1.Clinical value of C-reactive protein and serum lactate to the prognosis of acute abdomen in elderly patients

Objective To compare the clinical value of CRP and LAC in the evaluating of diagnosis and the prognosis of se‐vere acute abdomen diseases in the elderly patients .Methods Experimental group:The blood of 92 elderly patients diagnosed with critical abdomen mass with infection and followed by surgery were collected .Then ,extracted serum was used to measure the levels of CRP and LAC ,and these patients were divided into 4 groups on base of the degree of infection:SIRS group ,sepsis group ,serve sepsis group and septic shock group .Control group:92 serums of healthy persons were selected to measure the levels of CRP and LAC .Results The CRP and LAC significantly increased in the experimental group ,when compared with the control group(P<0 .05) .The level of LAC raised with the increase severity of sepsis(P<0 .05) among groups of sepsis ,serve sepsis and sepsis with shock ,but the level of CRP had no statistical difference(P>0 .05) .Conclusion Both CRP and LAC of elderly patients diagnosed with critical abdomen mass with infection have important referential meaning .Furthermore ,the CRP has better predictability in the early stage of abdomen mass with infection .However ,to predict the severity of infection level ,LAC seems more sensitive .
2.The neurophysiological changes associated with pulmonary function in patients with obstructive sleep apnea syndrome

Objective To observe neurophysiological changes and pulmonary function in obstructive sleep apnea syndrome (OSAS) and analyze their inter-relationship. Methods Sixty OSAS patients were studied. Their lung function and phrenic motor nerve conduction (PNC) were examined. Thirty cases without respiratory disorder served as controls. The lung function tests included percentage of the predicted value of vital capacity ( VC% ), percentage of the predicted value of maximal voluntary ventilation ( MVV% ), percentage of the predicted value of forced expiratory volume in one second ( FEV1% ), inspiratory capacity ( IC), and expiratory reserve volume (ERV). The phrenic nerve was stimulated electrically, and the latent period and the diaphragmatic compound muscle action potential (dCMAP) were recorded. Results The VC%, MVV%, FEV1% , IC and ERV of the patients with OSAS were significantly lower than those of the controls. There was no significant difference between the patients and the controls with regard to their PNC latency. In the OSAS patients the amplitude ratio of their dCMAPs was positively correlated with VC% , MVV% , FEV1% , IC and ERV, and negatively with an apnea hypopnea index (AHI).Conclusions PNC examination can provide valuable information for evaluating diaphragmatic dysfunction in those with abnormal lung function. Decreased dCMAP might be associated with abnormal lung function.
3.Associations of family functioning and parental styles with anxiety symptoms among high grade primary school students

Objective:
To investigate the prevalence of anxiety symptoms among high grade primary school students in Guangzhou and their correlation with family functioning and parental parenting styles, so as to provide theoretical basis and guidance for family based interventions for children s anxiety symptoms.
Methods:
From June to September 2022, a multi stage cluster random sampling method was employed to select 5 396 students from grades 4 to 6 in 13 primary schools in Guangzhou. The Family Functioning Assessment Scale for Children, Parental Bonding Instrument, and Spence Children s Anxiety Scale Short Version were used for the survey. Inter group comparison was analysed by t test or analysis of variance. Linear regression analysis was used to explore the effects of family function and parental parenting styles on anxiety symptoms in primary school students.
Results:
The average score for anxiety symptoms among high grade primary school students in Guangzhou was (11.63±10.88). In terms of parenting styles, mothers scored higher than fathers in the dimensions of "care" (25.63±4.92) and "control" (5.08±2.58) compared to fathers ( 24.74± 5.50, 5.00±2.51) ( t =15.80, 4.21, P <0.01). However, there was no statistically significant difference in the scores of parents on the dimension of "encouragement of independence"(12.98±4.06,13.05±3.95)( t =-1.77, P >0.05). After adjusting for general demographic characteristics, the results of linear regression analysis showed that primary school students family functioning ( B =0.47) and parental "control" ( B =0.67, 0.75) were positively associated with anxiety symptoms scores ( P <0.01). Meanwhile, parental "care" ( B =-0.53,-0.55) and "encouragement of independence" ( B =-0.62,-0.68) were negatively associated with anxiety symptom scores ( P <0.01). These associations remained statistically significant even after further adjusting for family functioning ( P <0.01).
Conclusions
Family functioning, parenting styles are closely associated with anxiety symptoms among high grade primary school students. Positive family functioning and parenting styles may mitigate childhood anxiety symptoms, whereas excessive parental control may increase the risk of childhood anxiety symptoms.
4.Recent advance in paroxysmal sympathetic hyperactivity

