2.Study on Enrichment Process of Hesperidin of Baohe Pills with Macroporous Resin

Objective: To study the technological parameters of the enrichment purification process of Baohe Pills with macroporous resin. Methods: With the enriched degree of hesperidin of Pericarpium Citri Reticulatae of Baohe Pills as a marker, the optimum technological parameters of the enrichment process were studied. Results: The optimum process is that 5mL of the extract of BaoHe Pills (500mg/mL) was adsorbed for 30min with a column of macroporous resin (R15mm?H100mm) and the resin was washed with 100mL of distilled water, then the hesperidin was eluted from the macroporous resin with 100mL of 50% ethanol. Conclusion: The elutive rate of hesperidin was above 95% by means of the macroporous resin. So this process enriching the active components of Baohe Pills is feasible.
3.Establishment of a risk assessment model for laboratory biosafety of Crypto-sporidium

Objective To evaluate the risk assessment model of Cryptosporidium laboratory,so as to provide the basis for laboratory personnel engaging in the operation of Cryptosporidium. Methods Firstly,the risk factors of Cryptosporidium infec?tion in laboratory were determined by the literature and Delphi,and then the weights of risk factors were determined by fuzzy an?alytic hierarchy process. A risk assessment model for laboratory biosafety of Cryptosporidium was established. Results Com?pared to the indexes,based on the risk assessment model,stool sample processing was the two steps in the laboratory with high risk of infection and high risk factors,with the combination weights of risk possibility and hazard rating were 0.111 and 0.107, respectively. Conclusion The risk assessment model established is feasible. It can be used to make some suggestions for the re?lated laboratory staff.
4.Clinical significance and outcomes of fetal ventriculomegaly

Objective To evaluate the clinical significance and prognosis of tetus with lateral ventriculomegaly.Methods We retrospectively analyzed 92 singleton pregnant women who were antenatally diagnosed with fetal ventriculomegaly by ultrasound in genetic conselling clinics,Department of Gynaecology and Obstetrics,Nanfang Hospital,Southern Medical University between August 2007 and August 2010.All participants were divided into three groups according to the width of the lateral ventricles:group A (10.0-12.0 mm,n=50),group B (12.1-14.9 mm,n=38) and severe ventriculomegaly group (≥15.0 mm,n=4).All fetuses were followed up.Chi-square test(or Fisher's exact test),Bonfferoni method,Mann Whitney or Kruskal-Wallis test were used for statistics.Results In group A,B and severe ventriculomegaly group,18% (9/50),65.8%(25/38)and 3/4 of fetuses were complicated with structural malformation,respectively (x2 =22.934,P =0.000),and statistical significance were found only between group A and B (x2 =20.798,P=0.000).The incidences of fetal chromosomal aberration were 4.0% (2/50),7.9% (3/38)and 0/4 in the three groups,respectively (x2=0.878,P = 0.645).Eventually,all four cases with severe cerebral ventriculomegaly were terminated.Among cases of mild ventriculomegaly,63 women continued the pregnancy (48 in group A and 15 in group B).The intrauterine improvement of group B was poorerthan that of group A (Z=-3.317,P =0.001).Respectively,three,ten and two cases of ventriculomegaly deteriorated,stabilized and regressed in group B,and the corresponding figures were 3,15 and 30 in group A.In group A,the prognosis of fetus with non-isolated ventriculomegaly was poorer than that of fetus with isolated ventriculomegaly (Z=-2.631,P=0.009).For neonates 14 days after birth (n=62),the rates of normal neonatal behavioral neurological assessment (NBNA) scoring were respectively 93.8% (45/48) and 71.4% (10/14) for groups A and B with statistical difference (Fisher's exact test,P =0.040).Bayley scales of infant development (BSID) used to evaluate infants at 12-month-old revealed that the psychomotor developmental index (PDI) between group A and group B had statistical difference (8,26 and 1 case of good,moderate and poor development in group A; one,seven and three cases in group B; Z=-2.203,P=0.043).However,the mental developmental index (MDI) between group A and B had no statistical difference.Twenty babies among the survived ones received magnetic resonance imaging (MRI) examination.The results showed that lateral ventricle width regressed in nine babies (45.0%) and progressed in one baby (5.0%).Ten cases (50.0%) did not change.The change of ventriculomegaly after birth was related to BSID evaluation.Better prognosis of fetuses were found in those with regressed ventriculomegaly (P=0.033).Conclusions Fetuses with mild ventriculomegaly (10.0 12.0 mm) have a favorable outcome.Further investigations,such as fetal development,chromosomal examination,intrauterine infection screening and MRI if necessary,are suggested for fetus with ventriculomegaly.Moreover,intensive intrauterine and postnatal follow-up is recommended.

