This article introduces common review types such as the systematic review, Meta-analysis, overview of systematic reviews, Meta-synthesis, scoping review, and integrative review, and distinguishes the similarities and differences of different types of reviews, and analyzes their advantages and limitations, in order to improve the scientific and professional level of nursing practice, and promote the development of evidence-based and nursing disciplines.
3.Clinical features of protracted bacterial bronchitis in children.

OBJECTIVETo analyze the clinical characteristics of protracted bacterial bronchitis (PBB) in children.
METHODThe clinical data of patients seen from October, 2010 to March, 2014 in Department of Respiratory Diseases of our hospital were retrospectively analyzed. Inclusion criteria were over 4 weeks cough, receiving fiberoptic bronchoscopy, positive bacterial culture and (or) the increased percentage of neutral granulocytes in bronchoalveolar lavage fluid (BALF).
RESULTTwenty eight patients were involved, 26 were male (93%) and two were female (7%). The median age of patients was 8.5 months. The median duration of cough was four weeks. The average length of hospital stay was (8.3 ± 3.9)days. The main clinical feature was wet cough in 28 cases, wet cough with wheezing was seen in 21 cases. The wet cough phase distribution was irregular in 21 cases. The crackles with wheeze (in 21 cases) was main signs of PBB. The percentage of CD3⁻ CD16⁺ 56⁺ cells increased in peripheral blood. The fiberoptic bronchoscopic manifestations of PBB were luminal mucosal edema. Eleven patients also had airway malacia. The neutrophil median in BALF was 0.2. The positive rate of bacterial culture of BALF was 36%. The main bacteria were Streptococcus pneumoniae (50%) and Haemophilus influenzae (30%). The main treatment for PBB patients included amoxycillin/clavulanate potassium and second-generation cephalosporins. The average duration of treatment was (17.3 ± 3.2)days, the prognosis was good.
CONCLUSIONPBB is common in male infants. Persistent wet cough with wheezing was the main characteristic of PBB. PBB is commonly accompanied by immune dysfunction and airway malacia, and the pathogens were Streptococcus pneumoniae and Haemophilus influenzae.

4.Establishment and evaluation of anesthesia nurses training system for postoperative delirium assessment

Objective:To establish training system for postoperative delirium (POD) assessment and evaluate the efficacy of training for anesthesia nurses.Methods:Sixteen nurse anesthetists of both sexes in our hospital were selected and received the systemic training for POD assessment.The training system included questionnaire survey, theoretical teaching, simulated visit, clinical observation, independent evaluation, centralized question-answering, evaluation of efficacy and random inspection.The level of POD knowledge tests were performed before the training and at the end of the fourth week of independent evaluation, respectively.At week 1 and 4 of independent evaluation, the diagnostic rate of POD and sensitivity and specificity of the assessment were calculated, and Kappa consistency analysis was used to assess the consistency between anesthesia nurses and training group in diagnosis of POD.In the first week of the third month after the end of training, the evaluation results were randomly inspected, the POD diagnosis rate was calculated between the anesthesia nurses and the training group, and the consistency analysis was conducted.Results:Compared with the scores of POD knowledge questionnaire and sensitivity of the assessment of the anesthesia nurses in the first week of training, the scores were significantly increased ( P<0.05), and no significant change was found in the POD diagnosis rate in the fourth week of training ( P>0.05). Compared with the training group, the diagnosis rate of POD of anesthesia nurses was significantly decreased in the first week of training ( P<0.05), and no significant change was found at the fourth week of training ( P>0.05). In the first and fourth weeks of training, the Kappa value of anesthesia nurses and the training group was 0.676 and 0.954 ( P<0.001), respectively.In the first week of the third month after the end of training, the Kappa value between anesthesia nurses and the training group in diagnosis of POD was 0.862 ( P<0.05). Conclusion:The training system of POD assessment has been successfully established, and the standardized anesthesia nurses training of POD has been achieved with good results.
5.Effect of Sizi Powder (四子散) Orientation Osmotherapy on Upper Limb Motor Function in Stroke Patients with Hemiplegia:A Randomized Controlled Study

