metastasis and pathological type (P <0.01 or <0.05).The expression of VEGF-C and nm23-H1 did not show a significant correlation with gender,age,tumor size,tumor site and depth of invasion (P > 0.05).Spearman correlation analysis showed that the expression of VEGF-C was negatively correlated with the expression of nm23-H 1 in colorectal carcinoma (r =-0.472,P <0.01).Conclusions The joint detection of VEGF-C and nm23-H1 expression is very promising in prediction of the prognosis of patients with colorectal carcinoma.However,whether it can be used as a marker in prognosis judgment need further investigation.
2.Efficacy of 5 Kinds of Interventions in the Prevention of Chronic Neurotoxicity Induced by Oxaliplatin:a Net-work Meta-analysis

OBJECTIVE:To systematically evaluate the efficacy of mecobalamin,ganglioside,mixture of calcium and magne-sium,reduced glutathione,mixture of calcium and magnesium combined with reduced glutathione in the prevention of chronic neu-rotoxicity induced by oxaliplatin, and to provide evidence-based reference for clinic treatment. METHODS:Retrieved from PubMed,clinicltrials.gov and CJFD,randomized controlled trials (RCT) about efficacy of 5 interventions in the prevention of chronic neurotoxicity induced by oxaliplatin were collected,and network Meta-analysis were performed by using ADDIS 1.16.5 af-ter extracting data and evaluating quality. RESULTS:A total of 34 studies were included,involving 3 236 patients. 4 studies were three-arm studies,30 studies were two-arm studies,totally 42 arms. Compared with placebo,the incidence of CIPN by 5 interven-tions was significantly reduced. Mixture of calcium and magnesium combined with reduced glutathione was the most effective and the followed by mecobalamin,reduced glutathione,ganglioside and mixture of calcium and magnesium,however,there were no significant differences among 5 interventions(P>0.05). CONCLUSIONS:5 kinds of interventions are all effective in the preven-tion of chronic neurotoxicity induced by oxaliplatin. Mixture of calcium and magnesium combined with reduced glutathione is the most effective. Duo to the low quality of included studies,large-scale and high quality RCTs are required for further validation of the conclusions.
3.Analysis and Evaluation of Personalized Medication Evidence Based on Pharmacogenetics

OBJECTIVE:To evaluate evidence foundation of phamracogenetics personalized medication,and to provide refer-ence for clinical application. METHODS:Using“phamracogenetics”“pharmacogenomics”and“gene polymorphism”as key words,related literatures and clinical guideline were retrieved from PubMed,CNKI,Wanfang database,and analyzed in respects of involved gene,site and drug types,etc. Evidences of package inserts of phamracogenetics biomarker were evaluated by using phamracogenetics practice and prevention evaluation guideline. RESULTS:8 276 papers,25 guidelines and 166 drug package in-serts are available for analysis. The phamracogenetics literatures mostly focus on the relationship between some one gene and differ-ent drugs. In guidelines,some one specific gene can guide clinical application of multiple drugs in different fields. In drug package inserts,general level of clinical evidence is not high;detectable biomarkers is inadequate in category,and detection rate is only 38.06% besides targeting preparation. CONCLUSIONS:Under the condition of low clinical evidence level the detection of pharma-cogenetics biomarker should be conducted carefully,and basic study should be further strengthened.
4.Efficacy observation on prevention of constipation after stroke through abdominal massage and navel therapy with fructus evodiae

Objective To study the efficacy on prevention of constipation after stroke through abdominal massage and navel therapy with fructus evodiae.Methods 120 patients of stroke with yin-type syndrome were divided into the observation group and the control group with 60 cases in each group.The control group was given routine nursing,the observation group was given abdominal massage and naval therapy with fructus evodiae based upon routine therapy.Comparisons were made about incidence rate of constipation after stroke and applications of non-retention enema with normal saline.Results There were statistical significances of incidence rate of constipation after stroke and applications of non-retention enema with normal saline.Conclusions Abdominal massage and navel therapy with fructus evodiae are good to constipation after stroke.
5.Influenza B virus monitoring and analysis of variation and evolution of hemagglutinin (HA) and neuraminidase (NA) genes in Jining city from 2017 to 2020

