Nowadays,the study of tumor stem cells has become the heatpoint in cancer research.Many experiments have successfully demonstrated the existence of tumor stem cells,which have been isolated from some solid tumors.As the research on the origin of tumor stem cell is developing,the knowledge of the occurrence and development of tumor has become more clear,which will influence the diagnosis and treatment for tumor significantly.Moreover it will bring benefit to the following-up after surgical operation and giving hopes to cancer sufferers.
2.Research Advances in Paenibacillus polymyxa and Their Bioactive Substances

Many beneficial bioactive substances were produced by Paenibacillus polymyxa such as antibiotics, antimicrobial proteins, plant hormones and flocculants. These bioactive substances also could be produced by Paenibacillus polymyxa making it show excellent prospect in biological control of plant diseases, treatment of mankind and animals. This article summarizes research advances in Paenibacillus polymyxa and their bioactive substances.
3.Progress in the study of tuberculosis diagnostic methods and their applied value

Tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis complex and is mainly manifested as pulmonary tuberculosis. Mycobacterium tuberculosis is characterized by high pathogenicity and drug resistance due to its high viability and lethality, and multidrug-resistant tuberculosis caused by it has become a global public health problem. Early and effective treatment is essential to prevent the emergence of drug-resistant strains. Commonly employed methods for the screening and diagnosis of TB include: clinical signs, imaging examinations (chest X-ray, computed tomography), sputum culture (L-J medium, BACTEC liquid culture system), and immunological tests (lipoarabinomannan antigen test, tuberculin skin test, interferon-gamma release test). In recent years, emerging molecular diagnostic methods such as GeneXpert MTB/RIF assay, loop-mediated isothermal amplification (LAMP), linear probe assay, and whole-genome sequencing have been used to diagnose and characterize TB. These methods not only identify Mycobacterium tuberculosis but also detect mutation sites associated with resistance to first-line anti-tuberculosis drugs (isoniazid, rifampicin, etc.), providing new possibilities for the diagnosis of drug-resistant Mycobacterium tuberculosis. In this paper, the advantages and disadvantages of some commonly used detection methods for tuberculosis are first reviewed, aiming at assisting clinicians to diagnose and treat the disease in a timely manner. Secondly, this paper explores the prospects of the latest high-throughput detection methods for drug-resistant Mycobacterium tuberculosis.

5.Therapeutic effect of balloon catheter dilatation with ice water on cricopharyngeal achalasia

Objective To observe the therapeutic effects of balloon dilatation with ice water and room temperature water on cricopharyngeal achalasia after brainstem stroke.Methods Forty dysphagic patients with cricopharyngeal achalasia after brainstem stroke were recruited.Twenty patients assigned to experimental group were treated by balloon dilatation with ice water and low frequency elecrtrical stimulation.Twenty patients assigned to control group were treated by balloon dilatation with room temperature water and low frequency electrical stimulation.Results After treatment,36 out of the 40 patients of the two groups could eat pasty food independently without choking.Videofluoroscopic swallowing study (VFSS) showed that the cricopharyngeal sphincter relaxed and the foods passed successfully when swallowing bolus,and no aspiration happened.In comparison,the level of cricopharyngeal opening,the number of patients with nasogastric tube remained and eating normally,and the scores of functional oral intake scale (FOIS) between the two groups showed no significant difference (P > 0.05).The average treatment sessions,average hospitalization days and average treatment cost in treatment group were significantly less than those in the control group(P <0.05).Conclusion Cricopharyngeal achalasia can be treated effectively by balloon dilatation with ice water or with room temperature water combined with low frequency electrical stimulation,but treatment course of balloon dilatation with ice water were significantly shorter than that of balloon dilatation with room temperature water.
6.Effect of indomethacin on insulin resistance after operation and metabolic response to surgical injury in patients with gastrointestinal tumor

ObjectiveTo investigate the effect of indomethacin on insulin resistance and metabolic response to surgical injury in patients with gastrointestinal tumor.MethodsFifty-eight cases with gastric cancer or colon cancer were divided into an indomethacin group (n =28) and a control group (n =30).All the operations were performed under general anesthesia.Patients in both groups were given parenteral nutrition 24 hours after operation for 5 ~ 7 days.The patients in the indomethacin group were treated with indomethacin suppository ( 100 mg/12 h).Fasting blood glucose (FBG),fasting serum insulin (FINS),creatinine (Cr),blood urine nitrogen (BUN),triglyceride (TG),free fatty acid (FFA) and C-reactive protein (CRP) of the two groups were detected on the day before operation,and 24,72,120 hours after operation.Insulin resistance index (HOMA-IR) was calculated by using the homeostasis model assessment (HOMA).The vital signs were observed in 72 hours after operation.ResultsThe vital signs in the indomethacin group were more steady.The levels of FBG,FINS,and InHOMA-IR of the control group 24 hours and 72 hours after operation were higher than before operation ( all P =0.000) and those of the indomethacin group ( all P <0.01 ).In both of the two groups,the levels of Cr,BUN,TG,and FFA were higher than those before operation,but declined over time.All the indexes in the indomethacin group 120 hours after operation decreased significantly compared with the levels 24 hours after operation ( all P =0.000 ),as well as with the levels in the control group 120 hours after operation ( all P < 0.05 ).No significant difference was found in the level of CRP between the two groups and between before and after operation.ConclusionIndomethacin could reduce the postoperative stress hyperglycemia and insulin resistance in patients with gastrointestinal tumor.
7.The value of calcification in CT differentiating benign and malignant thyroid lesions

