1.Common problems and countermeasures on medical equipment bidding in public hospitals

Objective To analyze the problems frequently found in medical equipment bidding at public hospitals and come up with countermeasures for building a scientific and standardized bidding procedure.Methods 168 cases of medical equipment bidding of the hospital in the recent five years served as objects of study,for a full-process longitudinal study.It covered the preparation of technical parameters,compilation of bidding documentation,bid opening/evaluation process,all the way to final acceptance and after-sales service,and complaints responses.Results In the 168 cases,imported equipments accounted for 86.90%;only 16.07% of the purchases involved consumables,yet their importance deserved high attention;59.52% of the equipments involved after-sales maintenance costs,calling for regular maintenance to ensure smooth operation of the equipments.Conclusions Quality of domestic equipments should be enhanced,for optimization of the ratio between domestic and imported equipments;equipment purchase,after-sales service and consumables purchase should be taken into account in general;scientific evaluation levels and standards should be established to enhance supervision,for the purpose of scientific and rational purchase of medical equipments.

3.Analysis of chemical constituents in Capparis spinosa by GC-MS

Objective To analyze the chemical constituents in fruits of Capparis spinosa by GC-MS analysis .Methods The compounds were extracted from Capparis spinosa by 70% ethanol after smash .After extraction by petroleum ether (boil-ing range was from 60 to 90 ℃) ,the compounds were analyzed by gas chromatography with a VF-5ms capillary column .The column was heated up from 80 ℃ to 300 ℃ ,15 ℃/min ,then maintained for 10 minutes ,vaporization temperature was 250 ℃ , carrier gas was Helium and the flow rate was 1 ml/min .The detection was carried out by mass spectrometry using electron im-pact ion source with 70 eV ionization voltages .The ion source temperature was 200 ℃ and split ratio was 30∶1 .The injection volume was 1 .0 μl .The content of the compounds was determined with peak area normalization method .Results 53 chroma-tographically peaks were separated and 48 chemical constituents were identified including hexadecanoic acid (21 .82% ) ,octade-canoic acid (7 .49% ) ,oleinic acid (42 .93% ) and monoolein (2 .39% ) .These chemical constituents were mainly saturated fatty acid ester ,unsaturated fatty acid ester and alkanes .Conclusion The method is rapid ,accurate and sensitive with the usage of GC-MS ,which could be used to identify main chemical constituents of Capparis spinosa in fruit .
4.Analysis of main problems and countermeasures in the construction of advanced stroke center in China

Objective:To understand main problems existing in the construction of advanced stroke centers in China and put forward solutions, for reference in promoting the standardization construction of advanced stroke centers and improving the efficiency of acute stroke treatment.Methods:The data were derived from relevant data of on-site export guidance in the construction of advanced stroke centers at 175 tertiary hospitals from 2020 to 2021, and the scores of on-site evaluation indicators for the establishment of stroke centers and their formal approval were compared and analyzed. Based on on-site investigation and expert consultation, the common problems existing in the construction of advanced stroke centers were summarized. All data were analyzed by descriptive analysis, the scores of on-site evaluation indicators were expressed by ± s, and paired t test was used for comparison between groups. Results:Compared with the total score(693.04±72.06) of on-site evaluation at the stage of project launch, the total score(747.94±78.10) of on-site evaluation for formal approval of stroke centers of 70 hospitals was higher, and the difference was significant( P<0.01). There were seven common problems in the construction of stroke centers in 175 hospitals, including insufficient attention paid by hospitals, lack of effective performance incentive policies, imperfect treatment procedures and medical norms, and so on. Conclusions:Experts on-site guidance plays an important role in the construction of stroke centers in China. At present, there were still problems to tackle in the construction of stroke centers in hospitals. In order to promote the standardized construction of stroke centers in China and improve the efficiency of stroke treatment, the authors suggest fuorther strengthening the importance attached by hospital leadership and the coordination and organization of functional departments, establishing stroke center models conforming to the actual situation of the hospital, seting up the post of brain and heart health manager, and improving the regional prevention and treatment level of acute stroke.
5.Effect of Marsdeniae Tenacissimae Caulis on Human Osteosarcoma Cells Based on JAK1/STAT3 Signaling Pathway

