Objective To elucidate the clinical characteristics and the outcome of acute renal failure (ARF) in the elderly. Methods Clinical data of 109 elderly patients(≥60 years) with ARF were analysed and compared with that of 175 younger adult ARF patients(18 40 years) in the same period. Results 109 out of 422 patients(25 8%) with ARF were more than 60 years. The mortality rate of ARF in these patients was 58 7%. ARF was induced mainly by prerenal factors (20 2%) and sepsis (15 6%). Complications such as infections (52 3% vs 29 7%), cardiovascular events (64 2% vs 38 3%), respiratory failure (43 1% vs 20 0%) and hyperkalemia (29 4% vs 15 4%) were more prevalent in elderly patient than that in the younger adult ARF patients ( P

Objective To investigate the intrapulmonary expression of tumor necrosis factor-? (TNF-?) gene during acute pancreatitis, as well as correlation between TNF-? and lung injury seen in many patients undergoing acute necrotizing pancreatitis (ANP). Methods Ninety SD rats were divided into three groups randomly. Acute edematous pancreatitis(AEP) and ANP were induced by retrograde infusion of sodium taurocholate (0.5% and 5%) into biliopancreatic duct respectively. Animals in the last group only underwent sham-operation. All the animals were sacrificed at certain time, plasma level of amylase and TNF-? was assessed. Objective To observe and compare efficacy and safety of vinorelbine (NVB) plus ifosfamide (IFO) (NI regimen) and NVB plus cisplatin(DDP) (NP regimen) in treatment of metastatic breast cancer.Methods 82 cases of recurrent and metastatic breast cancer after the failure of treatment with anthracyclines and taxanes,40 cases were treated with NI regimen (NVB 25 mg/m2,day 1,day 8,IFO 1.2 g/m2,day 1,day 2,day 3),and the other 42 cases received NP regimen (NVB 25 mg/m2,day 1,day 8,DDP 25 mg/m2,day 1,day 2,day 3).Both groups were constituted of 21 days one cycle.After 2 cycles of chemotherapy,the response rate and side effects were evaluated.Results All 82 cases were evaluated for objective response.The total response rate of NI group and NP group were 52.50 ％ (21/40) and 57.14 ％ (24/42).There was no significiant difference (P ＞ 0.05).By analysis of the correlative factors,the total response rate in treatment of triple-negative breast cancer of NI group was 43.75 ％ (7/16),and that of NP group was 44.44 ％ (8/18).The two rates were also approximative (P ＞ 0.05).Main side effects were both bonemarrow suppression and gastrointestinal reaction.Bonemarrow suppression rate at grade Ⅲ-Ⅳ of NI group [57.5 ％ (23/40)] had no significant difference compared with that of NP group[57.14 ％ (24/42)] (P ＞ 0.05).But grade Ⅲ-Ⅳ gastrointestinal rate of NI group [7.50 ％ (3/40)] was significantly lower than that of NP group [26.19％(11/42)](P ＜ 0.05).Conclusion NI regimen in treatment of advanced breast cancer is effective,and it shows a similar response rate compared with NP regimen.Also it is effective for triple negeative breast cancers.The toxicity can be tolerated,and the gastrointestinal reaction rate of NI regimen is lower.NI regimen is a good choose in treatment of advanced breast cancer.

Objective To evaluate the effect of early goal-directed fluid therapy with fresh frozen plasma in severe acute pancreatitis(SAP). Methods From January 2010 to June 2014,79 SAP patients were enrolled according to the continuous sampling method. All the patients were randomly divided into a control group who accept the traditional fluid therapy(group A),an experimental group 1 who accept early goal-directed fluid therapy(group B),and an experimental group 2 who accept the early goal-directed fluid therapy with fresh frozen plasma(group C). There were no significant differences of general conditions a-mong groups. The differences of ICU admission,mortality and occurrence rate of abdominal compartment syndrome(ACS)and MODS were compared among groups. Results Compared with group A,group B and C have a shorter length of ICU admission,a lower mortality and a lower occurrence rate of ACS and MODS (P < 0. 05). Compared with group B,group C have a shorter length of ICU admission,a lower mortality and a lower occurrence rate of ACS and MODS(P < 0. 05). Conclusion The method of early goal-direct-ed fluid therapy with fresh frozen plasma will contribute to shorten the length of ICU admission and reduce mortality and occurrence rate of ACS and MODS for patients with SAP.

