1.The immunomodulatory function and clinical applications of mesenchymal stem cells

BACKGROUND: Mesenchymal stem cells (MSCs) have the abilities of self-renewal, multidirectional differentiation and immunomodulation, and have become the focus of current research. OBJECTIVE: To summarize the immunomodulation of MSCs to different immune cells and the clinical applications of MSCs in the treatment of immune-related diseases. METHODS: The first author searched the PubMed and the CKNI databases for relative articles from January 1974 to December 2016. The key words were mesenchymal stem cells, immunomodulation, MSC1 and MSC2, autoimmune diseases in English and Chinese, respectively. Finally, 52 representative articles were included. RESULTS AND CONCLUSION: MSCs can inhibit the function of T lymphocytes, reduce the activation, proliferation and antibody secretion of B lymphocytes, affect the polarization of macrophages, inhibit the maturation of dendritic cells, inhibit the proliferation and toxicity of NK cells, so MSCs have the great potential in the treatment of immune-related diseases. However, MSCs exhibit the opposite immunomodulatory abilities under different inflammatory microenvironments, and moreover the definite and controllable mechanism of this phenomenon is still unclear, Therefore, future investigations may focus on the specific mechanism of MSCs in the clinical treatment of immune-related diseases.

3.Expression of the Four Mismatch Repair Genes Protein of Patients with Colorectal Cancer and Its Clinical Significance

Objective:To analyze the expression of the four mismatch repair genes protein (hMLH1,hMSH2,hMSH6 and hPMS2) of patients with colorectal cancer and its clinical significance.Methods:177 cases of patients with colorectal caner in the Third Affiliated Hospital of Guangzhou Medical University from January 2013 to December 2015 were randomly selected.Tested the expression of the hMLH1,hMSH2,hMSH6 and hPMS2 by immunohistochemistry,the relationship between protein expression and clinical parameters was analyzed.Results:Among 177 cases ofcolorectal cancer tissue,the deletion rate ofhMLH1 protein was 6.2% (11/177),the deletion rate of hMLH2 protein was 4.0%(7/177),the deletion rate of hMSH6 protein was 1.7%(3/177),the deletion rate of hPMS2 protein was 8.0%(14/177),the sum of the four values accounted for 19.8%(35/177) of all cases of colorectal cancer.The loss of expression of the four mismatch repair genes protein were correlated to tumor location (P<0.05),besides,the loss of expression of the hMLH1 and hPMS2 protein were correlated to degree of tumor differentiation (P<0.05),he loss of expression of the hMSH6 protein were correlated to depth of tumor invasion(P<0.05);But the loss was not correlated to age,sexes,lymph node metastasis and distant metastasis(P>0.05).Conclusion:The expression of loss phenomenon with mismatch repair protein appears in part of colorectal cancer,the loss phenomenon with mis match repair protein were correlated to tumor location and degree of tumor differentiation.Mutations of four genes in hMLH1,hPMS2,hMSH6 and bMSH2,to provide a reference value for the clinical judgment of prognosis and to develop a treatment plan.
4.Clinical analysis of 350 pregnant women with epilepsy

Objective To investigate the usage of antiepileptic drugs (AEDs),epileptic seizures,pregnancy outcomes and infant feeding practices among pregnant women with epilepsy,and provide scientific evidence for eugenics.Methods The clinical characteristics of 350 pregnant women with epilepsy from Shaanxi Provincial Epilepsy & Pregnancy Register from October 2012 to July 2016 were analyzed retrospectively.Results A total of 350 patients with 376 pregnancies were included in the final analysis.Among 376 pregnancies,272 pregnancies (72.3%) were under the treatment of AEDs.Of them,246 pregnancies (90.4%) were on monotherapy,26 (9.6%) on polytherapy.Only patients during 179 pregnancies (47.6%) took folic acid in the first trimester.Seizure frequency increased in patients during 123 pregnancies (32.7%),of whom patients during 43 pregnancies (35.0%) did not use AEDs.Although other patients during 80 pregnancies (65.0%) took AEDs,regular pharmacokinetic monitoring was absent.A total of 170 pregnancies (45.2%) attained seizure-free.Seizure frequency decreased in 42 pregnancies (11.2%) and remained unchanged in 24 pregnancies (6.4%).The rates of cesarean section and natural delivery were 43.8% (158/361) and 56.2% (203/361),respectively.Totally 4.5% (17/376) pregnancies had adverse pregnancy outcome,and 1.9% (7/376) had fetal malformations.The most common congenital malformation was heart malformation,which was observed in two offspring.One offspring developed dysostosis,one developed chromosome abnormality,and another developed cerebellum deformity.In addition,one twin developed conjoined deformities.Breastfeeding rate was 52.6% (190/361).Conclusions In Shaanxi province,AEDs compliance,pharmacokinetic monitoring and usage rate of folic acid still need to be improved in pregnant women with epilepsy.Clinical trial registration Chinese Clinical Trial Registry,ChiCTR-OOC-16009277
5.The mechanism of action of valsartan studied by HPLC-TOF/MS.

High performance liquid chromatography-time-off-flight mass spectrometer (HPLC-TOFMS) technology coupled with partial least squares discriminant analysis (PLS-DA) processed by SIMCA-P software was applied to investigate serum endogenous metabolites alternations of valsartan in spontaneous hypertension rats (SHR). And MetPA platform was used to connect identified potential biomarkers in corresponding metabolic pathways to find possible therapeutic mechanism of valsartan. Valsartan significantly declined the blood pressure of SHRs (P < 0.05) at fourth week. The metabolic profiling significantly changed and four metabolites involved in G protein-coupled pathway were identified. Metabolomics is able to detect holistic and microcosmic alternations in organism, so as to elucidate therapeutic mechanism of drugs.

