Objective To shorten the stat test turnaround time (TAT) and improve the quality of service in clinical laboratory.Method Stat test TAT related data of clinical laboratory in Tongji hospital in Wuhan from August 1st to September 30th were collected by laboratory information system and hospital information system.EXCEL and SPSS were used for data analysis.Median and the 90th percentile were calculated for TATs from order to collection, collection to transfer, transfer to reception, reception to inspection and inspection to report.Outlier rates were calculated for TAT from collection to reception, reception to report, and collection to report using 2 h, 2 h, and 4h as target TAT value, respectively.Meanwhile descriptive statistics were calculated for TAT from order to collection in different clinical department, TAT from collection to reception during different collection time frames, and TAT from reception to report during different reception time frames.Results 32 235 stat biochemistry test data were included in this survey.Among three periods cut by collection and reception time, TAT from order to collection were the longest (P50: 681 min,P90:1261 min), followed by TAT from collection to reception(P50:94 min,P90:169 min) and TAT from reception to report(P50:68 min,P90:111 min).TATs from order to collection were longer in gynecological tumor department and organ transplantation department while shorter in infection department and cardiac vascular department.The TATs from collection to reception of specimen collected during 2:00 to 3:59 were longer than other collection time.While the TATs from reception to report of specimen received during 6:00 to 7:29 were longer than other reception time.There was no significant correlation between the amount of emergency specimens collected and TAT from collection to reception in different collection time period.However, the amount of emergency specimens collected was significantly correlated with the TAT from reception to report in different reception time period.Conclusions Analysis of TAT data can be used to identify existing problems and provide improved directions to shorten TAT in clinical laboratories.

Objective To explore the development regularities and characteristics of child and adoles-cent body composition in Dalian,to provide scientific basis for their healthy growth and basic evidence for nutri-tional status and healthcare,and to accumulate dynamic date for evaluation of children's growth and development in the future.Methods Two primary schools、two secondary schools and one high school in Dalian were en-rolled,where four classes were taken from each grade,and 2 027students aged from 6-18 years were systematical-ly collected.The collected students were given height and weight tests,and body composition was examined with bioelectrical impedance analysis.Overweight and obesity were determined by body fat percent.Results (1 ) Height,weight,body fat percent,fat mass,lean body mass,muscles,bone mass,BMI and basal metabolism rate of the students increased from 6-18 years in Dalian,and the growth curves of boys and girls appear rising tenden-cy,while the percentage of total body water decreased.(2)The muscles,lean body mass and basal metabolism rate were obvious sex differences among all the age groups,and the others were sex difference in part of age. Body fat percent and fat mass of girls were greater than those of boys,while the other indexes of boys were grea-ter.(3)The obesity rate of girls in Dalian was 35.7%,while the boys'was 16.7%.Conclusion (1 )The find-ings were consistent with the growth characteristics of adolescent boys and girls aged 6-18 years in Dalian.(2) The muscles,lean body mass and basal metabolism rate were obvious sex differences among all the age groups, and the others were sex difference in part of age.(3)The obesity rate of students in Dalian was higher than the government standard.

?AIM:To compare the curative effect of conjoint fascial sheath( CSF) suspension and levator muscle resection for moderate or severe congenital ptosis.?METHODS: Forty - three patients ( 74 eyes ) with moderate or severe ptosis were treated by CSF suspension or levator muscle resection randomly, and followed up for 6mo. The normalization rates of the two operations were then compared by statistical method, and the complications of the two operations were analyzed.?RESULTS: The two operations appeared no significant difference on the normalization rate for moderate congenital ptosis (P>0. 05), while the normalization rate of CSF suspension on severe congenital ptosis was significantly higher than that of levator muscle resection (P<0. 05). Less complication was happened in the CSF suspension group than in the levator muscle resection group.?CONCLUSION:CSF suspension is more effective on the treatment of severe congenital ptosis than levator muscle resection, and has advantages such as less trauma, repeatable, and less complication.

Objective Construct the finite element model of vertebral pedicle screw fixation in thoracolumbar fractures can obtain the intuitive systematic mechanical effects under different motion conditions, it has a very important significance to the patients' rehabilitation, evaluation of system stability and prognosis.In this study, the biomechanical effects of different motion states of thoracic and lumbar spine fracture models were analyzed by the finite element analysis method.Methods We selected 1 orthopedic health volunteers from Nanjing General Hospital of Nanjing Military Region in June 2014, established the finite element model by the CT scan data of the healthy volunteer, used the geometric cutting method for fracture simulation and replacement, assembled posterior orthopedic internal fixation system of vertebral pedicle screw, simulated the different motion states and obtained the stress nephogram and displacement graph of the nail-stick system..Results For the six-nail and two-rod internal fixation system of posterior nail-stick system which set nails through injured vertebrae, we used the finite element operation method to simulate the biomechanical characteristics of different modes of motion in space, it combined by the movement of six directions(flexion, dorsiflexion, left side bend, right side bend, left twist, right twist).The maximum stress(94.99±1.72MPa) and the maximum displacement(0.1854±0.0052mm) and the of the dorsiflexion were significantly lower than that of flexion[(126.82±5.56)MPa、(0.2502±0.0050)mm]、left side bend[(152.18±9.13)Mpa、(0.3172±0.0048)mm]、right side bend[(159.58±13.54)Mpa、(0.3315±0.0051)mm](P<0.05).Conclusion The method of finite element analysis can obtain clear and intuitive biomechanical data, which provides effective evidence for the evaluation of surgical efficacy, the development of postoperative rehabilitation methods and the evaluation of the stability of thoracolumbar spinal system.

