PI3K/Akt pathway is a biological signal transduction pathway activated by phosphatidylinositol 3-kinase (PI3K).It plays a key role in cell proliferation,cell-cycle regulation,apoptosis initiation,and angiogenesis.In addition,the PI3K/Akt pathway is closely associated with the protection mechanisms of central nervous system damage.In-depth study of PI3K/Akt,downstream molecules and their regulation mechanisms,can provide some new ideas and methods for the treatment of brain injury.

3.Implementation of responsible system of attending physician and new partogram reduces cesarean section rate and alters cesarean section indications

Objective To analyze the changes of cesarean section rate (CSR) and indications of cesarean section (CS) after implementation of responsible system of attending physician (RSAP) and new partogram.Methods Totally,11 814 women,who delivered in the Department of Obstetrics,Beijing Obstetrics and Gynecology Hospital,Capital Medical University in the fourth quarter of 2012,2013 and 2014,were chosen.The mode of delivery,CSR,CS indications and matemal and fetal outcomes of these women were analyzed with Chi-square test.The RSAP was started in 2013,and both RSAP and the new partogram were applied in 2014 in our hospital.Results The CSRs in the fourth quarter of 2012,2013 and 2014 were 46.4%(1 502/3 235),40.4%(1 524/3 770) and 33.5%(1 612/4 809) (x2=138.312,P<0.05),and the converting CSR in the delivery room were 10.9%(212/1 945),7.8%(189/2 435) and 6.1%(208/3 414),respectively (x2=39.765,P<0.05).The proportion of assisted vaginal delivery in the fourth quarter of 2014 was higher than those of the former two years [4.9%(237/4 809) vs 3.4% (108/3 235) and 3.5%(131/3 770),x2=17.041,P<0.05].More and more CS deliveries emerged due to scarred uterus,multiple pregnancy,comorbidities and complications during pregnancy year by year,while those CS deliveries because of macrosomia,narrow pelvis,social factors,elderly primipara,umbilical cord entanglement,precious fetus and high myopia gradually decreased (all P<0.05).In the fourth quarter of 2014,the proportions of CS with fetal distress,abnormal labor,cephalopelvic disproportion and intrauterine infection were lower than those in 2013 (all P<0.05).No significant difference was shown in the proportion of postpartum hemorrhage and neonatal asphyxia among the three years (both P>0.05).In 2014,smaller percentage of neonates were transferred to the Pediatrics Department after birth than in 2012 and 2013 [10.2%(491/4 809) vs 12.0%(388/3 235) and 13.7%(516/3 770),x2=24.681,P<0.05].Conclusions The implementation of RSAP and the application of new partogram effectively reduce the CSR through strict control on indications of CS.

Precision medicine is currently being paid much attention and greatly promotes the diagnosis and treatment of various diseases,especially cancer.Surgery plays a pivotal role in the treatment of hepatocellular carcinoma (HCC) and precision hepatectomy stands as the most updated surgical idea.Application of the concept and techniques of precision medicine to further improve HCC surgery,consummate the skills and modalities of HCC surgical treatment,enrich the nature of precision hepatectomy,so as to make precise evaluation,early prediction,reduce postoperative recurrence and metastasis,find biomarkers of molecular targeted therapy and improve the prognosis is the prime task of HCC surgery.

0.05).Lymph node metas- tasis,macroscopic vascular invasion,TNM classification,Edmondson pathologic classification,tumor location and capsule,carbohydrate antigen 19-9 (CA19-9) might affect the outcome post-LT.Conclu- sion The overall survival of LT for CCC was dismal.We should select the recipients very carefully and strictly.Those CCC patients with large tumor,TNM class-Ⅲ,bi-lobe distribution or who can not be ruled out lymph node or vascular invasion,should be contraindicated for LT.
6.Content Determination of Paeoniflorin in Xiaohe Syrup by HPLC

OBJECTIVE:To establish a method for determining the content of paeoniflorin in Xiaohe syrup. METHODS:HPLC method was adopted. The Waters SunFire C18 column was used with the mobile phase of acetonitrile-0.1% phosphoric acid(12∶88, V/V)at the flow rate of 1.2 ml/min;the detection wavelength was 230 nm with column temperature at 30℃. Sample size was 10μl . R E-SULTS:There was a good linear relationship between the amount of paeoniflorin and peak area in the range of 0.061 0-1 . 2 2 0 8 μg (r=0.999 9). The RSDs of precision,stability and reproducibility tests were all less than 2% and the average recovery was 98.77%(RSD=1.09%,n=6). CONCLUSIONS:The method is simple,rapid and accurate and can be used for the content deter-mination of paeoniflorin in Xiaohe Syrup.
7."Re-evaluation of ""Shanghai Criteria"": indication of liver transplantation for hepatocellular carcinoma"

Objective To reevaluate Shanghai Criteria of liver transplantation (LT) for hepatocellular carcinoma (HCC).Method We evaluated the outcome of 433 consecutive patients with HCC who underwent LT from 2001 to 2013 at Zhong shan Hospital,Fudan University.The Kaplan Meier product-limit method with log rank test was used to evaluate survival rate according to different Criteria.Result The 5-year overall survival (OS) rate and relapse-free survival (RFS) rate were as follows:Milan Criteria (n =169; 72.5% and 79.0%),UCSF Criteria (n =223; 68.2% and 75.7%) ;Shanghai Criteria (n =255; 65.9% and 70.6%) ; Pittsburgh Criteria (n =419,55.6% and 61.2%).There was no significant difference in the 5 year OS and RFS between Shanghai Criteria group and Milan Criteria group (P>0.05).Conclusion Shanghai Criteria,which modestly expanded current criteria while still preserving similar long term survival,could be more fit for the situation of China.
8.Progress of research on chitosan-based microspheres in oral tissue lesions