ObjectiveTo observe the clinical efficacy and safety of Sizi Powder (四子散, SP) orientation osmotherapy in the treatment of stroke patients with hemiplegia. MethodsIn this study, 94 patients with post-stroke hemiplegia were randomly divided into a control group and a treatment group with 47 patients in each group. The control group received conventional treatment, nursing and rehabilitation training, while the treatment group was treated with SP orientation osmotherapy in addition to the treatments in the control group. The treatment consisted of 20 minutes per session, once daily, 5 times a week, 2 weeks per course, with a total of 2 courses. Before the intervention, after 2 weeks intervention and after 4 weeks intervention, the upper limb motor function was evaluated by using the Fugl-Meyer Assessment (U-FMA). The modified Ashworth Scale (MAS) was used to assess upper limb spasticity, and joint range of motion was measured for shoulder flexion, extension, abduction, internal rotation, external rotation, elbow flexion, wrist palmar flexion, dorsiflexion, ulnar deviation, and radial deviation. Clinical efficacy for spasticity was evaluated after 2 and 4 weeks of treatment. Skin conditions, including burns, allergic reactions, and blisters, were monitored during the treatment process. ResultsA total of 44 patients in the treatment group and 45 patients in the control group completed the study. The results showed that after 2 and 4 weeks of intervention, the U-FMA scores of both groups increased, with the treatment group showing higher scores than the control group (P<0.05). After 4 weeks of treatment, the MAS score distribution improved significantly in both groups (P<0.05), although no significant difference was found between the groups after 2 and 4 weeks treatment(P>0.05). The total effective rate of spasticity clinical efficacy in the treatment group after 2 and 4 weeks was 15.91% (7/44) and 40.91% (18/44), respectively, while in the control group, it was 11.11% (5/45) and 22.22% (10/45). There was no significant difference after 2 weeks treatment (P>0.05), but the treatment group showed a significantly higher total effective rate after 4 weeks treatment (P<0.05). After 2 and 4 weeks of treatment, the joint range of motion in the upper limbs was greater in both groups (P<0.05). After 2 weeks of intervention, the treatment group showed greater range of motion in shoulder flexion, extension, and abduction than the control group, and after 4 weeks of intervention, the treatment group demonstrated greater range of motion in shoulder flexion, extension, abduction, internal rotation, external rotation, and elbow flexion compared to the control group (P<0.05). During the treatment, one patient in the treatment group experienced mild skin allergy, while no other severe adverse reactions, such as intense pain, itching, blisters, or infection, were observed in any patients. ConclusionSP orientation osmotherapy can effectively improve upper limb motor function and joint range of motion in stroke patients with hemiplegia. After 4 weeks of treatment, it can reduce upper limb spasticity and improve clinical efficacy, with good safety.
6.An investigation of iodine status of 6 target populations in both urban and rural areas in Xiamen City 3 years after promotion of new standard iodized salt