Objective\ To prepare a digoxingen labelled cRNA probe for human tyrosine hydroxylase(hTH 2).Methods\ Using molecular cloning techniques the recombinant plasmid pGEMTH 2 was constructed.According to analysis of restriction endonucleases,the pGEMTH 2 contained the DNA fragment of hTH 2 gene and inserting sit preparation was correct for cRNA probe.From recombinant plasmid pGEMTH 2 prepared cRNA probe was identified by dot blot hybridization.Results\ The dot blot hybridization showed that the positive reaction sites were purple blue.Conclusion\ This probe prepared in present study was sensitive and reliable.This probe might provide a tool for identifying the effect of gene therapy in animal model of Parkinson disease.

Objective: To explore the clinicopathologic features of non-Hodgkin' s lymphoma (NHL) patients seen in our hospital in the recent ten years and to discuss the relationship between clinical data and the prognostic factors. Methods: The clinical and pathological features of 1,012 NHL cases were retrospectively analyzed. Results: The main pathological subtypes included 346 (34.1%) cases of diffuse large B-cell lymphoma (DLBCL), 185 (18.3%) cases of peripheral T-cell lymphoma (PTCL), 97 (9.6%) cases of follicular lymphoma (FL), 94 (9.3%) cases of extranodal mucosa-associated lymphoid tissue (MALT), 62 (6.1%) cases of nasal NK/T-cell lymphoma and 47 (4.6%) cases of T-lymphoblastic lymphoma. There were 619 (61.2%) cases classified as nodal lymphoma and 393 (38.8%) cases classified as extranodal lymphoma. For the whole group, the 5-year overall survival (OS) rate was 45.8%. The univariate analysis showed that the clinical stage, performance status (PS), B-symptom, age, tumor size, the serum lactate dehydrogenase (LDH) level, the number of extranodal involvement and IPI were correlated with prognosis of NHL(P＜0.05). The multivariate analysis showed that the source of T cells, Ⅲ/Ⅳ clinical stage, IPI (3～5) and increased LDH were correlated with poor prognosis (P＜0.05). Conclusion: The incidence of NHL in middle-aged people is relatively higher. Extranodal lymphoma and B-cell lymphoma are more frequent in Xinjiang. Immunophenotypes, clinical stages, IPI and the level of LDH expression are closely correlated with prognosis of NHL.

Objective To develop the knee osteoarthritis (KOA) TCM syndrome evaluation scale. Methods Resrarch team collected subjective symptoms from all kinds of syndrome types of KOA and classified the subjective symptoms, which formed the first version of item pool containing 24 subjective symptoms. Expert questionnaires were conducted to calculate the weight coefficient of items, and formed the item pool of the Knee Osteoarthritis TCM Syndrome Evaluation Scale. According to the patient's attention to the 11 items, the items were weighted.Results The Knee Osteoarthritis TCM Syndrome Evaluation Scale contained 11 items, including pain, swelling and stiffness with different weighted scores.Conclusions The Knee Osteoarthritis TCM Syndrome Evaluation Scale were introduced to assess the KOA outcomes on the basis of TCM syndromes. It showed the characteristic of TCM and TCM syndrome.It could provide an objective and standardized measurement for TCM treating KOA.

