According to the 'hygiene hypothesis', the intestinal microbes would not only regulate local immune status, but also affect systematic immune homeostasis, which may play an important role in the pathogenesis of allergic diseases. This review will mainly focus on investigating the intricate relationships among intestinal microbes, immune system and allergic diseases. Multiple factors affecting gut microorganisms may influence the immune homeostasis directly and later become risk factors of allergic diseases. This point of view may pave the way for the design of new interventions to the diseases.

Objective To explore the risk factors for in stent stenosis (ISR) in coronary artery disease in elderly patients undergoing postoperative percutaneous coronary intervention (PCI).Methods We chose 418 cases undergoing PCI in our hospital.There were 263 males and 156 females,aged from 60 to 79 years,average (71.3--8.9)years in this study.Under the same conditions of treatment strategies,we collected all possible risk factors for ISR in patients undergoing postoperative PCI,and statistics were analyzed.Results All patients were rechecked at 1 year after PCI.104 cases (24.9％) occurred ISR,and 314 cases (75.1％) were diagnosed without ISR.Multiple logistic analysis showed that,and sex had little effect on ISR incidence (P＞0.05).Body mass index(OR=9.54,95％CI:6.542～32.154,P=0.002),smoking history(OR=26.01,95％CI:5.532～122.238,P=0.000),hypertension(OR=15.72,95％CI:3.240～76.026,P=0.001),diabetes mellitus(OR=11.98,95％CI2.667～53.835,P=0.001),history of high cholesterol(OR=8.98,8,95％CI2.164～37.283,P=0.003),exercise time (OR 5.63,8,95 ％ CI2.004 ～ 25.385,P=0.013)had positive correlations with ISR incidence.Conclusions Effective control of risk factors including smoking,hypertension,diabetes,high cholesterol,proper exercise,and weight control can help prevent the development of ISR in elderly patients with postoperative PCI.

Objective To discuss the relationship between C-reactive protein and prognosis of acute coronary syndrome. Methods C-reactive protein of 60 acute coronary syndrome patients was evaluated. The individuals were divided into two groups:One group with higher CRP level and another group with normal CRP level. The following-up duration was 6 months. After correct therapy, the morbidity of re-angina, arrhythmia, heart failure, re-infarction and cardiac death was compared. Results The morbidity of re-angina, arrhythmia, heart failure, re-infarction, cardiac death was 46.3%(19/41), 43.9%(18/41),9.76%(4/41),22.0%(9/41),7.32%(3/41)respectively in higher CRP level group; The morbidity of re-angina, arrhythmia, heart failure, re-infarction was 15.8%(3/19),10.5%(2/19), 5.3%(1/19),5.3%(1/19)respectively in normal CRP group and there was no cardiac death accident. There was significant different between the two groups. Conclusions CRP plays an important role in the onset of acute coronary syndrome, and its level is related with the higher morbidity of re-angina, arrhythmia, heart failure, re-infarction and cardiac death.

Objective:To study the preparation technique and quality standard for new Shengfa pills. Methods: The best water extraction process of the medicinal herbs was optimized by orthogonal test using stilbene glucoside content and dry extract percent as the indices. The content of stilbene glucoside was determined by HPLC, and Hyssop and Chinese angelica in the preparation were identified by TLC. Results:The optimized preparation conditions were as follows:12-fold water was used in the 3-time extraction with 1 h for each time. The spots in TLC were clear without interference from the negative control. Conclusion:The preparation process is reasona-ble and feasible in techniques with controllable quality.

Objective To observe P27 expression and proliferation of mesangial cells of rats with Thy1 glomerulonephritis and to evaluate the interfering effect of valsartan, a specific angiotensin Ⅱ receptor antagonist, on Thy1 glomerulonephritis. Methods Rats were divided into normal control group, Thy1 glomerulonephritis group and valsartan treatment group. Following the onset of glomerulonephritis, renal morphological changes were observed on days 1, 3, 5 and 7, and proliferating cell nuclear antigen(PCNA) and P27 expression in glomerulus were measured by immunohistochemistry and Western blotting. Results High expression of P27 was found in quiescent mesangial cells of normal rats, but decreased expression in mesangial cells of rats with Thy1 glomerulonephritis, accompanying with the proliferation of mesangial cells. In valsartan treatment group, glomerular hypercellularity, mesangial matrix expansion and PCNA expression were significantly reduced as compared with those of the untreated rats with Thy1 glomerulonephritis from day 3 to day 7( P

The clinical and pathological data of eight cases of nasal solitary keratoacanthoma were analyzed and relevant literatures were reviewed. All of the cases were verified via pathological examination after local lesion excision, along with reconstruction of tissue defects in external nose. The patients were all primary healed. With a follow up from 8 months up to 7 years, only one case relapsed one year after first surgery, and given another operation, no recurrence happened thereafter. The other seven cases did not recur so far. The nasal solitary keratoacanthoma is a squamous tumor with potential malignancy, which manifests similarly with squamous cell carcinoma, and the correct diagnosis requires comprehensive judgment from both the clinical and histopathological characteristics. The disease shows favorable prognosis and low recurrence rate.

Objective To compare treatments of intra-articular calcaneal fractures with and without bone graft in foot and ankle surgery.Methods A comprehensive search was conducted on line for all English research articles published from January 1990 through December 2010 dealing with open reduction and internal fixation with and without bone graft for intra-articular calcaneal fractures.Two reviewers evaluated each study to determine whether it was eligible for inclusion and collected the data of interest.Meta-analytic pooling of group results across studies was performed for the 2 treatments.Results The systematic review identified 32 qualified primary studies with 1281 fractures.No significant differences were found in postoperative infection, osteoarthritis or subtalar fusion between the 2 treatments ( P ＞ 0.05) .The average full weight-bearing time in bone graft treatment was significantly lower (5.4 weeks) than in non-graft treatment (10.5 weeks) .The mean B(o)hler angle was significantly larger and long-term loss of collapse significantly lower in bone graft treatment than in non-graft treatment.The bone graft treatment had a lower American Orthopaedic Foot and Ankle Society (AOFAS) score (71.4 points vs.80.5 points) but a higher Creighton score (89.9 points vs.81.0 points) compared with non-graft treatment.Pooled mean results showed no significant differences in the weighted excellent-to-good rate between the 2 treatments (excellent 35%, good 40%, fair 21% and poor 4% for the bone graft treatment versus excellent 34%, good 42%, fair 14%, and poor 10% for the non-graft treatment).Conclusions Open reduction and internal fixation with bone graft for intra-articular calcaneal fractures may lead to better reduction of B(o)hler angle and early return to full weight-bearing without increased risk of postoperative infection.However, since more cases of joint depression and comminuted fracture have been included in the bone graft group in the present study, a prospective controlled comparative study with a large scale of samples is still needed.
