1.Progresses on mechanism of the relationship between gut microbiota and radiation injuries

Recently,researchers have paid attention to the relationship between gut microbiota and human health and attempted to investigate the effect of gut microbiota on radiation injury.More and more evidence showed that normal gut microbiota could maintain human health through Toll-like receptors,immune system and inflammatory reaction.Improvements of gut microbiota spectrum and its balance have become an effective strategy for the treatment of certain diseases.This paper reviewed the relationship between gut microbiota and radiation injury and underlying mechanisms,in order to provide novel theoretical evidence and guideline for the therapy of radiation enteritis and other diseases.
2.Mandibular movement characteristics in patients with skeletal class Ⅲ malocclusion during chewing movement

Objective: To investigate mandibular movement characteristics in patients with skeletal class Ⅲ malocclusion during chewing different foods. Methods: 14 skeletal class Ⅲ adult patients with mandibular asymmetry, 10 patients without mandibular asymmetry and 10 normal control cases were chosen to find out the different condylar and incisor movement area ratio based on the aid of ARCUSdigma mandibular moving track analysis system. Results: First,during chewing soft food, the condylar tracing length of skeletal class Ⅲ adult patients with mandibular asymmetry were significant different in sagittal and horizontal plane compared with the other two groups(P<0.05), while the incisor point trajectory difference was not statistically significant. Second, the condylar and incisor movement characteristic showed much more different among three samples during chewing brittle food(P<0.05). Third, in sagittal plane, the deflective condylar movement area ratio was significantly higher than that of the other two groups during chewing hand food(P<0.01). Conclusion: The mandibular movement in skeletal class Ⅲ adult patients are affected obviously by the shape and texture of food. Asymmetric lower mandible patients have a greater preponderance of this phenomenon.
3.Construction and application of multidimensional evaluation system for standardized residency training in a tertiary district hospital in Beijing

Objective:In order to improve the quality of standardized residency training in a tertiary hospital in Beijing, and to establish a multi-dimensional evaluation system based on post competency and professional accomplishment.Methods:The residents trained in our hospital were evaluated by questionnaires sent by third parties from the five aspects of evaluation by patients, peers, nurses, superiors and themselves. In this paper, SPSS 24.0 software was used to analyze the reliability and validity of the multidimensional evaluation scale. The evaluation results were analyzed by ANOVA and Pearson correlation analysis.Results:The hospital met the requirements for the post competency and professional accomplishment of the residents, and the overall level was good. However, the teamwork ability of some residents, especially the medical communication still needs to be improved.Conclusion:This study has enabled residents to pay more attention to the development of daily post competencies and professional accomplishment behaviors. It has provided an example basis for the further development of standardized residency training in the hospital, and carried out preliminary exploration for the design of training modules.
4.The Value of 3D-Reconstruction Technique with Spiral CT in Acetabulum Fracture

Objective To evaluate the usefulness of the transect imaging and 3D-reconstruction with spiral CT in acetabulum fracture.Methods 23 cases of acetabulum fracture proved by clinical plain radiograph,transect imaging and 3D-reconsection were reviewed retrospectively.3D-reconsection were obtained on the basis of spiral transect imaging with use of 2~3 mm collimation.3 pitch and 1.2~2 mm reconstruction interval.Results Fractures were demonstrated definitely with transect imaging.3D-reconstruction in all patients the transect imaging displayed the fractures and the displacement of the burden opex of acetablulum,the disruption within joint,the direction of fracture.Displacement of detached bone fragments and capitis femoris were display stereoscopically by 3D-reconstruction.Conclusion The transect imaging and the 3D-reconstruction of spiral CT technique with spiral CT could afford the more valuable messages.Surgeon will benefit greathy from their usefulness in the choice of the surgical intervention and operative planning.
5.AN OPTIMAL SELF-SCALING STRATEGY TO THE MODIFIED SYMMETRIC RANK ONE UPDATING

In the paper, the optimal self-scaling strategy to the modified symmetric rank one (HSR1) update, which satisfies the modified quasi-Newton equation, is derived to improve the condition number of the updates. The scaling factors are derived from minimizing the estimate of upper bounds on the condition number of the updating matrix. Theoretical analysis, and numerical experiments and comparisons show that introducing the optimal scaling factor into the modified symmetric rank one update preserves the positive definiteness of updates, and greatly improves the stability and numerical performance of the modified symmetric rank one algorithm.
6.Expressions and their Significance of Ki-67, p53, and survivin in esophageal cancer and precancerosis