Objective To investigate the effects and potential mechanisms of Marsdeniae Tenacissimae Caulis(Tongguanteng)injection and extract in human osteosarcoma cells proliferation,migration,invasion,and apoptosis.Methods MNNG/HOS,Saos-2 osteosarcoma cells,and normal bone marrow mesenchymal stem cells(BMSC)were cultured in vitro.Cells were incubated with different concentrations of Tongguanteng injection and Tongguanteng extract(40,60,80 mg/mL).Cell proliferation was evaluated by CCK-8 assay and plate colony formation assay.Cell migration and invasion were evaluated by scratch assay and Transwell assay.Cell apoptosis was evaluated by Hoechst33342 staining and Annexin-V/PI double staining assay.Bax,Bcl-2 and Caspase-3 mRNA expression were detected using RT-qPCR.The protein expressions of JAK1,p-JAK1,STAT3,p-STAT3 and MMP9 were detected by Western blot.Results Compared with the control group,both Tongguanteng injection and extract significantly decreased the survival rate of MNNG/HOS and Saos-2 cells,inhibited cell clone formation,migration,and invasion,induced cell apoptosis(P<0.05,P<0.01),promoted Bax mRNA and protein expression,inhibited Bcl-2 mRNA and protein expression,and up-regulated Caspase-3 mRNA and Cleaved Caspase-3 protein expression.Tongguanteng injection could significantly down-regulate the expressions of p-JAK1,p-STAT3 and MMP9 protein expression in Saos-2 cells(P<0.05,P<0.01).Conclusion Both Tongguanteng injection and Tongguanteng extract can significantly inhibit proliferation,migration and invasion of human osteosarcoma MNNG/HOS and Saos-2 cells,and induce apoptosis,with no significant difference in anti-tumor effect.The mechanism may be related to the inhibition of the activation of JAK1/STAT3 signaling pathway.
6.Research and clinical application of scrotal rapid rehabilitation bandage

[Objective] To explore the effectiveness of scrotal rapid rehabilitation bandage (scrotal girdle) in preventing postoperative complications of varicocele. [Methods] A total of 80 patients with varicocele who underwent microscopical varicocele ligation during Dec.2021 and May 2023 were divided into the experimental group (n=40) and control group (n=40) by the random number table method.All patients received wound dressing one week after surgery and stopped using it after wound healing.Additionally, the experimental group was treated with scrotal girdle for another 8 weeks.The preoperative and postoperative scores of scrotal edema on the 2nd, 4th and 6th day after surgery, satisfaction, length of hospital stay, frequency of dressing changes, pain intensity, complications, serum testosterone level and semen routine at the 12th week after surgery were compared between the two groups. [Results] The score of scrotal edema in the experimental group was (1.12±0.38) on the 6th day after operation, which was significantly lower than that in the control group (1.68±0.50) (P<0.05). The hospitalization time of the experimental group was significantly shorter than that of the control group, and the frequency of dressing changes was also significantly reduced, with statistical significance (P<0.05). The patient satisfaction was 95% in the experimental group higher than 80% in the control group (P<0.05). The sperm density was (54.12±6.23) ×106/L and the sperm motility rate was (66.45±0.44)% in the experimental group, while the sperm density was (41.53±8.51) ×106/L and the sperm motility rate was (57.85±0.47)% in the control group.The sperm density, sperm motility rate, and sperm malformation rate of both groups were significantly improved after surgery, and the sperm density and sperm motility rate of the experimental group improved more significantly than those of the control group, with statistical significance (P<0.05). [Conclusion] The scrotal rapid rehabilitation bandage is more advantageous than traditional external bandaging materials for patients with varicocele after operation, which can accelerate the postoperative rehabilitation of patients and has more clinical practicability.
7.Potential mechanism of Sophora flavescens against breast cancer via network pharmacology and molecular docking

