1.Changes of Plasma Somatostatin in Children with Septic Shock

Objective To explore the changes of plasma somatostatin(SST) in children with septic shock.Methods The level of plasma SST in children with septic shock (test group,n=21) on an empty stomach at shock stage,blood pressure and heart rate recovery stage,recovery stage(at that time clinical symptoms and signs disappeared,infection indicators such as blood routine and CRP returned to normal,about 6-12 days after admission) were detected by competive radioimmunassay,the level of SST in healthy children(healthy control group,n=25) on an empty stomach on morning was detected,too.The levels of plasma SST between septic shock concbined with paralytic ileus group and without paralytic ileus group were compared.Results 1.Level of plasma SST of test group at shock stage[(44.60?16.83) ng/L]was significantly lower than that of control group[(123.15?6.57) ng/L](t=-12.16 P0.05).The level of plasma SST of children with paralytic ileus [(28.10?7.0) ng/L] was significantly lower than that of children without paralytic ileus [(56.98?9.44) ng/L](t=-7.70 P
2.Therapeutic Effects on Single and Repeated - Dose Administration of Clonazepam for Seizure in Children

Objective To explore the change of degrade of clonazepam in serum with single and repeated - dose administration in children with seizure, and find a reasonable method for using the clonazepam. Methods Children with seizures were divided into single - dose paradigam, repeated - dose paradigam, and decreased - dose paradigam. The concentration of CZP in serum was determined by high performance liquid chromatography( HPLC). Results The serum concentrations of clanazepain in single - dose paradigam were (101.9?12.1),(76.9 ? 5.8),(50.7?2.9),(30.9?5.4),(21.5?6.8)?g/L,the time point that the blood samples collected were 15,30,60,120 and 480 min. The serum concentrations in repeated - dose paradigam were (97. 2 ? 6. 1),(130.4? 13. 4), (99. 4 ? 9.8),(79.6?2.4)?g/L,in decreased-dose paradigam were( 101.1 ?13.1),(123.1 ?6. 6), (99.4 ?9. 8), (79. 3 ? 2. 2)?g/L,in these two groups,the time point were 15,45,60 and 120 min. Conclusion Repeated administration of CZP with decreased dose may increase its effectiveness in treatment without substantially increasing toxicity.
3.A Meta analysis of Shensongyangxin capsule combined metoprolol in treatment of ventricular premature beats

Objective:To compare the therapeutic effects between Shensongyangxin capsule combined metoprolol and metoprolol monotherapy in treating ventricular premature beats.Methods:The related literature of Shensongyangxin capsule combined metoprolol in treatment of ventricular premature beats published from Aug 2005 to Aug 2013 were searched by computer from Cochrane Library,MEDLINE,EMbase,Wangfang Data,China National Knowledge Infrastructure (CNKI)and Chinese biomedical literature database (CBM),and were searched manually.RevMan 5.0 software was used to perform Meta analysis on data.Results:Symptomatic relief in treatment group was signifi-cantly better than that of control group [(Z=8.28,P <0.00001;OR=6.17,95%CI (4.01,9.48)];dynamic ECG improvement in treatment group was significantly better than that of control group [Z=9.37,P <0.00001;OR=3.41,95%CI (2.64,4.40)];there was no significant difference in incidence rate of adverse drug reaction be-tween two groups [Z=1.47,P =0.14;OR=0.70,95%CI (0.43,1.13)].All funnel schemas were symmetry in general,so there was small possibility of bias.Conclusion:Shensongyangxin capsule combined metoprolol possesses better therapeutic effect than metoprolol monotherapy in treating ventricular premature beats,and it doesn′t in-crease adverse drug reactions.
4.Changes of endoplasmic reticulum molecular chaperon GRP78 expression in spinal cord of ischemia/reperfusion injuried rat

Objective To evaluate the changes of GRP78 expression in the Spinal Cord of Ischemia/Reperfusion Injuried Rat.Methods Fifty five adult Wistar rats(250~300 g)were randomly divided into 2 groups as control group(n=5)and operation group(n=50).The spinal cord SCII model was established.The expression of GRP78 was detected in spinal cord tissue through immunohistochemistry(IHC)and Western blot analysis,and the neuronal apoptosis was detected through terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling(TUNEL)methods.Results The GRP78 expression was increased at 30 min of reperfusion,and peaked at 7 h,but began to decline at 11 h post-reperfusion,and reduced significantly at 23 h.The number of GRP78 positive neurons at 0.5,3,7,11 and 23 h groups was(19.4?1.34),(42.6?2.30),(82.4?2.07),(40.0?1.58)and(18.8?0.83),respectively and significantly higher than that of control group(P
5.Considerations on the construction of key disciplines in hospital

