1.Observations on the antihypertensive effect of aerobic exercise on primary hypertension in the elderly

目的观察有氧运动训练对老年性原发性高血压病血压的控制作用。方法160例老年性原发性高血压病患者分为轻度训练组、轻度对照组、中度训练组和中度对照组,各组40例。训练组采用有氧运动训练(步行或骑车) 60分钟/次,每周3次;中度患者配合药物治疗。比较训练3个月前后的收缩压和舒张压。结果训练后收缩压和舒张压与训练前相比,各训练组均有显著性差异(P<0.01或P<0.05),各对照组则无显著性差异。而训练组与相应对照组相比亦均有显著性差异(P<0.01或P<0.05)。结论有氧运动训练可有效降低或控制老年性原发性高血压。
2.Estimation of Internal Tumor Volume:A Phantom Study Based on Semiautomatic Standardized Uptake Value of the Background

Purpose To estimate and discuss the feasibility of the threshold value of the internal tumor volume (ITV) of lung cancer based on the standardized uptake value (SUV) of the background.Materials and Methods The phantom was designed with 3 vacuous models fixed at the bottom of a barrel. The 3 vacuous models were filled with18F-FDG (55.0 kBq/ml) which represented a tumor and the barrel was filled with18F-FDG (5.9 kBq/ml), which represented the background of the lung. The barrel moved sinusoidally with distances of 10.9 mm, 21.8 mm, and 43.7 mm to simulate breathing, and then the phantom 2D PET/CT data were acquired. The SUV in the background was recorded as ±SD, and+3SD was regarded as threshold value to obtain the measured ITVs. The Dice similarity coefficient (DSC) and volume recovery coefficient (VRC) were used to evaluate the accuracy of the ITVs measured by seven methods, including that by SUV 2.5, 35% SUVmax, 41% SUVmax, a×mSUV70+b×, 0.42×(SUVmax+) and Riegel et al. computed threshold value followed the gross tumor volume, tumor-to-nontumor ratio and motion extent.Results Nine ITVs of three lung cancer models were 134.3 ml, 166.1 ml, 223.5 ml, 86.6 ml, 108.5 ml, 150.7 ml, 32.3 ml, 43.8 ml and 63.6 ml, respectively. The ITV measured by Riegel et al. showed no difference with the real ITV (t=-0.48,P>0.05). The ITVs measured by the other six methods were significantly different from the real ITVs (t=-5.11-2.76,P<0.05). The DSC of the ITV measured by+3SD and the method of Riegel et al. were 0.75-0.92 and 0.69-0.93, respectively; and the VRC were 0.82-0.97 and 0.74-0.96, respectively (differences without significance:t=-0.73,P>0.05). Moreover, they surpassed the results of the other five methods.Conclusion The threshold value (+3SD) of the SUV determined by the background has potential value in the description of the ITV of lung cancer, so as to provide reference for radiotherapy.
3.Adhesive negative pressure suction apparatus in treatment of the wound infection after colorectal surgery

Objective To investigate the efficacy of adhesive negative pressure suction apparatus in treatment of wound infection after colorectal surgery.Methods From January 2010 to November 2011,consecutive patients with wound infection after colorectal surgery were randomized to the experimental group (n =24 ) and the control group (n =24).The patients in experimental group were treated with the adhesive negative pressure suction apparatus.The patients in the control group were treated with traditional wound therapy.The results of treatment were evaluated.Results Time from debriding the wound infection to secondary suture was significantly shorter in patients of the experimental group ( P < 0.05 ).The total number of dressing changes was very significantly less in patients of the experimental group ( P < 0.01 ).The total cost of dressing in experimental group was significantly less than the control group (P < 0.05 ).Conclusions The application of adhesive negative pressure suction apparatus in treatment of the wound infection after colorectal surgery can shorten the wound processing time,reduce the number of dressing changes and the total cost of dressing,thus has a high clinical value.
4.Diagnosis and surgical treatment of locally advanced thymic epithelial tumors