Objective To introduce the amino acid substitution for HP V16E749-57(HLA-A2-restricted CTL epitope) and to identify the novel epitopes.Me thods Quantitative method was used to evaluate the affinity of the substituted peptides.To determine the peptide candidates to be synthesized and identified,the molecular models of the HLA-A2-peptide complex and CTL epitope candidates b ound to the HLA-A2 molecule were established by computer molecular modeling.Pep tides were synthesized and purified with standard Fmoc assay,lactate dehydrogen ase (LDH) release assay was used to determine their abilities of inducing the ge neration of specific CTLs.Results Modified peptides met the requirements of H LA-A2-restricted CTL epitopes.Peptide RLHYNIVTF had the abilitiy of inducing th e generation of specific CTLs.Conclusions Compared with HPV17E749-57 the mod ified peptide RLHYNIVTF has a higher antigenicity and affinity to HLA-A2.So,pe ptide RLHYNIVTF may be used as one of the HLA-A2-restricted candidate epitopes,instead of HPV17E749-57,for peptide vaccine in the treatment of HPV infection.

AIM:To construct the recombinant eukaryotic expression plasmid pEGFP-C1-SR-A I for the high expression in 293T cells in order to identify functions of savenger receptor-A I(SR-A).METHODS:The primer was designed according to MSR1 cDNA and pEGFP-C1-SR-A I was constructed by standard molecular cloning technique and enzyme digestion.After sequencing,the plasmid was transfected into 293T cells by lipidosome method.The expression of scavenger receptor-A I was identified by RT-PCR and Western blotting.The foam cells were evaluated by the formation of lipid granules in the cells with oil red staining.Cell adhesion was analyzed by cell adhesion assay.RESULTS:24 h after transfection,SR-A I mRNA was highly expressed and the high level of the protein was detected.The ratio of foam cell formation was doubled,the efficacy of cell adhesion was enhanced two times compared to the control group and the empty vector group.CONCLUSION:The recombinant eukaryotic expression plasmid has been constructed successfully with enhancing the function of uptake ox-LDL and adhesion in 293T cells by overexpression of SR-A I.

Objective To explore the inner feelings of gestational diabetes at their primary use of the real-time blood glucose monitoring. Method Using the qualitative method, 10 gestational diabetic patients monitored their blood glucose using the real-time blood glucose monitoring for the first time and then were interviewed, the acquired results treated with phenomenological analysis. Result The inner feelings of the 10 patients were summarized including 5 themes:worry about the safety of the fetus, fear on alarming at monitoring, different views on four times input of finger blood glucose values, worry about inaccuracy in blood glucose monitoring data, changes in mental conditions and diets. Conclusion Health care workers should provide the gestational diabetic patients with targeted care strategies based on presence of psychological status and knowledge needs so as to improve their compliance, reduce the psychological burden of patients and control their blood glucose level.

Objective To investigate the correlation between expression of carcinoembryonic antigen (CEA) mRNA in preoperative peritoneal lavage fluid and prognosis of operation in patients with gastric cancer.Methods The expression levels of CEA mRNA in preoperative peritoneal lavage fluid of 68 patients with gastric cancer and 30 patients with gastric benign tumor were measured by reverse transcription polymerase chain reaction.The correlation between the expression level of CEA mRNA in preoperative peritoneal lavage fluid and clinicopathological features,prognosis of postoperative were analyzed.Results The expression level of CEA mRNA in preoperative peritoneal lavage fluid of patients with gastric cancre was 1.74 ± 0.25 and with gastric benign tumor was 0.19 ± 0.04,and there was statistical difference (P ＜ 0.05).The expression level of CEA mRNA in preoperative peritoneal lavage fluid had a correlation with the tumor infiltration depth,tumor differentiation,lymph node metastasis,distant metastasis and TNM stage (P ＜ 0.05).There was statistical difference in the recurrent / metastasis rate and survival rate 1 year after operation between low (34 cases) and higher (34 cases) expression level of CEA mRNA in preoperative peritoneal lavage fluid of gastric cancer patients:47.06％ (16/34) vs.82.35％ (28/34) and 79.41％ (27/34) vs.55.88％ (19/34),P ＜ 0.05.Conclusion The expression of CEA mRNA in preoperative peritoneal lavage fluid has close correlation with the clinicopathologic features,and the high expression of CEA mRNA indicats poor prognosis,indicating that CEA mRNA in preoperative peritoneal lavage fluid could be used as a prognostic marker in patients with gastric cancer.

Objective To explore the effect of empathy nursing on anxiety,depression and expectation of patients with myocardial infarction.Methods In our study,47 patients with myocardial infarction in the department of cardiology of our hospital from August 2013 to July 2014 were assigned as the control group and another 47 patients with the same disease during August 2014 to July 2015 as the study group.The control group received conventional nursing treatment and the study empathy nursing in addition to the conventional nursing.The two groups were compared in terms of anxiety,depression and expectation using Zung anxiety self rating scale (SAS),depression self rating scale (SDS) and Herth hope index (HHI).Results On admission,the patients in the two groups showed no differences in the levels of anxiety,depression and expectation (P＞0.05).After intervention,the levels of anxiety and depression in the study were significantly lower than those in the control group,but the level of expectation was higher (all P＜0.05).Conclusion Empathy nursing is effective for the relief of anxiety and depression in the patients with myocardial infarction as well as for their enhanced expectation for prognosis.
