1.Changes of vascular endothelial growth factor and tumor necrosis factor-α levels in serum of patients with primary hepatocellur carcinoma

Objective To investigate vascular endothelial growth factor(VEGF) and TNF-α levels in the serum of patients with primary hepatocellur carcinoma(HCC),and to evaluate their relevance with the development and metastasis of HCC.Methods VEGF and TNF-α levels in serum were examined by enzyme linked immunosorbent assay(ELISA) technology in 87 patients with HCC and 10 healthy individuals.Results Serum VEGF and TNF-α,levels in HCC patients were significantly higher than that in the contorl group.In addition,HCC patients infected with hepatitis virus(including HBV and HCV) showed a remarkable elevation in serum VEGF and TNF-α levels.The VEGF and TNF-α levels in HCC with metastasis were also higher than those without metastasis.Conclusion The serum VEGF and TNF-α levels in HCC patients might serve as the pivotal biological indices that could reflect HCC progressien and metastasis.

3.Clinical Values of Blood Serum Concentration of MHD in Children with Epilepsy of Different Ages

Objective To investigate effects of blood serum concentration of MHD in children with epilepsy of different ages. Methods Two hundred children with epile psy were divided into four groups according to age,group A:<3 years old in-fants;group B:3~6 years old preschoolers;group C:7~13 years old school-age chidren.Group D:14~18 years old adoles-cents,each group had 50 chidren.The serum concentration of active metabolite was detected by high-perform ance liquid chromatography (HPLC)method.Results The serum concentration of MHD increase with age,but there were no statistical significance (P>0.05).Conclusion The indiv idual variation in blood serum concentrations of MHD in children with epi-lepsy adminisered with oxcarbazepine is high,therefore monitoring the blood serum concentration is helpful to adj ust the dose of medication and to promote the implementation of the individualized dosing regimens.
4.Effect of proximal femoral intramedullary nailing combined with anti-osteoporosis drugs in the treatment of intertrochanteric fracture in elderly patients

Objective To investigate the clinical effect of the proximal femoral nail anti rotation nail combined with anti osteoporosis drugs in the treatment of intertrochanteric fracture in elderly patients.Methods From October 2014 to October 2015 in our hospital department of orthopedics for PFNA surgical treatment of 80 cases of intertrochanteric fractures in osteoporotic patients, according to the time of admission were divided into simple operation group (control group) and combined group (treatment group),with 40 cases in each group.Both the two groups were treated with PFNA surgery,treatment group were treated with ossification in three alcohol,calcium and calcitonin in the treatment of patients with hip.The fracture healing and hip function were compared between the two groups.Results The treatment group callus appear time,healing time were lower than the control group, the difference was statistically significant (P<0.05);One and three months after operation,the callus scoring of treatment group was higher than control groups (P<0.05);Six months after operation, the bone density of treatment group were higher than control group (P<0.05),the satisfactory rate of treatment group 90.00% was higher than the control group72.50% ( P<0.05 ) .Conclusion PFNA combined with anti-osteoporosis drugs in the treatment of elderly patients with intertrochanteric fracture is conducive to promote fracture healing , improve the condition of osteoporosis in patients with osteoporosis.
5.Comparison of Legionella Contamination of Cooling Water System and Freezing Water System in Central Air Conditioning System

Objective To compare the Legionella contaminative status of the cooling water system with freezing water system in central air conditioning system. Methods Legionella was tested in water and samples of all components from the cooling tower, condenser, swelling water box and evaporator of central air conditioning among 26 public places. Results The total detected rate of Legionella was 64.0% among 100 samples from two kinds of water system, 71 strains of Pneumophila legionella were separated. The detected rates were 75.0% and 52.1% in the cooling water systems and freezing water systems respectively. The amount of Legionella was more than 100 per milliliter in the cooling water systems. The detection rate of cooling tower water and swelling water box water was 100% in the air conditioning system run for over 10 years and 5-10 years. Concordance rate of consequence was 50.0% in cooling tower water and condenser water of the cooling water systems, the rate was 55.0% in swelling water box water and evaporator water of the freezing water systems. Conclusion This study suggested that more attention should be paid to the contaminative status of Legionella in freezing water system of central air conditioning system. Cooling tower and swelling water box should be the key parts in cleaning and disinfection of air conditioning system.
6. Enlightenment of “virtual reality” training program of US army medical staff to the construction of medical service in PLA overseas base of China

This paper introduces the characteristics and classification of “virtual reality (VR)” technology. Based on the “VR” training plan, training target, training stage and training mode of US army medical staff, combined with the current difficulties and challenges from the medical service work in PLA overseas base, this paper proposes some enlightenment for the construction and training of medical service force in base, so as to provide a reference for improving the medical service support capacity.
7.Arterial distribution of rectus abdominis masculocutaneous flap and its significance in breast reconstruction