Objective To compare of the difference about ectopic activation between autogenous bone graft and allograft from large segment tibia of rabbits.Methods Eighty healthy adult Chinese rabbits (6 months of age),weighing (2.5 ±-0.5)kg,were randomly divided into experimental group (allogeneic bone group) and the control group (autograft group),40 rabbits in each group.Another 10 rabbits were allogeneic bone donor.In experimental group,when 1.5 cm long rabbit tibial allograft were finishied,they were implanted into spatium intermusculare between the musculus rectus femoris and medial vastus muscle of the rabbit around the saphenous artery and were fastened to the femur by 1.0 mm Kirschner-wire.In control group,autologous tibias were done,the same as experimental group including length and position and method.Four weeks and 8 weeks and 12 weeks postoperative,respectively,the postmortem specimens were examined gross and immunohistochemistry and the expression of BMP-2 and collagen type Ⅰ of transplanted bone tissue were detected.Results BMP-2 mainly exist in cytoplasm of osteoblasts and chondrocytes undifferentiated mesenchymal cells.Collagen type Ⅰ primarily exist in the bone matrix around the pit of bone.The expression level of BMP-2 of experimental group in postoperative 4,8,12 and 16 weeks were 85.25 ± 4.47,109.44 ± 14.69,141.85 ± 9.45,116.25 ± 14.18,respectively,and the expression level of BMP-2 of control group were 103.78 ±-6.59,124.95 ± 14.94,145.46 ± 8.10,112.48 ± 13.27,respectively.The expression level of collagen type Ⅰ of experimental group in postoperative 4,8,12 and 16 weeks were 78.74 ± 7.99,95.95 ± 6.99,139.91 ± 4.32,137.76 ± 3.48,respectively,and the expression level of BMP-2 of control group were 88.87 ± 11.26,102.45 ± 2.82,140.76 ± 4.62,139.05 ± 4.55.Compared with control group,there was a significant difference in the expression level of the BMP-2 and collagen type Ⅰ of experimental group in postoperative 4,8 weeks (P ＜ 0.05),but,there was no significant difference in the expression level of the BMP-2 and collagen type Ⅰ of experimental group in postoperative 12,16 weeks (P ＞ 0.05).The amount increased gradually during 4 weeks,8 weeks,12weeks,peaked at 12 weeks,BMP-2 displayed a downward trend at 16 weeks,and collagen type Ⅰ basiclly maintain the level of 12 weeks.Conclusion Allograft segment could complete activation while they are implanted into spatium intermusculare containing famous blood supply within 3 months,there is no significant difference between autologous bone and allograft,it shows the feasibility of ectopic activation about allograft segment.

Objective To evaluate the accuracy of core needle biopsy (CNB) in diagnosing breast masses and its coherence with immunohistochemical (IHC) examination results of estrogen receptor (ER), progesterone receptor (PR) and Her2 protein between pre-and post-chemotherapy in invasive breast cancer. Methods The results of 516 CNB cases from June, 2005 to April, 2008 were analyzed retrospectively. The pathological examination was performed by two pathologists independently. Results 484 cases of malignant tumor, carcinoma in situ and phyllodes tumor were found in this group with the sensitivity of 96.7%. Sixteen cases of false negative (3. 3% ) were demonstrated by surgical biopsy. The accurate rate of CNB was not influenced by the maximum diameter of masses ( P = O. 423 ). The agreement rate of IHC results of ER, PgR and Her2 between pre- and post-chemotherapy were 90. 3%, 76. 8% and 82.5%, respectively. Conclusion CNB is a useful diagnostic method with a satisfactory accuracy in any size of breast masses. Given the histological heterogeneity of invasive breast cancer and the influence of ehemotherapy, the coherence of prechemotherapy IHC for ER, PgR and Her2 is not optimal with that of post-chemotherapy.

Objective To compare the accuracy and clinical effect between X-ray stereotactic vacuum-assisted biopsy (SVAB) and metal wire guided lumpectony.Methods From January 2010 to June 2014,681 cases of breast micro-calcification biopsy were performed.Among them,78 cases were performed with SVAB and 603 eases were performed with the method of stereotaetic metal wire guided lumpectomy.All cases were non-palpable breast lesions (NPBLs) and breast imaging-reporting and data system (BI-RADS) assessment categories 4.The diagnostic accuracy and clinical effect were compared.Results The sensitivity of both methods was 100 ％ with no misdiagnosis.The underestimation rate of SVAB was 12.5 ％.Compared with the method of metal wire guided lumpectomy,SVAB had many advantages,such as easy to use,quickly performed,low rate of local deformation and lower rate of operative complications.77.5 ％ patients benefited from SVAB by avoiding open surgery of benign disease.Conclusions SVAB is an accurate,safe and convenient method of biopsy.It can be recommended as the preferred method of micro-calcification (BI-RADS 4).Additional operation should be performed on patients with the pathological diagnosis of middle and high grade of dysplasia and any kind of carcinoma.
