OBJECTIVE To investigate the clinical symptom of hospital infection in hematology ward and analyze the risk factors about hospital infection,so as to control occurrence of hospital infection.METHODS Using prospective and retrospective investigating methods,all sufferers in hematology ward of our hospital from Jan to Dec,2005 were made statistical analysis.RESULTS Hospital infection rate was 11.96% and case infection rate was 12.96%.The infected sites were mainly respiratory tract,oral cavity and urethra,occupied 76.92% of all cases.Main pathogenic bacteria were Candida albicans,Pseudomonas aeruginosa,Escherichia coli,et al.CONCLUSIONS The occurrence of hospital infection in hematology ward is related to hospitalization time,underlying diseases,peripherial leucocyte count and using of antibiotic medicament.Main measures for preventing and reducing hospital infection are symptomatic treatment to underlying diseases,suitably using broad spectrum antibiotics,improvement of hospitalization conditions and shortening of hospitalization time.

Objective To explore the diagnostic and therapeutic effects of laparoscopy for early atypical tubal pregnancy.Methods Laparoscopy was conducted for diagnosing and treating 38 cases of early or atypical tubal pregnancy.For patients with blue and purple pregnant swellings seen clearly in the fallopian tubes,or those with one side of fallopian tube locally swollen and purple without obvious pregnant swellings observed,combination of fallopian tubes incision to take out embryo and salpingorrhaphy was performed.For those cases with normal fallopian tubes on both sides in appearance and without current desire of pregnancy,diagnostic uterine curettage was applied.After the diagnosis of tubal pregnancy was confirmed,30 mg of MTX was injected into ampulla of both sides.For patients with demand of reproduction,diagnostic uterine curettage was not performed.Results Five cases were misdiagnosed before operation,the misdiagnosis rate was 13%.Three cases were misdiagnosed by laparoscopy,and the rate was 8%.Fallopian tubes incision for embryo-taking under laparoscope combined with salpingorrhaphy were applied to 30 cases.Four cases were treated conservatively with injecting 30 mg of MTX into the fallopian tubes.The success rate was 100%.Blood ?-hCG was back to the normal level(4.2?3.1)days after surgery.Conclusions Laparoscopy is the optimal technique for the diagnosis and treatment of early atypical tubal pregnancy.

BACKGROUND: Previous studies demonstrated that eucommia bark can promote bone marrow stern cells (BMSCs) differentiated into osteoblasts, but relative mechanism is poorly understood. OBJECTIVE: To investigate the effects of eucommia bark water/methanol extracts on expressions of osteopontin (OPN) and osteoprotegedn (OPG) in rat BMSCs. METHODS: Totally 2 g eucommia bark powder were added into water or methanol to 16 mL and oscillated for 1 hour at room temperature. After soaked overnight, both extracts were centrifuged at 15 000 r/min for 10 minutes. Water extract was obtained from supernatant in water soaked powder. In methanol soaked powder, methanol extracts was obtained by concentrated supernatant in vacuo and resolved using 16 mL water. Water and methanol extracts were then filtered by 0.22 μm membrane, and conserved at -20℃. Six SD rats, aged 2 months, were selected, and the 3~(rd)passage of BMSCs were induced by water or methanol extracts with dilution of 1 × 10~(-2), 1 × 10~(-3), 1 × 10~(-4) and 1 × 10~(-5), respectively. PBS was added in the negative control group. All cells were cultured for 6 days. Expressions of OPN and OPG was measured by immunocytochemistry at 6 days with induction. The expression of OPN and OPG induced by water and methanol at 1 ×10~(-3) and 1×10~(-4) dilution was detected by RT-PCR. RESULTS AND CONCLUSION: Immunocytochemistrical results indicated that both water and methanol extracts of eucommia bark simulated OPN and OPG expression, in particular with dilution of 1×10~(-4). The methanol extracts had a stronger effect than water extract, but the expression of OPG did not change obviously. RT-PCR demonstrated that at the 3rd day of inducement, the level of OPN expression induced by water extract was higher than that of methanol extract, and no OPG expression was detected. Osteogenic differentiation of rat MSCs induced by eucommia bark water/methanol extracts relates to stimulating expression of OPN, which has no correlation to OPG. OPN expression induced by water extract is early than that of methanol extract.

