1.Study on the immune status of blood donors in Yangzhou after SARS-COV-2 vaccinating

【Objective】 To investigate the immune status of blood donors in Yangzhou area after SARS-COV-2 vaccinating. 【Methods】 Among 112 voluntary blood donors from August 29 to September 22, 2021, 111 were vaccinated with SARS-COV-2 vaccine.IgM antibody(by enzyme-linked immunocapture method), IgG antibody(by indirect method of combined immunoassay)and IgG antibody titer were detected. 【Results】 A total of 99.11% (111/112 ) voluntary blood donors were vaccinated, two-shot(n=103), one-shot(n=1) and three-shot (n=7) accounting for 91.96%, 0.89% and 6.25%, respectively.Eighty-eight (78.57%) were positive for IgG antibodies, and 14 (12.5%) were positive for IgM antibodies.No statistically significant difference was found in IgG and IgM positive yielding between males and females (P>0.05). The proportion (0.89%, 1/112) of positive IgM in blood donors with blood type A was significantly lower than that of other blood types (P<0.01). The IgG antibody titer of blood donors maintained rather high level within 6 months after vaccinating.47.66% of the donors presented antibody titer more than 160, and 5.60% had IgM antibody been detected within 1 month after vaccinating. 【Conclusion】 At present, the SARS-COV-2 vaccination effect in China is generally good.Since IgG antibodies cannot be detected after 6 months, it is suggested to perform IgG antibody testing for donors who have completed the second dose for more than 6 months.For those IgG antibody negative, booster shots should be conducted.For donors with high IgG antibody titer, their plasma may be considered to replace with COVID-19 convalescent plasma for the treatment of patients with rapid disease progression, or severe/critically ill patients diagnosed with COVID-19, so as to avoid the risk of COVID-19 re-spreading during convalescent plasma collection in blood centers. For blood donors with positive IgM antibodies, it is recommended to follow up the NAT results to minimize the risk of transmission.
2.Effects of health information literacy, perceived benefits and perceived risk on the willingness to use nursing skills of traditional Chinese medicine in elderly residents

Objective To explore the effects of health information literacy (HIL), perceived benefits and perceived risk on the willingness to use nursing skills of traditional Chinese medicine(TCM) in elderly residents. Methods From August to December 2022, 180 elderly residents were randomly selected from the Physical Examination Center of a grade A hospital in Yangzhou of Jiangsu Province by convenient sampling method. The questionnaire of general situation, Scale of HIL of TCM Health Care, and Questionnaire on the Acceptance of TCM Nursing Technology in Middle-aged and Elderly Residents in Community. The influence factors were analyzed by Chi-square test and multiple linear stepwise regression analysis, and the path relationship was constructed by structural equation model. Results A total of 151 valid questionnaires were collected. Education level, self-evaluation of health, the acceptance scores of participants receiving the TCM nursing skills or not, the health information literacy in TCM, perceived usefulness, perceived ease of use, and use concerns were the influencing factors of the willingness of elderly residents, which could explain 54.30% of the total variation. Perceived benefit and perceived risk both had mediated effects between health information literacy and willingness to use, accounting for 67.48% and 32.52% of the total effect, respectively. Conclusion In the process of the guidance and promotion of TCM nursing skills, medical institution should pay attention to the influence mechanism of health information literacy, perceived benefits and perceived risk on the use willingness of elderly residents, and improve the service model.
3.Diagnostic value of exhaled volatile organic compounds in pulmonary cystic fibrosis: A systematic review

Objective To explore the diagnostic value of exhaled volatile organic compounds (VOCs) for cystic fibrosis (CF). Methods A systematic search was conducted in PubMed, EMbase, Web of Science, Cochrane Library, CNKI, Wanfang, VIP, and SinoMed databases up to August 7, 2024. Studies that met the inclusion criteria were selected for data extraction and quality assessment. The quality of included studies was assessed by the Newcastle-Ottawa Scale (NOS), and the risk of bias and applicability of included prediction model studies were assessed by the prediction model risk of bias assessment tool (PROBAST). Results A total of 10 studies were included, among which 5 studies only identified specific exhaled VOCs in CF patients, and another 5 developed 7 CF risk prediction models based on the identification of VOCs in CF. The included studies reported a total of 75 exhaled VOCs, most of which belonged to the categories of acylcarnitines, aldehydes, acids, and esters. Most models (n=6, 85.7%) only included exhaled VOCs as predictive factors, and only one model included factors other than VOCs, including forced expiratory flow at 75% of forced vital capacity (FEF75) and modified Medical Research Council scale for the assessment of dyspnea (mMRC). The accuracy of the models ranged from 77% to 100%, and the area under the receiver operating characteristic curve ranged from 0.771 to 0.988. None of the included studies provided information on the calibration of the models. The results of the Prediction Model Risk of Bias Assessment Tool (PROBAST) showed that the overall bias risk of all predictive model studies was high, and the overall applicability was unclear. Conclusion The exhaled VOCs reported in the included studies showed significant heterogeneity, and more research is needed to explore specific compounds for CF. In addition, risk prediction models based on exhaled VOCs have certain value in the diagnosis of CF, but the overall bias risk is relatively high and needs further optimization from aspects such as model construction and validation.
4.Expression and significance of Survivin and Smac in ovarian mucinous tumors.

