1.Changes in dose requirements of rocuronium after right hemihepatectomy in donor for liying donor liver transplantation

Objective To investigate the changes in the dose requirements of rocuronium after right hemihepatectomy in donor of living donor liver transplantation. Methods Sixteen ASA Ⅰ patients of living donor liver transplantation, aged 21-49 yr, weighing 51-86 kg, scheduled for right hemihepatectomy, were enrolled in this study. Anesthesia was induced with iv midazolam and fentanyl, and TCI of propofol and rocuronium. Changes in adductor pollicis muscle were monitored by the train of four (TOF) stimulation. The patients were mechaincally ventilated after tracheal intubation when T1/Tc = 0. The target plasma concentration of rocuroniurn was reduced to 1.0 μg/ml after tracheal intubation and the concentration was regulated to maintain 0 < T1 ≤ 10%. Anesthesia was maintained with sevoflurane inhalation, TCI of propofol, intermittent iv injection of fentanyl. The infusion of rocuronium and inhalation of sevoflurane were stopped before the second time of cholangiography, the total time and amount of rocuronium administered and the recovery of muscle relaxation were recorded at this time (before hemihepatectomy). The infusion of rocuronium and inhalation of sevoflurune were rests-ted after right hemibepatectomy. After the peritoneum was closed, the infusion of rocuroniurn and inhalation of sevoflurane were stopped again, the total time and amount of rocuronium administered and the recovery of muscle relaxation were also recorded at this time (after hemihepatectomy). Results The amount of rocuronium administered was significantly reduced after hemihepatectomy compared with that before hernihepatectomy (P < 0.05). The amount of rocurunium administered after hemihepatectomy was (67 ± 13) % of that before hemihepatectomy. Conclusion Right hemihepatectomy can affect the metabolism of rocuronium in the donor of living donor liver transplantation.
2.Application of CD-4 theory in determining the width of implant in breast augmentation

Objective To introduce a new method (CD-4theory) to determine the width (W) of the implant and to explain the reasons why to do so in details.Methods From January 2006 to August 2015,the authors have found and applied CD-4 theory to determine the width (W) of breast implant in dual plane Ⅰ or Ⅱ breast augmentation cases through transaxillary or periareolar incision for 630 patients.CD was defined as the curved distance on skin from the midline of the sternal bone to the anterior axillary line (AAL) on the lateral chest wall through the horizontal level on inferior mammary fold (IMF).W =CD-4.0 cm or 3.5 cm.Results The 630 patients were treated by using both round and anatomic implants with width from 10.5 cm to 12.5 cm.Their CDs were from 14.5 cm to 17 cm.78% of the patients were followed up from 1 month to 5 years.Except for 5 patients were unilateral capsular contractions,all the other patients were satisfied with their nature new breast shapes and volumes.Their new intermammary cleavages without bras were between 1-2.5 cm and lateral borders of breast were on the area of the anterior axillary line (AAL).Conclusions Width of the implant is equal to CD-4.0 cm when doing dual plan Ⅰ or Ⅱ breast augmentation.For the very thin patient,the width should be 3.5 cm.
3.Safety of nasogastric tube versus nasojejunal tube feeding in early enteral nutrition in acute pancreatitis: a Meta-analysis

Objective To evaluate the safety of nasogastric tube feeding and nasojejunal tube feeding in early enteral nutrition treatment of acute pancreatitis.Methods Using key words,subject headings,and citation tracing,we searched literatures reporting randomised controlled trials on early enteral nutrition treatment of acute pancreatitis through nasojejunal tube and nasogastric tube in the following databases:PubMed,Embase,Cochrane library,Wanfang,China National Knowledge Infrastructure (CNKI),and VIP published since the founding of the databases up to 2016.Meta-analysis was performed with the selected literature.Results Seven randomised controlled trials with 367 patients were included.Meta-analysis showed that the nasogastric tube group was not inferior to the nasojejunal tube group in the incidence of recurrent abdominal pain,gastrointestinal adverse reaction,the total length of hospital stay,and mortality.Conclusion Enteral nutrition via nasogastric tube is safe and well tolerated,may be a safe approach of nutrition treatment for acute pancreatitis.
4.Effect and mechanism of salvianolic acid B in attenuating elevated portal pressure in a rat model of portal hypertension induced by endothelin-1

OBJECTIVE: To investigate the effects of salvianolic acid B (SA-B) on portal hypertension induced by endothelin-1 in rats. METHODS: Twenty-eight Sprague-Dawley rats were randomly divided into four groups: ET-1 group, ET-1+SA-B group, ET-1+ET(A)R blocker (BQ-123) group and ET-1+ET(B)R blocker (BQ-788) group. The rats of ET-1+SA-B group underwent intragastrical administration of salvianolic acid B for five days before ET-1 injection, while in three other groups' drinking water was given. In BQ-123 group or BQ-788 group, an intravenous injection of BQ-123 or BQ-788 via femoral vein was administered 30 minutes prior to ET-1 injection. Then changes of portal pressure, cervical artery pressure and heart rate were monitored continuously. RESULTS: After ET-1 injection, the portal pressure of all rats in the ET-1 group increased significantly, while slightly in groups that pretreated with SA-B, BQ-123 or BQ-788. CONCLUSION: SA-B can attenuate the elevated portal pressure induced by ET-1 with effect similar to ETR blocker.
5.Effect of dexmedetomidine on perioperative stress reaction and cellular immunity of patients with breast cancer radical surgery

