1.Influence of intensive drug program on blood pressure, cardiac function and laboratory parameters of elderly patients with type H hypertension

Objective To investigate the influence of enalapril maleate and folic acid combined with Vitamin B6 (VB6)+Vitamin B12 (VB12) on blood pressure, cardiac function and laboratory parameters of elderly patients with type H hypertension. Methods One hundred and fifty-six elderly patients with type H hypertension were chosen in the period from January 2015 to March 2016 in Traditional Chinese Medicine Hospital of Baoji City and randomly divided into both groups including control group (78 patients,enalapril maleate and folic acid used alone) and observation group (78 patients,VB6+VB12 on the basis of control group); and the levels of blood pressure, cardiac function, homocysteic acid (Hcy), human soluble intercellular adhesion molecule-1(sICAM-1), nitric oxide (NO) and endothelin-1 (ET-1) before and after treatment and the incidence of major cardiovascular events of both groups were compared. Results The levels of 24 h systolic blood pressure mutation rate (24 h SBPV), 24 h diastolic blood pressure variability (24 h DBPV), day systolic blood pressure mutation rate (dSBPV), day diastolic blood pressure variability (dDBPV), night systolic blood pressure mutation rate (nSBPV) and night diastolic blood pressure variability (nDBPV) after treatment in observation group were significantly lower than those in control group and before treatment (P < 0.05). The levels of interventricular septum thickness (IVST), left ventricular end diastolic diameter (LVDd), left ventricular end systolic diameter (LVDs) and left ventricular ejection fraction (LVEF) after treatment of observation group were significantly better than those ofcontrol group and before treatment (P<0.05). The levels of Hcy, sICAM-1, NO and ET-1after treatment of observation group were significantly better than those of control group and before treatment (P < 0.05). Conclusions Intensive drug program in the treatment of elderly patients with type H hypertension can efficiently control blood pressure level, improve cardiac function, regulate the levels of Hcy, sICAM-1, NO and ET-1 and be helpful to prevent major cardiovascular events.
2.Predictive Value of Combining the Level of Antithrombin Ⅲ and Procalcitonin for SA-AKI

Objective To investigate the predictive effect of antithrombin Ⅲ(AT-Ⅲ)combined with procalcitonin(PCT)on sepsis-associated acute kidney injury(SA-AKI).Methods A total of 96 patients with sepsis who met the inclusion criteria were in-cluded,all patients were divided into the AKI group(n=42)and non-AKI group(n=54),and the data between the two groups were compared.We used the receiver operating characteristic(ROC)curves to evaluate the predictive effect of antithrombin Ⅲ level,procalCI tonin level,and combination on SA-AKI.Multivariate Logistic regression analysis was used to study the influencing factors of SA-AKI.Results The proportion of PCT,CRP,fibrinogen,SOFA score,APACHE Ⅱ score,vasoactive drug use,and blood purification applica-tion in the AKI group was higher than that in the non-AKI group(P<0.05),as well as the AT-Ⅲ,and platelet count were lower than those in the non-AKI group(P<0.05).The area under the ROC curve for predicting SA-AKI in patients with sepsis was 0.876(95%CI:0.793-0.935,P<0.01),the sensitivity was 78.6%,and the specificity was 88.9%.Multivariate Logistic regression anal-ysis showed that AT-Ⅲ(OR=0.907,95%CI:0.860-0.956)and PCT(OR=1.037,95%CI:1.002-1.074)were independent influencing factors for SA-AKI.Conclusion AT-Ⅲ combined with PCT could be used to predict SA-AKI and has good predictive ef-ficacy.

Objective To characterize the baseline status of Chinese diabetic patients based on data derived from Chinese cohort from SOLVETM study.Methods Patients with type 2 diabetes initiating basal insulin detemir at the decision of the physician were eligible for the study.Data on demographics,medical history,glycemic profile and treatment regimen at baseline were collected by physicians.Results A total of 3272 patients [female 42%,male 58%,mean age (56.2 ± 10.8) years] were included in the study.Their BMI was (25.3 ± 3.3) kg/m2.The duration of diabetes was 4.0 (0.1-27.0) years,and the duration of treatment with oral antidiabetic drugs (OADs) was 3.0(0.0-20.2) years.The proportions of subjects with diabetic macro-and micro-vascular complications were 15.8% (515 cases) and 27.1% (866 cases),respectively.The hemoglobin Al c (HbAl c) at baseline was (8.33 ± 1.70) %,and the fasting blood glucose (FPG) was (9.5 ± 2.6) mmol/L.Conclusions A large proportion of patients with type 2 diabetes remain in poor glycemic control,and the prevalence of diabetic complications is high,which requires optimal therapeutic strategy for the patients with suboptimal glycemic control.
