OBJECTIVE:To explore the treatment effect and complication prevention of various stent materials for intracranial arteriostenosis.METHODS:A computer-based online search of Science Direct and Ei database (1998-01/2009-10) was performed for English articles with the key words of "intracranial arteriostenosis,stent placement" In addition,CNKI and CBM database (1998-01/2009-10) were searched for related Chinese articles with the same key words in Chinese.Moreover,related works were manually searched.Studies regarding various stent materials for intracranial arteriostenosis were included,including basic and clinical experiments.RESULTS:Multicenter studies of worldwide show that stent implantation displays obvious advantages in treating carotid artery stenosis compared with carotid endarterectomy.Recently,with developing of balloon and stent compliance,as well as stent technology,and application of various novel materials and cerebral protection device,stent has become a safe and effective therapy for intracranial arteriostenosis.However,complications following stent implantation,such as restenosis,ischemic stroke,cerebral hemorrhage hypertransfusion syndrome,and cerebral angiospasm,limit its application.CONCLUSION:The safety and efficacy of intracranial stent implantation remains validation of multicenter and perspective studies.High incidence of restenosis following stent implantation requires further improvement.

Objective To systematically analyze the effectiveness and security of Chinese medicinal ointment treating eczema for the purpose of making an objective and creditable evaluation and guiding clinical usage. Methods Data in the VIP database (1989-2008) and CHKD (1994-2008) were researched and literatures on the treatment of eczema were included. Randomized control trails (RCTs) of therapeutic effects were compared between Chinese medicinal ointment and western medical treatment. Jadad Scale was adopted and RevMan4.2.7 was used to conduct Мeta analysis. Results 11 RCTs were adopted, including 1415 cases.① Heterogeneity Test: It was found through analysis with fixed effect model that Q= 16.57, and P=0.08>0.05,which indicated that the effective between studies are homogenous. ② Total effect size' ORunited was 0.17,with the 95% confidence interval [0.29,0.58]. ③ x2=5.04 (P<0.01), P<0.05, the total ORunited had statistic significance, which indicated that the treatment of eczema with the Chinese Medicinal Ointment was obviously better than that of western medicinal Ointment. Conclusion It is a effective method to treat eczema with Chinese Medicinal Ointment, and there is no serious ADR. However, because of publication bias and RCTs of universal lower quality, the credibility of the conclusion was reduced.

Objective To present experience on the diagnosis and treatment of bladder outlet obstruction (BOO) caused by bladder neck fibrosis. Methods 30 cases of BOO caused by bladder neck fibrosis were studied. All the patients were treated by transurethral partial resection or incision of the cervix vesicae combined with alpha 1 adrenoceptor antagonists such as tamsulosin and terazosin or parazosin. Results Urination has been improved in all.The patients have been followed up for 3 to 30 months.On repeated uroflowmetry,the 30 patients showed the urine flow rates have been more than 15ml/s. IPSS scores was 5.3?1.7(25.4?4.2 preoperative) and quality of life scores was 1.4?0.6(4.1?0.8 preoperative).Fibroblast proliferation was noted in all the 30 and obvious chronic inflammation in 18. Conclusions Clinical symptoms, pressure rate measurement and cystoscopy are the reliable diagnostic methods.Transurethral partial resection or incision of cervix vesicae combined with alpha 1 adrenoceptor antagonists is effective.

BACKGROUND:The type B synoviocytes induced by transforming growth factor β1(TGF-β1) have the potential to differentiate into chondrocyte,which can remain the phenotype in vitro. However,whether the transfected cell combined with scaffold can form cartilage tissues need further research. OBJECTIVE:Rabbit type B synoviocytes was transfected by liposome method in vitro,combined the cells with Pluronic-F127,and then implanted into nude mouse to construct tissue engineering cartilage. Additionally,to explore the feasibility of synoviocytes differentiate into chondrocytes.DESIGN,TIME AND SETTING:The randomized animal experiment of cytology observation. The experiment was performed at the Medical Research Center,the Second Affiliated Hospital of Sun Yat-sen University between April 2007 and May 2008.MATERIALS:Healthy New Zealand white rabbits,aged 3 months,and 12 BALB/c nude mice,aged 4 weeks,weighted 20 g.METHODS:The synovial membrane tissues were taken out from the rabbit knee,isolated by enzyme digestion,and then transfected. The positive cloning was screened by G418,and the expression of TGF-β1 and collagen Ⅱ was detected. The Pluronic-F127 was dissolved at 4℃ and prepared fluid with concentration of 30%. The fluid was mixed with cells. At the same time,there were 2 groups as the control group:chondrocytes with Pluronic-F127,and synoviocytes transfeced by pcDNA3.1(+)and Pluronic-F127. The cell density was 5x1010/L. Each compound (0.2 mL) was injected into the subcutaneous of the nude mouse back. Rats were sacrificed at weeks 4,6 and 8 to harvest samples.MAIN OUTCOME MEASURES:Cell growth curve;phenotype change of the cells after transfection;histological observation of the tissue in the subcutaneous of the nude mouse back. RESULTS:Cell growth curve demonstrated that the living activity of the transfected cells was temporary decreased,which returned into normal level at days 6,7 after transfection. At day 4 after transfection,TGF β1 were positive expressed,and at day 7,the collagen Ⅱ staining were positive. The compounds in the subcutaneous of the nude mouse back formed immature chondroid tissues at week 4,which turned to mature chondroid tissue at week 8,and the collagen Ⅱ staining were positive.CONCLUSION:The transfected synoviocytes can express the phenotype of chondrocytes in vitro,and form chondrocyte-likecells. The transfected synoviocytes with Pluronic-F127 can form chondroid tissues in nude mice.

