1.Caries status and related factors among 12-year-old children in Yangpu District, Shanghai

ObjectiveTo investigate the prevalence and risk factors of dental caries and its related influencing factors in 12-year-old children in Yangpu District, Shanghai, and to provide relevant data support for the formulation of oral health policies and measures for children. MethodsIn 2021, 839 12-year-old children were randomly selected from five ordinary junior high schools in Yangpu District. The investigation included an oral health examination and a questionnaire survey on factors related to dental caries. Logistic regression analysis was used to analyze the influencing factors of dental caries. ResultsAmong the 839 participants, data from 830 were included. Among them, 426 were male (51.3%) and 404 were female (48.7%). Of the 830 children, 203 had dental caries, resulting in a caries prevalence of 24.46%, a caries supplemental filling ratio of 32.6%, and a DMFT of 0.63. Multivariate logistic regression analysis revealed that a mother’s education level of college or above (OR=0.849,95% CI: 0.732‒0.984), brushing teeth twice a day (OR=0.827,95%CI: 0.703‒0.973), and the caregiver brushing teeth twice a day (OR=0.843, 95%CI: 0.726‒0.983) were effective factors in reducing the caries rate. Factors associated with childhood dental caries included consuming sweets ≥1 time per day (OR=1.397,95%CI: 1.197‒1.342) and female gender (OR=1.482,95%CI: 1.347‒1.671). ConclusionThe prevalence of dental caries among 12-year-old children in Yangpu District is 24.46%. Gender, maternal education, the frequency of children's toothbrushing, the frequency of sweet consumption, and the caregiver’s toothbrushing frequency are the main factors affecting the prevalence of dental caries in children.
2. Preparation of resveratrol-DPPC liposomal dry powder inhalations and its antibacterial activities

Objective To prepare the resveratrol-DPPC liposomal dry powder inhalations (RDLDPIs), and solve the problem of lower bioavailability of resveratrol after oral administration. Methods The RDLDPIs were prepared by film-dispersion and freeze drying. The formulation was optimized by orthogonal design. The particle size, entrapment efficiency, electric potential, in vitro release, and lung deposition were characterized. The broth microdilution method was used to evaluate the antibacterial activity and determine the minimal inhibitory concentration (MIC) in vitro. Results The optimized ratio of DPPC/cholesterol, resveratrol/DPPC, and mannitol/DPPC was 3:1, 1:3, and 2:1 in the prescription of RDLDPIs, respectively; And hydration time was 15 min. The entrapment efficiency of RDLDPIs was (69.8 ± 1.6)%, the drug loading was (2.4 ± 0.9)%, the particle size was (191.5 ± 4.5) nm, and the zeta potential was (12.4 ± 1.5) mV. The aerodynamic particle size of the powder was (3.2 ± 0.2) μm and the in vitro pulmonary deposition ratio was 28.1% in vitro. The antibacterial activity against Staphylococcus aureus, Acinetobacter baumannii, Streptococcus pneumoniae, and Pseudomonas aeruginosa was evaluated in vitro. The crude drug group had no antibacterial activity in four species of bacteria, while RDLDPIs had antibacterial activity of Staphylococcus aureus and Acinetobacter baumannii, and MIC was 4 mg/mL and 2 mg/mL, respectively. Conclusion The prepared RDLDPIs have small size and uniform distribution with good stability and high lung deposition rate and in vitro antibacterial activity against Staphylococcus aureus and Acinetobacter baumannii, which is expected to be an substitute for traditional antibiotics for the treatment of bacterial pneumonia.
3.Epidemiological characteristics and prediction of varicella trends in Yangpu District, Shanghai, from 2005 to 2022