Objective To study the effect of dexmedetomidine on perioperative stress reaction and postoperative cellular immunity of patients with breast cancer radical surgery.Methods We selected 108 cases of breast cancer patients undergoing elective radical surgery under general anesthesia in our hospital from Janu-ary 2012 to June 2015.These patients were divided into dexmedetomidine group (group D)and the control group (group C)by random number table method,54 cases in each group.Patients in group D were given dexmedetomidine 1.0 μg/kg by intravenous drip within 15 min before anesthesia and maintained 0.5 μg.kg -1 .h -1 after anesthesia induction intubation until operation finish.Patients in group C were given the same capacity of normal saline instead of dexmedetomidine,and the rest of the anesthesia was same as group D.The plasma norepinephrine (NE)and epinephrine (E)and cortisol (COR),serum interleukin-6 and T lymphocyte subsets (CD3 +,CD4 +,CD8 +,CD4 +/CD8 +)and NK cells levels were determined before anes-thesia (T0 ),postoperative 4 h (T1 ),1 d (T2 ),3 d (T3 ),and 7 d (T4 ).Results The level of plasma NE, E,COR and serum IL-6 of the D group in different time had no statistical significance compared with T0 time (P ﹥ 0.05).The plasma level of E and NE in T1 -T3 ,plasma COR in T2-T3 and serum IL-6 in T2-T4 of the C group were increased significantly (P ﹤ 0.05).The level of plasma E and NE in T1 -T3 ,plasma COR in T2-T3 and serum IL-6 in T2-T4 of the D group were decreased significantly compared with C group (P ﹤ 0.05).The level of CD3 + and NK cells in T1 -T3 of the two groups decreased significantly compared with T0 (P ﹤ 0.05).The level of NK and CD3 + in T1 -T3 and CD4 +,CD4 +/CD8 + in T2-T3 of group D increased significantly com-pared with group C (P ﹤ 0.05).Conclusion Dexmedetomidine of intravenous drip during general anesthesia can effectively reduce stress reaction in perioperative period of breast cancer patients,and improve patients immune status.
6.Research on Effect Factors to Function of Human-machine System as Displaying with LCD.

Objective To provide the experimental data for standard establishment and ergonomics research on manned spaceflight displays basing on relation of six effect factors to function of human-machine system as displaying with LCD,and the best factor-level-composition is chosen.Methods An experimental system was developed.The orhoganal experiment means was adopted.Twenty subjects were asked to finish three tasks: to view parameters of the whole screen interface,to seek local parameters,to identify the fault events quickly.Three indices: reaction time,error times and times of unable to recognize were analyzed.Results The indices were much different for different factors at different levels.According to the indices of mean reaction time,extreme difference of view-angle was the lowest and view-distance the highest.Conclusion The order of the display effects of different factors on human-machine system is successively: viewdistance,illuminant level,character height,color of character/background and viewangle.
8.Clinical treatment and analysis of minimally invasive percutaneous nephrolithotomy

Objective To investigate the complications of minimally invasive percutaneous nephrolithotomy (PCNL) and their countermeasures.Methods The datum of 265 cases who underwent PCNL in our hospital.Among the 265 cases of PCNL,serious complications occurred in 13 cases.Of the 13 cases,4 cases had massive hemorrhage,2 case had colon perforation,1 case had serious urinary extravasation,6 cases had febricity.Results The incidence of serious complications was 4.91%(13/265),all patients were cured by corresponding treatment and no death in the cases.Conclusions Adequate preoperative preparation in PCNL and strict surgical indications were good for complications reduction.Serious complications should be detected early and treated correspondingly.
9.Analysis of the in vitro quinolones resistance in Helicobacter pylori strains isolated in Zhejiang prov-ince

Objective To investigate the in vitro quinolones resistance in Helicobacter pylori ( HP) strains isolated in Zhejiang province and to provide references for the clinical application of quinolones in the eradication of HP infection.Methods A total of 713 HP strains were isolated from several hospitals located in different regions of Zhejiang province from April to June 2013.Drug sensitivity test was performed to measure the resistance of those HP strains to levofloxacin and moxifloxacin by using the agar dilution method recommended by the Clinical and Laboratory Standards Institute ( CLSI) .The antibiotic solutions were added to the agar and diluted into corresponding critical concentrations.The mixed liquors were placed in dilution plates and added with the bacteria suspension.If there were bacterial expansion on the plates after incuba-tion, the strains were considered to be drug-resistant strains.Results The resistance rates of 713 HP strains to levofloxacin and moxifloxacin were 19.50%.In addition to the Jinhua area, drug resistance rates in the other 7 areas including Hangzhou, Wenzhou, Shaoxing, Jiaxing, Taizhou, Lishui and Zhoushan were above 15%.The highest drug resistance rate of HP was found in Wenzhou, which was significantly higher than that in Jinhua area (26.40%vs 9.52%, P<0.05).No significant differences in the resistance rate of HP to quinolone were found between Wenzhou area and the other regions in Zhejiang.Conclusion Com-pared with amoxicillin and furazolidone, higher resistance rates of HP to levofloxacin and moxifloxacin were observed in Zhejiang province.Because the mechanisms of drug resistance to levofloxacin and moxifloxacin were same and the drug targets within HP to which the two antibiotics binding were similar, levofloxacin and moxifloxacin shared common characteristics of drug resistance among HP strains, indicating that prescription of the two antibiotics in replacement should be avoided in clinical treatment of HP drug-resistant strains.
