Objective To investigate the protective effect of mucopolysaccharide polysulfate cream(MPC) and micro needle array drug delivery technologarray(MNADDT) on the blood vessel of arteriovenous fistula.Methods Eighty cases maintenance hemodialysis patients with autogenous arteriovenous fistula were randomly divided into control group(with the treatment of MPC) and study group(with the treatment of MPC plus MNADDT),40 patients in each group.Both groups were studied for the duration of 12 months.The vascular complications of puncture pain sense,puncture failure times,phlebitis,hardening of the arteries,internal fistula stenosis or embolism,pseudoaneurysm,swollen hands comprehensive syndrome of two groups were observed,and the changes of arteriovenous fistula blood flow,blood vessel diameter by Doppler ultrasound were observed in the two groups for 12 months.Results (1)The incidence of puncture pain(2.71±0.11 vs.2.76±0.14,t=2.21,P<0.05),puncture failure times(38 times vs.73 times,χ2=11.425,P<0.05),phlebitis(2 cases vs.8 cases,χ2=4.341,P<0.05) and blood vessel sclerosis(2 cases vs.9 cases,χ2=5.446,P<0.05) of the treatment group were significantly lower than that of the control group during the study(P<0.05).(2)Blood flow of arteriovenous fistulain of control group before and after treatment were (859.7±148.3) ml/min and (946.5±169.2) ml/min respectively,and the difference was significant(t=2.356,P<0.05).While blood flow of arteriovenous fistulain,cephalic vein diameter,head of venous flow velocity of study group before and after treatment were (824.6±171.5) ml/min and (1 218.1±241.9) ml/min,(5.59±0.74) mm and (6.02±0.57) mm,(94.23±27.35) cm/s and (115.85±36.63) cm/s,respectively,and the differences were significant(t=8.212,2.382,2.877,P<0.05),there were no significantly changes in the above indexes in the study group after treatment(t=5.3612,2.152,2.281,P<0.05).Conclusion MPC plus MNADDT can reduce the vascular complications of arteriovenous fistula,improve blood vessel diameter and increase blood flow.

In response to such setbacks as inefficient medical service,costly human resources and low rate of return on capital,due to the pursuit ofMega hospitalsand extensive management of hospitals,Wuxi No.2 People's Hospital introduced the delicacy management chain to improve its management and service.This article studied its practice of delicacy management chain as follows.By means of the six gears of organization,clinic,outpatients,medical technicians,nursing,and logistics,the management chain pursues such management goals as flat management,attending-focused clinical medicine,centralized outpatient service,integrated medical examinations,vertical nursing management,and logistics outsourcing.These efforts are designed for the optimization of both scale and construction efficiency.

The authors probed systematically into the delicacy management measures adopted by the hospital in its internal mechanism reform, innovative medical service model, human resource optimization, and its performance appraisal system reform. Discussions in the paper include such topics as flat management for hospitals,intensification,centralization and integration in diagnose and treatment, as well as vertical nursing management, and the outsourced logistic services. The methods and paths to achieve these goals are summarized as “preciseness, accuracy, carefulness, and strictness”.

Objective To observe the effect of hyperthyroidism decoction on thyroid hormone and thyroid pathological changes of ex-perimental hyperthyroid rats. Methods 40 rats were randomly divided into four groups: Normal group,hyperthyreosis model group, western drugs control group and hyperthyroidism decoction group. The model rats were constructed by drenching Youjiale. Hyperthyroidism decoction group were given hyperthyroidism decoction of respective dosage, western drugs control group were given propylthiouracil. The serum contents of T3, T4, FT3, FT4 and TSH were determined by radio-immunity method, the thyroid pathological changes effected by hyperthyroidism de-coction were observed. Results Compared with model group, the contents of serum T3, T4, FT3, FT4, of hyperthyroidism decoction group were decreased, while the contents of serum TSH were increased. Hyperthyroidism decoction may improve thyroid pathological changes of ex-perimental hyperthyroid rats. Conclusion Hyperthyroidism decoction can effectively improve the thyroid function of experimental hyperthy-roid rats.

Objective To identify the diagnostic criteria and investigate the incidence and distribution of the patellofemoral arthritis in aged people in Wuchuan region of the Inner Mongolia of China. Methods A population-based cross-sectional survey was conducted among 1030 residents aged 50 years and over of Wuehuan County, Inner Mongolia. Survey participants, mostly farmers, reported heavy physical activity, completed an interview-based questionnaire and Skyline View weight-bearing films were evaluated for KeU-gren and Lawrence (K/L) grade. Radiological patellofemoral arthritis (RPFA) and symptomatic patellofemoral arthritis (SPFA) were defined. Statistical package SPSS 14.0, crosstabs chi-squae test and Excel were adopted for statistical analysis. Results One thousanod and twenty-five participants were included in this study and 2050 knee joint Skyline View weight-bearing films were analyzed. The incidence of RPFA was 8.72%(6.8% in men, and 10.5% inwomen). The incidence of SPFA was 6.33% (4.8% in men, and 8.0% in women).The RPFA incidence of age groups 50～, 60～,≥70 was 3.8%, 10.6%, 22.9% respectively, while the incidence of SPFA was 2.7%, 7.7%, 17.3% respectively. The Three age groups' SPFA/RPFA was 71.7%, 72.0%, 75.6% respectively. The incidence of RPFA was 6.00/0, 7.3%, 11.0%, 11.3%, and the incidence of SPFA was 4.2%,5.2%, 7.3%, 9.6% and the four SPFA/RPFA was 70.0%, 71.3%, 66.7%, 84.8% in people whose BMI was less than 18.5, 18.5～, 23.0～ and higher than ≥25.0 kg/m~2 respectively, 73.6% people had joint pain among RPFA.Conclusion The prevalence of symptomatic patellofemoral arthritis in the rural areas of China is 6.8%, the incidence of that in women is higher than in men, and elder people suffer a high incidence. And the heavier the people are, the higher the incidence of patellofemoral arthritis.
