3.Analysis of drug resistance on extended spectrum β-lactamases-producing Escherichia coli and Klebsiella pneumoniae

Objective Analysis of clinical distribution and susceptibility to antimicrobial agents in producing extended spectrumβ-lactamases (ESBLs)of Escherichia coli(ECO)and Klebsiella pneumoniae (KPN).Methods The samples from 2013 April to 2014 December in Changji Prefecture People′s Hospital were cultured and identified,drug sensitivity test did by VITEK2compat au-tomatic bacteria analysis,phenotypic confirmatory test was did on producing ESBLs ECO and KPN,and the clinical distribution and drug resistance was analyzed.Results 1 576 strains of ECO and KPN isolated 306 strains of producing ESBLs strains and 343 strains of multi drug resistant bacteria,the isolation rates were 1 9.4% and 21.8% respectively.In 576 strains of ECO,there were 1 77 strains of producing ESBLs strains and 202 strains of multi drug resistant bacteria were isolated,the separation rates were 30.7% and 35.1% respectively,generic drug resistant strains were not found,and those producing ESBLs strains were isolated mainly from the urine specimens (39.5%),and mainly from the ICU (37.3%)and (29.9%)the surgical ward.There were 129 strains of producing ESBLs strains,141 strains of multi drug resistant bacteria and 4 generic drug resistant strains were isolated from 1 000 strains of KPN,the separation rates were 12.9%,14.1% and 0.4% respectively,and those producing ESBLs strains were isolated mainly from sputum specimens (83.7%),and mainly from the ICU (42.6%)and(29.5%)in the department of inter-nal medicine.Isolation of producing ESBLs ECO and multi drug resistant bacteria were higher than KPN,the difference were statis-tically significant (P <0.05).The sensitivities of ECO and KPN ESBLs strains to carbapenems imipenem andaminoglycosides ami-kacin and compound preparation piperacillin/tazobactam were good,the other antimicrobial drug resistance rates were more than 50.0%.Conclusion ESBLs ECO mainly causes urinary tract infections,KPN mainly causes respiratory tract infection.Detection rates of producing ESBLs ECO and KPN were high,and the multi drug resistance is serious,should strengthen the monitoring of drug resistance on producing ESBLs strains,and according to the results of drug susceptibility,standardize the rational use of anti-microbial agents,and to strengthen the management of antimicrobial agents,delay the occurrence of bacterial resistance.
4.Single-incision Laparoscopic Surgery Versus Conventional Multi-port Laparoscopic Surgery for Tubal Pregnancy

Objective To study the feasibility of transumbilical single-incision laparoscopic surgery ( SILS) in the treatment of tubal pregnancy, as compared with conventional multi-port laparoscopic surgery. Methods Sixty-two patients diagnosed as tubal pregnancy undergoing laparoscopic surgery at this hospital between September 2013 and June 2014 were selected for investigation. There were 29 cases of transumbilical SILS ( SILS group) and 33 cases of conventional multi-port laparoscopic surgery ( MPLS group) . We retrospectively reviewed the medical records of all patients and analyzed the surgical outcomes, including operative time, blood loss, surgical complications and hospital stay. Results In both groups, all procedures were performed without failure.No conversion to open surgery or additional skin incision was needed.There were no differences between the SILS and MPLS groups in the operation time [(51.5 ±10.8) min vs.(47.3 ±9.4) min, t=1.637, P=0.107], the mean estimated blood loss [(15.5 ±10.5) ml vs.(18.4 ±12.2) ml, t=-0.996, P=0.323], the absolute decrease of hemoglobin from preoperative to postoperative [(14 ± 5) g/L vs.(13 ±4) g/L, t=0.874, P=0.386], the postoperative hospital stay [(3.5 ±0.9) d vs.(3.8 ±0.8) d, t=-1.390, P=0.170], and the incidence of postoperative fever (2 cases vs.3 cases, χ2 =0.000, P=1.000).Follow-up for 1-3 months ( mean, 1.8 months) in all the cases showed smooth recovery and no postoperative complications. Conclusion SILS is a feasible and safe approach in the treatment of tubal pregnancy.
5.Effect of lysophosphatidylcholine on cholesterol efflux from macrophage foam cells

AIM: To explore the effect of lysophosphatidylcholine(LPC) on cholesterol efflux from macrophage foam cells, and to offer experimental evidence for research on prevention and treatment of atherosclerosis.METHODS: Peritoneal macrophages and LDL were seperated from mice and serum of healthy volunteers, respectively. The foam cells were derived from macrophages in the presence of Acylated LDL (AcLDL). Cholesterol efflux from cells and LDH activity were measured by enzymetic fluorometry and LDH kit, respectively.RESULTS: After incubated with LPC for 24 hours, cholesterol efflux from macrophage foam cells increased significantly compared to control, and cellular cholesterol was lower than that in control group. At the same time, medium LDH activity of LPC group was not increased obviously. CONCLUSION: Within the dosage of 10-80 ?mol/L, LPC can promote cholesterol efflux from macrophage foam cells in a dose-dependent manner, and this effect has nothing to do with cytotoxity of LPC.