ObjectiveTo analyze the epidemiological characteristics of varicella in Yangpu District, Shanghai from 2005 to 2022, predict the trend of varicella in Yangpu District in 2023, and provide evidence for prevention and control of varicella outbreaks. MethodsInformation of varicella cases reported in Yangpu District from 2005 to 2022 was obtained from the China Information System for Disease Control and Prevention. Descriptive statistics was used to characterize the varicella epidemiology. An autoregressive integrated moving average (ARIMA) model was established by using the number of cases per month from 2005 to2022 to predict the trend of varicella epidemics in Yangpu District in 2023. The varicella incidence in 2022 was used to evaluate the fitness of the ARIMA model. ResultsFrom January 2005 to December 2021, a total of 11 527 cases of varicella were reported in Yangpu District, Shanghai. After excluding duplicates and clinical diagnoses, 11 413 cases were included into the analysis. The annual average incidence rate was 51.87/105, the age of onset was mainly under 20 years old (66.5%), and the occupation was mainly students (49.7%). The ARIMA (1,1,0)×(0,1,1)12 model was constructed and showed a good fitness while using monthly reported varicella cases in 2022 for model fitting. It was predicted that 1 089 cases of varicella would be reported in Yangpu District in 2023. ConclusionIt is predicted that varicella cases in Yangpu District will increase in 2023. It is recommended to continue promoting delayed varicella vaccination to maintain a high level of vaccination rate. Before the peak of the epidemic, health education regarding varicella should be strengthened, and measures for epidemic prevention and control should be reinforced to prevent varicella outbreaks.
4.The inhibitory effect of calcium dobesilate on cisplatin induced HK-2 cell apoptosis and its mechanism

We observed the effect of calcium dobesilate (CaD) on apoptosis induced by cisplatin in human proximal tubular epithelial cells (HK-2) and explored the possible mechanism. Based on HK-2 cells apoptosis model induced by cisplatin, CCK-8 method was used to detect the effect of CaD on the proliferation of HK-2. Apoptosis was detected by flow cytometry. Reactive oxygen species (ROS) assay was used to evaluate the level of oxidative stress. The mitochondrial membrane potential was measured by JC-1 method. The expression levels of p53, caspase-3, bcl-2 and bax in cisplatin-induced HK-2 were detected by Western blot. The expression of renal injury factor 1(KIM-1) and neutrophil gelatin-related apolipoprotein (NGAL), markers of acute kidney injury, were detected by ELISA. The results showed that CaD could reduce the oxidative stress level induced by cisplatin and inhibit apoptosis in renal tubular epithelial cells. Cisplatin can up-regulate the protein expressions of p53, caspase-3, bax, KIM-1 and NGAL, and reduce the expression of bcl-2. After using CaD, the protein levels of KIM-1, NGAL, p53, caspase-3 and bax were significantly reduced, while the levels of bcl-2 were increased. This study has shown that CaD can alleviate cisplatin-induced HK-2 injury and inhibit HK-2 apoptosis, which may be related to the regulation of bax/bcl-2/caspase-3 apoptosis signaling pathway.
5.Risk factors of stroke-associated pneumonia in patients with acute ischemic stroke treated with intravenous thrombolysis

Objective To analyze the risk factors of stroke-associated pneumonia (SAP) in patients with acute ischemic stroke (AIS) treated with intravenous thrombolysis.Methods A total of 798 patients with AIS who received intravenous thrombolysis therapy with alteplase from the Department of Neurology,Yangpu Hospital,Tongji University from January 2016 to December 2020 were retrospectively selected and divided into pneumonia group and non-pneumonia group according to the presence or the absence of SAP.The clinical of the two groups were compared.Multivariate logistic regression analysis was used to analyze the independent risk factors of SAP.Results Of 798 cases,115 cases occurred SAP.By multivariate logistic regression analysis,age≥70 years(OR=2.846,95%CI 2.311~5.083,P<0.001),diabetes(OR=2.291,95%CI 1.601~3.945,P=0.003),chronic obstructive disease(OR=2.064,95%CI 1.759~3.528,P=0.005),NIHSS score at admission>8(OR=1.814,95%CI 1.502~2.452,P<0.001),unconsciousness(OR=2.325,95%CI 2.005~3.404,P=0.008),dysphagia(OR=2.457,95%CI 2.221~4.017,P<0.001),hemorrhagic transformation(OR=1.828,95%CI 1.653~2.523,P=0.012) were independent risk factors of SAP in AIS patients treated with intravenous thrombolysis.Conclusion Age≥70 years,diabetes,chronic pulmonary disease,NIHSS score at admission,unconsciousness,dysphagia,hemorrhagic transformation were independent risk factors of SAP in AIS patients treated with thrombolysis.
6.Peripheral blood indicators in medical radiation workers in Yangpu District of Shanghai, 2017-2019

