1.Significance of atrial fibrillation/atrial flutter in prognosis in patients with primary percutaneous coronary intervention

Objective Previous studies have reported that the incidence of atrial fibrillation (AF)/atrial fluttler in patients with AMI who were treated with thrombolysis is 7%-10%. However, little is known concerning the incidence of AF/atrial fluttler and its effects on the prognosis of patients with AMI who are treated with PCI. Methods A total of 668 consecutive patients were studied in Beijing Chaoyang Hospital from Nov. 2000 to Jan. 2004. Patients were categorized into 2 groups according to the presence of AF/ atrial flutter. Results The results showed that the incidence of AF/atrial fluttler was 10% and that the patients with AF/atrial fluttler were older, were in higher Killip classes, had higher rates of previous myocardial infarction and previous cerebrovascular diseases, multivessel disease, and had poorer reperfusion of the infarc-related artery than those without AF/atrial fluttler. 2.Adhesive negative pressure suction apparatus in treatment of the wound infection after colorectal surgery

Objective To investigate the efficacy of adhesive negative pressure suction apparatus in treatment of wound infection after colorectal surgery.Methods From January 2010 to November 2011,consecutive patients with wound infection after colorectal surgery were randomized to the experimental group (n =24 ) and the control group (n =24).The patients in experimental group were treated with the adhesive negative pressure suction apparatus.The patients in the control group were treated with traditional wound therapy.The results of treatment were evaluated.Results Time from debriding the wound infection to secondary suture was significantly shorter in patients of the experimental group ( P < 0.05 ).The total number of dressing changes was very significantly less in patients of the experimental group ( P < 0.01 ).The total cost of dressing in experimental group was significantly less than the control group (P < 0.05 ).Conclusions The application of adhesive negative pressure suction apparatus in treatment of the wound infection after colorectal surgery can shorten the wound processing time,reduce the number of dressing changes and the total cost of dressing,thus has a high clinical value.
3.Radical surgery of lung cancer with chest wall invasion and reliable methods of chest wall reconstruction

Objective To study the efficacy of radical surgery for the invasion of chest wall by lung cancers and the different methods of chest wall reconstruction. Methods A retrospective analysis of clinical data was conducted in 27 cases of lung cancer with chest wall invasion in this hospital. 24 cases received lobectomy, 3 cases received pneumonectomy, and all cases received wall reconstruction. The size of chest wall defect after resection ranged from 6.5 cm × 5.4 cm × 5.0 cm to 15.5 cm × 12.5 cm ×10.0 cm. The number of rib resection for each patient ranged from 1 to 4. The reconstructive methods were as follows: using polyester fabric to reconstruct costal pleura, using plexiglass, stainless steel wire and grid-like titanium plate to reconstruct bone defect, and using nearby muscle flap and pedunculated muscle flap to reconstruct muscle defect. Results Pathologic diagnosis was non-small cell lung cancer,including 22 cases of squamous cell carcinomas and 5 adenocarcinomas. There were pulmonary infections in 6 cases, chest hemorrhage in 1 case and chest purulence in 1 case. One-year survival was 77. 9 %,3-yearsurvival was 58.3 %, and 5-year survival was 29. 1 %. Conclusion The lung cancer with chest wall invasion is advisable to make active use of radical surgery. The different methods and repair materials are chosen to reconstruct the chest wall defect, supplemented by comprehensive treatments.
4.Effect of early comprehensive rehabilitation therapy on dysphagia in stroke patients

Objective To explore the effect of early comprehensive rehabilitation therapy on dysphagia after stroke. Methods A total of 120 stroke patients with dysphagia were collected from December 2006 to May 2009 and divided into 3 trial groups and a control group randomly.No treatment was given to patients in the control group.Patients in trial group 1(T1)were given rehabilitation training,while patients in trial groups 2(T2)and 3(T3)were treated with VitalStim and electrical acupuncture,respectively,in addition to the rehabilitation training.A standardized swallowing assessment (SSA) and the swallowing quality of life(SWAL-QOL) scale were used to evalu-ate all the patients before and after 4 weeks of treatment. ResuIts No statistically significant difference was re-vealed before the treatment among the groups in terms of the patients'sex,age,course of disease,SSA or SWAL-QOL results.Statistically significant improvement was observed after treatment in the 3 trial group,but not in the control group with regard to the SSA and SWAL-QOL scores compared with those before treatment.The trial groups all had higher scores than the control group after treatment.while T2 and T3 had higher scores than T1 after treatment.There was no statistically significant difference between groups T2 and T3.Conclusions Early compre-hensive rehabilitation therapy can improve swallowing and the quality of life of stroke patients with dysphagia.Reha-bilitation combined with neuromuscular electrical stimulation provides effects similar to that of training combined with electrical accupuneture,and is more effective than simple training in treating dysphagia.
5.A study of body image,prosthesis satisfaction and quality of life of the amputees from 5·12 Wenchuan earthquake

