Objective To study the immunosuppression mechanism induced by ultraviolet (UV) and cis urocanic acid. Method The auto lymphocyte proliferation test with Langerhans′ cell (LC) in guinea-pig was performed. Results In exposure to low dose of UVB (25 J/m2) radiation, the inhibition rate of lymphocyte proliferation stimulated by LC was 10.5％ , the inhibition rates of UVB radiation in doses of 50 J/m2 and 100 J/m2 were 22.4％ or 50％ , respectively. The lymphocyte proliferation was almost completely suppressed by 200 J/m2 UVB radiation, while the inhibition of LC function by cis urocanic acid was weak. Conclusion UVB significantly inhibits LC auto-stimulation in dose-dependent way, which may play an important role in UVB induced immunosuppression.

Objective:To investigate the effect of of simvastatin on bone formation and bone mineral density(BMD) during the retention after orthodontic tooth movement. Methods:Orthodontic tooth movement of upper first molar was performed in 40 rats with coil spring for 21 days. 40 rats were randomly allocated into 5 groups: basic control group,negative control group and 3 simvastatin groups(2.5 mg?kg-1, 5.0 mg?kg-1 and 10.0 mg?kg-1respectively).Rats in basic control group were killed when appliances were removed after 21 days.The experimental groups were administered simvastatin daily from 1 day before appliances removed for 4 weeks.The negative control group received the isotonic saline as control.4 weeks later all animals were anesthetized and killed. Objective To investigate the expression and significance of Toll-like receptors-4 in peripheral blood and placental tissue in gestational diabetes mellitus (GDM) patients.Methods From February 2013 to February 2015.a total of 30 cases of gestational diabetes mellitus patients(GDM group) and 30 cases of normal pregnant people(health group)were selected as research objects.Peripheral blood and placental tissue of two groups were collected.The expression of TLR4 mRNA in peripheral blood mononuclear cells was detected by RT-PCR,the expression of TLR4 protein in placenta tissue was detected by immunohistochemistry.Results The expression of TLR4 mRNA in peripheral blood mononuclear cells of the observation group was significantly higher than that of the health group[(0.63±0.12) vs.(0.32±0.07)],the difference was statistically significant(t=12.223,P<0.05).The expression of TLR4 in villous trophoblast cells,decidual cells and amniotic epithelial cells in GDM group was significantly higher than that in the health group(Z=2.325,2.374,2.162,P<0.05).Conclusion The expression of TLR4 in peripheral blood mononuclear cells,villous trophoblast cells and decidual cells in gestational diabetes mellitus patients significantly enhanced,suggesting that TLR4 might be related to the e gestational diabetes mellitus.

To evaluate the efficacy and safety of resection and cryotherapy combined with amniotic membrane transplantation (AMT) for the treatment of vernal keratoconjunctivitis (VKC) with giant papillae (GP). Eight patients (16 eyes involved) with VKC, characterized by GP on the upper tarsal conjunctiva, underwent resection and cryotherapy in combination with AMT. The follow-up lasted for 3-22 months. The results showed that corneal shield ulcers and superficial punctuate keratitis healed during the first week after surgery and did not recur. Fourteen eyes (87.5 %) were symptom-free 1 month after surgery, and no GP, ectropion, trichiasis and other complications were noted, but the blood vessels of upper tarsal conjunctiva could not be clearly seen and a little conjunctival scar was observed. Recurrence of GP was observed in 2 eyes (12.5 %), with the area being less and irritation milder as compared with those before the operation. Among the two eyes, one eye was treated by cyclosporine eyedrops with improvement, but the other eye showed no improvement after the treatment, and underwent a second surgery with a cotton patch soaked in fluorouracil applied onto the supratarsal area after resection and cryotherapy. Four months after the treatment the patient presented no symptoms and GP did not recur. It is concluded that the resection and cryotherapy combined with AMT is an effective and safe treatment for VKC with GP.