AIM:To observe the expression of Smad7 and Smad ubiquition regulatory factor-Smurf2 in rat glomerular mesangial cells (GMC) stimulated by the high concentration of glucose,and to investigate the effect of the ubiquition on Smad signaling by adding MG132 as a proteasome differential inhibitor. METHODS:Cultured rat GMC were divided into normal group (the concentration of glucose:5.6 mmol/L),high glucose group (20 mmol/L,30 mmol/L,respectively),therapy group (30 mmol/L glucose with MG132). The expressions of Smurf2 and Smad7 in each group were measured by indirect immunofluorescence and laser scanning confocal microscope. Objective To understand the functional status and radiation protection requirements of helical tomotherapy (TOMO) unit when it is applied in clinic treatment so as to promote the construction of TOMO quality control system.Methods A helical tomotherapy unit with a type of tomotherapy Hi-Art was employed as the radiotherapy unit.A series of tests and analyses on dosimetric and radiation safety performance parameters of the TOMO unit were conducted with the use of a A1SL ionization chamber,a set of solid water equivalent phantoms and other test equipments.The performance parameters were compared with that of a conventional 6 MV accelerator with a type of ARTISTE.Results The deviations of static and dynamic output dose were-1.51% and 1.18%,respectively.The values of PDD10/PDD1.5 and PDD20/PDD1.5 were 0.608 and 0.318 within the range of X-ray quality parameters calibration.Under the different field conditions of 40 cm × 5 cm,40 cm × 2.5 cm and 40 cm × 1 cm,the stability of transverse/longitudinal dose distribution curve was no more than 1.20% and the offset distance of Jaw width was smaller than 0.5 mm.Radiation leakage ratio of the treatment rotating center from the muhileaf collimator (MLC) was 0.20%.No matter the average or the maximum value of the leakage radiation from couch plane was less than 0.02%.Conclusions The performances and testing requirements of TOMO unit are different from a conventional electron linear accelerator,the quality control index of TOMO unit should be set seperately and tested strictly in terms of the specific requirements of the unit by complying with the QC programme.The reliable quality control system should be established and effectively implemented to serve the radiotherapy quality and radiation safety effectively in the tomotherapy.
8.Analysis of SCI paper publication of doctoral students of Capital Medical University in recent ;five years

Objective To estimate the incentive and guidance of degree policy for the doctoral student's capacity-building. Methods Totally 1185 published SCI papers completed by 1 013 doc-toral students from 2008 to 2012 in Capital Medical University were filed and impact factors of these SCI journals taken as analysis objects were organized using Microsoft Office Excellsoftware. ANOVA was used for data procession and t-test was used for statistical analysis. P<0.05 signifies statistically significant difference. Results This study showed that 97%doctoral students can complete SCI paper publication in compliance with requirements of doctoral degree policy. Quality of SCI papers of aca-demic degree PhDs was higher than that of professional degree PhDs(P=0.00). Level of SCI papers of PhDs in basic disciplines was higher than that of PhDs in clinical disciplines (P=0.00). In addition, this study found that higher leveled SCI papers originated primarily from the national key disciplines and co-first authors. Conclusions The appropriate degree policies play an important guiding role in enhancing the quality of doctoral students' education. Degree-granting units should make more specific guidance policies according to different types of disciplines and cultures, such as raising published arti-cle requirements to the clinical professional PhDs, improving and standardizing relevant degree poli-cies about SCI papers published by co-first authors, strengthening disciplinary conditions building, etc.
9.Effect of Transcutaneous Electrical Acupoint Stimulation on Emergence Agitation in Children Undergoing Cochlear Implantation

Objective To study the effect of transcutaneous electrical acupoint stimulation (TEAS) on emergence agitation in children undergoing cochlear implantation.Methods Sixty patients (age range,1-4 years) scheduled to undergo pediatric cochlear implantation were enrolled and randomized into the TEAS group (group T,n =30) that received continuous TEAS and the control group (group C,n =30) that did not receive TEAS.Anesthesia time,operation time,change in blood flow dynamics during the perioperative period,extubation time,and PACU stay time were recorded.The incidence of emergence agitation and postoperative nausea and vomiting (PONV) was monitored.Results No significant intergroup differences were observed in age,sex,weight,operation time,and PACU stay time (P > 0.05).The MAP and HR in group T were lower than those in group C at different time points such as 30 min after the start of surgery,the end of the surgery,the extubation and 5 min after extubation,and the difference was statistically significant (P < 0.05).The FPS-R,FLACC,and PAED scores in group T were lower than those in group C at different time points within 30 min after exmbation (including the time of extubation),and the difference was statistically significant (P < 0.05).Extubation time,anesthesia time,and incidence of PONV were lower in group T than in group C,and the difference was statistically significant (P < 0.05).Conclusion TEAS combined with general anesthesia significantly reduces the incidence of emergence agitation in children undergoing surgery for congenital hearing loss;it also helps control hypotension,reheve dramatic hemodynamic changes during the waking period,and reduce the incidence of PONV.
10.Study of gait footprint parameters of children and its value in diagnosis and treatment of cerebral palsy

Objective To study the gait footprint parameters of the normal children and the cerebral palsy (CP) children, and to explore its clinical value. Methods A total of 2 800 normal children aged 3 to 10 years and 139 spastic CP children aged 3 to 5 years were recruited in this study. The normal children were divided into seven groups with one year interval, and were measured with regard to the length of foot and step, step width and foot angle of footprint of every age group with self made oil printed carpet.