Objective:To analyze the variation and evolution characteristics of hemagglutinin (HA) and neuraminidase (NA) genes of influenza B virus circulating in Jining from 2017 to 2020.Methods:Throat swab specimens were collected from patients with influenza-like symptoms in sentinel hospitals and influenza outbreaks in Jining from 2017 to 2020 and tested for influenza B virus nucleic acid. After virus isolation, 20 representative strains of influenza B virus were selected to sequence the full length of HA and NA genes. Phylogenetic trees were constructed and the molecular characteristics were analyzed using bioinformatics software. Results:A total of 4 575 specimens were collected and 842 of them were positive for influenza virus, including 398 (8.7%, 398/4 575) influenza B virus-positive specimens. The positive rate of influenza B virus was 47.27% (398/842). The isolated influenza B virus strains of Victoria (BV) and Yamagata (BY) lineages from 2017 to 2020 shared 98.7%-98.8% and 98.5%-99.1% homology in HA gene with vaccine strains, respectively. The BV lineage strains isolated from 2018 to 2020 belonged to Victoria clade 1A branch and the BY lineage strains isolated from 2017 to 2018 belonged to Yamagata clade 3 branch. Mutations were detected in several antigenic sites, but not in the sites related to NA inhibitor resistance. Conclusions:Mutations in several antigenic sites caused antigenic changes in influenza B virus of BV and BY lineages, which might be related to the outbreaks of influenza B virus infection in Jining during 2017 to 2020.
6.Analysis on salary satisfaction of medical staff in the county-level public hospitals of Hei-longjiang province

Objective:To measure the salary satisfaction level of medical staff in county-level public hospitals and provide reference for the design of policy reforms. Method:Eight county-level public hospitals were investigated. All analyses were performed using SPSS 19. 0 via statistical description and binary logistic regression. Results: The total salary satisfaction of the medical staff was 48 . 8%, and the average score was 54 . 05 ± 18 . 04 . The welfare satis-faction is the lowest (38. 8%), and the average score is 7. 98 ± 3. 51. Logistic regression analysis shows that the ac-ademic degree, department category, working hours per week, acceptance of bribes, and officially budgeted posts are the main factors affecting overall compensation satisfaction. Conclusion:The overall salary satisfaction level of medi-cal staff in county-level public hospitals is very low, and the welfare satisfaction is the lowest. Establishing the rea-sonable level of compensation, the rise of compensation, and the welfare mechanism are feasible choices to improve salary satisfaction.
7.Validity and reliability of the Chinese version of the Kutcher Adolescent Depression Scale

Objective:To assess the validity and reliability of the Chinese version of the eleven-item Kutcher Adolescent Depression Scale (KADS-11)in Chinese adolescents,calculate its optimal cut-off value and the sensi-tivity and specificity,and explore the possibility of providing a useful tool to assess the severity of adolescent de-pressive symptoms.Methods:Totally 3180 students aged 11 -17 years were selected from schools in 6 provinces and Shanghai.All of them were asked to complete the KADS-11 and Children Depression Inventory (CDI). Students whose CDI scores were above 19 (including 19)were diagnosed with the DSM-IV criteria of depressive disorder,73 students from Shanghai sample were assessed with KADS-11 and CDI to analyze the test-retest reliabil-ity 1 month later.Results:Exploratory factor analysis showed that KADS-11 had 2 factors,and confirmatory factor analysis tested proved the 2-factor model fit better than the one-factor model.The KADS-11 total scores were posi-tively correlated with CDI total scores (r =0.74,P <0.01 ),and the KADS-11 scores were higher in depressive group than those in non-depressive group.The mean area under the curve (AUC)of KADS-11was 0.94,the mean area under the curve of each item ranged from 0.7 to 0.9.The optimal cut-off point of KADS-11 was total score≥9,sensitivity and specificity were 89% and 90% respectively.The Cronbach's alpha coefficient of the KADS-11 was 0.84,the spilt-half reliability coefficient was 0.71 (P <0.01),and the test-retest coefficient was 0.77 (P <0.01).Conclusion:The KADS-11 is appropriate for Chinese adolescents because of its good validity,reliability and diagnosis accuracy,it could be used to assess depressive symptoms for adolescents.
8.Influence of class III magnetic appliances on the craniofacial morphology and histology in Rhesus monkeys.