Objective To explore body image(BI),prosthesis satisfaction and quality of life(QOL)of amputees from Wenchuan earthquake and the relationship among them to provide evidence for the intervention.Methods A total of 21 amputees accepted a survey regarding QOL,BI,and satisfaction with prosthesis.The scale of SF-36 was used to assess QOL.Amputees Bodily Image Scale(ABIS)was;used to assess BI.A self-design questionnaire wag used to evaluate the prosthesis satisfaction of amputees.Results The scores of SF-36,ABIS,and prosthesis satisfaction wag 41.90 ±15.00,64.58 ±10.60,and 59.52±11.06,respectively.The ABIS scores in women(65.07 ±12.10)were hisher than that in men(63.20 ±5.0)(P<0.05).There was a positive correlation between score of QOL and prosthesis satisfaction,but a negative correlation between QOL and BI.The negative correlations were also observed between BI and Vitality,BI and mental health.Prosthesis satisfaction had positive correla tions with education level.QOL and mental health.Conclusion The body image disturbance(BID)and dissatis faction with prosthesis may cause negative influences on the amputees'quality of life.Relevant intervention should be provided of amputees handling their prostheses and body images correctly and raising QOL.
6.Neurogranin overexpression on peripheral blood mononuclear cells from patients with systemic lupus erythematosus

Objective To investigate the mRNA expression level of neurogranin on peripheral blood mononuclear cells(PBMCs)from patients with systemic lupus erythematosus(SLE).Methods Top 20 tags of SLE PBMCs SAGE library were searched from normal lymphocytes SAGE library including navie-T,Th1,Th2, CD8+T, NK and B cells,and their abundance was compared.The mRNA expression level of neuro-granin,a differential over-expressed tag,was detected in 35 cases of SLE and 15 normal controls by reversetranscription-polymerase chain reaction (RT-PCR).Results Neurogranin tag could only be detected in SLE PBMCs SAGE library,but was hardly found in normal lymphocyte SAGE library.However,either SLE pa-tients or normal controls showed a detectable mRNA level of neurogranin on PBMCs by RT-PCR.The mRNA level of neurogranin in active SLE patients was significantly increased than those in controls(P<0.001).but only slightly increased in inactive SLE patients (P>0.05).Conclusion Neurogranin,as a novel proapototic factor,is overexpressed on PBMCs of SLE patients.It may be involved in the regulation of abnormal immune responses in lupus.
7.Expression of tumor necrosis factor alpha and its relationship with infiitrating lymphocytes in lichen planus lesions

Objective To investigate the expression of tumor necrosis factor alpha(TNF-α)and its relationship with infiltrating lymphocytes in lichen planus(LP).Methods Tissue specimens were obtained from the lesions of 60 patients with LP and normal skin of 20 human controls.Immunohistochemical SP method was used to detect the expression of TNF-α,and infiltrating lymphocytes were counted in TNF-α-positive tissue sections.Results TNF-α was expressed in 72% of the LP specimens but in none of the control specimens(P < 0.01).Positive staining for TNF-α was mainly located in the membrane of prickle cells,cytoplasm or membrane of dermal infiltrating lymphocytes.The expression of TNF-α in LP was uncorrelated with age,sex or disease course(all P > 0.05),but was positively correlated with infiltrating lymphocyte number (rs =0.47,P < 0.01).Conclusion TNF-α seems to play a certain role in the pathogenesis of LP.
8.Advanced glycation end products modulate osteoclastic acidification by inhibiting the expression of V-ATPase a3 and CIC-7