Objective To study the diagnostic value of calcification in differentiating benign and malignant thyroid lesions. Methods CT images of 318 consecutive patients with pathologically proven thyroid lesions were retrospectively reviewed by two radiologists. The following characteristics of calcification on CT images were evaluated: (1) size (≤2 mm indicating microcalcification and > 2 mm or shelly and irregular shape indicating macrocalcification, and both features indicating mixed calcification), (2) number (single or multiple) and (3)location (internal or edge). χ~2 test was used for statistical analysis. Results Oft he 318 cases, 48 were diagnosed as malignant (papillary carcinoma 26, follicular carcinoma 7, medullary carcinoma 3 and microcarcinoma 12) and 270 were benign (nodular goiter 36, adenoma 170, nodular goiter with adenoma 38 and adenoma with Hashimoto's thyroiditis 26). Calcification was found in 60 cases (18.9%). Among them 21 (papillary carcinoma 12,microcarcinoma 6,follicular carcinoma 2 and medullary carcinoma 1) were malignant(43.8%) and 39(nodular goiter 6, adenoma 13,nodular goiter with adenoma 19 and adenoma with Hashimoto's thyroiditis 1) were benign (14.4%) (P < 0.01). Sensitivity and specificity for diagnosing thyroid carcinoma were 43.8% (21/48) and 85.6% (231/270), respectively. Microcalcification was found in 37 cases (malignant 8, benign 29) and macrocalcification was found in 23 cases(malignant 13, benign 10) (P < 0.01) . Sensitivity and specificity of macrecalcification for diagnosing thyroid carcinoma were 61.9% (13/21) and 74.4% (29/39), respectively. Single calcification was found in 31 cases (malignant 13, benign 18) and multiple calcification was found in 29 cases(malignant 8, benign 21) (P >0.05). Internal calcification was found in 15 cases of malignant lesions(71.4%) and 12 of benign lesions(30.8%); Edge calcification was found in 6 cases of malignant and 27 of benign, (P <0.01). Sensitivity and specificity of internal calcification for diagnosing thyroid carcinoma were 71.4% (15/21) and 69.2% (27/39), respectively. Conclusion Internal calcification or(and) macrocalcification of the thyroid lesions may strongly suspect thyroid carcinoma and fine-needle aspiration or surgery should be further performed.
8.Evaluation of Three-dimensional Fast Recovery Fast Spin-Echo MRCP in Differentiating Benign from Malignant Causes of Biliary and Pancreatic Duct Dilatation

Objective To evaluate the diagnostic value of MR cholangiopancreatography(MRCP) using three-dimensional fast recovery fast spin-echo(3D FRFSE) in differentiating benign from malignant causes of biliary and pancreatic duct dilatation.Methods 106 cases suspected biliary and pancreatic duct dilatation underwent 3D FRFSE MRCP.The images were prospectively analysed by two skilled radiologists,and the results were compared with that of surgery,biopsy or follow-up.Results MRCP was successful performed at one time in all cases.On MRCP,the visual rate of intra-and extrahepatic bile duct was 100% and that of main pancreatic duct was 93.4%.80 patients had benign lesions: bile duct lithiasis(n=66),papillitis(n=6),descending part of duodenum diverticulitis(n=2),duodenal adenomatoid hyperplasia(n=1) and chronic pancreatitis(n=5).26 patients had malignant lesions: extrahepatic cholangiocarcinoma(n=9),ampullary carcinoma(n=5),gallbladder carcinoma(n=4) and pancreatic head carcinoma(n=8).The sensitivity,specificity and accuracy in differentiating benign from malignant causes of biliary dilatation were 92.3%,96.3% and 95.3%,respectively.Conclusion 3D FRFSE MRCP plays an important role in diagnosis of differentiating diagnosis of the causes of biliary dilatation in clinical practice.
9.A systematic review of hip fractures in elderly people