OBJECTIVETo investigate the expressions and significances of Survivin and Smac in ovarian mucinous tumors.
METHODSA total of 55 paraffin-embedded specimens of primary ovarian mucinous tumors were collected. SABC was used to detect protein expression of Survivin and Smac genes. Immunoelectron microscopy using colloidal gold labeling was performed to determine the subcellular localization and patterns of Smac protein expression.
RESULTS(1) The cytoplasmic expression rates of survivin in benign, borderline and malignant ovarian mucinous tumors were 2/20, 12/15 and 20/20 respectively, which presents an improving trend.There were significant differences of survivin expression between benign vs. borderline lesions (P < 0.01), and benign vs. malignant tumors (P < 0.01). The nuclear expression rates of survivin in benign, borderline and malignant ovarian mucinous tumors were 1/20, 6/15 and 5/20, respectively, which presents a.declining trend.There was significant difference of survivin expression between benign vs. borderline tumors (P < 0.05). The positive expression rates of Smac among the three groups were 19/20, 9/15 and 3/20, respectively. There was significant difference among the three groups (P < 0.01 or < 0.05). There was a negative correlation between Survivin and Smac (r = -0.153, P < 0.01). (2) Colloidal gold labeling study demonstrated that mitochondrion intramembranous storage of Smac granules in the three groups were 24.1 ± 7.2, 11.1 ± 1.9 and 5.2 ± 1.7, respectively, and there were significant differences among the three groups (P < 0.01 or < 0.05). The extramemebranous Smac granules were 4.7 ± 3.0, 2.9 ± 1.0 and 1.7 ± 1.3, although without significant difference among the three groups (P > 0.05).
CONCLUSIONSWith the malignant development of ovarian mucinous tumors, the expressions of Survivin are up-regulated, and the expressions of Smac are down-regulated. Smac proteins exist mainly in an inactive intramembranous storage form inside of mitochondria.

5.Investigation of apoptosis mechanism of arsenic trioxide on oral squamous cell carcinoma.

OBJECTIVETo probe the possible mechanism of growth-inhibitory and apoptosis of oral squamous cell carcinoma by arsenic trioxide.
METHODSThe induction of apoptosis in two tongue squamous carcinoma cells treated by arsenic trioxide was investigated. The morphology changes of the cells was observed under electron microscope. The mitochondrial transmembrane potential was detected using rhodamine 123 and flow cytometry. The cell cycle was detected by flow cytometry, and p16, p53, BCL-2, Caspase-3, and PARP changes were examined by western blot.
RESULTS1. The antiproliferative effect on the oral squamous carcinoma cells by arsenic trioxide was carried out through two ways: induction of apoptosis and toxicity damage. 2. Activation of the caspase-3 and PARP, while no changes of p16, p53, BCL-2 occurred. 3. Mitochondrial transmembrane potential collapse and G(2)-M stagnation were correlated with apoptosis of oral squamous cell carcinoma.
CONCLUSIONS1. Tubulins and mitochondria may be the chief action position of arsenic trioxide, which is the start-up factors of mechanism. 2. Activation of the caspase-3 proteolytic pathway may be one of the pivotal ways of apoptosis procedure induced by arsenic trioxide.

6.Value of C-PLAN index as a prognostic indicator of advanced esophageal cancer patients treated with immune checkpoint inhibitors

Objective To evaluate the prognostic value of C-reactive protein (CRP), performance status (PS), lactate dehydrogenase (LDH), albumin (ALB) and derived neutrophil-to-lymphocyte ratio (dNLR) composite index (C-PLAN) as a prognostic indicator in advanced esophageal cancer patients treated with immune checkpoint inhibitor (ICI). 7.Clinical and immune response characteristics among vaccinated persons infected with SARS-CoV-2 delta variant: a retrospective study.

OBJECTIVES:
This study aimed to observe the clinical and immune response characteristics of vaccinated persons infected with the delta variant of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in Yangzhou, China.
METHODS:
We extracted the medical data of 129 patients with delta-variant infection who were admitted to Northern Jiangsu People's Hospital (Yangzhou, China) between August and September, 2021. The patients were grouped according to the number of vaccine doses received into an unvaccinated group: a one-dose group and a two-dose group. The vaccine used was SARS-CoV-2-inactivated vaccine developed by Sinovac. We retrospectively analyzed the patients' epidemiological, clinical, laboratory, and imaging data.
RESULTS:
Almost all patients with delta-variant infection in Yangzhou were elderly, and patients with severe/critical illness were over 70 years of age. The rates of severe/critical illness (P=0.006), fever (P=0.025), and dyspnea (P=0.045) were lower in the two-dose group than in the unvaccinated group. Compared to the unvaccinated group, the two-dose group showed significantly higher lymphocyte counts and significantly lower levels of C-reactive protein (CRP), interleukin-6 (IL-6), and D-dimer during hospitalization and a significantly higher positive rate of immunoglobulin G (IgG) antibodies at admission (all P<0.05). The cumulative probabilities of hospital discharge and negative virus conversion were also higher in the two-dose group than in the unvaccinated group (P<0.05).
CONCLUSIONS
Two doses of the SARS-CoV-2-inactivated vaccine were highly effective at limiting symptomatic disease and reducing immune response, while a single dose did not seem to be effective.