Objective:To explore the impact of occupational ionizing radiation exposure on blood indicators including white blood cell( WBC), red blood cell (RBC), hemoglobin (Hb), platelet (PLT) were analyzed. Methods:A total of 237 medical radiation workers in Yangpu district, Shanghai were recruited and divided into observation group and control group, according to individual average dose of external exposure. The annual effective dose in observation group was 0.357 7-4.704 3 mSv, and the median dose was 0.536 8 mSv (0.441 2-0.893 8). The annual effective dose in control group was 0.031 2-0.350 8 mSv, and the median dose was 0.199 2 mSv (0.143 8-0.252 8). 7.Characteristics of hospitalized accidental injuries in Yangpu District, Shanghai in 2021

ObjectiveTo analyze the data of hospitalized accidental injuries with registered residence in Yangpu District, Shanghai, to describe their characteristics, and to provide an evidence for formulating accidental injuries prevention and control strategies. MethodsStatistic analysis was conducted on the data of accidental hospitalized injury cases in Yangpu District. The incidence rate (per 100 000 population) and the hospital stay were used to analyze the characteristics of hospitalized accidental injuries. ResultsA total of 4 924 hospitalized accidental injury cases were reported in Yangpu District, Shanghai. The incidence rate was 468.77/105, the ratio of male to female was 1∶1.41. Among them, the age ≥65 group had the highest incidence rate, accounting for 55.83% of all cases. The incidence rate of female was 1.97 times higher than that of male (χ2=287.61, P<0.05). The top five causes of injuries were falls, traffic-related, accident blunt injuries, injury by sharp instrument and fire or burns. The incidence rate of falls in female was higher than in male (χ2=176.65, P<0.05). The incidence rates of sharp instrument and blunt injuries in male were higher than in female(sharp instrument χ2=13.45, P<0.05; blunt injuries χ2=9.10, P<0.05). Altogether, the incidence rates of falls and traffic-related accident increased with age group (falls χ2trend=1 593.07, P<0.05; traffic related χ2trend=106.82, P<0.05). Fire or burns and drowning had a median length of hospital stay of 8 and 14 days, respectively. ConclusionFalls is the leading cause of hospitalized accidental injuries (accounting for about 74% of the total number of hospitalized accidental injury cases), with a higher incidence rate among elderly people aged ≥65 years old, and higher female than in male. Although the incidence rates of burns and drowning are not high, the hospital stay is relatively long. Therefore, accidental injury prevention and intervention should be targeted at key accidental injuries (such as falls, fire or burns, and drowning) and key populations (such as elderly female aged ≥65 years old), to reduce the likelihood and the related loss of accidental injuries.
8.Changing trend in diabetes-related death and probability of premature mortality among residents in Yangpu District of Shanghai from 2002 to 2020

ObjectiveTo determine the trend in diabetes-related death and probability of premature mortality among residents in Yangpu District of Shanghai and provide evidence for the formulation of relevant intervention strategies. MethodsMortality and demographic data were collected among residents in Yangpu District of Shanghai from 2002 to 2020. Statistical analysis was conducted using chi-square test with SPSS 21.0 and Excel 2010. Joinpoint regression was used to determine annual percent change (APC). The mortality was standardized by utilizing the world standard population in 2000. ResultsA total of 25 091 cases of diabetes-related deaths were reported in Yangpu District, Shanghai from 2002 to 2020. The average annual crude mortality of diabetes-related diseases was 122.10/105, which was 116.13/105 in males and 128.23/105 in females. The difference between males and females in crude mortality was statistically significant (P<0.05). Moreover, primary causes of diabetes-related deaths were diabetes, cardiovascular and cerebrovascular diseases, and tumors. Among diabetic deaths, peripheral circulatory complications accounted for 50.79%, followed by renal complications (16.05%). The crude mortality in males, females, and total of diabetes-related diseases showed an upward trend, while the standardized mortality remained stable with an upward trend in male and a downward trend in female. Furthermore, the crude mortality in males, females, and total of diabetes complicated with cardiovascular and cerebrovascular diseases showed an increasing trend. In contrast, both the standardized mortality in males and in total showed an increasing trend, while that in females remained stable. The overall crude mortality of diabetes was on the rise, which was increasing in males and stable in females. The overall standard mortality of diabetes was on the decline, which was increasing in males while declining in females. In addition, the probability of premature mortality caused by diabetes-related diseases, cardiovascular and cerebrovascular diseases complicated with diabetes, and diabetes decreased from 2002 to 2020 with no statistical significance. Males showed an upward trend while females showed a downward trend. ConclusionThe mortality of diabetes-related diseases, cardiovascular and cerebrovascular diseases complicated with diabetes, and diabetes among residents in Yangpu District of Shanghai is on the rise. Similarly, standardized mortality and probability of premature mortality in males for all three diseases are also on the rise. It warrants more attention to the health of male diabetes patients and targeted measures to reduce the disease burden.
9.Research progress on valgus impacted proximal humeral fractures.