The clinical treatment of locally advanced thymic epithelial tumors still faces many challenges, a multidisciplinary treatment model based on surgery is the main treatment method. In recent years, the surgical treatment of locally advanced thymic epithelial tumors has progressed rapidly, especially in the field of minimally invasive treatment. But controversies still exist and there are concepts needed to be clarified and principles to abide by. This article focuses on current status and recent progress in diagnosis and treatments of locally advanced thymic epithelial tumors.
5.Analysis on dissolved content of main constituents of Maxing Shigan Decoction and its powder

AIM:To analyze the dissolved content of main constituents of Maxing Shigan Decoction(Herba Ephedrae,Semen Armeniacae amarum,Gypsum Fibrosum,etc.) and powder applied in experiment.The experiment was carried out simulantly in vitro;i.e.drug passed through metabolically man-made stomach liquid and stomach-intestine liquid. METHODS:The experiment of decoction and its powder of Maxing Shigan Decoction were carried out respectively.main ingredients of component drugs in compound Maxing Shigan Decoction were chosen to analyze the dissolved content by the aid of GC and Complexometry. RESULTS:There was a distinctive difference in dissolved content of main ingredients. CONCLUSION:The law of dissolved content change provides a quick and efficient scientific basis for clinical dosage and determination of quality standard.
6.Change of Total Content of Magnolol and Honokiol in Cortex magnoliae Officinalis before and after Ginger mix frying

Objective:To establish the content standards of Cortex Magnoliae Officinalis with ginger mix frying by determing the change of total content of magnolol and honokiol in Cortex Magnoliae Officinalis before and after ginger mix frying. Methods: The total content of magnolol and honokiol in Cortex Magnoliae Officinalis before and after ginger mix frying were determined by HPLC according to the determination method of Cortex Magnoliae Officinalis in China Pharmacopeia (2000). Results: There was 13-14% decrease in total content of magnolol and honokiol of Cortex Magnoliae Officinalis after ginger mix frying. Conclusions: It is suggested that the total content of magnolol and honokiol of Cortex Magnoliae Officinalis after ginger mix frying shouldn't lower than 1.6%.
7.A study of body image,prosthesis satisfaction and quality of life of the amputees from 5·12 Wenchuan earthquake

Objective To explore body image(BI),prosthesis satisfaction and quality of life(QOL)of amputees from Wenchuan earthquake and the relationship among them to provide evidence for the intervention.Methods A total of 21 amputees accepted a survey regarding QOL,BI,and satisfaction with prosthesis.The scale of SF-36 was used to assess QOL.Amputees Bodily Image Scale(ABIS)was;used to assess BI.A self-design questionnaire wag used to evaluate the prosthesis satisfaction of amputees.Results The scores of SF-36,ABIS,and prosthesis satisfaction wag 41.90 ±15.00,64.58 ±10.60,and 59.52±11.06,respectively.The ABIS scores in women(65.07 ±12.10)were hisher than that in men(63.20 ±5.0)(P<0.05).There was a positive correlation between score of QOL and prosthesis satisfaction,but a negative correlation between QOL and BI.The negative correlations were also observed between BI and Vitality,BI and mental health.Prosthesis satisfaction had positive correla tions with education level.QOL and mental health.Conclusion The body image disturbance(BID)and dissatis faction with prosthesis may cause negative influences on the amputees'quality of life.Relevant intervention should be provided of amputees handling their prostheses and body images correctly and raising QOL.
8.A protocol for the perioperative management of thymectomy for myasthenia gravis patients