Paroxysmal sympathetic hyperactivity (PSH),which recognized in a subgroup of survivors of severe acquired brain injury,is a syndrome of simultaneous,paroxysmal transient increases in sympathetic (elevated heart rate,blood pressure,respiratory rate,temperature,sweating) and motor (posturing) activity.These symptoms are not specific and can occur in sepsis,epilepsy,malignant hyperthermia,hydrocephalus and many other complications,which is related to confused diagnosis and delayed treatment of PSH.In addition,Patients with PSH have prolonged ICU stays and added healthcare costs.This article discusses the pathophysiological mechanism,clinical diagnostic criteria,and current drug treament measures of PSH,so as to guide rapid diagnosis.
5.Correlation between percentage of body fat and simple anthropometric parameters in children aged 6-9 years in Guangzhou

Objective To evaluate the accuracy of simple anthropometric parameters in diagnosing obesity in children in Guangzhou. Methods A cross-sectional study, including 465 children aged 6-9 years, was carried out in Guangzhou. Their body height and weight, waist circumference (WC) and hip circumference were measured according to standard procedure. Body mass index (BMI), waist to hip ratio (WHR) and waist-to-height ratio (WHtR) were calculated. Body fat percentage (BF%)was determined by dual-energy X-ray absorptiometry. Multiple regression analysis was applied to evaluate the correlations between age-adjusted physical indicators and BF%, after the adjustment for age. Obesity was defined by BF%. Receiver operating characteristic (ROC) curve analyses were performed to assess the diagnostic accuracy of the indicators for childhood obesity. Area under-ROC curves (AUCs) were calculated and the best cut-off point that maximizing'sensitivity+specificity-1'was determined. Results BMI showed the strongest association with BF% through multiple regression analysis. For'per-standard deviation increase' of BMI, BF%increased by 5.3%(t=23.1, P<0.01) in boys and 4.6%(t=17.5, P<0.01) in girls, respectively. The ROC curve analysis indicated that BMI exhibited the largest AUC in both boys (AUC=0.908) and girls (AUC=0.895). The sensitivity was 80.8% in boys and 81.8% in girls, and the specificity was 88.2%in boys and 87.1%in girls. Both the AUCs for WHtR and WC were less than 0.8 in boys and girls. WHR had the smallest AUCs (<0.8) in both boys and girls. Conclusion BMI appeared to be a good predicator for BF%in children aged 6-9 years in Guangzhou.
6.Prevalence of psoriatic arthritis in Chinese population with psoriasis: A multicenter study conducted by experienced rheumatologists.

BACKGROUND:
Reports on the prevalence of psoriatic arthritis (PsA) among Chinese patients with psoriasis are very limited. This study, conducted by rheumatologists, estimated the prevalence of PsA in a large number of Chinese patients with psoriasis.
METHODS:
Consecutive patients with a confirmed diagnosis of psoriasis attending nine dermatology clinics in five hospitals were recruited. All psoriasis patients were asked to complete a questionnaire comprising 16 questions to identify possible cases of PsA. All patients with one or more positive answers to the questionnaire were evaluated by two experienced rheumatologists.
RESULTS:
A total of 2434 psoriasis patients, including 1561 males and 873 females, were enrolled. Both the questionnaire and rheumatologists' examinations were completed in the dermatology clinics. The results identified 252 patients with PsA, comprising 168 males and 84 females. The overall prevalence of PsA among psoriasis patients was 10.4% (95% confidence interval [95% CI], 9.1%-11.7%). By sex, the prevalence was 10.8% (95% CI, 9.2%-12.5%) for males and 9.6% (95% CI, 7.7%-11.9%) for females and there was no significant sex difference in the prevalence of PsA (P = 0.38). Of the 252 PsA patients, 125 (49.6%, 95% CI, 41.3%-59.1%) were newly diagnosed by rheumatologists. Consequently, the prevalence of undiagnosed PsA among psoriasis patients was 5.2% (95% CI, 4.4%-6.2%).
CONCLUSION
The prevalence of PsA in the Chinese population with psoriasis is about 10.4%, which is almost double that of previous reports in the Chinese population, but lower than that in Caucasians.