In a hospital, key disciplines are often regarded as the representative of service and reputation of the hospital. It is therefore of great significance to strengthen the construction of key disciplines to ensure sustainable development of the hospital in the increasingly fierce competition of medical market. This paper discusses how to strengthen construction of key disciplines from four aspects, namely, to increase the awareness of focal discipline construction among hospital staffs, and establish "brand awareness" ;to develop a team of high level professionals, and develop or introduce academic leaders; to improve academic research level and promote the technology progress ;and to strengthen management by objectives.
6.Study of level Ⅰb delineation, dose analysis and regional recurrence in level Ⅰb lymph node-negative nasopharyngeal carcinoma patients treated by intensity-modulated radiotherapy

Objective To investigate the level Ⅰb contouring,dose analysis and regional recurrence in level Ⅰb lymph node-negative (Ⅰb-negative) nasopharyngeal carcinoma (NPC) patients treated by intensity-modulated radiotherapy (IMRT).Methods One hundred ninety newly-diagnosed,Ⅰb-negative NPC patients treated by IMRT were enrolled.Level Ⅰb contouring and dose prescribing in this cohort were classified into planned prophylactic irradiation (PPI) group (56 cases) and non-planned prophylactic irradiation (non-PPI) group (134 cases).The mean dose (Dmean) of the level Ⅰb bilaterally and submandibular glands (SMGs) was recorded for comparison.Results After a median follow-up of 47 months,there was no level Ⅰb regional recurrences noted in the entire group.The mean doses of level Ⅰb and the submandibular glands were significantly lower in the non-PPI group than those in the PPI group as follows:(50.81±5.37) Gy vs (59.68±3.32) Gy for the left level Ⅰb,(51.55±5.02) Gy vs (59.66±3.85) Gy for the left submandibular gland,(51.55±5.02) Gy vs (59.66±3.85) Gy for the right level Ⅰb and (57.25±4.69) Gy vs (63.41±2.88) Gy for the right submandibular gland (all P =0.000).Conclusion In this retrospective analysis of non-randomized single institute data,it seems unlikely that PPI to level Ⅰb is necessary in Ⅰb-negative NPC patients treated by IMRT.
7.Clinical analysis of 11 patients with relapsing polychondritis

Objective: To improve the understanding of the clinical feature and early diagnosis of relapsing polychondritis(RP). Method: Eleven patients with RP were analyzed retrospectively and the reported literatures were reviewed. Result: Nine cases had initial involvement of auricular cartilage while two polyarthritis. The treatment of 2RP patients with respiralory tract involvement was invalid and 1 patient died. Nine cases were initially misdiagnosed, with a misdiagnosis rate of 81.82%. Conclusion: RP involves cartilage and connective tissue. The prognosis with respiralory tract involvement is poor. RP is a sort of paroxysmal and progressive inflammation involving cartilages all over the body with a variety of clinical manifestations. Early diagnosis of RP is based on full understanding of its clinical features.
8.STUDY ON THE ANTIOXIDATIVE AND ANTIAGING EFFECT OF THE BIOACTIVE EXTRACT FROM AGROCYBE AEGERITA

Objective To investigate the antioxidative and antiaging effects of the bioactive extract from Agrocybe aegerita. Method Fruit flies and D-galactose induced aging mice were fed with the bioactive extract from Agrocybe aegerita. The lifespan of fruit fly and spleen index, brain index,SOD activity in serum and lipofuscin level in 1iver of the mice were measured, and its acute toxic effect was studied. Results The bioactive extract from Agrocybe aegerita could significantly increase the mean lifespan and half death time of fruit flies. The percentage of life prolongation was 41.81% compared with the control. Compared to the models, SOD activity in serum,spleen index and brain index of male mice fed with 800mg/(kg.d) of the bioactive extract from Agrocybe aegerita were respectively increased by 42.36%, 33.33% and 17.85%,while lipofuscin content of liver decreased by 50.06%. 20g/(kg.d) of the bioactive extract from Agrocybe aegerita was safe for mouse. Conclusion The bioactive extract from Agrocybe aegerita have obviously antioxidative and antiaging effects with little side effect.
10.Transfection and expression of Smad7 inhibits transforming growth factor-?_1 effects on renal tubular cells

Objective To explore the effect of Smad7 gene transfer on tubular cell cycle arrest, tubular apoptosis and fibronectin (FN) synthesis by transforming growth factor ? 1(TGF ? 1) induced. Methods Mouse Smad7 gene was transfected into renal tubular cells in primary cell culture by using Tfx 50 cationic liposome. Tubular cell proliferation was measured by MTT and cell cycle was observed by flow cytometry. The level of FN secretion in the supernatant was determined by ELISA. Results At 48 h after administration of 10 ng/ml TGF ? 1 to renal tubular cells, cell proliferation declined, G 0/G 1 arrested in cell cycle, and cell FN secretion increased significantly. These abnormalities were attenuated by liposome mediated Smad7 gene transfection. Conclusion Transfection and expression of Smad7 can markedly inhibit TGF ? 1 responses in renal tubular cells, which is helpful for further studies of Smad7 gene therapy in vivo .
