Acrylonitrile is most commonly used aliphatic nitrile compounds characterized by the structural formula R-C=N and used mossy to make acrylic fibers, plastics, synthetic rubber, and wall coverings. In recent, because of its extensive usage and the rapid expansion of the chemical industry, many poisonings have been reported and many studies on its health effects have been performed. Acute toxicity resembles cyanide poisoning and results mainly in effects on the nervous system. High exposure also can cause temporary damage to red blood cells and the liver and can cause lead to death. Because long-term occupational exposure to the acrylonitrile has been with cancer in humans, the U.S EPA classifies acrylonitrile as probable carcinogen. For this reason, The federal government has developed regulations and advisories to protect individuals firm the potential health effects of acylonitrile in the environment, but there are few studies, case reports and regulations of the government in our country. We experienced acute poisoning caused by acrylonitrile inhalation that occurred in an industrial accident. So, we report this case with literature reveiw.

BACKGROUND: Studies on the reparation of peripheral nervous system are not only a key focus in medical research, but also a major topic for biologists, biochemical engineers and materialogy specialists. OBJECTIVE: To investigate the individual difference of ideal muscle bridging neurologic defect configuration on nerve regeneration. DESIGN, TIME AND SETTING: The control experiment was performed at the Shenyang Medical College from June 2004 to September 2005. MATERIALS: Ten common dogs weighing 20-30 kg were used in this study. METHODS: After intravenous anesthesia, muscle strips in the caudal right sartorius and middle musculus flexor digitorum superficialis of dogs were sliced into two segments. One was observed by dissection under a stereoscope. Another received Haematoxylin & Eosin (HE) Staining and transverse area of muscle fiber of distal and proximal ends was analyzed by image analysis. The same muscle strip was sutured at 2 cm neurologic defect region of autologous left and right limbs. Number of medullated nerve fibers was measured after osmic acid staining. MAIN OUTCOME MEASURES: Morphological difference of muscle bridge, transverse area of muscle fiber of distal and proximal ends, and number of medullated nerve fibers at bridging femoral nerve defect of left and right limbs. RESULTS: The muscle fiber in sartorius paralleled the longitudinal axis of muscle strip. The angle between muscle fiber in musculus flexor digitorum superficialis and the longitudinal axis of muscle strip was big. 80% muscle fiber in sartorius was equa1 to the length of the muscle bridge. Muscle fiber in the musculus flexor digitorum superficialis was shorter than the length of muscle strip. Muscle fiber in the musculus flexor digitorum superficialis was thick, whereas muscle fiber in sartorius was thin. OBJECTIVETo investigate the causes and managements of postoperative neurological complications in pedicle screw internal fixation for the treatment of degenerative scoliosis (DS).

METHODSThe data of 325 patients with degenerative scoliosis underwent pedicle screw internal fixation was retrospectively analyzed from February 2000 to April 2013. There were 22 patients with postoperative neurological complications. Of them, 16 cases complicated with numbness or pain of lower limb and 6 cases with obvious sensation and motor function decreasing in lower limb. The patients were treated with trophic nerve, dehydration, glucocorticoids, reoperation according to the causes of disease. Postoperative at 3, 6 months and 1 year later, according to VAS scoring and muscule power improvement,the recovery of nerve injury was assessed.

RESULTSPostoperative at 3,6 months and 1 year later,VAS scoring of 16 patients with slightly nerve injury was 2.81 +/- 0.66, 1.94 +/- 0.77, 0.63 +/- 0.62, respectively, and the symptoms had obviously improved than 1 week after operation (P < 0.05). Postoperative at 3 months, among 6 patients with severe nerve injury,muscule power improved in 2 cases and no-improved in 4 cases, with VAS scoring of 4.83 +/- 1.17; postoperative at 6 months,muscule power still had not improved in 3 cases,with VAS scoring of 4.17 +/- 0.75; both of the VAS scoring had not significant difference than 1 week after operation (P > 0.05). One year later, there was no muscule power improvement in 2 cases,with VAS scoring of 3.00 +/- 1.26, there was significant difference than 1 week after operation (P < 0.05).

CONCLUSIONThe causes of postoperative neurological complication in internal fixation for the treatment of dengenerative scoliosis includes: dragging and torsion injury of spinal marrow and nerve root because of excessive orthopedic of scoliosis; inderect injury of nerve root because of malposition of pedicle screw; nerve functional impairment caused by spinal cord ischemia. Avoiding the above factors could decrease the complication and early discovery and treatment could decrease the adverse outcomes.

OBJECTIVE: To explore the influence of sublingual immunotherapy on Th2 type immunoreaction in patients with allergic rhinitis within 12 months' treatment. METHOD: Forty three patients with allergic rhinitis were followed-up within 12 months after receiving sublingual immunotherapy. The changes of clinic symptoms and immunoreaction were analyzed. VAS score was applied to evaluate the efficacy of clinic treatment. TIgE and sIgE in serum were tested to evaluate the systemic immunoreaction. Eosinophil counting in nasal discharge was applied to evaluate the local nasal mucosa immunoreaction. RESULT: VAS scores were decreased significantly one month after the starting point of treatment, companied by the decrease of EOS count in nasal discharge (P < 0.05). The changes of TIgE and sIgE in serum within 12 months of sublingual immunotherapy were not significant (P < 0.05). The levels of IL-4 and IL-13 in nasal lavage were decreased significantly after 12 months' treatment. CONCLUSION The bias of Th2 type immunoreaction existed in nasal mucosa immune system in patients with allergic rhinitis could been rectified partially after receiving sublingual immunotherapy for 12 months, but the significant changes were not observed in systemic immune system.

This study established an HPLC fingerprint of Tibetan medicine Shaji Gao from different habitats and lay a foundation for Shaji Gao varieties identification and preparation process. The chromatographic condition was as follow: Agilent zorbax SB-C18 (4.6 mm x 250 mm, 5 μm) eluted with the mobile phases of acetonitrile and 0.4% phosphoric acid water in gradient mode. The flow rate was 1.0 mL x min(-1), and the detection wavelength was set at 360 nm. The fingerprints of 15 batches Shaji Gao were carried out by similarity comparation, 7 chromatographic peaks were extracted as the common peaks of fingerprint, 3 peaks were identified, which were quercetin, kaempferol and isorhamnetin. The similarity degrees of 14 batches of samples were above 0.9 and 1 batch of samples was below 0.9. This is the first established fingerprint of Shaji Gao by using HPLC. This method has good precision, stability and repeatability that it could provide basis for quality control and evaluation of Shaji Gao.

Authors evaluated the accuracy of preoperative pelvic CT scan staging and its effects on management in 12 biopsy proved rectal cancer patients. Authors also studied postoperative CT in 5 patients to detect disease recurrence and metastasis. Preoperative CT staging was identical to surgical and/or pathological staging in 9 patients (75%), but it was underestimated in two cases and overestimated in one instance than in surgical stagings. In 7 cases, CT scan did not alter original choice of procedures. However, preoperative CT staging gave definitive informations to change management plans in 5 cases otherwise the treatment would be difficult and inadequate. Postoperative CT showed local recurrence in one and liver metastases in 2 cases. One of them was not detected at exploratory laparotomy.
