Objective To analyze the current situation of TB control in Hefeng County and probe into the role of medical institutions in TB control. Methods In-depth interviews were conducted with health workers of TB prevention, TB patients and their family members; data on registered TB patients were collected; the current situation of TB control was analyzed and countermeasures were put forward. Results The TB epidemic was pretty serious in Hefeng County, with a high morbidity rate and a low cure rate. According to an epidemiological survey in 1990, the TB morbidity rate in the county was 1000/100000. Conclusion Hefeng County failed to give full play to the role of hospitals in TB control and the treatment of TB patients was separated from their management. Hence the deplorable results of TB control. The following countermeasures were presented: ①giving full play to the role of general hospitals in TB control; and ②perfecting the three-level medical networks of prevention and healthcare.

Xylanase is the key enzyme to degrade xylan that is a major component of hemicellulose. The enzyme has potential industrial applications in the food, feed, paper and flax degumming industries. The use of xylanases becomes more and more important in the paper industry for bleaching purposes. Xylanases used in the pulp bleaching process should be stable and active at high temperature and alkaline pH. Thermophilic and alkalophilic xylanases could be obtained by screening the wild type xylanases or engineering the mesophilic and neutral enzymes. In this paper, we reviewed recent progress of screening of the thermophilic and alkalophilic xylanases, molecular mechanism of thermal and alkaline adaptation and molecular engineering. Future research prospective was also discussed.

Objective T o investigate the m ain point of long backbone fracture caused by blunt force in forensic clinical identification and to provide a reference for the inspection and appraisal practices of such injury. Methods N inety-nine cases of adult long backbone fractures were collected from January 2006 to D ecem ber 2013 in G utian C ounty of Fujian Province. A ccording to the term s of fracture loca-tion, m ode of injury, type, the data were sum m arized. Results In the 99 cases, there were 36 cases caused by hitting, kicking, and falling and 63 cases caused by vehicle collision. T he m ajority of the for-m er was ulna, and those of the latter were tibia and fibula. T he types of fracture were transverse one, short oblique one, long oblique one, and spiral one. Conclusion D ifferent types of long backbone frac-ture, not only causing stress load of fractures as well as structural differences related to each segm ent.

Objective To assess the carotid elasticity using wave intensity(WI) in subjects with different plasma glucose level. Methods 107 subjects were enrolled in this study, according to their plasma glucose levels, subjects were categorized as normal plasma glucose group (group A),normal plasma glucose in higher level group (group B) and pre-diabetes group (group C).Carotid WI examination was performed and analyzed. The parameters included magnitude of the peak during late systole (W2),negative area during the mid-ejection (NA),and stiffness parameter (β),pressure strain elasticity modulus (Ep),arterial compliance(AC),augmentation index(AI),one point pulse wave velocity(PWVβ).Results Compared to group A,W2,β,Ep,PWVβ increased significantly while AC decreased in group C(P <0.05),but there was no obvious difference of NA between two groups.Furthermore, this statistically difference was not found in group B(P >0.05).Conclusions Carotid elasticity have been altered in pre-diabetes group which can be evaluated by WI,but no marked change is observed in normal plasma glucose of higher level group.

Objective To investigate the effect of berberine on gut microbiota and T helper cell 17 (Th17), regulatory T cell (Treg) cell in sleep deprived rats. Methods SD rats were randomized into control group, model group, low-dose and high-dose group (BBR1 and BBR2, 100 mg/kg and 200 mg/kg, administrated orally).Sleep deprived rat model was established by the small-platforms-over-water method. The number of bacteria in rectum content of rat was detected. The ratio of Th17/Treg was evaluated by flow cytometry. The expression of interleukin 17 (IL-17), RAR-related orphan receptor (ROR) C, and Forkhead Box Protein P3 (Foxp3) mRNA was evaluated. Results The counting of Clostridium perfringens was elevated (P<0.05), the amount of other microbiota decreased (P<0.05), Th17/Treg ratio(P<0.05), IL-17 and RORC expression enhanced (P<0.05), Foxp3 expression decreased (P<0.05) in the gut of model rats.In contrast, treatment with berberine inhibited the proliferation of Clostridium perfringens(P<0.05), and promoted the growth of other microbiota (P<0.05), and dampened Th17/Treg ratio (P<0.05), regressed IL-17 and RORC expression, augmented Foxp3 expression. Conclusions Various doses of berberine can counteract gut microbiota imbalance and Th17/Treg imbalance induced by sleep deprivation.

Objective Myofascial pain syndrome (MPS) is a common muscle function disturbance. This study was to investigate the clinical value of acoustic radiation force impulse (ARFI) imaging in assessing the therapeutic effect of acupuncture for patients with trapezius MPS.Methods A total of 116 patients diagnosed with trapezius MPS received acupuncture therapy once a day for 10 days in Pudong Hospital from August 2015 to June 2016.Using ARFI imaging, we recorded the shear wave velocity (SWV) of the trapezius and obtained the short-form McGill Pain Questionnaire (SF-MPQ) scores of the patients before, at the end of, and at 7, 15 and 30 days after treatment. Then, we analyzed the interaction and correlation between the SWV values and SF-MPQ scores of the patients at different time points. Results Both the SF-MPQ scores and SWV values were first decreased, falling to lowest values (5.19 points and 1.43 m/s) at 7 days after treatment, and then increased again. The interaction between the SF-MPQ scores and SWV values were significantly different at different time points (Finteraction=2.553, Pinteraction=0.038), and there was a strong linear correlation between the two indexes (r=0.755, P=0.000).Conclusion ARFI imaging combined with SF-MPQ has a certain clinical value in assessing the effect of acupuncture on trapezius MPS.

ObjectiveTo study the videourodynamic characteristics in spina bifida patients and to discuss the urodynamic risk factors causing the upper urinary tract deterioration.Methods33 cases with spina bifida were evaluated with non-complete synchronized videourodynamic test.Filling-phase function of bladder was evaluated with bladder sensation, detrusor activity, compliance, relative safe capacity, detrusor leak point pressure and presence of vesicoureteral reflux;voiding phase function was evaluated with detrusor pressure, the synergy between detrusor and external sphincter, the pressure-flow study.Patients were classified into upper urinary tract damage group and non-damage group according to the imaging and renal function test. The urodynamic parameters between the two groups were compared statistically.Results17 cases were found upper urinary tract damage(51%)in which 11 with vesicoureteral reflux and 6 was non-reflux hydronephrosis.Bladder compliance and relative safe capacity were significantly different between the two groups.ConclusionSpina bifida patients complicate a high incidence of upper urinary damage, especially vesicoureteral reflux.Low compliance and small relative safe capacity may be the major urodynamic risk factors causing upper urinary tract damage.