T-2 toxin is a secondary fungal metabolite that belongs to the trichothecene mycotoxin family.T-2 toxin can lead to the structural and functional changes of cartilage cells and cartilage cell degeneration and necrosis.With strong cytotoxicity,T-2 toxin can cause definite damage to cartilage cells.In this paper,we reviewed recent studies on the effects of T-2 toxin on chondrocyte apoptosis,ultra structural changes and extracellular matrix in vitro.
6.Combined treatment with recombinant human growth hormone and stanazolol improves growth and final adult height in girls with Turner's syndrome

Forty girls with Turner's syndrome (TS),aged (12.6 ± 1.9) years,were treated with daily subcutaneous injection of recombinant human growth hormone (rhGH,1.0 ~ 1.1 IU · kg-1 · w-1) and oral stanozolol (0.02 ~ 0.04 mg · kg-1 · d-1) for 1 ~ 5 years.Growth velocity (GV),height standard deviation score (SDS) by reference of healthy Chinese girls (HtSDSNor) and height SDS by reference of untreated Chinese TS girls (HtSDSTs)were evaluated regularly.Of the forty girls studied,thirteen had discontinued the treatment after a mean duration of (2.9 ± 1.2)years when GV was less than 2 cm/year or when patients were satisfied with the achieved height.Final adult height (FAH) or near-final height,which was defined as the most recent available height after discontinuation of treatment,and the height gained in the thirteen girls were evaluated.Estrogen therapy was started at the age of(16.0 ± 1.1) years.HtSDSNor increased from-4.2 ± 1.0 to-3.4 ± 1.0 in the first year,and-2.8 ± 1.0,-2.4 ± 0.8,-2.5 ± 0.5,-2.3 ±0.3 respectively in the 2nd,3rd,4th,and 5th year.The change in HtSDSTs was similar to HtSDSNor.It was increased from 0.1 ± 0.9 to 1.0 ± 0.9 in the first year,and to 1.5 ± 0.8,1.9 ± 0.6,1.7 ± 0.4,1.7 ± 0.2 in the subsequent 4 years.The predicted adult height (PAH) in 13 girls was (142.8 ± 4.2) cm before treatment.FAH was (151.7 ± 4.1) cm,which was significantly higher than PAH (P<0.01),and the mean height gain was (8.9 ± 2.8) cm (5.1 ~ 12 cm).FAHSDSNor was increased to-1.6 ± 0.8 from-3.8 ± 0.8.For girls with TS around 9 years of age,combined therapy with rhGH and low dosage of stanazolol may significantly increase growth velocity and improve final adult height.
7.Expression of IL-10+CD19+ regulatory B cells in peripheral blood of patients with chronic hepatitis B inaction

Objective To investigate the population and role of IL-10+ CD19+ regulatory B cell (Breg) in patients with chronic hepatitis B.Methods Patients with acute hepatitis B (AHB) (n =28),chronic hepatitis B (CHB) (n =31) and normal subjects (n =25) were collected from Changshu No.2 People's Hospital between 2011 June and 2012 October.Peripheral blood mononuclear cells (PBMC) were isolated and stimulated with CpG ODN 2006 and PMA.Flow cytometry was used to analyze the population of IL-10-CD19 + Breg,CD4 + CD25high Treg,and ELISA was used to analyze the concentration of IL-10 in culture supernatant.Results The population of Breg in Peripheral blood of the CHB group [1.28% (1.05%-2.20%)] was higher than that in the AHB group [0.87%(0.55%-1.22%)] and the HCs group [0.89% (0.51%-1.37%)] (P =0.001,0.006),and the difference between the AHB group and the HCs group was not statistically significant (P=0.669).Breg in the CHB group [14.30% (10.70%-16.70%)] was higher than that in the AHB group [10.30% (7.05%-13.30%)] and the HCs group [10.40%(6.85%-12.60%)] (P =0.003,0.001),treg in the CHB group [5.80% (4.20%-9.10%)] was also higher than that in the AHB group [4.05% (2.53%-5.40%)] and the HCs group [4.50% (2.55%-5.50%)] (P <0.001,P =0.005),and there was no significantly difference between the AHB group and the HCs group (Breg:P =0.796 ; Treg:P =0.227).Spearman correlation analysis showed that Breg was positively correlated with Treg in the CHB group (r =0.50,P =0.004),however there was no significantly correlation in the AHB group and the HCs group (r =-0.15,P =0.462; r =0.09,P =0.669).The concentration of IL-10 in the CHB group was higher than that in the AHB group and the HCs group (P < 0.001),and the difference between the AHB group and the HCs group was not statistically significant (P=0.341).Spearman correlation analysis showed that IL-10 were positively correlated with the population of Breg in the CHB group (r =0.409,P =0.022).Conclusion The poluations of regulatory B cell and regulatory T cell increased in patients with chronic hepatitis B,and Breg cell might play the immune regulation role through secreting IL-10 in chronic HBV infection.
8.TK2-related mitochondrial DNA depletion syndrome:two cases report and review of literature