AIM　To explore the effect of berberine in inhibition of proliferation and induction of apoptosis of gastric cancer cell MGC-803.METHODS　The MGC-803 cell morphology and cell counting were studied by Trypan blue, MTT, Methyl green pyronin stain assay, the MGC-803 cell DNA changes were investigated by flow cytometry and agarose gel electrophoresis.RESULTS　Berberine could significantly inhibit the growth of MGC-803. After 48 hours of treatment with different concentration berberine (8, 4, 2, 1 mg*L-1), the inhibitive rate of MGC-803 were 86.7%, 65.9%, 20.0%, 0%, respectively. Meanwhile, MGC-803 showed typical apoptosis features: cell shrinkage, nuclear condensation, DNA fragmentation and formation of apoptotic bodies. A typical subdiploid peak before G0/G1 phase was observed by flow cytometry. The Trypan blue stain achromatophilia rates of death cells, which with well membranes, were between 60%～82%. Agarose gel electrophoresis analysis, which showed that the chromosomes of MGC-803 were broken by berberine before the structures of membrane were damaged, while DNA broke into long fragments rather than small fragments, supported that the activation effect of berberine on nuclease was mild.CONCLUSION　Berberine could significantly inhibit proliferation of gastric cancer cell MGC-803 and induced apoptosis, and the cytotoxicity of berberine on MGC-803 was weak.

Objective To investigate the effect of tannic acid on glomerular mesangial cells (GMC),and to clarify the mechanism of tannic acid in improving the pathological changes of diabetic nephropathy (DN)from the aspect of oxidative stress and micro-inflammation. Methods The glomerular mesangial cells were treated with glucose (30 mmol·L-1 )or advanced glycosylation end-products (AGEs)bovine serum albumin(BSA)(250 mg·L-1 )and then different concentrations of tannic acid (10,20,40 and 80μmol·L-1 )were added into the GMC.The cells cultured by normal glucose or treated with BSA were used as control groups and then the level of malonic dialdehyde (MDA), glutathione peroxidase (GSH-Px ), superoxide Dismutase (SOD ), CAT (Catalase ) activities and 8-hydroxy-2′-deoxyguanosine(8-OHdG)levels in the culture supernatant 48 h after culture were determined by colorimetry and ELISA method. The expressions of intercellular cell adhesion molecule-1 (ICAM-1 ) protein, monocyte chemotactic protein 1 (MCP-1 ) and ICAM-1 mRNA in GMC were detected by immunohistochemical staining and RT-PCR method.Results Compared with high glucose and AGEs groups,the MDA levels in tannic acid groups were reduced significantly(P<0.05);the activities of GSH-Px,SOD and CAT were increased significantly(P<0.05 or P<0.01);the 8-OHdG levels in annic acid groups were significantly reduced (P<0.05). Compared with high glucose and AGEs groups,the expressions levels of ICAM-1 protein in 40 and 80μmol· L-1 tannic acid groups were decreased (P<0.05 ). The mRNA expressions levels of MCP-1 and ICAM-1 were significantly lower than those in high glucose group (P<0.01 ).Conclusion Tannic acid could protect GMC against the damage of oxidative and inflammatory mediators,thereby delaying and improving the glomerular lesions of DN.