Objective To investigate the expressions and clinical significance of Ki-67,p53,and survivin in esophageal cancer and precancerosis.Methods The expressions of Ki-67,p53,and survivin proteins were detected by immunohistochemical staining in 40 normal esophageal mucosa,136 precancerosis (42 mild atypical hyperplasia,43 moderate atypical hyperplasia,and 51 severe atypical hyperplasia),and 68 esophageal cancer tissues.The correlation of three proteins expressed in esophageal carcinoma tissues was analyzed.Results The positive expression rate of Ki-67 was 0 (0/40)for normal epithelium,35.7% (15/42) for mild dysplasia,51.2% (22/43) for moderate dysplasia,74.5% (38/51) for severe dysplasia,92.6% (63/68) for squamous carcinoma,respectively.The positive expression rate of p53 protein was 0 (0/40) for normal epithelium,28.6% (12/42) for mild dysplasia,46.5% (20/43) for moderate dysplasia,52.9% (27/51) for severe dysplasia,67.6% (46/68) for squamous carcinoma,respectively.The positive expression rate of survivin protein was 0 (0/40) for normal epithelium,38.1% (16/42) for mild dysplasia,55.8% (24/43) for moderate dysplasia,64.7% (33/51) for severe dysplasia,89.7% (61/68) for squamous carcinoma,respectively.Rank correlation analysis showed that abnormal expressions of Ki-67,p53,and survivin were correlated significantly with the pathological grading of the lesions (r =0.637,0.454,0.590,P <0.01).The expressions of Ki-67,p53,and survivin were positively correlated in esophageal carcinoma (r =0.407,0.646,P < 0.01).Conclusions The abnormal expressions of Ki67,p53,and survivin were associated with the processes of the esophageal canceration,and the joint detection with three parameters has important clinical value.

8.Early enteral nutrition versus parenteral nutrition for patients with acute pancreatitis: a meta-analysis

Objective To analyze the impacts of early enteral nutrition (EN) and parenteral nutrition (PN) on the prognosis of acute pancreatitis.Method By searching relevant literature between January 1996 and August 2013 in Chinese and English databases including Wanfang,VIP,CNKI,PubMed,the Cochrane Library,CBM,and EMABSE,we compared the prognosis of acute pancreatitis after EN or PN in terms of casefatality rate,infections,multiple organ failure,and other complications.Result Compared with PN,early EN significantly reduced the case-fatality rate [OR =0.37,95% CI (0.23,0.58),P < 0.000 1],decreased the infection rate [OR =0.24,95% CI (0.15,0.39),P <0.000 01],shortened hospital stay [MD =-9.87,95% CI (-10.84,-8.89),P < 0.000 01],and reduced complications [OR =0.26,95% CI (0.12,0.58),P =0.001 0] in patients with acute pancreatitis,although the incident of multi-organ failure showed no significant difference [OR =0.35,95% CI (0.10,1.19),P =0.09].Conclusions For patients with acute pancreatitis,early EN is superior than PN in terms of case-fatality rate,infection rate,hospital stay,and complications.Therefore,it should be applied in such patients whenever condition allows.

10.Effect of Qinghuang Powder combined Chinese herbs for Pi-strengthening and Shen-reinforcing on HIF-1alpha in bone marrow mononuclear cells of myelodysplastic syndrome patients: an experimental research.

OBJECTIVETo study the effect of Qinghuang Powder (QHP) combined Chinese herbs for Pi-strengthening and Shen-reinforcing (CHPSSR) on hypoxia-inducible factor 1alpha (HIF-1alpha) in bone marrow mononuclear cells of myelodysplastic syndrome (MDS) patients.
METHODSChanges of HIF-1alpha in bone marrow mononuclear cells of MDS patients were detected in 25 MDS patients treated by QHP combined CHPSSR using flow cytometry. Meanwhile, 13 healthy subjects were recruited as the control group. Changes HIF-1alpha levels in various serial bone marrow mononuclear cells were detected.
RESULTS(1) Among the 25 patients in the treatment group, there were 19 patients effective and 6 patients ineffective, with the total effective rate being 76%. (2) Compared with before treatment, levels of peripheral blood WBC, Hb, PLT, and ANC significantly increased in the treatment group after treatment, showing statistical difference (P < 0.05, P < 0.01). (3) Compared with before treatment, the HIF-1alpha mean fluorescence intensity was enhanced in bone marrow lymphocytes, monocytes, granulocytes, and nucleated red blood cells of the treatment group after treatment (P < 0.05, P < 0.01). Compared with the control group, the HIF-1alpha mean fluorescence intensity was weakened in bone marrow lymphocytes, monocytes, and nucleated red blood cells of the treatment group before treatment; while it was obviously enhanced in granulocytes (P < 0.01). But after treatment the HIF-1alpha mean fluorescence intensity increased more in the granulocytes of the treatment group than in those of the control group (P < 0.01), but there was no statistical difference in bone marrow lymphocytes, monocytes, or nucleated red blood cells.
CONCLUSIONQHP combined CHPSSR could improve HIF-1alpha levels in bone marrow lymphocytes, monocytes, granulocytes, and nucleated red blood cells of MDS patients, thus improving Hb levels of MDS patients, and improving their anemia and correlated symptoms.