BACKGROUND:The effect of advanced glycation end products (AGEs) on bone resorption is controversial. Our previous study has shown that bone resorption is significantly inhibited when AGEs present with pre-osteoclast cells RAW 264.7, while the effect of AGEs on osteoclastic acidification remains unknown. OBJECTIVE:To investigate the effect of AGEs on osteoclastic acidification and the underlying mechanism. METHODS:RAW 264.7 cells were induced by RANKL (15μg/L;normal group) to generate osteoclasts, and AGEs (50-400 mg/L;experimental group) or bovine serum albumin (100 mg/L;control group) were added at the beginning of the induction. The effect of AGEs on bone resorption was evaIuated by anaIyzing the area of bone resorption on the Osteo Assay Surface plates, and the effect of AGEs on osteoclastic acidification was evaluated by acridine orange staining. Furthermore, the expression levels of V-ATPase a3 and CIC-7 were detected to investigate the underlying mechanism. RESULTS AND CONCLUSION:The bone resorption area in the AGEs group was significantly decreased compared with the normal group (P<0.05). Acridine orange staining reveaIed that the red fluorescence (620 nm) intensity in the AGEs group was significantly decreased compared with the normal group (P<0.05), and this inhibitory effect became obvious with the increase of AGEs concentration. Immunocytochemistry, western blot assay and PCR findings showed that the expression levels of V-ATPase a3 and CIC-7 in the AGEs group were decreased significantly compared with the normal group (P<0.05). To conclude, AGEs exert inhibitory effect on osteoclastic acidification, probably by inhibiting the expression of V-ATPase a3 and CIC-7.
9.Effect of different concentrations of propranolol gel on infantile hemangiomas

Objective To evaluate the efficacy and safety of different concentrations of propranolol gel in treatment of infantile hemangiomas(IH).Methods Sixty-three consecutive infants with hemangiomas from Mar.2011to Sep.2012 in Department of General Surgery,Fuzhou General Hospital of Nanjing Military Command,were divided randomly into A,B,C groups and respectively treated with propranolol hydrochloride 1% (20 g ∶ 200 mg,A group),2% (20 g:400 mg,B group),3% (20 g:600 mg,C group),evenly applied to the surface of the hemangiomas,3 times a day.Follow-up treatment was performed for 6 months.Details of adverse reactions,gender,age,tumor location,complications,and hemangioma size,texture,color and recurrence after stopping were recorded,and hospital review and efficacy evaluation were performed every month.Results After 6 months of treatment,A group had effective treatment in 11 patients(52.38%),ineffective treatment in 10 patients (47.62%),B group had effective treatment in 12 patients (57.14%),ineffective treatment in 9 patients (42.86%),and C group had effective treatment in 19 patients (90.48%),ineffective treatment in 2 patients(9.52%).After taking color Doppler ultrasound examination revealed IH thickness revealed significantly thinner.Conclusions External use of propranolol hydrochloride gel is an effective option for superficial hemangiomas,And 3% concentration of propranolol gel is superior to the efficacy of the other 2 drugs.
10.The preliminary study of the differential diagnosis of ascites using Golgi glycoprotein 73 and alpha fetal protein

Objective To investigate the concentration of Golgi glycoprotein 73(GP73) in ascites, then study the value of GP73 and alpha fetal protein(AFP)in the differential diagnosis of ascites.Methods Totally 96 malignant ascites specimens , 35 benign ascites specimens and their paired serum specimens were collected in the Tumor Hospital Affiliated to Xinjiang Medical University from November 2012 to November 2013.GP73 in serum and ascites were detected by ELISA and AFP in ascites was measured by electrochemical luminescence.The application values of GP 73 and AFP were evaluated through ROC curve.Results In benign ascites group , the difference of GP73 between the ascites ( 16.06 ±9.53 ) ng/ml and the serum (13.69 ±8.87) ng/ml was statistically significant (t=5.026,P<0.001), which showed a good linear correlation (r=0.966,P<0.001).In malignant ascites group, the difference between the ascites (28.37 ±12.02) ng/ml and the serum (16.06 ±9.53) ng/ml was statistically significant (t=10.641,P<0.001), and also showed a good linear correlation (r=0.933,P<0.001).The cutoff values of GP73, AFP and GP73*AFP (GP73 multiplied AFP) for diagnosing the malignant ascites were 17.1 ng/ml, 13.1 ng/ml and 321.The area under ROC curve ( AUC) were 0.795, 0.753 and 0.902 respectively.The AUC of GP73*AFP is the largest.The sensitivity of three index diagnosing the ascites , were 89.6%( 86/96 ) , 88.5%(85/96) and 86.4%(83/96); and the specificity were 60.0%(21/35), 54.2%(19/35) and 85.7%( 30/35 ) respectively.Conclusions Detection of GP73 in ascites specimens has clinical value , GP73*AFP was the best indicators for differential diagnosis of benign and malignant ascites compared to GP73 and AFP.
