1.Effect if Dexamethasone and Some Drugs on Shope Rabbit Papilloma.

In 1933 Shope described an infectious papilloma of wild cattontail rabbits. Shope rabbit papilloma is a virus-induced epidermal neopIasm indigenous to western cottontail rabbits and readily transmissible to domestic rabbits by inocuiation with an cell-free extract of wild cottontail papilloma. Shope papilloma induced in the domestic rabbits may spoataneously, remain benign, or progress to carcinoma. Kidd et al. (1936) noted that the regression is not correlated with the titer of virus neutralizing antibody. The possibility that papilloma regression is brought about by a mechanism anaIogous to that operating in allograft rejection was suggested by kidd et al. (1938) with the view of the histological resemblance between papilloma regression and rejection of allografted tissue. That regression is mediated by an immune mechanism is strongly indicated by the findings of Evans et al.(1962) that the proportion of rabbits showing regression is substantially increased by vaccination of rabbits with their own papilloma tissue or with papilloma tissue from another rabbits. McMichael (1967) demonstrated that suppression of regression was achieved by use of large doses of corticosteroid. The effect of 5-fluorouracil on human warts was mentioned as being effective by Goldman et al. (1963). In Korea resinous extract from Paulownia coreana has long been used empirically in the treatment of human warts. In this investigation effects of dexamethasone administration on the development and the spontaneous regression of Shope papillomas in domestic rabbits were studied. At the same time, observations were made to evaluate the comparative effectiveness of topical application of 5% 5-fluorouracil ointment and resinous extract from Paulownia coreana on Shope papilloma in the domestic rabbit. The results are summarized as follows: l. Administration of dexamethasone significantly reduced the spontaneous regression of Shope papillomas in domestic rabbits by 12 weeks after virus inoculation. 2. Topical application of 5% 5-fluorouracil ointment on Shope papillomas cured the tumors by 4 weeks of drug application. However, similar treament with resinous extract from Paulownia coreana was without any demonstrable effects. 3. Marked losses of body weights became manifest in dexamethasone treated rabbits. 4. Reinoculation of Shope papilloma virus into 2 rabbits whose papillomas had regressed 3 months ago failed to produce papillomas.

2.Surveillance for Spectinomycin Resistant Neisseria Gonorrhoeae.

In 1982, we reported a case of recalcitrant urethritis caused by a strain of spectinomycin resistant PPNG. It caused a great fear that it might be the signal of an end of spectinomycin era, The disk-prediffusion method was used on all isolates from the Choong-Ku VD Clinic between August 1984 and December 1985 to screen out strains of N. gonorrhoeize resistant to spectinomycin, with disks containing 100pg of spectinomycin, Strains with inhibition zone greater tha.n 25 mm were considered susceptible, We could not disclose a single strain which wa.s resistant to spectinornycin, The reason why we do not see any more of spectinomycin. N. gonorrhoeae requires some speculation, It is possible that the mutant which had resistant to spectinornycin might not have teen quite stable and, after series of passage through human hosts, mutated back to original spectinornycin sensitive states. It is suggested that continual surveillance for spectinomycin resistant N. gonorrhoeae should be conducted.

3.Treatment of Gonorrhoea.

Because of increasing resistance of circulating N. gonorrhoeae and frequent failures in the treatment of gonorrhoea, intensive work on gonorrhoea has become of paramount importance. During January 1980-April 1984, at the Choong-Ku VD Clinic in Seoul, 3,340 male patients with uncomplicated gonococcal urethritis were treated with various treatment regimens. Diagnosis of gonorrhoea and declaration of a treatment failure were made on the basis of positive urethral culture. In 1984, the prevalence of Penicillinase Producing N. gonorrhoeae (PPNG) was about 30%, The pretreatment minimun inhibitory concentration of various antibiotics were quite high. Even for non-PPNG urethritis standard penicillin regimens gave unsatisfactory results. For PPNG urethritis, only spectinomycin, cefoperazone and cefotaxim-probenecid regimens gave satisfactory results. No spectmomycin resistant strain of N. gonorrhoeae has been found since 1982 at the Choong-Ku VD Clinic. As an agent of single drug therapy, spectinomycin seems to be one of the most cost effective drugs in the treatment of uncomplicated gonorrhoea in men.

4.A case report of orthodontic treatment for adult patient with upper prognathism.

