1.Practice and thinking of quality management of organ procurement organization

BACKGROUND:With the introduction of related policies on organ procurement and distribution, the organ procurement and standards of the organ procurement have a clear operational criteria, as wel as how to al ocate organs. But the work processes and organizational positioning on organ procurement organization are not enough elaborated and normal. OBJECTIVE:To analyze the experience of Guangzhou General Hospital of Guangzhou Military Command as the organ procurement organization units in quality management, timely review working progress and explore areas for improvement, which provide the corresponding countermeasures for enhancing quality management and establishing the efficient and professional organ procurement organization. METHODS:A computer-based online search in CNKI database and PubMed database was performed with the keywords of“organ donation, organ transplantation, Organ Procurement Organization, quality management”in English and Chinese by screening titles and abstracts. RESULTS AND CONCLUSION:According to relevant laws and regulations, Guangzhou General Hospital of Guangzhou Military Command formulates a standard evaluation standard and sets up the team, thus achieving the reunification of the responsibilities and powers. At the same time, they improve the relevant organ donation,acquisition, preservation, registration, and design different quality management plans according to these different aspects. In addition, In addition, the hospital hold regular coordinator training and make it standardization. Through a series of quality management measures, the hospital organ procurement organization has gradual y regularized and achieved some results. It is recommended to further strengthen scientific research, achieve the unification of efficiency and effectiveness of scientific research;improve the awareness of the hospital leaders, consummate the mechanism of organ procurement;focus on personnel training, improve the professional quality of the organization members;enhance information management, achieve multi-disciplinary col aboration;build quality evaluation system, achieve the whole process of quality management;emphasize on medical ethics, and promote the harmony between the hospital and the patient.
2.A Primary Study on HPLC Fingerprint of Pogostemon cablin

Objective To study the fingerprint determination method of Pogostemon cablin by HPLC.Methods A RP-HPLC method was established with the mixed solution of methnol and 0.1 % acetic acid aqueous solution as mobile phase(gradient elution),flow rate at 1.0 mL/min,detection wavelength at 254 nm,analytic time being 50 min and column temperature at 25 ℃.Results The results of comparative analysis of 10 batches samples showed that 15 common peaks were shared by Pogostemon cablin.The common peaks were classified to two regions and the second was the main characteristic region which had an amount of characteristic peaks.All samples had the similar fingerprint features.Conclusion The method is accurate,repetitive and can be applied for the identification and quality control of Pogostemon cablin.
3.Relationship between CYP2C19 gene polymorphism and clopidogrel resistance after PCI in patients with coronary heart disease/

Objective :To explore influence of CYP2C19 gene polymorphism on clopidogrel resistance after percutane‐ous coronary intervention (PCI) in patients with coronary heart disease (CHD).Methods :A total of 100 CHD pa‐tients ,who were treated and received PCI in our hospital ,were selected .There were 24 cases with clopidogrel re‐sistance (CR ,CR group) and 76 cases without CR (NCR ,NCR group).According to CYP2C19 genotype ,patients were divided into rapid metabolism CYP2C19*1/*1 (n=49) ,medium metabolism CYP2C19*1/*2 (n=28) and*1/*3 (n=11) ,and slow metabolism CYP2C19*2/*2 (n=9) and *2/*3 (n=3).Relationship among differ‐ent genotypes ,CR ,maximum platelet aggregation rate (MPA) and incidence of major adverse cardiovascular events (MACE) were analyzed .Results :With rapid metabolism CYP2C19*1/*1 as the base ,there was significant rise in CR risk in medium metabolism (CYP2C19*1/*2 and *1/*3 ,OR=4. 16 ,5. 03 , P<0.05 both) and slow metab‐olism (CYP2C19*2/*2 and *2/*3 ,OR=7.04 ,17. 6 , P<0.01 both ) ,medium metabolism increased by 4. 16 and 5. 03 times respectively ,while slow metabolism increased by 7. 04 and 17. 60 times respectively .Compared with rapid metabolism genotype ,there were significant rise in MPA and incidence rate of MACE in medium and slow me‐tabolism genotypes , P<0.05 or <0. 01 ;incidence rate of MACE in CR group was significantly higher than that of NCR group (20. 8% vs.5. 3%, P=0.02).Conclusion : CYP2C19 gene polymorphism possesses certain influence on CR after PCI in CHD patients .Those with medium or slow metabolism genotypes are more likely to suffer from CR ,higher MPA and incidence rate of MACE .
4.Intra-articular injection of different concentration of dexmedetomidine combined with ropivacaine on anal-gesic effect of knee arthroscopy in elderly patients in the department of orthopedics

Objective To investigate the analgesic effect of different concentrations of medetomidine and ropivacaine on knee arthroscopy in elderly patients in the orthopedics department. Methods 72 patients were di-vided into 4 groups,and from February 2016-September 2017 ,different concentrations of dexmedetomidin and ropivacaine were given to the patients after the operation,and compared the analgesic effects and adverse reactions of each group. Results The duration of analgesia in groups C and D were longer than group A and B and there were significant differences in the amount of fentanyl in the 4 groups after 24 h(F = 79.633,P < 0.05 )and there was no significant difference in the incidence of adverse reactions between the 4 groups(P > 0.05);1,3,6, 10,16,24 h after operation,The VAS scores in group C and D were significantly lower than those in group A and B(P < 0.05 ). Conclusion Dexmedetomidine plus ropivacaine can effectively improve the analgesic effect after knee arthroscopy,0.50 g/kg was the best concentration of dexmedetomidin.
5.Spatial-resolved metabolomics reveals tissue-specific metabolic reprogramming in diabetic nephropathy by using mass spectrometry imaging.

Detailed knowledge on tissue-specific metabolic reprogramming in diabetic nephropathy (DN) is vital for more accurate understanding the molecular pathological signature and developing novel therapeutic strategies. In the present study, a spatial-resolved metabolomics approach based on air flow-assisted desorption electrospray ionization (AFADESI) and matrix-assisted laser desorption ionization (MALDI) integrated mass spectrometry imaging (MSI) was proposed to investigate tissue-specific metabolic alterations in the kidneys of high-fat diet-fed and streptozotocin (STZ)-treated DN rats and the therapeutic effect of astragaloside IV, a potential anti-diabetic drug, against DN. As a result, a wide range of functional metabolites including sugars, amino acids, nucleotides and their derivatives, fatty acids, phospholipids, sphingolipids, glycerides, carnitine and its derivatives, vitamins, peptides, and metal ions associated with DN were identified and their unique distribution patterns in the rat kidney were visualized with high chemical specificity and high spatial resolution. These region-specific metabolic disturbances were ameliorated by repeated oral administration of astragaloside IV (100 mg/kg) for 12 weeks. This study provided more comprehensive and detailed information about the tissue-specific metabolic reprogramming and molecular pathological signature in the kidney of diabetic rats. These findings highlighted the promising potential of AFADESI and MALDI integrated MSI based metabolomics approach for application in metabolic kidney diseases.