【Objective】To observe the effect of Huangqi Sijunzi Decoction(HSD) combined with parenteral nutrition in promoting postoperative recovery of gastric cancer patients.【Methods】Eighty gastric cancer patients were randomized into two groups.On the 3rd day after operation,the two groups received parenteral nutrition,and the treatment group(group A) was given HSD additionally.The anal exsufflation time,incision infection rate,biochemical indexes and immune indexes of the two groups were observed.【Results】The anal exsufflation time was(2.40?0.33) days in the treatment group and(3.82?0.26) days in the control group,the difference being significant(P0.05).The increase of pre-albumin in the treatment group was obvious as compared with that in the control group(P0.05).The increase of immune indexes was not obvious in the control group(P

Objective To evaluate the efficacy and safety of maintenance treatment with Qingpeng ointment in the prevention of recurrence of localized eczema.Methods Totally,197 patients with localized eczema were topically treated with halometasone cream for 2-4 weeks.The patients who achieved clinical cure were then randomly classified into two groups to receive maintenance treatment with Qingpeng ointment (Qingpeng group,n =69) or vitamin E-containing cream (emollient group,n =68).Patients were followed up for 8 weeks for the detection of recurrence after the discontinuation of maintenance treatment.Results During the follow up,the recurrence rate in the Qingpeng group was significantly lower than that in the emollient group (19.70％ vs.37.31％,x2 =5.06,P ＜0.05).Significant differences were also observed in the duration of remission between the Qingpeng group and emollient group ((43.92 ± 15.70) vs.(32.60 ± 14.40) days,t =2.23,P＜ 0.05).The incidence rate of adverse reactions was 1.45％ in the Qingpeng group during the maintenance treatment,and no adverse reactions were observed in the emollient group.Conclusion The maintenance treatment with Qingpeng ointment can delay the recurrence of localized eczema to some extent with a favorable safety.

Objective To analyze the relationship between the expression of protection of telomeres 1 (POT1) and the pathogenesis of acute myeloid leukemia (AML). Methods 62 patients with de novo AML (case group) and 10 patients with iron deficiency anemia (control group) were enrolled in this study. The quantitative real-time polymerase chain reaction (PCR) and Western blot were used to detect the expression of POT1 in AML patients. Results There were 62 de novo AML patients, including 2 cases M1, 14 cases M2, 12 cases M3, 14 cases M4, 17 cases M5, 2 cases M6 and 1 case AML without classification. According to the risk stratification, high risk group (24 cases), medium risk group (22 cases) and low risk group (16 cases) were divided. Compared with that in the controls, POT1 expression levels in patients with AML were significantly decreased both in mRNA and protein level (P< 0.05). The relative expression levels of POT1 mRNA and protein in patients with M2, M4 and M5 were significantly lower than those in the controls (P< 0.05). The expression levels of POT1 in high risk group, medium risk group and low risk group were significantly decreased than those in the controls (P<0.05). Compared with that in the controls, The relative POT1 mRNA expression was significantly decreased in M3 patients (P< 0.05), but not in protein level. POT1 protein expression was showed both in the cytoplasm and nucleus. There was no significant difference of the expression of POT1 protein between cytoplasm and nucleus (P> 0.05). Conclusions POT1 may be involved in the pathogenesis of AML. POT1 protein expresses in both cytoplasm and nucleus, and the regulatory mechanism may be related to the telomere length.

Objective To summarize the clinical manifestations and treatment of Churg-Strauss syndrome (CSS) involving large and medium-sized arteries. Method Six cases were investigated retrospec-tively and literature were reviewed. Results The pathological category of large-and medium-sized arteries involved in CSS included occlusion, stenosis, thrombesis, dissection, aneurysm, arteritis, and periarterifis. The involved arteries included coronary artery, central retinal artery, mesenteric artery, vertebral artery, basilar artery, carotid artery, aorta, and arteries of extremities. The large-and medium-sized arteries involvement in patients with CSS might not associated with age, duration of disease, and the titer of perinuclear-antineutrophil cytoplasmic antibodies. Treatment with corticosteroid and immunosuppressive agents combined with surgical interventions or anticoagulation if needed might improve the prognosis in the early stage. It was particularly hazardous for coronary involvement in patients with CSS. Conclusion CSS can involve many kinds of large-and medium-sized arteries. The pathological category of large and medium-sized arteries involved in CSS is diverse. More attention should be paid to large and medium-sized arteries involvement in patients with CSS.