Chitosan-based microspheres use chitosan as the main material to obtain particles with special structures through microsphere processing technology. They have the ability of slow and controlled release of drugs and the role of scaffolding, which have great application prospect in stomatology, but the application of chitosan-based microspheres is still in the research stage and has not yet been applied in clinical practice. This article reviews progress of domestic and foreign research on chitosan-based microspheres, in aspects of treatment of oral and jawbone tissue defects, periodontal diseases, dental pulp diseases and nerve tissue injury, in order to provide reference for follow-up research.
9.Role of TAK1 mediated cell autophagy after global cerebral ischemia-reperfusion in rats

Objective To study the effects of TGFβ-activated kinase-1 (TAK1)mediated cell autophagy after global cerebral ischemia-reperfusion (IR ) in rats.Methods Seventy-two male Kunming rats were randomly divided into six groups:control group (group C),sham operation group (group S),ischemia-reperfusion group (group IR),TAK1 shRNA lentivirus group (group T),nega-tive lentivirus group (group Y)and normal saline group (group NS)(n = 12 each).The rats in groups T,Y and NS received cerebral ventricles injection of TAK1 shRNA lentivirus,negative lenti-virus and normal saline 10 μl two weeks before preparing animal model.Using thread embolism of the right middle cerebral artery occlusion (MCAO)to cause focal ischemia for 2 h and released for 24 h for reperfusion in groups IR,T,Y and NS.The common carotid arteries were separated but not liga-ted in group S,the rest of the procedure as the same as group IR.The rats of each group were evalua-ted by neurological severity scores (NSS)24 h after reperfusion,the cerebral infarct volume was measured with the method of TTC and the expression of TAK1,LC3Ⅱ/LC3Ⅰ,Beclin1 and p62 pro-tein in rat hippocampus were determined by using Western blot.Results The infarct volume and NSS in groups IR,T,Y and NS were significantly higher than those in group C (P <0.05).The infarct volume and NSS in group T were significantly lower than those in group IR (P <0.05).TAK1 pro-tein of hippocampus in groups IR,Y and NS was significantly higher than that in group C (P <0.05).TAK1 protein of hippocampus in group T were significantly lower than that in group IR (P <0.05).LC3Ⅱ/LC3 Ⅰand Beclin1 protein of hippocampus in groups IR,T,Y and NS were signifi-cantly higher than those in group C,and the p62 protein of hippocampus in groups IR,T,Y and NS was significantly lower than that in group C (P <0.05).The LC3 Ⅱ/LC3 Ⅰand Beclin1 in group T were significantly lower than those in group IR,and the p62 protein of hippocampus in group T was significantly higher than that in group C (P < 0.05 ).Conclusion TAK1 mediated cell autophagy takes part in the mechanism of brain ischemia-reperfusion injury in rats.
10.Observation of Pattern Changes of Syndrome in TCM of 258 Cases of Acute Stroke and Therapeutic Effect Evaluation of Small Compound Prescriptions in Stroke Unit with Treatment Based on Syndrome Differentiation

Objective: To investigate into the etiological factor,distribution of syndrome(patterns),pathogenesis features as well as the curative effects of small compound prescriptions of(Traditional) Chinese Medicine(TCM) in the treatment of acute stroke in stroke unit.Methods:(Cases) corresponding to diagnosis standards were randomly divided into control group(102 cases) and treatment group(156 cases).Conventional treatment methods were given to cases in control group.On the base of control group,small compound prescriptions were added to in treatment group,successively 14-21 days.The relation between types and time and TCM scale(TCMS),the NIH stroke scale(NIHSS) and Barthel index(BI) were observed in the two groups.Results: Stroke was age-related,the older,the more stroke patients.Stroke was closely related to hypertension.Ascending hyperactivity of liver yang was main type in the previous three days,this type(decreased) with the time going on.On the contrary,the type of wind phlegm and blood stasis was secondly main type in the previous three days,it increased with the time going on.Types of ascending hyperactivity of liver yang,wind phlegm and blood stasis as well as phlegm heat and constipation covered 95 per cent of all patients in the previous 21days.In the two groups,the mean scale of TCM and NIH decreased,and BI increased after treatment,this statistical difference could also be seen between the two groups.Conclusion: Excessive types are main types in the acute stage of stroke.Types of ascending hyperactivity of liver yang,wind phlegm and blood stasis as well as phlegm heat and constipation are its main types.Transferring of ascending hyperactivity of liver yang into endogenous wind may be trigger of reaction link of wind,fire,phlegm and blood stasis in TCM theory.Small compound prescriptions of TCM aiming to clearing away liver fire to stop wind,to activate blood for removing blood stasis and phlegm,to removing phlegm and heat from discharge can reduce scale of TCM and NIH,increase BI in the treatment of patients in stroke unit,which provide beneficial proofs for evaluation of effects of TCM.