OBJECTIVE:
To review the advancement made in the understanding of valgus impacted proximal humeral fracture (PHF).
METHODS:
The domestic and foreign literature about the valgus impacted PHF was extensively reviewed and the definition, classification, pathological features, and treatment of valgus impacted PHFs were summarized.
RESULTS:
PHF with a neck shaft angle ≥160° is recognized as a valgus impacted PHF characterized by the preservation of the medial epiphyseal region of the humeral head, which contributes to maintenance of the medial periosteum's integrity after fracture and reduces the occurrence of avascular necrosis. Therefore, the valgus impacted PHF has a better prognosis when compared to other complex PHFs. The Neer classification designates it as a three- or four-part fracture, while the AO/Association for the Study of Internal Fixation (AO/ASIF) categorizes it as type C (C1.1). In the management of the valgus impacted PHF, the selection between conservative and surgical approaches is contingent upon the patient's age and the extent of fracture displacement. While conservative treatment offers the advantage of being non-invasive, it is accompanied by limitations such as the inability to achieve anatomical reduction and the potential for multiple complications. Surgical treatment includes open reduction combined with steel wire or locking plate and/or non-absorbable suture, transosseous suture technology, and shoulder replacement. Surgeons must adopt personalized treatment strategies for each patient with a valgus impacted PHF. Minimally invasive surgery helps to preserve blood supply to the humeral head, mitigate the likelihood of avascular necrosis, and reduce postoperative complications of bone and soft tissue. For elderly patients with severe comminuted and displaced fractures, osteoporosis, and unsuitable internal fixation, shoulder joint replacement is the best treatment option.
CONCLUSION
Currently, there has been some advancement in the classification, vascular supply, and management of valgus impacted PHF. Nevertheless, further research is imperative to assess the clinical safety, biomechanical stability, and indication of minimally invasive technology.

10.Food and nutrients intake among 223 residents in Yangpu District from 2011 to 2015

Objective To analyze the intake and changes of food and nutrients in 223 residents of Yangpu District from 2011 to 2015. Methods From the China Nutritional Transition Cohort Study in 2011 and 2015, the data in the study was obtained on Yangpu District′s residents aged 2 and above.A dietary survey was conducted using the 3-day 24-hour dietary recall and condiment weighing method to obtain data on food and nutrients intake per reference man per day and the nutrient source of energy, and to analyze the intake status of different years. Results The intake of vegetables, fruits, livestock and poultry, aquatic products, eggs, milk and dairy products, soybeans and nuts, edible oil and salt per reference man per day was lower in 2015 than in 2011.The insufficient intake proportions of cereals and tubers, vegetables, fruits, aquatic products, eggs, milk and dairy products, soybeans and nuts, and edible oils were 56.0%, 84.0%, 98.2%, 56.0%, 70.2%, 97.8%, 85.3% and 91.1%, respectively, the excessive intake proportion of livestock and poultry meat was 51.6%;the intake of energy, protein, fat and micronutrient per reference man per day in 2015 was lower than in 2011, and the insufficient intake proportion of energy, protein, vitamin A, vitamin B1, vitamin B2, vitamin C, and calcium were 84.0%, 56.4%, 97.3%, 93.8%, 93.3%, 90.2% and 91.6%, respectively.In 2011 and 2015, it was found that residents who had the proportions of energy supply from carbohydrate lower than 50% accounted for 64.6% and 49.8%, those who had the proportions of energy supply from protein higher than 15% accounted for 76.2% and 40.4%, and those who had the proportions of energy supply from fat higher than 30% accounted for 76.7% and 72.9%. Conclusion In 2011-2015, the food and nutrients intakes among community residents in Yangpu District generally decrease, showing the coexistent problems of insufficient and excessive food intake, and the insufficient intake is serious.The insufficient intake of energy, vitamins and calcium is still a severe problem.And the dietary structure is not rational; the proportions of energy supply from three major nutrients are unbalanced.It is suggested that nutrition education for residents should be enhanced.