Objective To study the age peak of incidence,pathologic sites,therapeutic effect and complications of different treatment for hip fractures in the elderly.Methods Hip fractures,therapy,elderly were used as key words to search articles both in English and Chinese from 1979 to 2017.The articles were filtrated by title,abstract and full text and 32 of them were left.All the patients were objects of this study,but the repeated cases were excluded.The patients' data such as age of onset,gender,fracture type,operation method,follow-up time,and complications were collected and analyzed by systematic analysis.Results Data of 2 758 patients in 32 papers had been collected for analysis.There were 1791 femoral neck fractures and 967 intertrochanteric fractures,with a ratio of 1.85:1.The patients' age of onset was ranging from 54 to 92 years old,with an average of 76.7 years old,and the age peak of incidence was from 70 to 79 years old.The sex ratio was 1:1.66 (1 037 males:1721 females).All the patients had been followed up for 0.5 to 18 years,(average,7.8 years).Among 927 femoral neck fractures who were treated by 3 lag screws,there were 278 cases of non-union (29.9%),139 femoral head necrosis(15.1%) and 19 internal fixation loosening (2.0%);but for 183 femoral neck fractures who were treated by dynamic hip screws (DHS),there were 51 cases of non-union (28.1%),31 femoral head necrosis(17.0%),3 internal fixation losening(1.8%),and 27 cutting effect (15%).Then for 400 femoral neck fractures who were treated by hemiarthroplasty,there were 6 cases of dislocation(1.5%),14 shaft fracture (3.5%),8 deep infection (2.0%);while for 281 femoral neck fractures who were treated by total hip arthroplasty,there were 14 cases of dislocation (5.0%),11 shaft fracture (4.0%),and 10 deep infection (3.5%).On the other hand,for 354 intertrochanteric fractures who were treated by proximal femoral nail or proximal femoral nail antirotation internal fixation (PFN or PFNA),there were 18 cases of trochanter fracture (5.0%),28 hip varus (8.1%),32 displacement (9.0%);and for 210 intertrochanteric fractures who were treated by Gamma nail,there were 17 cases of trochanter fracture (7.9%),11 cutting effect (5.0%),6 displacement (3.0%);and for 135 intertrochanteric fractures who were treated by DHS,there were 8 cases of cutting effect (6.1%),7 displacement (5.2%);and for 101 intertrochanteric who were treated by proximal femoral locking plate,there were 5 cases of displacement (5.0%);Also for 89 intertrochanteric fractures who were treated by hemiarthroplasty,there were 2 cases of dislocation(1.8%),3 shaft fracture (3.0%),1 deep infection(1.5%);and for 78 intertrochanteric fractures who were treated by total hip arthroplasty,there were 4 cases of dislocation (4.8%),3 shaft fracture (4.0%),2 deep infection (2.5%).Conclusion The elderly hip fractures' age peak of incidence is from 70 to 79 years old,and femoral neck fracture takes up a majority of it.Female patients are more than male patients.The femoral neck fractures are mainly treated by 3 lag screws internal fixation;non-union and femoral head necrosis are the common complications postoperation.While the intertrochanteric fractures are mainly treated by proximal femoral nails;displacement,hip varus and trochanter fractures are the common complications postoperation.The artificial hip replacement is the effective remedy measure for secondary femoral head necrosis after hip fractures.
10.Vertical Dissemination of Escherichia coli and Klebsiella pneumoniae Isolates Producing Extended-spectrum Beta-lactamases in Hospital

OBJECTIVE To investigate the epidemic situation of vertical dissemination of the Escherichia coli(ECO) and Klebsiella pneumoniae(KPN) isolates producing extended-spectrum beta-lactamases in the hospital.METHODS The fingerprints of the isolates were obtained by pulsed-field-gel-electrophoresis(PFGE),and were analyzed with software Quantity One.RESULTS The similarity of 10 isolates and 7 isolates among the 21 ECO(isolates) from ICU was more than 90%,and 100%,respectively;the similarity of 3 KPN isolates was 100%.The similarity of 9 isolates among the 20 ECO(isolates) was more than 90% and the similarity of 4 isolates among the 6 KPN isolates was more than 90% in the neurosurgery ward.The similar coefficients of 3 isolates and 2(isolates)(among) the 10 ECO isolates from the ward for cadre were more than 90% and 100%,(respectively.) The similar(coefficients) of 2 isolates among the 7 ECO isolates from the gastroenterology ward and 2(isolates)(among) the 18 ECO isolates from the respiratory ward were more than 90%.The isolates whose similarity was more than 90% were also found in other wards.CONCLUSIONS There are vertical disseminations of single clone of ESBL-producing organisms in several wards,especially in the ICU.It is necessary to strengthen the management of(infection)-control.