A 23 year-old female with skeleto-dentoalveolar protrusion of maxilla, minor broken contact points between anterior teeth, and missing of lower 1st molars, has been treated with multibanded edgewise technique. After treatment of 14 months, she has gained functional overbite-overjet relationship and facial harmony due to the retraction of upper anterior teeth. Root resorption was slight. Especially, us ing the space of missed lower 1st molars instead of extracting lower premolars, expected and favorable results were obtained.

5.Glomangiomyoma on the Face.

We report a rare case of glomangiomyoma in a 25-year-old man who had an asymptomatic, 1.5 × 1.5cm sized, round, skin-colored, soft mass on the right cheek. Histopathologically, the lesion was consistent with glomangiomyoma which showed irregularly dilated vessels lined by endothelial cells and several outer layers of glomus cells, and smooth muscles distributed within the tumor. The tumor cells were characterized immunohistochemically by the presence of smooth muscle actin, muscle specific actin, and vimentin. To our knowledge, this is the first case report of glomangiomyoma on the facial location in the Korean literature.

6.Multiple Eccrine Hidrocystoma Treatedwith 1% Topical Atropine Sulfate.

We report a case of multiple eccrine hidrocystoma (MEH) in a 57-year-old woman who had asymptomatic, discrete, skin-colored, shiny, firm papulovesicles on the face. These lesions enlarged during summer or when the patient did housework, and decreased in sue during winter or rest. Histologic examination showed a dilated, unilocular, cystic, invaginated structure within the middle dermis. The wall of the cyst generally consisted of two layers of Rat or cuboida1 epithelial cells. Decapitation secretion and myoepithelial cells were not observed. The lesions improved markedly with 1% topical atropine sulfate.

Entrapment neuropathy of the suprascapular nerve is rare and frequently overlooked in the differential diagnosis of shoulder pain. There have been few published reports on a ganglion compressing the suprascapular nerve. We experienced a case of entrapment neuropathy of the suprascapular nerve by a ganglion that compressed the inferior branch of suprascapular nerve at the spinoglenoid notch.

10.Extracellular Matrix and Astrocytic Response during Regeneration following Cryogenic Injury in Adult Rat Cerebral Cortex.

Astrocytes are the most numerous cellular elements in the cerebrum, and they normally have a very slow turnover rate. But during regeneration after injury, they proliferate markedly resulting in astrogliosis. The extracellular matrix in the central nervous system is present in the vessel walls and in the external glia limitans as a basal lamina. The presence of an intact extracellular matrix framework is important in regeneration after injury. Understanding the properties of astrocytic proliferation will be helpful to find out new treatment for functional recovery in the central nervous system. In this study, after cryogenic injury was performed on the cerebral cortex in rats, changes in astrocytes and the extracellular matrix were observed using light microscopy, immunohistochemical stain for glial fibrillary acidic protein(GFAP), proliferating cell nuclear antigen(PCNA), fibronectin, laminin, and type IV collagen, autoradiography and electron microscopy. The results were as follows; 1) The coagulative necrosis, which followed cryogenic injury on the cerebral cortex was healed, forming a new pia mater above the lesion. 2) Some of the PCNA positive cells were astrocytes and some of the GFAP positive cells showed a positive reaction to PCNA. 3) Proliferating astrocytes labelled by autoradiography or immunohistochemical stain for PCNA reached maximal numbers 3days after the injury and they were no longer found 2 weeks after injury. 4) In autoradiography with immunohistochemical stain for GFAP, about 1% of GFAP positive astrocytes were labelled by autoradiography and in double immunohistochemical stain for PCNA and GFAP, about 8-16% of GFAP positive astrocytes were also stained by PCNA. 5) In immunohistochemical stain for fibronectin, laminin and type IV collagen, laminin and type IV collagen were present in the newly formed blood vessel walls and fibronectin showed a diffuse positive reaction within the lesion. The new pia mater was formed within 2 weeks after the injury. 6) On electron microscopic examination, basal lamina material was found in the vessel wall 1 week after the injury and at 2 weeks, a nearly complete and continuous basal lamina was formed although the thickness was uneven. According to these findings, astrocytes in the cerebral cortex of adult rats proliferate very early in the regenerative period after cryogenic injury. At 2 weeks after the injury, this regeneration ceases and the damaged basal lamina of pia mater and vessel wall were reconstituted.