OBJECTIVETo investigate the influence of class III forces in craniofacial morphology and sutural histology in Rhesus monkeys.
METHODSSix male Rhesus monkeys in mixed dentition were employed in this study, two as 90-day experimental group, two as 45-day experimental group and the other two as control respectively. The experimental monkeys were casted magnetic orthopedic appliance-III (MOA-III), the control monkeys received no treatment. Intraoral photographs, casts and cephalometric radiographs of each animal were obtained at the beginning and at the end of the experiment. Fluorescent and haematoxylin-eosin staining were used to observe the histological changes in craniofacial sutures. RESULTS; In experimental groups, the maxillomandibular relationship changed more significantly in 90-day experimental group than 45-day experimental animals. But changes were scarcely noted in the control. The bony depositions of the sutures were distinctly found during the experimental period, especially in zygomaticomaxillary, transverse palatine, pterygopalatine and zygomaticotemporal sutures.
CONCLUSIONThis study indicates that this kind of magnetic appliance could cause skeletal and dental changes of maxilla and mandible, and change the arrangement of connective tissues in craniofacial sutures, and lead to significant new bone deposition in craniofacial sutures.

9.Clinical and genetic characteristics of congenital hypogonadotropic hypogonadism in boys

Objective:To analyze the clinical and genetic characteristics of congenital hypogonadotropic hypogonadism (CHH) in boys.Methods:Cross-sectional study.Clinical data, laboratory data and genetic results of boys who were genetically diagnosed with CHH at the Department of Endocrinology of Shenzhen Children′s Hospital from December 2019 to February 2023 were collected in this retrospective study.Their clinical manifestations, hormone levels and gene mutations were analyzed.The non-normal distribution was represented by the median.The rank sum test was used to compare the non-normal distribution data between the two groups.Results:A total of 27 boys were genetically diagnosed with CHH, with the age at first diagnosis ranging from 0.3 to 16.6 years old.All these children presented with micropenis (100%), of whom 16 were complicated with cryptorchidism (59.3%), 9 with microrchidia (33.3%), 7 with simple micropenis (25.9%), and no had simple cryptorchidism.Three children had cardiovascular dysplasia.The median of basal luteinizing hormone(LH) level was 0.09 IU/L, and 92.5%(25/27) of children had the basal LH level below 1.00 IU/L.The median of peak LH level after gonadotropin-releasing hormone(GnRH) stimulation was 1.42 IU/L, and 96.2%(26/27) of children had the peak LH level below 4.00 IU/L.The median of serum inhibin B was 41.15 μg/L, and the median of serum anti-Müllerian hormone(AMH) was 12.62 mg/L.The serum AMH level of children with cryptorchidism was significantly lower than that of children without cryptorchidism (10.02 mg/L vs.50.50 mg/L, P<0.05). A total of 12 gene mutations were detected in the 27 children, of which 1 was biallelic mutation.The most common gene mutations were in CHD7 and ANOS1 genes (7 children each, both accounting for 51.8%), followed by FGFR1 gene (3 children, 11.1%). After short-term treatment by GnRH pump or subcutaneous injection of recombinant human follicle stimulating hormone in 4 children, the levels of serum inhibin B and AMH increased significantly, and the testicular volume also increased. Conclusions:CHH is a congenital disease with different clinical manifestations at different ages.The main manifestations in childhood are micropenis and cryptorchidism, and some children have microrchidia.Its diagnosis in prepuberty is difficult, but genetic testing is of great significance for early diagnosis.
10.Case report of fetal alcohol syndrome

The clinical characteristics of a case of fetal alcohol syndrome (FAS) diagnosed by Shenzhen Children′s Hospital were summarized.The patient was 6 years and 4 months old, and admitted to the hospital because of her " slow growth of height for more than 6 years" . There was a history of alcohol exposure in the fetus.The infant was born with low body mass, and grew slowly in height and body mass after birth.She was diagnosed with FAS due to typical facial features of FAS, microcephalia, poor memory and narrative ability.The effect of alcohol exposure during pregnancy on fetus is permanent, and abstinence is the only way to prevent FAS.In this paper, the clinical characteristics of FAS were summarized and the literature was reviewed in order to improve the clinical understanding of the disease.