Objective To study arterial distribution of rectus abdominis musculocutaneous flap and to evaluate whether it can be divided into several units for reconstruction. Methods The arteries of the rectus abdominis musculocutaneous flap were studied on 60 sides of cadavers by dissection and angiography. Results The superior epigastric artery (SEA) and the inferior epigastric artery (IEA) continued in a longitudinal direction. Most of their branches took on a typical spiral configuration and communicated with each other within muscle above the level of umbilicus. Many perforating arteries penetrated through the anterior rectus sheath to get to the overlying skin, but the highest concentration of major perforators were in the paraumbilical area. The inferior epigastric artery was more significant than superior epigastric artery in supplying the skin of the musculocutaneous flap. Based on thefstudies of the vascular anatomy of muscles, we could classify arterial distribution into 3 types: type Ⅰ (SEA 26.5 %, IEA 34.6%) revealed a single main intramuscular artery: type Ⅱ (SEA 64.7 %, IEA 48.1%) had two major intramuscular branches; type Ⅲ (SEA 8.8%, IEA 17.3 %) revealed three intramuscular branches. Our anatomic studies showed that the superior and inferior epigastric artery bifurcated or divided into more than two main branches in the majority of cases (SEA 73.5%, IEA 65.4%). Conclusion The rectus abdominis musculocutaneous flap could often be divided into several regions for breast construction which is based on the distribution of each branch of the artery.
8.Risk factors of in-stent restenosis in coronary artery disease patients with CYP2C19*1/*1

Objective To explore the risk factors of in-stent restenosis in coronary artery disease (CAD) patients with CYP2C19*1/ *1. Methods The population diagnosed with CAD subsequently underwent PCI, followed by CYP2C19 genotype examination. 127 patients with CYP2C19*1/ *1 followed up 1 year after PCI in our hospital were divided into ISR group (n = 47) and control group (n = 76) according to absence or presence of in-stent restenosis (the restenosis was defined as the reduction of lumen diameter ≥ 50%). The basic clinical data, laboratory results and medication and PCI of the two groups were retrospectively analyzed to figure out the etiological risk factors of in-stent restenosis in CAD patients with CYP2C19*1/ *1. Results Based on bivariate analysis, we found several independent variables (P > 0.05) including age, hypertension, BMI, smoking and drinking history. The independent variables with results of bivariate analysis of P < 0.05 included DM, physical exercise, TG, TBIL, FIB, and the stent length, diameter and number. A multivariate logistic regression analysis showed that such variables including DM,TG,FIB, stent length were independent predictors of ISR (P < 0.05). On the contrary, exercise and stent diameter may be the protective factor (P<0.05). Conclusions Clinical treatment for patients with high risk factors including improved life style, strengthened biochemical indicators monitoring, a reasonable treatment and appropriate stent placement according to individual coronary artery lesions , is expected to reduce the incidence of in-stent restenosis and improve the prognosis.
9.Clinical observation of the treatment based on Peitushengjin theory in patients with AECOPD with Ⅱ respiratory failure

Objective to observe the effect of lung and spleen mixture on lung spleen deficiency AECOPD with Ⅱ respiratory failure of TCM symptoms integral,nutrition,blood gas and related indicators.Methods in accordance with AECOPD with Ⅱ type of respiratory failure need to use the noninvasive or have a respirator assisted ventilation,and conform to the type of TCM for patients with spleen-deficiency type,40 cases were divided into two groups,control group to the conventional western medicine therapy and Chinese medicine group on the basis of western medicine treatment for reasons of Yifeijianpi mixture,oral or tube feeding,a week for a period of treatment,observation of patients before and after treatment of TCM symptoms and physical signs integral,APACHE Ⅱ score,serum albumin,pre-albumin,neutrophil percentage,CRP and blood gas analysis and related indicators was conducted.Results The results show that the TCM group can improve the patients' TCM symptoms and physical signs integral,there was significant difference between the two groups after treatment (P< 0.05);compare the serum albumin,pre-albumin,transferrin,neutrophil percentage,CRP and arterial blood gas between two groups of patients after treatment,Chinese medicine group is superior to the control group,with statistical significance (P< 0.05);And compared with control group,the APACHE Ⅱ improvement situation of Chinese medicine group were superior to control group after the treatment after a week,with statistical significance (P< 0.05).Conclusion on the basis of using conventional western medicine treatment,treatment based on the theory of ridging raw gold formulae of Yifeijianpi mixture has good effect in the treatment of AECOPD combined type Ⅱ respiratory failure,and can improve the nutritional status of patients and the indicators of arterial blood gas analysis.
10.Effect of Levothyroxine Replacement Therapy on Improving Diffused Left Ventricular Myocardial Lesions in Patients With Hypothyroidism

Objective: To study the effect of levothyroxine replacement therapy on improving diffused left ventricular myocardial lesions and cardiac function in patients with hypothyroidism. Methods: Our research included 2 groups: Hypothyroidism group,n=20 newly diagnosed patients and Control group, n=17 normal healthy subjects. Diffused left ventricular myocardial lesions were quantitatively evaluated by non-invasive cardiac MRI T1 mapping technique. Left ventricular myocardial T1 value and left ventricular function before and after levothyroxine replacement therapy were compared; the relationship between T1 value and thyroid function and the relationship between T1 value and the indicators of left ventricular function were analyzed. Results:①Compared with Control group, Hypothyroidism group had increased left ventricular myocardial T1 value, P<0.01, while decreased cardiac index (CI) and left ventricular peak filling rate (PFR), allP<0.05.②In patients with hypothyroidism, left ventricular myocardial T1 value was negatively related to serum FT3 level (r=-0.52,P=0.0006) and PFR (r=-0.43,P=0.0085).③Compared with pre-therapeutic condition, the patients with recovered normal thyroid function showed obviously decreased left ventricular myocardial T1 value and improved CI, PFR. Conclusion: Levothyroxine replacement therapy may improve diffused left ventricular myocardial lesions in patients with hypothyroidism, and therefore improve the cardiac function.