8.Clinical Value of Four-hole Unilateral Dissecting Lobectomy and Mediastinal Lymph Node Dissection in the Treatment of Early Non-small Cell Lung Cancer.

BACKGROUND:
Thoracoscopic lobectomy combined with mediastinal lymph node dissection has been considered as one of the standard surgical procedures for early lung cancer. After 20 years of development, thoracoscopic lobectomy has reached a consensus on reliability and minimally invasive. At present, thoracoscopic lobectomy has a variety of incisions, which gradually evolve into four holes based on three holes, and two or one hole as the operative approach. The aim of this study was to evaluate the clinical value of four-hole unilateral dissecting lobectomy and mediastinal lymph node dissection in the treatment of non-small cell lung cancer (NSCLC). The aim of this study was to investigate the clinical value of anatomical lobectomy with mediastinal lymphadenectomy under four-hole completely video-assisted thoracoscopic surgery (C-VATS) in the treatment of non-small cell lung cancer.
METHODS:
The patients undergoing lobectomy with mediastinal lymphadenectomy for NSCLC were identified in the Department of Thoracic Surgery, Yangzhou First People's Hospital, Yangzhou University from March 2015 to July 2016. Preoperative clinical diagnosis of peripheral-type early NSCLC. The patients were randomly divided into four-hole monophasic group (experimental group) and three-hole group (control group) according to the number of hospitalization before surgery. According to inclusion and exclusion criteria, the 39 cases assign in experimental group and 34 cases in the control group, including 36 males and 37 females; aged 38 to 84 years. The mean operation time, average blood loss, lymph node dissection group, average drainage, average extubation time and postoperative complications were compared between the two groups for statistical analysis.
RESULTS:
The two groups of patients were successfully completed surgery, no death after surgery. Mean bleeding in the two groups, the number of lymph node dissection group, the average postoperative drainage, the average time of extubation, postoperative complications, with no significant difference. The average operation time of the four-hole unidirectional group was shorter than that of the three-hole group. The difference was statistically significant (P<0.05).
CONCLUSIONS
The safety and efficacy of a four-hole one-way operation under VATS are satisfactory. The operation is smooth during operation, which shortens the course of operation and deserves the clinical promotion.

9.Generation of transgenic mice expressing human lysozyme in mammary gland.

OBJECTIVETo evaluate the feasibility of generating animal mammary gland bioreactors expressing human lysozyme (hLYZ).
METHODSThe recombinant vector p205C3-hLYZ, as a result of connecting the hLYZ cDNA with the mammry gland expression vector p205C3, was used to generate transfer genic mice by microinjection.
RESULTSA total of 136 F0 mice were obtained, of which 7 (2 females and 5 males) and 4 (1 females and 3 males) were found to contain the transfer-gene by PCR and Southern blotting respectively. The results of Western blotting indicated that the expressed protein had the same molecular weight as that of normal hLYZ. From the F1 generation on, the mice mated only with their brothers or sisters and a colony of F7 transgenic mice was obtained. Among the offspring, the female transgenic mice maintained and expressed the transfer-gene stably with an expression level as high as 750 mg/L. The expressed protein had strong tissue specificity, and in addition to the mammary glands, some degree of ectropic expression in the spleens and intestines of the transgenic mice was confirmed by dot blotting assay.
CONCLUSIONThese data indicate that the mice mammary gland bioreactors expressing hLYZ have been successfully generated.

10.Expressions of STAT3, VEGF and C-myc in seminoma and their significance.

OBJECTIVETo understand the role of the JAK/STAT3 signal transduction pathway in the pathogenesis of seminoma by studying the expressions of signal transducers and activators of transcription-3 (STAT3), vascular endothelial growth factor (VEGF) and C-myc in seminoma.
METHODSWe examined 38 paraffin specimens of seminoma and 10 samples of normal human testes by immunohistochemical staining using the antibodies of STAT3, VEGF and C-myc, and observed the staining intensity under the light microscope.
RESULTSThe positive expression rates of STAT3, VEGF and C-myc were 76.3%, 71.1% and 84.2%, respectively, with statistically significant differences from the corresponding protein expressions in the normal testis tissues (P < 0.01), which gradually increased with the clinical stages of tumor, nodes and metastasis (TNM) classification. Different correlations were observed among the STAT3, VEGF and C-myc proteins in seminoma: STAT3 positively correlated with VEGF (r = 0.640, P < 0.01) and C-myc (r = 0.408, P < 0.05); C-myc positively correlated with VEGF (r = 0.459, P < 0.01).
CONCLUSIONThe JAK/STAT3 signal transduction pathway can facilitate the development and metastasis of seminoma by activating the expression of VEGF and meanwhile induce the malignant proliferation of primary reproductive cells and promote the pathogenesis and progression of seminoma by activating the expression of C-myc.