Objective To explore the risk factors of initial surgery and postoperative complications of Crohn's disease (CD).Methods The retrospective case-control study was conducted.The clinical data of 227 patients with CD who were admitted to the Cangzhou Central Hospital from April 2011 to July 2015 were collected.Treatment principles included reducing the clinical symptoms,promoting healing of intestinal mucosa under endoscopy,delaying CD progression and preventing intestinal exhaustion and related complications.The medication was performed in the early period.The resection of partial intestines was applied to patients if there was poor effect of medication or combined with intestinal obstruction,intestinal fistula,digestive tract perforation,abdominal abscess and complex anal fistula.Observation indicators:(1) treatment situation,(2) follow-up situation,(3) related factors analysis affecting initial surgery of patients with CD,(4) related factors analysis affecting postoperative complications of patients after initial surgery for CD.Follow-up using regular telephone interview and outpatient examination was performed up to May 2016.Follow-up included the wound infection,abdominal abscess,intestinal obstruction,anastomotic fistula and pulmonary infection.Measurement data with normal distribution were represented as-x ± s and comparison between groups was analyzed using the t test.Count data were represented as the proportion and analyzed by the chi-square test.The univariate analysis was done using the chi-square test and Kruskal-Wallis test,and multivariate analysis was done using the Logistic regression model.Results (1) Treatment situation:of 227 patients,68 underwent initial surgery and 159 didn't undergo surgery.The duration from diagnosis to initial surgery in 68 patients was (4.7 ± 2.5) months.Of 68 patients with surgery,28 received the emergency surgery and 40 received the selective surgery.Operation time and volume of intraoperative blood loss were (175 ±44) minutes and (285 ± 110) mL,respectively.The side-to-side anastomosis was conducted in 47 patients and non-side-to-side anastomosis in 21 patients.Other 159 patients without surgery received the medication of mesalazine,hydrocortisone,methotrexate and infliximab.(2) Follow-up situation:68 patients with initial surgery were followed up for 5-61 months,and 22 had postoperative complications.Of 9 patients with anastomotic fistula,6 had enterocutaneous fistula (5 patients with enterocutaneous fistula were improved by selective surgery,and the other patient was progress to acute diffuse peritonitis and then was improved by peritoneal lavage,adequate drainage and nutritional support therapy after emergency surgery).Three patients with anastomotic abscess were improved by adequate drainage.Six patients with secondary intestinal obstruction were improved by conservative treatment.Three patients with abdominal abscess were improved after antiinflammatory treatment and adequate drainage.Two patients with wound infection were improved by regular dressing change.Two patients with pulmonary infection were improved by anti-inflammatory and phlegm conservative treatment.(3) The related factors analysis affecting initial surgery of patients with CD.The results of univariate analysis showed that age of diagnosis,smoking history and behavior of disease were the related factors affecting initial surgery of patients with CD (Z =-2.120,x2 =5.082,50.512,P＜ 0.05).The results of multivariate analysis showed that A3 of age of diagnosis,B2 and B3 of pattern of disease were the independent risk factors affecting initial surgery of patients with CD [OR =15.624,10.535,28.509,95％ confidence interval (CI):4.856-29.375,3.609-17.637,8.526-79.228,P ＜ 0.05].(4) The related factors analysis affecting postoperative complications of patients after initial surgery for CD.The results of univariate analysis showed that preoperative levels of albumin (Alb) and hemoglobin (Hb),emergency surgery,operation time and anastomotic method were the related factors affecting postoperative complications of patients after initial surgery for CD (x2 =10.757,7.639,6.773,4.309,16.346,P ＜ 0.05).The results of multivariate analysis showed that preoperative Alb≤28 g/L,Hb≤ 100 g/L,emergency surgery and non-side-to-side anastomosis were the independent risk factors affecting postoperative complications of patients after initial surgery for CD (OR =9.592,8.849,6.538,12.645,95％CI:2.209-25.235,2.034-24.773,1.846-15.893,3.935-38.873,P ＜ 0.05).Conclusions The age of diagnosis ＞ 40 years,B2 and B3 of CD are high risk group of initial surgery.The poor preoperative nutritional status,emergency surgery and non-side-to-side anastomosis are independent risk factors affecting postoperative complications of patients after initial surgery for CD.