Objective To study the clinical characteristics,muscle pathological features,diagnosis and prognosis of TK2-related mitochondrial DNA depletion syndrome(MDS).Methods Clinical and laboratory data of 2 cases of TK2-related myopathic MDS were reported.And data of previously reported 58 TK2-related MDS cases were reviewed.Results Total 60 patients consisted of 35 male and 25 female.The age of onset ranged from the birth to the age of 74 years old,and 54 of the patients were attacked at the age younger than 3 years old.Muscle weakness and hypotonia were detected in all patients,with 40 patients(including the newly diagnosed 2 cases) manifested as pure myopathic form,and 20 patients with other multiple organs involvement.Serum creatine kinase was mildly increased (211-6 500 IU/L) in 53 patients.Elevated serum lactic acid level (2.3-12.0 mmol/L)was observed in 24 patients.Muscle biopsy was available from 55 patients,and ragged red fibers and/or cytochrome C oxidase (COX)-negative fibers were detected in 48 out of them.Nine out of 11 patients received electronic microscope study showed proliferation of abnormal mitochondria.Respiratory chain enzymatic activities in skeletal muscle were reduced in 31 out of 33 patients.Marked mtDNA content reduction was observed in 36 out of 41 patients (4%-25% of age-and tissue-matched controls).A total of 42 TK2 mutations were found in 60 patients,including 2 novel mutations c.923A > G and c.619-2A > T in this study.Conclusions The most common clinical manifestations of TK2-related MDS are severely,rapidly progressing myopathy with infantile or early childhood onset.As the detection rate of characteristic pathologic features in muscle is high,muscle biopsy is important for the diagnosis of TK2-related MDS.
9.Clinical efficacy of free expanded deltopectoral perforator flaps for aesthetic repair of large facial scars

Objective To investigate the effect of expanded deltopectoral perforator flap as a free skin flap for distant transfer to aesthetically repair large facial scars via microsurgical technique.Methods 20 cases of large facial scars were treated with free expanded deltopectoral skin flaps via microsurgical technique.At the first stage,24 large volume expanders were inserted into the subcutaneous pockets in the deltopectoral zone.4 to 6 months later,the second stage operation was performed when the flaps were expanded fully and facial scars removed;deltopectoral perforator flaps harvested choosing the second or the third thoracic artery perforating branches as the pedicle;the surface of the wound was covered with the flaps.Results All of the 24 flaps survived completely and the wounds healed well after transplantation.The 6 months to 2 years' follow-up results showed that the color and texture of the flaps matched the recipient place well without swelling.The superficial and deep sensation recovered very fast.Conclusions It is safe,practical and effective that the delto-pectoral perforator flaps are used to repair large facial scars avoiding the shortcomings of traditional pedicle transfer such as more procedures,longer fixed time,or more wasted expanded skin.Free transfer of expanded delto-pectoral perforator flaps can achieve ideal function and aesthetic effects.
10.Pathogenic Bacteria Distribution and Drug Resistance in Inpatients of Burn Department

OBJECTIVE To analyze the pathogenic bacteria distribution and drug resistance in inpatients of burn department,and instruct clinical application of antibiotics reasonably.METHODS Pathogenic bacteria were isolated from inpatients of burn department from 2003 to 2006.K-B slip diffusion method was taken to carry out the sensitive test.Rate of drug resistance of the pathogenic bacteria was analyzed.RESULTS Totally 1325 strains were isolated,among them 846 strains were Gram-negative bacteria,464 strains were Gram-positive ones,and 15 strains were fungi.The percentage of these three groups was 63.85%,35.02% and 1.13%,respectively.The main strains of the Gram-negative bacteria were Pseudomonas aeruginosa,Escherichia coli,Acinetobacter baumannii,Enterobacter cloacae and Klebsiella pneumoniae,and that of the Gram-positive bacteria were Staphylococcus aureus,Enterococcus and Staphylococcus epidermidis.The major strain of fungi was Candida albicans.The pathogenic bacteria tested showed high drug resistance.The detection rate of the meticillin-resistanct S.aureus(MRSA) was 82.12%.The detection rate of the ESBLs from the K.pneumoniae and the E.coli was 60.17% and 41.89%.CONCLUSIONS It was showed that the major pathogenic bacteria infected of the inpatients of burn department are Gram-negative bacteria.The pathogenic bacteria are the multidrug-resistant ones.Enhanced monitoring on pathogenic bacteria distribution and drug resistance analyses among inpatients of burn department could benefit for the guide of clinical rational administration and depression of multidrug-resistant bacteria.