Objective To analyze the iodine nutritional status among pregnant women,breasffeeding women,0-2 years old infants,8-10 years old children,18-45 years old women and men in both urban and rural areas of Xiamen 3 years after promotion of new standard iodized salt.Methods The Huli and Xiang'an were selected as the urban and rural study sites in Xiamen City,from March 2015 to March 2016.Each study site was further divided into five geographic locations (east,south,west,north and center).In each location,one township was randomly selected.In each town,2 tap water samples,60 salt samples of residents,and urine samples of 40 children aged from 8-10 years old,20 pregnant women,12 breastfeeding women,0-2 years old infants,18-45 years old women and men were collected to determine the iodine level.The water iodine and urinary iodine were detected by arsenic cerium catalytic spectrophotometry;salt iodine was detected by direct titration.Results The medians of salt iodine in urban and rural were 23.0 and 25.3 μg/L,respectively.The coverage rates of iodized salt in urban and rural area were 99.3% (298/300) and 94.0% (282/300),respectively.The consumption rates of qualified iodized salt were 93.7% (281/300) and 90.7% (272/300),respectively.The medians of tap water iodine in urban and rural were 4.1 and 4.4 μg/L,respectively.In urban and rural areas,the medians of urinary iodine of pregnant women were 167.6 and 119.6 μg/L,respectively.The medians of urinary iodine of breastfeeding women were 121.6 and 101.2 μg/L,respectively.The medians of urinary iodine of infants were 165.9 and 110.5 μg/L,respectively.The medians of urinary iodine of children were 208.9 and 167.2 μg/L,respectively.The medians of urinary iodine of women aged 18-45 were 132.5 and 154.5 μg/L,respectively.The medians of urinary iodine of men aged 18-45 were 131.7 and 154.5 μg/L,respectively.Conclusions After 3 year promotion of new standard iodized salt,the iodine status of pregnant women in urban,breastfeeding women,0-2 years old infants,children aged 8 to 10 and adults aged 18 to 45 is adequate,but the pregnant women in rural has showed iodine deficiency.So surveillance and health education should be continuously strengthened in the future.Scientific salt iodization should continue to ensure the amount of iodine intake.

OBJECTIVE:
To analyze the current status of diagnosis and management of acute appendicitis (AA) in China.
METHODS:

Different centers collected and summarized their data in 2017 and sent back the questionnaires for summary.
RESULTS:
A total of 8 766 AA patients were enrolled from 43 medical centers, including 4 711 males (53.7%) with median age of 39 years and 958 (10.9%) patients over 65 years old. Of 8 776 patients, 5 677 cases (64.6%) received one or more imaging examinations, and the other 3 099 (35.4%) did not receive any imaging examination. A total of 1 858 (21.2%) cases received medical treatment, mainly a combination of nitroimidazoles (1 107 cases, 59.8%) doublet regimen, followed by a single-agent regimen of non-nitroimidazoles (451 cases, 24.4%), a nitroimidazole-free doublet regimen (134 cases, 7.2%), a triple regimen of combined nitroimidazoles (116 cases, 6.3%), nitroimidazole alone (39 cases, 2.1%) and nitroimidazole-free triple regimen (3 cases, 0.2%). Of the 6 908 patients (78.8%) who underwent surgery, 4 319 (62.5%) underwent laparoscopic appendectomy and 2589 (37.5%) underwent open surgery. Ratio of laparotomy was higher in those patients under 16 years old (392 cases) or over 65 years old (258 cases) [15.1%(392/2 589) and 10.0%(258/2 589), respectively, compared with 8.5%(367/4 316) and 8.0%(347/4 316) in the same age group for laparoscopic surgery, χ²=91.415, P<0.001; χ²=15.915,P<0.001]. Patients with complicated appendicitis had higher ratio of undergoing open surgery as compared to those undergoing laparoscopic surgery [26.7%(692/2 589) vs. 15.6%(672/4 316), χ²=125.726, P<0.001].The cure rates of laparoscopic and open surgery were 100.0% and 99.8%(2 585/2 589) respectively without significant difference (P=0.206). Postoperative complication rates were 4.5%(121/2 589) and 4.7%(196/4 316) respectively, and the difference was not statistically significant (χ²=0.065, P=0.799). The incidence of surgical site infection was lower (0.6% vs. 1.7%, χ²=17.315, P<0.001), and hospital stay was shorter [6(4-7) days vs. 6(5-8) days, U=4 384 348.0, P<0.001] in the laparoscopic surgery group, while hospitalization cost was higher (median 12 527 yuan vs. 9 342 yuan, U=2 586 809.0, P<0.001).
CONCLUSIONS
The diagnosis of acute appendicitis is still clinically based, supplemented by imaging examination. Appendectomy is still the most effective treatment at present. Laparoscopic appendectomy has become the main treatment strategy, but anti-infective drugs are also very effective.