Objective To analyze the nursing interventions of two kinds of percutaneous nephrolithotomy (PCNL),so that we can provide personalized service for patients with PCNL and improved quality of nursing. Methods 140 cases of patients undergoing PCNL were chosen from January 2010 to January 2012.70 of them received standard PCNL,another 70 cases were given tubeless PCNL.The same nursing method was given before the operation,and personalized nursing was given according to difference in operation method.A self-designed questionnaire was conducted for satisfaction degree with nursing service of all patients on discharge. Results Satisfaction degree with psychological care,catheter nursing and complication nursing of tubeless PCNL was higher than standard PCNL. Conclusions Two methods of PCNL have become conventional therapy for upper urinary calculus,and careful nursing is very important,especially for tubeless PCNL.

BACKGROUND: Homogenate meals, as a replacement of normal diet, can be used in either hospitals or families. The homogenate meal as nutritious supplements should contain multiple components when it is applied in senior patients with chronic diseases because of the longterm application and complex complications.OBJECTIVE: To investigate the impact of longterm tube feed of finished product of homogenate meal on the nutritious supplements of senior patients.DESIGN: A cross-sectional study based on diagnosis.SETTING, PARTICIPANTS and METHODS: Totally 24 senior patients with chronic diseases who had been used self-made finished products of homogenate meal for more than 1 year were selected for nutritious supplement from Guangzhou General Hospital of Guangzhou Military Area Command of Chinese PLA. The changes of nutritious index at before and 4, 8 and 12 months after supplement were observed.MAIN OUTCOME MEASURES: Anthropometric measurement index and laboratorial index of 24 senior patients with chronic diseases before and after intervention were compared.RESULTS: Serous albumin at before nutritious supplements, and 4, 8 and 12 months after supplements were[ (36.4 ± 4.03), (36.60 ± 4.42),(38.20 ± 3.77), and(40.40 ± 3. 33) ] g/L respectively. Phosphorus and peripheral lymphocytes were significantly heightened while blood sugar began to decrease since 4 months after intervention, which all had significant differences compared with that of before intervention( t =2. 196 -2. 356,P <0.05; t=2.958-3.431. P <0.01) .Anthropometric measurement index also improved compared with that of before intervention.CONCLUSION: Longterm application of finished product of homogenate meal can improve the nutritious status of senior patients with chronic diseases.

Objective:To explore the effect of modified multifunctional dressing bag with glauber's salt for relieving swelling and pain in early patients with limb fracture.Methods:A total of 111 patients with limb fracture in Tongde Hospital of Zhejiang Province were divided into two groups according to the admission time. The control group consisted of 53 patients with limbs fracture using common mirabilite bag with glauber′s salt and ice bag for external application admitted from January to August 2018, and the observation group consisted of 58 patients with limbs fracture using modified multifunctional dressing bag with glauber′s salt admitted from September 2018 to January 2020. The degree of pain, the time of appearance of dermatoglyphia, the cases of severe swelling, waiting time for operation, compliance and comfort of the patients were observed within 72 hours after injury.Results:The degree of pain, the time of appearance of dermatoglyphia, the waiting time, the cases of severe swelling for surgery in the observation group were (1.8±0.7) points, (4.5±1.2) days, (5.7±1.2) days,1 case, respectively, which were all lower than those in the control group (2.2±0.8) points, (6.0±1.5) days, (6.5±1.4) days, 19 cases, the differences between the two groups were statistically significant ( t values were -2.6,-5.6,-3.3, χ2 value was 21.8, all P<0.05). The total compliance rate in the observation group was 82.76% (48/58), higher than that in the control group 56.60% (30/53), the differences were statistically significant ( χ2 value was 9.34, P<0.05). The comfort of the observation group was 82.76% (48/58), significantly higher than that of the control group 3.77% (2/53), the difference was statistically significant ( χ2 value was 69.8, P<0.05). Conclusions:The modified multifunctional dressing bag with glauber′s salt can reduce the swelling of limbs, reduce the appearance of skin blisters, stop bleeding and pain, and shorten the waiting time for operation, at the same time, it improves the compliance and comfort of the patients.