Objective To develop a standard and procedural protocol for the perioperative management of thymectomy for myasthenia gravis(MG) patients and thus to reduce the incidence of MG crisis.Methods From June 1996 to March 2016,466 MG cases received thymectomy we continuously explored key technologies of surgical treatment for MG 466 patients,there were 209 male cases and 259 female cases,with age ranging from 5 to 77 years and chief complaint history ranging from 12 days to 18 years.Symptoms included drooping eyelids,double vision,weakness,shortness of breath,coughing,dysarthria,and difficulties in swallowing and chewing.According to the modified Osserman classification,there were 248 type Ⅰ MG cases,58 type Ⅱa MG cases,66 type Ⅱb MG cases,71 type Ⅲ MG cases,and 23 type Ⅳ MG cases respectively.116 cases received thymecotomy via full sternotomy,204 cases via J type semi-sternotomy,and 146 case via thoracoscopy (including 13 cases via sub-xiphoid approach).Results Perioperatively one case died of sudden death,another patient died of respiratory failure after the second operation for metastatic thymoma,with a mortality rate of 0.42% (2/466);13 cases had M G crisis (13/466);Six cases underwent tracheotomy (6/466);2 cases had plasmapheresis hypotonic syndrome (accounting for 3.4% in plasmapheresis cases) and were reoperated to stop bleeding.Postoperatively pathological diagnosis was made,including three thymic atrophy cases,272 thymic hyperplasia cases,178 thymoma cases,and 13 thymic cyst cases.Conclusion A standard and procedural protocol for the perioperative management of thymectomy for MG patients can be developed,which can reduce the morbidity of MG crisis and the incidence of tracheotomy.
9.Effect of different concentrations of propranolol gel on infantile hemangiomas

Objective To evaluate the efficacy and safety of different concentrations of propranolol gel in treatment of infantile hemangiomas(IH).Methods Sixty-three consecutive infants with hemangiomas from Mar.2011to Sep.2012 in Department of General Surgery,Fuzhou General Hospital of Nanjing Military Command,were divided randomly into A,B,C groups and respectively treated with propranolol hydrochloride 1% (20 g ∶ 200 mg,A group),2% (20 g:400 mg,B group),3% (20 g:600 mg,C group),evenly applied to the surface of the hemangiomas,3 times a day.Follow-up treatment was performed for 6 months.Details of adverse reactions,gender,age,tumor location,complications,and hemangioma size,texture,color and recurrence after stopping were recorded,and hospital review and efficacy evaluation were performed every month.Results After 6 months of treatment,A group had effective treatment in 11 patients(52.38%),ineffective treatment in 10 patients (47.62%),B group had effective treatment in 12 patients (57.14%),ineffective treatment in 9 patients (42.86%),and C group had effective treatment in 19 patients (90.48%),ineffective treatment in 2 patients(9.52%).After taking color Doppler ultrasound examination revealed IH thickness revealed significantly thinner.Conclusions External use of propranolol hydrochloride gel is an effective option for superficial hemangiomas,And 3% concentration of propranolol gel is superior to the efficacy of the other 2 drugs.
10.Clinicopathological study on lymph node metastases in the mesorectum of rectal cancer

Objective To investigate the patterns of mesorectum lymph node (LN) metastases of rectal cancer. Methods Rectal cancer specimens obtained by total mesorectum excision were treated with lymph node revealing solution to retrieve all the nodes, and examined by routine pathology and immunohistochemical staining. Results A total of 443 LN in 26 specimens were harvested, with 128 nodes(28 9%)in 23 cases (88 5%) found positive. Positive nodes with the size less than 5 mm accounted for 59%. Among the 23 metastatic cases, there were 14 cases with tumors locating in the posterior wall, 69 out of 71 positive nodes were found along the superior rectal artery. In the other 9 cases, tumors were found in the lateral wall, 29 out of 57 positive nodes were found around ipsilateral branches of superior rectal artery,7 were around the contralateral branches, 4 were around the ipsilateral branches of middle rectal artery. Conclusions Most positive LN were less than 0 5 cm in diameter. LN metastasis of rectal cancer have close relationship with tumor location. Tumors in the posterior wall tend to spread upward along the superior rectal artery, while tumors in the lateral wall may have upward and lateral LN metastases simultaneously, with most metastatic LN found ipsilaterally.