Objective To study the impact of QseBC on the motility of Salmonella enterica serovar Typhi ( S.Typhi ) . Methods The motility of wild-type ( WT) and null mutants (ΔqseB and ΔqseC) at mid-log phase was investigated by swimming assay.Quantitative RT-PCR was carried out to calculate the transcriptional variation of flhD and qseB among WT,ΔqseB andΔqseC.QseB overexpressing strain was constructed to compare its motility and flhD expression with the wild-type control.Results The result of motility assay showed that the motility of ΔqseB was similar to that of the WT strain , while the motility of ΔqseC was much lower than that of WT .qRT-PCR revealed that compared with WT , the expression of flhD was significantly decreased in ΔqseC while the expression of qseB was increased considerably .The motility of QseB overex-pressing strain was lower .Conclusion The expression of flhD may be regulated by QseBC which has an effect on the motil-ity of S.typhi, and the overexpression of QseB may inhibit the motility .

Objective To explore the predictive value of heart-type fatty acid-binding protein (HFABP) in predicting the cardiovascular events after none-cardiac thoracic surgery in aged patients.Methods Sixty eight aged patients were admitted from February 2012 to August 2014 and accepted none-cardiac thoracic surgery.The patients'general information and the incidence of postoperational cardiovascular events were recorded.Serum H-FABP was measured.The receiver operating characteristic (ROC) curve was constructed and the predictive effect of serum H-FABP for prognosis was analyzed.Results Eighteen patients were suffered from postoperative cardiovascular events (PCE).The level of H-FABP after surgery in patients with PCE was higher than that with none-PCE (P ＜ 0.05).The serum concentration of H-FABP after surgery and NT-proBNP before surgery was the independent risk factors for the elderly patients undergoing non-cardiac thoracic surgery.The area under ROC of H-FABP was 0.840 (95％ CI:0.716 ～0.964,P ＜0.01).The sensitivity and specificity of predicting postoperational cardiovascular event with serum concentration of H-FABP ＞5.26μg/L was 77.8％ and 84.0％ (OR 15.944,95％ CI:4.239 ～59.978,P ＜0.01).Conclusions The level of H-FABP after surgery is an independent risk factor for the occurrence of PCE for the elderly patients undergoing non-cardiac thoracic surgery.It can predict the risk of PCE after non-cardiac thoracic surgery in aged patients.

Objective To observe the effect of intact vagus nerve stimulation(IVNS)on liver inflammation and mRNA expression of suppressors of cytokoine signaling(SOCS)in rats. Methods Eighty Sprague Dawley rats were randomly divided into control group(n=20),sham group(n=20),LPS group(n=20,with intravenous injection of LPS to induce system inflammatory response)and LPS +IVNS group(n=20,with intact vagus nerve stimulation after LPS administration).The level of TNFαand IL-10 in the liver was detected with enzyme linked immunosorbent assay at 0,2,4,6 hours post-injection time.mRNA expressions of SOCS1 and SOCS3 in the liver were also determined with reverse transcription and polymerase chain reaction(RT-PCR). Resuits The liver TNFαin LPS+INVS group was lower than that of LPS group after LPS injection at 2,4 and 6 hours(P＜0.05).Compared with LPS group,liver IL-10 level in LPS+IVNS group was higher(P＜0.05)at post-injection time points(4 and 6 hours).After four hours of LPS challenge,mRNA expressions of both SOCS1 and SOCS3 were significantly increased.Compared with LPS group,mRNA expression of SOCS3 in LPS+INVS group was upregulated more significantly(P＜0.01),while mRNA expression of SOCS1 showed insignificant difference. Conclusions IVNS can depress liver inflammation and the anti-inflammatory mechanism involves SOCS signal transduction pathway.

Objective To summarize the experience of surgical management of carotid tumors ( CBT ) and application of shunt between common and internal carotid artery intraoperatively. Methods Thirty patients of CBT (mean age:39. 2 ±2. 3 years old,10 male and 20 female, 15 in left, 14 in right and 1 in both sides) who underwent surgical resection, were retrospectively reviewed. The average size of CBT was 4.9 ±0.3 cm. The diagnosis was established by ultrasound, CT, MRI or carotid arteriography. 16 patients underwent surgical resection of CBT, 10 patients underwent additional ligation of external carotid artery, and 4 patients underwent additional intraoperative shunt between common and internal carotid artery. Results Surgical procedures were successfully performeded in all 30 patients with CBT. Intraoperative shunts were successfully used between common and internal carotid artery in 4 patients. The postoperative complications included hoarseness (15) , bucking (11), crooked tongue ( 17) , dyspnea (1), dysphagia(3). There was no hemiplegia and death. Conclusion Surgical resection is the choice of treatment of carotid body tumor. The application of intraoperative shunt between common and internal carotid artery in complicated Shamblin Ⅲ stage is safe and effective.

Objective To investigate effect of dexmedetomidine on postoperative analgesia pump dose and effect.Methods 50 cases of patients with abdominal surgery under general anesthesia were selected.According to the postoperative analgesic drugs were divided into control group and experimental group, 25 cases in each group were given corresponding drug treatment.After treatment, the visual analogue scale, comfort score, adverse reaction rate and dosage of analgesic drugs were detected and compared.Results Compared with the control group,the VAS score were lower(P <0.05),the BCS score were higher(P<0.05),the adverse reaction rate were lower(P<0.05),the dosage of analgesic pump were lower(P<0.05). Conclusion Dexmedetomidine can significantly reduce postoperative pain degree of patients, reduce the incidence of adverse reaction, reduce analgesic dosage of the drug pump.