Objective To analyze the main active components and potential molecular mechanism of Sophora flavescens against breast cancer based on network pharmacology and molecular docking. Methods The chemical constituents were collected and screened by TCMSP, ETCM database and literature review. The targets of active ingredients were predicted by Swiss Target Prediction database. Breast cancer-related targets were collected by GeneCards, TTD, Drugbank and OMIM. The anti-breast cancer targets of Sophora flavescens were screened by Venny 2.1.0 software. Cytoscape software was used to construct the network diagram of Sophora flavescens-key active ingredients-targets. STRING database was used to analyze the common targets, and PPI network diagram was constructed. GO function enrichment analysis and KEGG pathway enrichment analysis of key target proteins were performed by DAVID database and Hiplot online platform. Schrodinger software was used to calculate the molecular docking between the active ingredients and targets. Molecular biological methods were used to verify the key targets. Results A total of 36 active components with clear structures were screened from Sophora flavescens. 70 anti-breast cancer targets of Sophora flavescens were screened out. 12 core targets including EGFR, AKT1, ESR1, SRC, CYP19A1, AR and ABCB1 participate in endocrine resistance, EGFR tyrosine kinase inhibitors and estrogen signaling pathways in breast cancer. Moreover, the docking score between the core component and the key target AR is the highest. In vitro experiments showed that the extract of Sophora flavescens can inhibit the proliferation of breast cancer cells, induce cell apoptosis and up-regulate AR protein expression. Conclusion It was revealed that Sophora flavescens plays an anti-breast cancer role by regulating complex biological processes through multiple components acting on multiple targets and signaling pathways. The upregulation of AR protein by Sophora flavescens may become a new therapeutic strategy for the treatment of breast cancer.
8.Prevalence and genetic characteristics of Cryptosporidium infections among HIV-positive individuals in Jiangxi Province

Objective To investigate the prevalence of Cryptosporidium infection and the distribution of parasite species and genotypes among HIV-positive individuals in Jiangxi Province. Methods HIV-positive individuals' sociodemographic and clinical data were collected from three AIDS designated hospitals in Jiangxi Province from January 2022 to March 2023. Subjects' stool samples were collected, and genomic DNA was extracted from stool samples. Nested PCR assay was performed based on the small subunit ribosomal RNA (SSU rRNA) gene of Cryptosporidium, and Cryptosporidium gp60 gene was amplified in stool samples positive for the SSU rRNA gene. The second-round PCR amplification product was checked with 1.5% agarose gel electrophoresis, and the products of suspected positive amplifications were sequenced, followed by sequence alignment. The phylogenetic tree was created using the Neighbor-Joining method with the software MEGA 11.0, to characterize the species, genotypes and sub-genotypes of Cryptosporidium. Results A total of 382 HIV-positive individuals were enrolled, with two cases identified with Cryptosporidium infection (0.52% prevalence), and both cases had no abdominal pain or diarrhea. Following sequencing and sequence alignment, the gene sequences of these two Cryptosporidium isolates shared 99.76% and 99.88% similarity with the gene sequence of C. meleagridis isolates. Phylogenetic analysis based on the Cryptosporidium SSU rRNA gene sequence identified the species of these two Cryptosporidium-positive stool samples as C. meleagridis. Following nested PCR amplification of the Cryptosporidium gp60 gene, sequencing and sequence alignment, the two C. meleagridis isolates were characterized as III eA17G2R1 and III bA25G1R1a sub-genotypes, and the sub-genotype III bA25G1R1a was firstly described in humans. Conclusion The prevalence of Cryptosporidium is low among HIV-positive individuals in Jiangxi Province. The likelihood of Cryptosporidium infection cannot be neglected among HIV-positive individuals without diarrhea.