6.Gene mutation and enzymatic activity analysis of α-galactosidase A in a patient with Fabry disease

Objective To analyse the clinical characteristics,gene mutation and enzymatic activity of αgalactosidase A(α-GalA)in a 15-year-old male patient with typical Fabry disease,whose mother was without any clinical manifestations.Methods Clinical features and laboratory data were collected from the patient and his mother.Genomie DNA was extracted from peripheral blood of the patient.his mother,and a healthy control subject.Seven exons of the GLA gene were amplified by PCR.PCR products were purified.cloned into T vector,and then sequenced.The enzymatic activity of α-GalA Was measured by fluorimetrie substrate assay. Results DNA sequencing results showed that a missense mutation of 10036-10038delAAG in exon 7 WaS identified in the patient,resulting in the replacement of 374 lysine and 375 glyeine by arginine,which Was not previously reported.The patient Was a hemizygote with gene mutation,his mother WaS a heterozygote carrying gene mutation,and the healthy control without mutation.α-GalA enzymatic activity assay showed that the enzymatic activity of the patient with GLA gene mutation was only 50%of the healthy control subject,while the enzymatic activity of the patient's mother Was about 70%of the heahhy control SObject.Conclusiolls Detecting GLA gene mutation and α-GalA enzymatic activity in patients with Fabry disease who have been clinically diagnosed seelns to be helpful in finding other patients in the family and in further understanding the molecular pathogenesis of that disease.
7.Exploring the associated rules of traditional Chinese medicine and western medicine on urinary tract infection with text mining technique

Objective To explore the associated rules between western medicine and traditional Chinese medicine (TCM) on urinary tract infection (UTI) with text mining technique.Methods The data set on UTI was downloaded from CBM database.The regularities of Chinese patent medicines (CPM),western medicines and the combination of CPM and western medicines on UTI were mined out by data slicing algorithm.The results were showed visually with Cytoscape2.8 software.Results The main function of CPM was focused on clearing heat and removing toxicity,promoting diuresis and relieving stranguria.For western medicine,antibacterial agents was often used and it was also frequently used together with CPM such as Sanjinpian.Conclusions Text mining approach provides an important method in the summary of the application regularity for disease in both TCM and western medicine.

OBJECTIVE:To prepare famotidine sodium chloride injection METHODS:The famotidine sodium chloride injection was prepared with aspartic acid as solutizing agent and sodium chloride injection as solvent The contents of famotidine and its related substances were determined by means of HPLC Influencing factor and the stability of famotidine were studied and the therapeutic effect was observed RESULTS:The content of famotidine was slowly reduced while temperature rising and time prolongine The content of famotidine was 98 6% at the end of the 6th month at 40℃ with accelerating experiment Its content was 98 4% after one year storage under room temperature Its content was 98 5% at the 10th day after illumination with 4 500Lx light in accelerating experiment The content of related substances was less than 2% The total effective rate was 93 75% CONCLUSION:Famotidine sodium chloride injection was stable in property and had definite therapeutic effect,and its clinical application was simple and free from contamination
9.Combined detection of capsule associated protein 10 and virulence-associated DEAD-box RNA helicase 1 in the clinical diagnosis of Cryptococcus neoformans meningitis

Objective To establish the quantitative detection of capsule associated protein 10 (CAP10)and virulence-associated DEAD-box RNA helicase 1(VAD1)genes in Cryptococcus neoformans (CN) and compare the diagnostic values of single gene test and combined gene test in CN meningitis.MethodsTwenty-three CN meningitis patients with fungal culture or ink staining or CN antigen detection positive in cerebrospinal fluid (CSF) were recruited and patients with craniocerebral trauma were recruited as controls.Standard plasmids were constructed using standard CN strain.Real time fluorescent quantitative polymerase chain reaction (RT-FQ-PCR) was established to detect the mRNA expressions of CAP10 and VAD1 genes in the CSF of patients with CN meningitis,which were compared with the results of CSF ink staining,fungal culture and antigen detection.The diagnostic values of single gene test and combined gene test were compared by chi square test.Results Among 23 CN meningitis patients,22 (95.6%) were CAP10 mRNA positive detected by RT-FQ-PCR,which was significantly higher than both ink staining (16/23,69.6%,x2 =4.167,P<0.05) and fungal culture (15/23,65.2%,x2=5.143,P<0.05),respectively; but not significant different from antigen detection (21/23,91.3%,x2=0.500,P>0.05).There were also no statistical significant differences between combined detection of CAP 10 + VAD1 and CAP 10 or VAD1 single gene test (P>0.05).ConclusionRT-FQ-PCR detection is successfully established using virulence genes as target,which is superior to the conventional methods.
10.Expression of CD40 and CD40L on peripheral blood mononuclear cells in patients with condyloma acuminatum.

To observe the expression of CD40/CD40L on peripheral blood mononuclear cells (PBMC) in patients with condyloma acuminatum (CA), flow cytometry was employed to examine the expression of CD40 and CD40L on PMBC in 36 patients with CA and 20 healthy controls. Our results showed that mean level of CD40 expression in CA patients was significantly lower than that in the controls (6.58% +/- 2.74% vs 14.81% +/- 6.12%, t = 5.703, P < 0.05); the average level of CD40L in CA patients was also significantly lower than that in the controls (0.73% +/- 0.54% vs 2.67% +/- 2.43%, t = 3.532, P < 0.05). Our results suggest that the reduced costimulatory interaction of CD40 and CD40L in CA patients may be one of the important factors responsible for the low cellular immunity.
